Not all women seek breast enlargement. Some want them to decrease, because they have problems with fatigue, back discomfort or simply do not feel comfortable with their appearance.

By José Manuel Meza It is no secret to anyone that one of the aesthetic procedures that are most in demand in plastic surgery clinics is breast augmentation; however, there are also cases of people who do not want big breasts and on the contrary, want to reduce them.

“There are patients who, due to motherhood and breastfeeding, have noticed a growth in their breasts, but there are also those who are genetic. In both cases, the weight eventually creates certain problems in terms of shoulder pain, because the bra leaves marks on their back.

“That’s one of the reasons why patients come to get a breast reduction. While many seek the increase, some women prefer a small breast,” said Ernesto Castellanos Muñoz.

The plastic surgeon commented that there are busts which can exceed up to one kilogram, which can be annoying.

“In my personal experience, I have come to remove volumes of up to 800 grams. It is a world of weight for the patient.

“But most of the average is 200 to 250 grams which are removed per bust for this type of surgery,” he said.

The vast majority, said the doctor, has an ideal cup C for the measurement of your breasts; nevertheless, everything depends on the comfort and appearance that a patient wants to have, so the measurements can have variations when it comes to removing the tissue from her bust.

Contrary to what one might think, more and more people are looking for this type of surgical intervention.

No todas las mujeres buscan aumentarse los senos.

Algunas quieren que disminuyan, porque presentan problemas de cansancio, malestar de espalda o sencillamente no se sienten cómodas con su apariencia.

Por José Manuel Meza Para nadie es un secreto que uno de los procedimientos estéticos que mayor demanda tienen en las clínicas de cirugía plástica es el aumento de busto; sin embargo, también existen los casos de personas que no quieren pechos grandes y por el contrario quieren reducirlos.

“Hay pacientes que por la maternidad y el amamantamiento han notado un crecimiento en sus senos, pero también las hay por genética. En ambos casos el peso les crea con el tiempo ciertos problemas en cuanto a dolor en los hombros, porque se les marca el brasier y en la espalda.

“Esa es una de las razones por la cual acuden las pacientes a hacerse una reducción de busto.

Mientras muchas buscan el aumento, algunas mujeres prefieren un pecho pequeño”, mencionó Ernesto Castellanos Muñoz.

El cirujano plástico comentó que hay bustos los cuales pueden llegar a excederse hasta en un kilogramo, lo cual puede ser molesto.

“En mi experiencia personal he llegado a quitar volúmenes de hasta 800 gramos. Es un mundo de peso para la paciente.

“Pero la mayoría en el promedio son de 200 a 250 gramos lo que se retira por busto para este tipo de cirugía”, manifestó.

La gran mayoría, expresó el médico, tiene un ideal de una copa C para la medida de sus senos; no obstante, todo depende de la comodidad y apariencia que quiera tener una paciente, por lo que las medidas pueden tener variaciones a la hora de retirar el tejido de su busto.

OPERACIÓN EN AUMENTO

Contrario a lo que pudiera pensarse, cada vez son más quienes buscan este tipo de intervención quirúrgica.

De acuerdo con el especialista, la reacción de las personas después de haberse sometido a este procedimiento es de una gran satisfacción.

“A pesar de que queda una cicatriz grande, la famosa ancla o la “T” invertida. A las pacientes de busto grande lo que menos les importa son las cicatrices, porque literalmente se quitaron un peso de encima”, resaltó.

Y reconoció que, paradójicamente, son casos de personas que están buscando el servicio de un cirujano plástico no tanto por estética, sino por comodidad.

“Es una cirugía de reconstrucción. Es cierto que buscamos que el busto aparte de reducirlo hay que ponerlo en una posición anatómica normal, levantarlo, pero también se pre-

tende que el busto quede lo más bello posible.

“Que la paciente no solamente se sienta satisfecha porque quedó más ligera, sino porque le quedó un busto bonito”, indicó.

Técnicamente cuando la persona tiene senos flácidos y caídos, este médico comentó que se retira el exceso de piel y tejido mamario.

“Debido a ese excedente, muchas pacientes toman una postura jorobada”, agregó.

CAMBIOS NATURALES DE SENOS

Castellanos Muñoz detalló que luego de un embarazo pueden presentarse tres escenarios en las mamas de una persona.

“Que el busto quede en su tamaño original de la edad adulta, aunque es raro. También puede haber una pérdida de volumen en algunos casos, pero también ganancia de tamaño, y es cuando buscan este tipo de cirugía, porque no están a gusto”, aseguró.

La ventaja de esta cirugía es que, a diferencia de Estados Unidos, aquí en México el costo es muy accesible, comparado con los precios de USA.

“Los precios pueden, incluso, variar de cirujano a cirujano, y aunque cada vez la cirugía plástica tiende a ser más accesible, la única recomendación que yo haría al público es que busquen a un cirujano plástico certificado”, manifestó el director de la clínica Silhouette en Reynosa, Tamaulipas.

Para que el paciente tenga esa seguridad Castellanos Muñoz sugirió consultar la Asociación Mexicana de Cirugía Plástica, donde aparece un listado con todos los médicos avalados para realizar estos procedimientos.

No es mucho el dolor de la cirugía de disminución de senos ni tampoco se compara con la molestia que representa llevar un excedente de peso todo el tiempo.

Además, otra ventaja es que el especialista hace pruebas y estudios de manera anticipada a la intervención quirúrgica, como el ultrasonido y la mamografía, lo cual le permite a una paciente descartar o detectar cualquier enfermedad en sus pechos.

Against obesity: advanced endoscopy

I

n our daily life, we are facing a health event very important and negative, which affects the population, it is obesity. Mexico and the United States occupy first and second place respectively. Obesity triggers diseases that cause a very high expenditure on health systems, including hypertension, diabetes, heart attacks, etc.

Among the many attempts to combat obesity are diets, physical exercise programs, special devices, reducing creams, medications, food supplements, appetite suppressants, reductive specialized clothes, medical procedures such as the intragastric balloon, until you reach to remove part of the stomach by means of large internal surgeries although of minimal invasion through the skin.

Endoscopic gastroplasty in the sleeve or endoscopic gastric sleeve is a new type of procedure to lose weight. This procedure reduces the size of the stomach through a suture device by means of a simple endoscopy. This procedure can be an option if you have considerable obesity (a body mass index of 30 or more), and diet and exercise have not worked.

Contra la obesidad: endoscopía avanzada

E

n nuestro diario vivir nos encontramos ante un acontecimiento de salud muy importante y negativo, que afecta a la población, se trata de la obesidad. México y Estados Unidos ocupan el primer y segundo lugar respectivamente. La obesidad desencadena enfermedades que causan un gasto muy alto en los sistemas de salud, entre ellas la hipertensión, diabetes, infartos cardiacos, etc.

Dentro de los tantos intentos para combatir la obesidad se encuentran dietas, programas de ejercitación física, aparatos especiales, cremas reductoras, medicamentos, suplementos alimenticios, inhibidores del apetito, ropas especializadas reductivas, procedimientos médicos pasajeros como el balón intragástrico, hasta llegar a quitar parte del estómago por medio de grandes cirugías internas aunque de mínima invasión a través de la piel.

La gastroplastía endoscópica en manga o manga gástrica endoscópica es un nuevo tipo de procedimiento para bajar de peso. Con este procedimiento reduce el tamaño del estómago a través de un dispositivo de sutura por medio de una sencilla endoscopía. Este procedimiento puede ser una opción si tienes obesidad considerable (un índice de masa corporal de 30 o más), y la dieta y el ejercicio no te han dado resultado.

La manga gástrica endoscópica reduce el tamaño del estómago sin necesidad de cirugía, utilizando una sonda delgada y flexible para crear un estómago más pequeño.

Es reversible y sin necesidad de cirugía abdominal, reduce la cantidad de alimento que un individuo puede ingerir, disminuyendo a solo 3-4 onzas la capacidad del estómago.

Es impresionante cómo esta sencilla técnica permite lograr reducir el tamaño del estómago SIN afectar su función, ya que es indispensable que todo el estómago intervenga en realizar la digestión para una vida sana. With obesity comes diseases and metabolic disorders that lead to problems with glucose, cholesterol, and triglycerides, bypass or gastric sleeve surgery can help solve this issues.

By Beatriz Flores Currently, bariatric surgeries in addition to helping in weight loss have multiple benefits and are of great help in the control of other conditions, such as metabolic disorders.

As its name says, metabolic disorders are a series of alterations in metabolism, that is, alterations in the processes that the body performs to obtain or produce energy.

“It has to do with the digestive tract, food or certain hormones, there are many metabolic alterations, basically they are alterations of glucose, cortisol, insulin, high blood pressure, cholesterol levels and triglycerides, all related to obesity,” the specialist in laparoscopic surgery, Oscar Treviño explained.

Although there are many surgical procedures focused on weight loss, the doctor said that not all are focused on correcting metabolic disorders such as gastric bypass and gastric sleeve.

“The procedure that is one hundred percent guaranteed for this is the gastric bypass. In it, we make a bridge with a cross of certain organs that produce hormones and this influences the function of certain substances that develop the metabolic syndrome,” he explained.

He added that in some cases the gastric sleeve can help, which works restrictively by limiting gastric capacity.

To know which procedure is appropriate, an individualized study protocol is made to each patient in which a detailed clinical history is made.

It includes age, sex, gender, race, genetic influence, that is if it has a genetic load to develop some type of disease.

He explained that those who have mental illnesses that are not treatable and people who do not control their consumption of alcohol will not be able to submit to them.

“Also those who do not understand the procedure or the change that they will suffer and patients older than 65 years or younger than 16 years, mostly because of their maturity, the young patient hardly understands the changes that all this implies. Sometimes they require psychological support or some kind of guidance,” he said.

He warned that the metabolic syndrome is the most severe that exists in terms of obesity and pointed out that the number one cause worldwide of death in obese people are cardiovascular diseases.

Actualmente las cirugías bariátricas además de ayudar en la pérdida de peso tienen múltiples beneficios y son de gran ayuda en el control de otros padecimientos, como son los trastornos metabólicos.

Como su nombre lo dice, los trastornos metabólicos son una serie de alteraciones del metabolismo, es decir, alteraciones de los procesos que realiza el organismo para obtener o producir energía.

“Tiene que ver con el tubo digestivo, la alimentacion o ciertas hormonas, hay mu-

chas alteraciones metabólicas, básicamente son alteraciones de la glucosa, cortisol, insulina, alta presión, niveles de colesterol y triglicéridos, todos relacionados con la obesidad”, detalló el especialista en cirugía laparoscópica, Oscar Treviño.

Aunque existen muchos procedimientos quirúrgicos enfocados en la pérdida de peso, el médico precisó que no todos están enfocados en corregir alteraciones metabólicas como lo hacen el bypass gástrico y la manga gástrica.

“El procedimiento que está cien por ciento avalado para ello es el bypass gástrico.

Agregó que en algunos casos puede ayudar la manga gástrica, que funciona de manera restrictiva al hacer una limitación de la capacidad gástrica.

Para saber cuál procedimiento es el adecuado, se hace un protocolo de estudio individualizado a cada paciente en el cual se hace una historia clínica detallada.

En ella se incluye edad, sexo, género, raza, influencia genética, es decir, si tiene carga genética para desarrollar algún tipo de enfermedad.

“Los pacientes que tienen madre y padre diabéticos o con problemas de colesterol tienen una tendencia alta, arriba del 60 por ciento de desarrollar alguno de estos padecimientos”, comentó.

Agregó que también se realizan estudios de laboratorio de rutina y algunos otros especiíficos para detectar si hay alteraciones o mal funcionamiento de órganos y todos esos factores son tomados en cuenta para decidir cuál procedimiento quirúrgico se realizará.

NO SON MILAGROSAS

A pesar de que estas cirugías están destinadas a eliminar la obesidad y de tener gran efectividad en contra de los trastornos metabólicos, no actúan únicamente por sí mismas.

El paciente deberá cambir hábitos y seguir ciertas recomendaciones para alcanzar sus objetivos.

“Las cirugías son herramientas, digamos que tienen indicaciones de uso y si no se usa correctamente no va a dar los resultados adecuados. No es nada más de que te opero y ya; el paciente debe cambiar acciones y tener un control continuo”, advirtió.

Detalló que con las cirugías el paciente logrará disminuir su aporte de medicamentos para controlar sus enfermedades y bajar niveles de colesterol, insulina, entre otros, pero si el paciente se apega al manejo integral se cura al cien por ciento.

“Es común que el corazón de un paciente hipertenso batalle para distribuir todo el aporte sanguíneo a un cuerpo grande. Conforme va disminuyendo su peso el corazón sufre menos y ya no es necesario el mismo aporte de medicamentos y se va bajando las dosis hasta que deja de tomarlo”, comentó.

Añadió que el cambio en la presión arterial es un poco más tardado, ya que baja de forma lenta y quienes son diabéticos, al mes utilizan dosis muy bajas de insulina.

CIRUGÍA ÚNICA

Advirtió que hay pacientes que por alguna mala evaluación o capricho propio piden determinada cirugía y no se logran los resultados esperados, de cuaqluier forma, dijo, este tipo de procedimientos no se pueden hacer dos veces.

“Hay quienes, eligen una manga gástrica, cuya funcionalidad es de dos años y medio en cuanto a pérdida de peso, de ahí en adelante si las condiciones o los hábitos alimenticios siguieron igual, ese estómago vuelve a crecer, le caben más alimentos, se estanca y ya no perderá peso, al contrario, aumentará”, advirtió el especialista.

Está contraindicado realizar un procedimiento del mismo tipo cuando no funcionó el primero; lo que está indicado es que un procedimiento restrictivo sea convertido a un bypass gástrico.

“Ahí ya se habla de cirugía de conversión y se realiza una sola vez. Hay diferentes tipos de bypass gástrico y dependiendo de eso es si en un futuro puede haber alteraciones que requieran alguna modificacion del procedimiento”, explicó el médico.

PREPARADOS PARA EL CAMBIO

A pesar de que cualquier persona puede someterse a este tipo de cirugías, hay algunas cosas que deben tomarse en cuenta para saber si el paciente es candidato para ello.

Explicó que quienes tengan enfermedades mentales que no sean tratables y personas que no controlen su consumo de alcohol, no podrán someterse a ellas.

“También quienes no comprendan el procedimiento o el cambio que van a sufrir y pacientes mayores de 65 años o menores de 16 años, más que nada por la madurez,

el paciente joven difícilmente entiende los cambios que implica todo esto. A veces requieren apoyo psicológico u algun tipo de orientación”, expuso.

Advirtió que el síndrome metabólico es de lo más severo que existe en cuanto a la obesidad y precisó que la causa número uno a nivel mundial de muerte en personas obesas son las enfermedades cardiovasculares.

“Es importante tener en cuenta que un paciente con obesidad no tiene una calidad de vida óptima y de cantidad tampoco, pues se estima que vive hasta 15 años menos que un paciente normal”, expresó.

Many people who are channeled with the psychiatrist refuse to accept it, while those who already attend, hide it for fear of being labeled as crazy.

Many decide to go with the psychologist, who has a little more acceptance in society, although it is not adequate to meet certain cases.

The psychiatrist is a surgeon with a specialty in psychiatry and they are trained to treat mental and behavioral pathologies, explained Dr. Alejandro Caballero Torres.

Aún en estos tiempos en que la información fluye y parece llegar a todos, se sigue estigmatizando a quien acude con el psiquiatra, considerándolo un loco y al médico, como a alguien que “droga” a sus pacientes.

Por Beatriz Flores Muchas personas que son canalizadas con el psiquiatra se niegan a aceptarlo, mientras que quienes ya asisten, lo ocultan por temor a ser etiquetados como locos.

Muchos deciden acudir con el psicólogo, quien tiene un poco más de aceptación en la sociedad, aunque no sea el adecuado para atender ciertos casos.

El psiquiatra es un médico cirujano con especialidad en psiquiatría y está capacitado para atender patologías mentales y conductuales, explicó el doctor Alejandro Caballero Torres.

Reconoció que aunque hay un poco de mejoría en la percepción que la gente tiene sobre un psiquiatra y la salud mental, aún es un tema tabú debido a la mala información que en ocasiones “proporcionan hasta los mismos médicos”.

“Ellos mismos te dicen que con el psiquiatra no porque te va a drogar, pero ahora se sabe que la atención mental es tan importante como la física”, mencionó.

No es lo mismo un psiquiatra que un piscólogo, además de la cantidad de años de estudio y formación, el primero es un médico y el segundo un licenciado.

“La psiquiatría es una especialidad médica y se encarga de la atención de problemas en la fisiología o en la química cerebral; un psicólogo es un profesionista que hace una carrera universitaria de cuatro años, una licenciatura y generalmente atiende a través de habilidades técnicas para modificar conductas”, expuso.

El psiquiatra también utiliza dichas técnicas gracias a la amplitud de su formación.

NIÑOS Y ADULTOS NO SON TAN DIFERENTES

Tanto niños como adultos que lo necesiten van con el psiquiatra y en los menores de edad la atención inicia a partir de los 4 años.

Las consultas pueden ser en familia o individuales y se atienden pacientes con problemas de tipo afectivo, emocional o mental.

“Niños y adultos pueden desarrollar las mismas enfermedades, pero dependerá del grado de madurez mental cómo se dé, un cerebro va a madurar plenamente después de los 12 años y en algunos casos hasta después de los 20 y eso hace que la percepcion de una enfermedad se viva de manera distinta”, explicó.

Muchas veces se le resta importancia a los problemas mentales de los niños, sin embargo, su salud dependerá del

ambiente en el que viven y se desarrollan.

“Padres que discuten, infidelidades, inseguridad, entre otras cosas, alteran su salud mental, generalmente también desarrollan problemas de conducta, de aprendizaje o de ansiedad”, detalló.

Expresó que alrededor del 30 por ciento de sus pacientes son niños y que la mayor parte son adultos.

De ellos, son más las mujeres que acuden, pues considera que ellas pueden expresar mayormente sus emociones, mientras que los hombres se enfrentan al machismo que los conduce a conductas erróneas.

MALES ACTUALES

Muy probablemente la causa principal de consulta es el estrés, la angustia y la ansiedad generalizada.

A veces, trastornos obsesivos compulsivos, de pánico, fobias y problemas derivados con la percepción de inseguridad que se vive no solamente en la ciudad, sino en el país.

“Trastornos afectivos, las depresiones como consecuencia del estrés debido a la falta de oportunidades, un trabajo inestable, problemas familiares, pérdidas importantes, muertes, la terminación de un noviazgo, es lo que más frecuentemente tenemos aquí”, precisó

el especialista.

Sin embargo, importantes enfermedades mentales como la esquizofenia, trastornos bipolares, paranoias y trastornos delirantes deben ser atendidos por el psiquiatra quien elaborará un estudio diagnóstico para otorgar un tratamiento adecuado que incluirá apoyo farmacológico, psicoterapéutico y tal vez hospitalización.

El médico Alejandro Caballero Torres dijo que todo comienza en la familia y está relacionado con la formación, valores, ética y civismo que se fomenten en ella.

“Desgraciadamente se han perdido y puede ser debido a las modas que se presentan, también de la influencia de otros paises a través de la música, televisión, videojuegos e Internet”, comentó.

Extendió la invitación a las personas que consideren que están viviendo un proceso que esté afectando alguna área de la mente a acudir a consulta con el especialista indicado.

“Quiero que recuerden que no solamente por sentirse nervioso es por lo que un paciente viene, sino que hay alteraciones en muchas funciones mentales, como la memoria, el afecto, la forma de relacionarse con las personas y cómo experimentamos a través de los sentidos”, precisó.

Also called uterine myomatosis is one of the most common tumors in the uterine genital tract, said the specialist in Gynecology and Obstetrics, Dr. Teresa Tijerina Ortega.

“They are tumors of smooth muscle that appear in the womb or uterus up to 50 percent of women can suffer from them and depend on their location, will be the symptoms that arise, although most of the time it is asymptomatic and can go unnoticed”, she explained.

Its size ranges from three millimeters to 10 or 15 centimeters and the cause is due to hormonal effects.

“The female hormones, estrogen, and progesterone are what make these tumors grow in patients between 25 and 45 years, which is the fertile age where hormones are most active,” she explained.

Having presented menstruation very early, for example, at 9 or 10 years, or the onset of menopause late, at 49 or 50 are factors to develop them and also the use of contraceptives.

She explained that fibroids can be classified as submucosal, that is, under the surface of the uterine lining; subcerosos, under the outer covering of the uterus or intramurals, those that grow inside the muscular wall.

One of the most frequent symptoms, according to the specialist, is abnormal bleeding, which may occur outside or during menstruation but more abundantly.

It can be open or laparoscopic surgery, in which small incisions are made to perform it, although they are simple procedures, as with all surgical procedures there are risks, with bleeding being the greatest.

The specialist commented that after the surgery they can grow again, but the hormonal treatments will make a block to prevent it.

“There are treatments such as gonadotropin analogs that cause a menopause in the patient and will stop growing the fibroids, they are also indicated prior to surgery when they are very large tumors to reduce them and make it easier to remove them,” she explained.

Hysterectomy, said Tijerina Ortega, is an operation through which the uterus is removed, the ovaries can be removed or not, according to the doctor’s instructions.

The gynecologist said that if the patient no longer wants to have children and has very heavy bleeding, she can be a candidate, so a protocol will be made to do it, a valuation and any type of cancer should be ruled out.

“Any woman who hears that the womb will be removed is shocked. Many psychological aspects come into play, but surgery is not indicated in all cases, if I have a young patient without children, the matrix cannot be removed; It will depend on each person and of course that is well convinced of it”, she added.

Pueden medir de tres milímetros a 15 centímetros y el síntoma más común es el sangrado y aunque son benignos deben recibir tratamiento médico.

Las tumoraciones uterinas siempre son motivo de preocupación pues las pacientes de inmediato lo asocian con algún tipo de cáncer, sin embargo, los miomas o fibromas son benignos.

También llamada miomatosis uterina es una de las tumoraciones más comunes en el tracto genital uterino, mencionó la especialista en Ginecología y Obstetricia,Teresa Tijerina Ortega.

“Son tumores de músculo liso que aparecen en la matriz o útero y pueden padecerlos hasta el 50 por ciento de las mujeres y dependiendo de su ubicación, serán los síntomas que se presenten, aunque la mayoría de las veces es asíntomático y pueden pasar desapercibidos”, detalló.

Su tamaño va de los tres milímetros hasta los 10 o 15 centímetros y la causa es por efectos hormonales.

“Las hormonas femeninas, estrógenos y progesterona, son las que hacen que crezcan estas tumoraciones en pacientes de entre 25 y 45 años, que es la edad fértil en donde las hormonas están más activas”, explicó.

El haber presentado la menstruación de forma muy temprana, por ejemplo, a los 9 o 10 años, o la aparición de la menopausia de forma tardía, a los 49 o 50 son factores para desarrollarlos y también el uso de anticonceptivos.

Explicó que los miomas se pueden clasificar en submucosos, es decir, bajo la superficie del revestimiento uterino; subcerosos, bajo la cubierta externa del útero o intramurales, los que crecen dentro de la pared muscular.

Uno de los síntomas más frecuentes, según la especialista, es el sangrado anormal, el cual puede presentarse fuera de la menstruación o durante ella pero de forma más abundante.

“Es muy común que las mujeres que llegan con sangrados tengan anemia debido a la cantidad excesiva de sangre que se pierde, un nivel normal de hemoglobina es de 10, muchas personas llegan con 2, no es ni la mitad de lo que deben tener”, mencionó.

Debido a la anemia puede presentarse fatiga, poco rendimiento en sus actividades y dolor de cabeza.

Advirtió que cuando hay hemorragias muy severas pueden llegar a necesitar una transfusión sanguínea ya que corren el riesgo de presentar un choque hipovolémico.

También recibe el nombre de shock hemorrágico y ocurre cuando baja el volúmen sanguíneo al grado que el corazón no tiene suficiente sangre para bombear al resto del cuerpo y fallan los órganos por falta de oxígeno.

Si los miomas crecen mucho y por fuera de la matriz, pueden presionar los intestinos o la vejiga y esta compresión puede provocar colitis.

La ginecóloga dijo que, aunque de forma muy rara, puede presentarse una compresión durante las relaciones sexuales y ocasionar dolor, incomodidad y molestias.

Expresó también que si después de un año de tener relaciones sexuales la paciente no logra embarazarse, pueden ser un factor para la infertilidad.

“Esas tumoraciones que se encuentran por dentro de la matriz no permitirán que se implante el embarazo”, comentó.

DIAGNÓSTICO Y TRATAMIENTO

El diagnóstico se realiza por medio de un ultrasonido que puede ser transvaginal

u abdominal con los que se detectarán tumoraciones desde tres a cinco milímetros hasta los más grandes.

“El 50 por ciento no requiere tratamiento son asintomáticas solamente debe vigilarse que no vayan a crecer y el tratamiento depende de la edad y sintomatología”, expresó.

Si la paciente presenta hemorragias se puede administrar medicamento o realizar un procedimiento quirúrgico.

El tratamiento médico a base de hormonas puede ser en inyecciones, tomado o por medio del dispositivo intrauterino (DIU) y se recomienda a mujeres jóvenes que aún no han tenido hijos o desean tener más.

Con ello se logrará terminar con los sangrados y disminuir el tamaño de los miomas, expresó la doctora.

“El tratamiento médico es para pacientes que no tienen síntomas tan abundantes o quienes no puedan someterse a una

cirugia, si está cercana a la menopausia el tratamiento hormonal es la solución porque si la paciente deja de menstruar se acaba el problema de los miomas”, precisó.

En cuanto a los tratamientos quirúrgicos existen dos opiciones: la miomectomía y la histerectomía.

La miomectomía es un procedimiento quirúrgico por medio del cual se quitan los fibromas o miomas, dejando el útero intacto, explicó la doctora.

Puede ser cirugía abierta o laparoscópica, en la cual se hacen pequeñas incisiones para realizarla, aunque son procedimientos sencillos, como todo procedimiento quirúrgico hay riesgos, siendo el mayor el sangrado.

La especialista comentó que después de la cirugía pueden crecer nuevamente, pero los tratamientos hormonales realizarán un bloqueo para impedirlo.

“Hay tratamientos como los análogos de gonadotropina que causan una menopausia en la paciente y dejarán de crecer los miomas, están indicados también previos a la cirugía cuando son tumoraciones muy grandes para reducirlos y que sea más fácil extirparlos”, detalló.

La histerectomía, comentó Tijerina Ortega, es una operación por medio de la cual se extrae el útero, pueden quitarse los ovarios o no, según las indicaciones del médico.

La ginecóloga dijo que si la paciente ya no desea tener hijos y tiene sangrado muy abundante, puede ser candidata a ella, por lo que se realizará un protocolo para hacerlo, una valoracion y deberá descartarse cualquier tipo de cáncer.

“Cualquier mujer que escucha que le van a quitar la matriz se impacta. Entran muchos aspectos psicológicos en juego, pero la cirugía no está indicada en todos los casos, si tengo una paciente joven sin hijos no se le puede quitar la matriz; dependerá de cada persona y por supuesto que esté bien convencida de ello”, agregó.

La especialista destacó la importancia de acudir con el ginecólogo ante la presencia de cualquier sangrado anormal y no restarle importancia, pues el tratamiento a tiempo es lo mejor para la salud.

En cada uno de los casos es necesario evaluar el nivel de declive en las cejas a través de una exploración.

Mencionó que tanto hombres y mujeres pueden ser candidatos a esta operación si el diagnóstico así se lo indica.

“Cuando existe un grado de caída en las cejas normalmente la molestia principal es que los pacientes sienten los párpados pesados. Como si existiera algo de peso encima, aunque son dos zonas anatómicas diferentes.

“No hay que confundir cuando está suelta la piel del párpado con la caída de las cejas. Muchas veces van de la mano y en ocasiones se combina una cirugía de párpado con elevación de las cejas”, explicó.

Campos Martínez dijo que en promedio son más las mujeres quienes solicitan esta cirugía en una proporción de uno a 20 en cuanto a los hombres y recalcó la importancia de ofrecer un trabajo que ofrezca efectos duraderos.

“Hablamos de que la persona se vea bien no dos meses, sino cinco o siete años.

En algunas técnicas de rejuvenecimiento facial podemos decir que se logran resultados tan buenos como de 10 a 12 años”, resaltó.

Y reconoció que el hecho de que en el caso de las cejas el área a operar sea reducida no lo exime como cirujano plástico a llevar a cabo un riguroso protocolo.

“Nunca hay que minimizar las áreas porque la cicatriz o la zona anatómica es pequeña. También es importante recalcar que al ser una cirugía que se puede realizar con anestesia local no implica que resulte fácil, sencilla y que deje de ser compleja.

“El que sea una intervención ambulatoria no la hace libre de riesgos, por eso es necesario que sea efectuada por un mé-

dico certificado en cirugía plástica, estética y reconstructiva”, ponderó.

Indicó que después de la operación de cejas, es normal que la persona pueda estar hinchada.

“Es una zona anatómica que tiende mucho a la inflamación y es inherente al procedimiento. Afortunadamente podemos hacerla de tal manera que el proceso sea controlado, que no sea exagerado y que tampoco distorsione alguna de las estructuras.

“Se hace con tal cuidado que debe ser una inflamación moderada, aunque hay personas que tienden a inflamarse un poco más, pero debe ser algo mesurado”, puntualizó.

RECOMENDACIONES

Los pacientes que son sometidos a una cirugía de elevación de cejas deben mantener varios días de reposo en casa.

“Ninguna técnica de rejuvenecimiento facial que vaya con esta finalidad es totalmente rápida en su recuperación que al día siguiente la persona ya pueda reincorporarse a sus actividades.

“Necesita mantener un cierto reposo y el tiempo de recuperación es variable de

The recommendation is familiar: the consumption of omega 3 fatty acid is good for the heart.

That is the reason why millions of people from different parts of the world resort to fish oil supplements.

What is a macular hole?

T

he macular hole is when a tear or opening in the macula forms.

As the hole is formed, things in the central vision will be blurred, wavy or distorted and as it grows in size, a dark or blind spot appears in the central vision.

What are the causes of a macular hole?

Age is the most common cause of the macular hole. As it ages, the vitreous begins to shrink and become detached from the retina. In general, the vitreous detaches without problems. However, sometimes the vitreous can stick to the retina. This causes the macula to stretch and form a hole.

A macular hole may also form when the macula swells due to another eye di-

sease or caused by an eye injury.

How is a macular hole treated?

Surgery called vitrectomy is the best way to treat a macular hole. The ophthalmologist removes the vitreous that is pulling on the macula, then places a gas bubble inside the eye. This bubble helps to flatten the macular hole and keep it in place wh0ile the eye is healing. The gas bubble disappears by itself slowly.

The vision will improve as the macular hole closes. It may take several months until the hole has finished healing. The vision that you recover will depend on the size of the macular hole. It also depends on how long you had the hole before undergoing surgery.

¿Qué es un agujero macular?

E

l agujero macular es cuando se forma un desgarro o abertura en la mácula. A medida que se forma el agujero, las cosas en la visión central se verán borrosas, onduladas o distorsionadas y a medida que crece en tamaño, aparece un punto oscuro o ciego en la visión central. ¿Cuales son las causas de un agujero macular ?

La edad es la causa más común del agujero macular. A medida que envejece, el vítreo comienza a encogerse y a desprenderse de la retina. En general, el vítreo se desprende sin problemas. Sin embargo, a veces el vítreo puede quedarse pegado a la retina.

Esto hace que la mácula se estire y se forme un agujero.

También puede formarse un agujero macular cuando la mácula se hincha debido a

otra enfermedad ocular o causado por una lesión en el ojo. ¿Cómo se trata un agujero macular?

La cirugía llamada vitrectomía es la mejor manera de tratar un agujero macular. El oftalmólogo retira el vítreo que está jalando de la mácula, luego coloca una burbuja de gas en el interior del ojo. Esta burbuja ayuda a aplanar el agujero macular y mantenerlo en su lugar mientras se cura el ojo.

La burbuja de gas desaparece por sí sola lentamente.

La visión mejorará a medida que el agujero macular se cierre. Pueden pasar varios meses hasta que el agujero haya terminado de sanar. La visión que recupere dependerá del tamaño del agujero macular. También depende de cuánto tiempo tuvo el agujero antes de someterse a la cirugía.

Conjunctivitis is an inflammation of the conjunctiva, said doctor Luis Jian Mo, is the superficial gelatinous layer that covers the white of the eye, called sclera.

Conjuntivitis: No siempre es

contagiosa

Todo el mundo le teme a la conjuntivitis. Mucho se habla del intenso dolor, ardor y comezón que provoca, por lo que de inmediato se aísla a quien la padece, pero dependerá de su tipo si se contagia o no.

Por Beatriz Flores Son muchas las razones por las que una persona puede desarrollar conjuntivitis, esa enfermedad de los ojos que se caracteriza por un intenso color rojo, inflamación, lagrimeo, secreciones, comezón y sensibilidad en los ojos.

La conjuntivitis es una inflamación de la conjuntiva, la cual, dijo el médico Luis Jian Mo, es la capa gelatinosa superficial que cubre lo blanco del ojo, llamada esclera.

Contrario a lo que se piensa mayormente, la conjuntivitis no siempre es contagiosa.

“Las conjuntivitis contagiosas se dividen en agudas, que pueden ser bacterianas y virales, las más frecuentes son virales que son como una gripa del ojo y en muchas ocasiones el paciente tiene el antecedente de haber tenido o tiene una gripe al mismo tiempo”, explicó.

Dijo que las que no se contagian son de tipo alérgico o por una reacción autoinmune, es decir, una reacción exagerada de las defensas del organismo.

En este grupo también se encuentran las conjuntivitis tóxicas medicamentosas, que generalmente ocurren cuando el paciente se auto receta, sobre todo al usar gotas de antibióticos o vasoconstrictoras, utilizadas regularmente para los ojos rojos.

Explicó que el período de incubación de la conjuntivitis infecciosa viral, es similar al de un virus que causa la gripe, que va de 48 a 72 horas y los síntomas pueden estar presentes entre una y dos semanas.

En el caso de la bacteriana, puede durar entre siete y 10 días, mientras que las de tipo alérgico o por reacción dependerá del tipo de alergia y su exposición al agente que la causa.

sobre todo si trabaja con muchas personas o en lugares cerrados.

NIÑOS SON MÁS PROPENSOS

El especialista destacó que los niños son más propensos a desarrollar conjuntivitis alérgica y conjuntivitis vernalis, la cual se presenta mayormente en primavera u otoño.

También los lugares cerrados o con hacinamiento, como escuelas o guarderías, favorecen el contagio entre los menores.

Sin embargo, los adultos no escapan a ello lo cual puede suceder al tener contac-

to con gran cantidad de personas o lugares carentes de ventilación.

El saludar de manos y después tocarse la cara o los ojos, así como compartir toallas, maquillaje, lentes e incluso la almohada, son acciones que fomentan su propagación.

Es de suma importancia no autorrecetarse, pues al no conocer el tipo de enfermedad que se padece, podría empeorarse la situación. El médico advirtió que ante cualquier molestia ocular se debe acudir con el especialista para que recete el tratamiento adecuado.

“Se debe tratar de no autorrecetarse y acudir con su oftalmólogo de confianza, porque sus ojos son invaluables”, comentó.

According to specialists, oral cleaning is essential to prevent tooth decay and other health problems.

Dr. Ana Rubí Reyes Ibarra mentions that although this is a topic that may seem logical, many people do not know exactly the implications of not brushing properly and not go to the dentist either.

“What happens when the teeth do not receive the proper cleaning is that they change color, there are spots, mobility and bad breath is generated, apart from a red gum with bleeding,” she illustrated.

And she argued that bad breath most of

‘La limpieza es vital para la

salud dental’

De acuerdo con especialistas, la limpieza bucal es fundamental para evitar el deterioro de los dientes y otros problemas de salud.

Por José Manuel Meza Nuevo Progreso, Tam. Visitar al dentista puede alargar la vida de las piezas bucales, atender problemas de desgaste e, incluso, ayudar a detectar enfermedades como el cáncer.

La doctora Ana Rubí Reyes Ibarra menciona que aunque este es un tema que puede parecer lógico, mucha gente desconoce con exactitud las implicaciones que tiene no cepillarse de manera adecuada y no acudir tampoco al dentista.

“Hay personas que pasan cuatro o cinco años sin una limpieza dental y cuando tienen el problema es agudo y vienen con movilidad en sus piezas.

“La importancia de la limpieza dental es prevenir la pérdida de dientes y la enfermedad de encías”, indicó.

La principal causa de no ir a una consulta, agregó Reyes Ibarra, es que los pacientes sienten miedo.

“Nosotros mismos provocamos ese temor al no brindarles la confianza. Siempre hay que explicarles lo que van a sentir, que no les va a doler y que tenemos un aparato que vibra, que habrá mucha agua, pero que tenemos otro instrumento que la estará succionando.

“Uno tiene que brindarle la seguridad al paciente, de que levante su mano y nos avise si siente alguna molestia”, comentó.

La doctora, quien tiene su clínica en la primera cuadra de la calle Coahuila, de Nuevo Progreso, Tamaulipas (cruzando el puente internacional a mano izquierda, en el local 6), expresó que hay pacientes muy constantes en el aseo y protección de su dentadura; sin embargo, señaló que la gran mayoría tiende a descuidarse.

“Mucha gente me pregunta cada cuando puede acudir a su limpieza dental, pero esto depende: si es una persona constante en su cepillado diario, y siempre y cuando su técnica la realice bien, sus visitas serán eventuales dos veces al año.

“En cambio, si es un paciente que su aseo dura dos minutos y una vez al día, pues para mantener la dentadura bien tiene que venir cada mes, de lo contrario va a presentarse un deterioro”, manifestó.

La entrevistada recalcó que la limpieza bucal no solamente es esencial para prevenir la caries y la movilidad dental, sino también para detectar otras patologías.

“Hay causas que ocasionan el no atender nuestra boca, aparte de la pérdida de dientes, y son enfermedades del corazón, porque como estamos generando sarro y bacterias, lo terminan afectando.

“También producen malestares digestivos, muchas personas ignoran todo eso, pero son problemas que se están agudizando y cada vez los vemos más aquí en la consulta”, describió.

ALGUNOS SÍNTOMAS

Reyes Ibarra hace referencia de algunas manifestaciones cuando alguien presenta problemas bucales.

“La encía también se daña. Mucha gente nos dice que no viene al dentista porque solamente tiene sangrado de vez en cuando, pero si eso sucede es por una inflamación, la cual es causada por el sarro dental.

“Lo que ocurre cuando los dientes no reciben el aseo es que cambian de color, hay manchas, movilidad y se genera un mal aliento, aparte de una encía roja con sangrado”, ilustró.

Y agregó que el mal aliento la mayoría de la veces deriva de la falta del aseo. El 99 por ciento es por eso, insistió.

Dijo que la limpieza dental es un procedimiento simple que tarda de 15 a 20 minutos; sin embargo, si el paciente acude con sangrado de encías o enfermedad periodontal (que es la movilidad de las piezas); o con un acumulamiento de sarro de ocho a diez años, muy probablemente será necesaria la

Otro aspecto muy importante que resaltó la doctora es que las personas que toman antibióticos, anticonvulsivos; medicamentos para problemas mentales y diabéticos, todos ellos tienden a generar una inflamación excesiva de encías y deben estar yendo mínimo cada dos o tres meses a revisarse.

“Son pacientes que presentan mucha pérdida de hueso y necesitan ponerle mucha atención a sus dientes y no faltar a sus visitas dentales”, subrayó Reyes Ibarra.

Añadió que las caries y otras erupciones prematuras de muelas del juicio, alguna movilidad, malformación o un problema de cáncer pueden ser detectados desde un consultorio dental.

“Si una persona trae una tumefacción o una bolita que está cambiando de color, tamaño en paladar o en boca, nosotros lo vemos al momento de la visita.

“Y procedemos a hacer una prueba, mandamos biopsia con patólogo y ya se diagnostica”, añadió.

Comentó que por lo general los pacientes asocian al cáncer con otras áreas del cuerpo, pero la doctora Reyes Ibarra dijo que también puede presentarse en la boca.

“No hay edad para un carcinoma bucal y existen pacientes que nunca en su vida fumaron y han tenido cáncer de este tipo”, aseguró la dentista egresada por la Universidad de Matamoros.

“It can present with dermatological alterations, which is the most common, excessive sensitivity to the sun, the presence of lesions on the back of the nose, generically called butterfly wings; oral ulcers, joint problems, reflect an arthritis picture; the presence of neurological abnormalities or compulsive crises.

“It also manifests with effusions in serous cavities such as the pericardium, that is, the fibrous membrane that surrounds the heart; or show hematologic signs such as hemolytic anemia or low platelets; but the most serious is the renal disorders, where there is the retention of substances that affect the kidney.

“This is in late stages, but there are clinical criteria to make the diagnosis of lupus erythematosus. It is not necessary that all the symptoms are in the same patient to detect the disease, that is why we rely on laboratory studies,” described the interviewee.

With the advent of new technological tools, today it is easier to make a quick assessment of the case.

PREVAILS DISAGREEMENT

Until a few years ago, lupus was practically an unknown disease, but there are still many sectors of the population that ignore its existence, even when it has already claimed many lives.

“It is often confused with a prognosis similar to a cancerous problem. Until 1950 the mortality of lupus was very high, but

with the appearance of new therapeutic approaches, that figure decreased significantly.

“I believe that the public, in general, should be more informed to seek early attention,” Dr. Cruz Álvarez insisted.

This doctor said that a lupus patient assisted in a timely manner, for example, can have the same average life of a person suffering from diabetes or hypertension, but everything depends on the stage in which they are and diagnose.

“We need to avoid damage to what we call the white organ, such as the lung, brain, heart, liver, etc.,” he added.

The rheumatologist explained that the lupus antibodies hurt the organs and tissues, for which reason he emphasized making the patient an integral assessment and giving him reliable information and solid bases so that he can improve his quality of life.

“The relationship with the use of stem cells or alternative therapies has not been shown to improve the survival of the patient.

“The diagnosis is very important and the integral management also. Patients with lupus can lead a good life. Even to procreate family, as long as the disease is under control, the important thing is to avoid having long-term complications and suffer kidney damage or other,” he said.

While in private clinics in the United States the annual cost of the disease amounts to 35 thousand dollars.

The WHO estimates that 5 million people suffer from it around the globe, but many people with limited resources can not access medicines and their health worsens more quickly.

“Here we have to put on a scale what is cost-benefit. When talking about high-cost medicine, this would have to be viewed from a patient’s point of view of the quality of life.

“They are drugs that we call immunosuppressants or those that modify the immune response, including steroids or what the public knows as cortisone, but there is also a wide range of drugs called immunomodulators.

SOME FACTORS

It has been associated that the disease could have hereditary patterns and viral infections, but what is proven is that the recrudescence of lupus is related to prolonged exposure to sunlight and even with the intake of some foods.

“Ultraviolet rays have a certain range of action in patients with sun sensitivity or skin changes. It is advisable to avoid them using sunscreen that we call SPF, with a minimum of 50 or up to 100 values to avoid the reactivation of the disease.

“In relation to dietary management it has been seen that the intake of some products such as alfalfa can induce a modification of the immune response.

“Patients with lupus should not have a special diet, as long as they do not have a kidney condition, because then they should have a diet low in sodium and some proteins,” said Cruz Alvarez.

The active participation of the patient and his attachment to the treatment are indispensable to carry out an adequate control.

De acuerdo con la Organización Mundial de la Salud (OMS) afecta primordialmente a personas de tez oscura, pero también a latinos y aunque su origen no está del todo definido, existen tratamientos para controlarlo.

Según explica el reumatólogo Luis Javier Cruz Álvarez esta enfermedad tiene predominancia en el sexo femenino y normalmente aparece entre la segunda y cuarta década de la vida, además de que los rayos solares pueden estimularla.

“Tiene una prevalencia de aproximadamente cuatro mujeres por cada hombre, pero la idea es que la gente esté bien enterada y evaluada para no generar confusión”, mencionó.

Y es que el diagnóstico del lupus, dijo, siempre ha sido un dilema para el médico de primer contacto.

Para evitar caer en noticias falsas y resolver algunas dudas –ante cualquier sospecha de la enfermedad– recomendó consultar los sitios en Internet del Consejo Mexicano de Reumatología y del Colegio Europeo de Reumatología.

“Sin embargo, el diagnóstico del lupus eritematoso debe de ser abordado de preferencia por el médico internista o el reumatólogo.

“Desafortunadamente desde el inicio de la enfermedad y hasta que es evaluada por parte de un especialista a veces el tiempo que ha pasado es considerable y las secuelas pueden ser mayores, por eso es importante atenderse de manera oportuna”, comentó. ¿CÓMO SE DIAGNOSTICA?

una persona lo sufre o no.

“Puede presentarse con alteraciones dermatológicas, que es lo más común, sensibilidad excesiva al sol, presencia de lesiones en el dorso de la nariz, denominadas genéricamente como alas de mariposa; úlceras orales, problemas articulares, reflejan un cuadro de artritis; presencia de anomalías neurológicas o crisis compulsivas.

“Asimismo se manifiesta con derrames en cavidades serosas como el pericardio, es decir, la membrana fibrosa que envuelve al corazón; o mostrar signos hematológicos como anemia hemolítica o una baja de plaquetas; pero lo más grave son los trastornos renales, en donde existe la retención de sustancias que afectan al riñón.

“Esto es en etapas tardías, pero hay criterios clínicos para hacer el diagnóstico del lupus eritematoso. No es necesario que todos los síntomas estén en un mismo paciente para detectar la enfermedad, por eso nos apoyamos en estudios de laboratorio”, describió el entrevistado.

Con el advenimiento de nuevas herramientas tecnológicas, hoy en día es más sencillo hacer una rápida evaluación del caso.

PREVALECE DESCONOCIMIENTO

Hasta hace algunos años el lupus era prácticamente una enfermedad desconocida, pero aún hay muchos sectores de la población que ignoran sobre su existencia, incluso cuando ya ha cobrado muchas vidas.

“Suele confundirse con un pronóstico similar a un problema canceroso. Hasta 1950 la mortalidad del lupus era altísima, pero con la aparición de nuevos enfoques terapéuticos esa cifra disminuyó sensiblemente.

“Considero que le público en general debe de estar más informado para buscar una atención temprana”, insistió el

pueden acceder a los medicamentos y su salud empeora con mayor rapidez.

“Aquí tenemos que poner en una balanza lo que es costo–beneficio. Al hablar de una medicina de alto costo,esto tendría que verse desde un punto de vista de calidad de vida del paciente.

“Son medicamentos que llamamos inmunosupresores o aquellos que modifican la respuesta inmune, incluyendo esteroides o lo que el público conoce como cortisona, pero también hay una gran gama de medicamentos denominados inmunomoduladores.

“Hoy en día éstos tienen un costo variable y pueden estar accesibles a través de alguna institución”, sugirió el doctor.

ALGUNOS FACTORES

Este médico dijo que un paciente de lupus asistido de una manera oportuna, por ejemplo, puede tener el mismo promedio de vida de una persona que padece diabetes o hipertensión, pero todo depende de la etapa en la que se encuentre y diagnostique.

“Necesitamos evitar daños a lo que denominamos órgano blanco, como el pulmón, cerebro, corazón, hígado, etcétera”, añadió.

El reumatólogo explicó que los anticuerpos del lupus lastiman a los órganos y tejidos, por lo cual hizo énfasis en hacerle al paciente una evaluación de forma integral y otorgándole una información fidedigna y con bases sólidas para que pueda mejorar su calidad de vida.

“La relación con el uso de células madre o las terapias alternativas no han demostrado que mejoren la vida del paciente.

“El diagnóstico es muy importante y el manejo integral también. Los pacientes con lupus pueden llevar una buena vida.

Incluso llegar a procrear familia, siempre y cuando la enfermedad se encuentre bajo control, lo importante es evitar que tengan complicaciones a largo plazo y sufran daños renales u otros”, subrayó.

COSTO–BENEFICIO

El tratamiento para atender el lupus es oneroso. En promedio algunos sistemas hospitalarios de carácter público en latinoamérica erogan hasta 6 mil pesos mexicanos mensuales por cada paciente y 40 mil, cuando se atraviesa por una crisis.

Mientras que en clínicas particulares de Estados Unidos el costo anual del padecimiento asciende a los 35 mil dólares.

Se ha asociado que la enfermedad pudiera tener patrones hereditarios e infecciones virales, pero lo que sí está comprobado es que el recrudecimiento del lupus está relacionado con la prolongada exposición a la luz solar e incluso con la ingesta de algunos alimentos.

“Los rayos ultravioleta tienen un determinado rango de acción en pacientes con sensibilidad al sol o alteraciones dérmicas. Es conveniente evitarlos utilizando protector solar que denominamos FPS, con un mínimo 50 o hasta 100 valores para evitar la reactivación de la enfermedad.

“En relación al manejo dietético se ha visto que la ingesta de algunos productos como la alfalfa pueden inducir una modificación de la respuesta inmune.

“Los pacientes con lupus no deben llevar una dieta especial, siempre y cuando no presenten una afección renal, porque entonces sí deberán tener una dieta baja en sodio y algunas proteínas”, señaló Cruz Álvarez.

La participación activa del paciente y su apego al tratamiento son indispensables para llevar un adecuado control.
