Endoscopy is the most specific and efficient method in the diagnosis of gastroparesis.That is done by means of an endoscope that is a thin and flexible tube that is introduced through the mouth and the pharynx and transmits real images of the interior of the digestive system.

In addition to recognizing the disease, it can identify and treat associated local complications such as hemorrhage or obstruction.

Another method is the gastro-duodenal esophageal transit, this is a radiology study with which it manages to visualize the lack of emptying and the probable causes.

TREATMENTS

If the patient suffers from diabetes, their sugar levels should be corrected and this will help to improve the symptoms. It is also recommended to eat soft foods in small portions frequently, which can also help to improve the symptoms.

On the other hand some cholinergic drugs, erythromycin and prokinetic are used for this disease. Botox (botulinum toxin) injected by endoscopy can also be applied to the stomach outlet.

As well as perform surgery to create a new communication between the stomach and the intestine. This is an invasive procedure, which requires incisions in the abdomen and several days of hospitalization.

In the world level, minimally invasive treatment is carried out through an endoscopy. The method is called G-POEM or gastric POEM for its acronym in English that means endoscopic peritomy myotomy of the stomach.

Through an endoscopy a cut is made in the mucosa of the stomach about two inches before the outlet of the stomach called the pylorus, the formation of a tunnel begins in the submucosal space (below the mucosa) advancing until it pas-

ses through.

Subsequently, the section of the muscle fibers is made until the sphincter is cut and finally the initial incision is closed with staples called hemoclips, all through the endoscope.

The day after the procedure, a radiographic study is carried out to confirm the proper passage of a contrast medium from the stomach to the intestine and to start feeding with liquids.

This method of endoscopic treatment is carried out in a hospital environment where it has all the necessary resources for its development. With the help of a wide variety of equipment such as CO2 insufflator, high pressure solution installation equipment known as Water Jet, with more sophisticated electrocoagulation equipment that currently exists Erbe Vio 300D and with the use of a variety of attachments called “Knifes” or endoscopic knives all of Japanese origin, it is possible to perform the myotomy or sphincter cut without the need for either open or laparoscopic surgery.

Primer G-POEM en México,

se realizó con éxito

En Reynosa

Por primera vez en todo México se realizó el procedimiento endoscópico denominado G-POEM; el encargado del mismo fue el doctor Juan Carlos Caramés, líder en procedimientos de Endoscopia Intervencionista; ante la supervisión del doctor Michel Kahaleh, líder mundial en Endoscopia.

Por Pamela Escobar Dicho procedimiento se llevó a cabo el pasado 27 de agosto, catalogado a su vez, todo un éxito tanto en el desarrollo del mismo, como en la evolución clínica de la paciente.

Confirmando con ello que gracias a la alta precisión lograda a través de los procedimientos endoscópicos, se está revolucionando la forma de abordar y resolver muchas enfermedades digestivas sin cirugía, es decir, sin heridas en el abdomen.

Tras su realización la paciente fue egresada del hospital luego de 24 horas regresando a sus actividades de rutina sin limitaciones y comiendo de forma normal.

SÍNTOMAS

El G-POEM es un procedimiento endoscópico para corregir la gastroparesia, un padecimiento que provoca la disminución en la capacidad del estómago para vaciar su contenido al intestino, a veces a consecuencia de una obstrucción mecánica o bloqueo en éste último.

Para identificar si se padece gastroparesia algunos de los síntomas pueden ser distensión abdominal o sensación de llenado rápido con los alimentos, náuseas, vómitos de contenido alimentario, deshidratación, hipoglucemia, es decir, azúcar baja y pérdida de peso, este último se presenta cuando la enfermedad es crónica.

LA CAUSA

SU DIAGNÓSTICO

ALIVIAR LA GASTROPARESIA

Aunque se desconoce la causa exacta que origina la gastroparesia, ésta puede ser provocada por una interrupción de las señales nerviosas al estómago.

Esta afección es una complicación común ocasionada a su vez en pacientes con diabetes; a consecuencia de esclerodermia.

También puede presentarse después de cirugías que se realizan en el esófago o estómago en las que se cortan los nervios vagos, o bien, por el uso de algunos medicamentos anticolinérgicos (para las náuseas).

La endoscopia es el método más específico y eficiente en el diagnóstico de la gastroparesia. Misma que se realiza mediante un endoscopio que es un tubo delgado y flexible que se introduce a través de la boca y la faringe y transmite imágenes reales del interior del sistema digestivo.

Además de reconocer la enfermedad permite identificar y tratar complicaciones locales asociadas como la hemorragia o la obstrucción.

Otro método es el tránsito esófago gastro duodenal, éste es un estudio de

radiología con el que logra visualizar la falta de vaciamiento y las probables causas.

TRATAMIENTOS

En caso de que el paciente sufra diabetes se deben corregir sus niveles de azúcar y esto ayudará a mejorar los síntomas, además se recomienda ingerir alimentos blandos en pequeñas porciones frecuentemente, que también puede ayudar a mejorar los síntomas.

Por otro lado algunos medicamentos colinérgicos, eritromicina y procinéticos se utilizan para esta enfermedad.

También se puede aplicar Botox (toxina botulínica) inyectada por endoscopia en la salida del estómago.

Así como realizar una cirugía para crear una nueva comunicación entre el estómago y el intestino. Esto es un procedimiento invasivo, que requiere incisiones en el abdomen y varios días de internamiento hospitalario.

En la actuliadad a nivel mundial el tratamiento de mínima invasión se lle-

va a cabo mediante una endoscopia. El método se denomina G- POEM o POEM Gástrico por sus siglas en inglés que significan Miotomía endoscópica per oral del estómago.

A través de una endoscopia se realiza un corte en la mucosa del estómago unos cinco centímetros antes del orificio de salida del estómago llamado píloro, se inicia la formación de un túnel en el espacio submucoso (por debajo de la mucosa) avanzando hasta atravesarlo.

Posteriormente se realiza la sección de las fibras musculares hasta cortar el esfínter y por último se cierra la incisión inicial con unas grapas llamadas hemoclips, todo ello a través del endoscopio.

El día después del procedimiento se realiza un estudio radiográfico con el que se confirma el paso adecuado de un medio de contraste del estómago al intestino y se inicia la alimentación con líquidos.

Este método de tratamiento endoscópico se lleva a cabo en un medio hospitalario donde se cuenta con todos los recursos necesarios para su desarrollo.

Con la ayuda de una gran variedad de equipos como son insuflador de CO2, equipos de instilación de solución a gran presión conocido como Water Jet, con equipo de electrocoagulación más sofisticado que existe actualmente Erbe Vio 300D y con la utilización de una gran variedad de aditamentos llamados “Knifes” o cuchillos endoscópicos todos de procedencia japonesa, se logra realizar la miotomía o corte del esfínter sin la necesidad de una cirugía ya sea abierta o laparoscópica.

Con la realización de estos procedimientos se está a la vanguardia en los tratamientos de mínima invasión utilizando Endoscopia Terapéutica Avanzada.

An alternative to identify the type of disease that is suffered is the use of nuclear medicine, in addition to being able to receive curative therapy in the same way, as reported by Dr. Erika Barragán Pineda.

The specialist in nuclear medicine explained that this technique uses small amounts of radioactive materials called radiosondes, which are usually injected into the bloodstream, inhaled or swallowed.

“The radiosonde travels through the area examined and delivers energy in the form of gamma rays that are detected by a special camera and a computer to create images of the inside of your body,” he said.

He added that nuclear medicine provides unique information that can not usually be obtained using other imaging procedures and that it also offers the possibility of identifying diseases in the early stages.

WHAT DOES IT CONSIST OF?

Barragán Pineda explained that nuclear medicine is a specialty that is a branch of the diagnostic image, however, bases its findings on physiological changes in the body.

“Unlike X-rays in which we see anatomy, for example, if a person has a tumor in a kidney you can see its dimensions, the space it occupies; and if you study it through the nuclear procedure you can identify the repercussion that this tumor has on renal function,” she said.

She pointed out that radiotracers (radiopharmaceuticals) are used to analyze physiology; unlike other image modalities; the radioactive material is adminis-

tered to the patient, mainly the so-called (99mTc) pinnacle 99 metatalio and is marked with special molecules that follow a specific route.

THE PROCEDURE

The patient is given intravenously a radiotracer that looks like calcium in the body, bound to a radioactive molecule; this substance is fixed to the surface of the bone to be able to photograph with special equipment and identify the gamma radiation that comes from that radioisotope.

“You should wait two hours, during which the patient has to hydrate and take at least two liters of water; the phenomenon of hydration favors the material injected, sticking to the entire surface of the bone,” she said.

To that phenomenon of flame chemo absorption, allows to see the integrity of the bone, and if there are injuries, as in the case of metastases in people suffering from cancer.

“That’s why a bone scan can identify metastases in bone four or even 10 weeks before they appear on an x-ray or tomography; for that reason it allows to detect diseases at early stages,” she said.

She also stated that nuclear medicine is an ideal aid to give an opportune diagnosis of multiple diseases.

The function of Nuclear Medicine has a great and wide variety, mainly for the study of the oncological patient since it can identify extension to bones, brain or other organs, but also serves as an auxiliary in non-oncological pathologies.

Barragán Pineda listed that nuclear me-

dicine in internal medicine is useful for the diagnosis of infarction, or to identify the viability of the myocardium that is infarcted and if the patient serves a catheterization; as well as in specialties such as urology, nephrology or pneumology.

She added that in pediatrics its use is very large because it is useful to detect diseases due to esophageal reflux, very com-

mon in children; to know if that disease is causing the child to aspirate and if the bones have any alteration.

“The wonder of these studies is that the patient only comes with us, it is explained to them, they are given the radioactive material once they agree to do the study and we just have to wait for the drug to be fixed. in the case of bone scintigra-

phy,” she specified.

She also clarified that the patient only experiences the pain of the picket when the material is placed in the vein, because in the case of reflux, the radioactive material is mixed in the food; finally the necessary time passes, the images similar to those of the tomography are made, and the patient does not experience any discomfort.

OTHER USES

Dr. Erika Barragán Pineda said that there are multiple procedures of nuclear medicine and that most of these are used for the timely diagnosis of some diseases, but also for other infectious processes or fractures. In addition to serving to give treatment, because in one branch is diagnostic and the other is therapeutic.

“The most common case in which nuclear medicine treatment is used is for example thyroid cancer, once the patient is operated on, the thyroid is removed and ideally he has to visit the nuclear medicine doctor to receive an abrasive dose of iodine, already with it eliminates the possibility that the cancer at the time reappears,” she said.

Another treatment to remove bone pain, with yttrium 90; while for the articulation with samarium or strontium; recently there are new therapies such as in prostate cancer that extends to the bones; if the patient is a candidate for nuclear medicine therapy, he or she can receive treatment with a radioisotope called radio 223; the material is fixed in the metastatic lesions and destroys them.

RISKS OF NUCLEAR MEDICINE

According to the specialist, there is no risk in using nuclear medicine, as she said that the only discomfort suffered will be the picketing by intravenous administration if this is the case.

“There is only one radiotracer that has the capacity to produce allergy, the macroaggregate of albumin because it is a protein derivative and can be pre-medicated to the patient; because all the rest are analogous components of something that we have in the body.

“Like calcium, we can not be allergic to the calcium we have in the body; the iodine that we use, no one can be allergic because its chemical structure is identical to that found in the table salt that we consume,” she concluded.

She explained that only in the treatment used for patients with thyroid problems requires special preparation, because it must be explained to the patient that in some cases, conventional treatment or hormone supplementation must be suspended, either by hyperthyroidism or hypothyroidism, respectively.

“We use a gamma camera, it is called that, precisely because of its ability to detect the gamma radiation that comes from the radioisotopes we use,” she said.

She added that it is a team very similar to the one that makes a tomography and its function is to identify the radiation that comes from the organ that is going to be studied, registering the photons that are leaving the body marked by a radiotracer.

“Through a complex process they turn from analytical to digital and with principles that have to do with nuclear medicine, it is transformed into an image that allows identifying the site where the tracer accumulates, if it were the case, or, if it is missing the accumulation of it,” she documented.

Dr. Erika Barragán Pineda consults at Servicios Oncológicos de Reynosa and it is precisely she who is in charge of the interpretation of the studies since she mentioned that this type of procedure must be interpreted by a specialist in nuclear medicine who has the endorsement of the Nuclear Physicians Council.

“We rely on a technician and a nurse to carry out the study. The nurse injects the radioactive material and the technician acquires the images in the equipment and I interpret them and the patient is given a medical information in addition to some images,” she said.

She explained that nuclear medicine in a very complete area since it has two branches, the diagnosis and the one used as treatments in its therapeutic branch.

“It is a useful tool, to be used as the medicine of the future because we base our findings on molecular biology, but today we know that it is current medicine, because it is something that will allow us to make a diagnosis very early on. the diseases of our patients,” she added.

And it was that before you had to visit large cities to access it today is within reach; with trained doctors, specialists in this and who have international courses that endorse their knowledge, with which there is an important option for the diagnosis or treatment of any disease.

Gracias a la Medicina Nuclear es posible ofrecer tratamientos en su rama terapéutica así como obtener un diagnóstico a tempranas etapas de múltiples padecimientos.

Una alternativa para identificar el tipo de enfermedad que se padece es el uso de la medicina nuclear, además de poder recibir terapia curativa por la misma vía, así lo informó la doctora Erika Barragán Pineda.

La especialista en medicina nuclear, detalló que en dicha técnica se utilizan pequeñas cantidades de materiales radiactivos llamadas radiosondas, que generalmente se inyectan en el torrente sanguíneo, se inhalan o se tragan.

“La radiosonda viaja a través del área examinada y entrega energía en la forma de rayos gamma que son detectados por una cámara especial y una computadora para crear imágenes del interior de su cuerpo”, comentó.

Agregó que la medicina nuclear proporciona información única que generalmente no se puede obtener utilizando otros procedimientos de toma de imágenes, y que además ofrece la posibilidad de identificar enfermedades en etapas tempranas.

¿EN QUÉ CONSISTE?

Barragán Pineda explicó que la medicina nuclear es una especialidad que es rama de la

“Se debe esperar dos horas, durante las cuales el paciente se tiene que hidratar y tomar al menos dos litros de agua; el fenómeno de hidratación favorece que el material inyectado, se pegue sobre toda la superficie del hueso”, recalcó.

A ese fenómeno se llama quimio absorción, permite ver la integridad del hueso, y si existen lesiones, como en el caso de las metástasis en personas que padecen cáncer.

“Por eso una gammagrafía ósea puede identificar a las metástasis en el hueso de cuatro o hasta 10 semanas antes que aparezcan en una radiografía o tomografía; por ello permite detectar enfermedades a etapas

tempranas”, apuntó.

Manifestó además que la medicina nuclear es un auxiliar ideal para dar un diagnóstico oportuno de múltiples enfermedades.

La función de la medicina nuclear tiene una gran y amplia variedad, principalmente para el estudio del paciente oncológico ya que puede identificar extensión a los huesos, al cerebro o a otros órganos, pero también sirve como un auxiliar en patologías no oncológicas.

Barragán Pineda enlistó que la medicina nuclear en medicina interna es útil para el diagnóstico de infarto, o identificar la viabilidad del miocardio que se infarta y si le sirve al paciente un cateterismo; así como en especialidades como urología, nefrología o neumología.

Añadió que en pediatría su uso es muy grande porque es útil para detectar enfermedades por reflujo esofágico, muy común en los menores; para saber si esa enfermedad esta haciendo que el niño se aspire y si los huesos tienen alguna alteración.

“La maravilla de estos estudios es que el paciente sólo acude con nosotros, se les explica de que se trata, se le administra el material radiactivo una vez que está de acuerdo en hacerse el estudio y sólo hay que esperar a que se fije el fármaco en el caso de la gammagrafía ósea”, especificó.

Aclaró además que el paciente sólo experimenta el dolor del piquete cuando se le pone el material en la vena, pues en el caso de reflujo dicho material radiactivo se mezcla en los alimentos; finalmente pasa el tiempo necesario se hacen las imágenes similares a las de la tomografía; y el paciente no experimenta ninguna molestia.

OTROS USOS

La doctora Erika Barragán Pineda precisó que existen múltiples procedimientos de la medicina nuclear y que la mayor parte de éstos se emplean para el diagnóstico oportuno de algunas enfermedades, pero también para otros procesos infecciosos o fracturas. Además de servir para dar tratamiento, pues en una rama es diagnóstica y la otra es terapéutica.

“El caso más común en el que se emplea tratamiento de medicina nuclear es por ejemplo el cáncer de tiroides, una vez que el paciente se opera, le quitan la tiroides e idealmente tiene que visitar al medico nuclear para recibir una dosis abrasiva de yodo, ya con ello se elimina la posibilidad de que el cáncer al tiempo vuelva a aparecer”, subrayó.

Otro de los tratamientos para quitar el dolor óseo, con itrio 90; mientras que para el articular con samario o estroncio; recientemente hay nuevas terapias como en el cáncer de próstata que se extiende a los huesos; si el paciente es candidato a una terapia por medicina nuclear, puede recibir un tratamiento con un radioisótopo que se llama radio 223; el material se fija en las lesiones metastásica y las destruye.

RIESGOS DE LA MEDICINA NUCLEAR

De acuerdo con la especialista no se corre nin-

gún riesgo al utilizar la medicina nuclear, pues dijo que el único malestar sufrido, será por el piquete de la administración intravenosa, en caso de ser así.

“Solamente hay un radiotrazador que tiene la capacidad de producir alergia, el macroagregado de albúmina, porque es un derivado protéico y se puede pre medicar al paciente; pues todos los demás son componentes análogos de algo que tenemos en el cuerpo.

Como el calcio, no podemos ser alérgicos al calcio que tenemos en el cuerpo; al yodo que usamos nosotros nadie puede ser alérgico porque su estructura química es idéntico al que se encuentra en la sal de mesa que consumimos”, concluyó.

Detalló que solamente en el tratamiento utilizado para los pacientes con problemas de tiroides se requiere de preparación especial, porque se debe explicar al paciente que en algunos casos, se debe suspender el tratamiento convencional o bien la suplementación hormonal, ya sea por hípertiroidismo o hipotiroidismo, respectivamente.

tención de las imágenes que arrojan los radiotrasadores del material nuclear.

“Nosotros utilizamos una gamma cámara, se denomina así, precisamente por su capacidad para detectar la radiación gamma que proviene de los radioisotopos que utilizamos”, externó.

Agregó que es un equipo muy similar al que realiza una tomografía y su función es identificar la radiación que viene del órgano que se va a estudiar, registrando los fotones que están saliendo del cuerpo marcados por un radiotrazador.

“A través de un proceso complejo se convierten de analíticos a digitales y con principios que tienen que ver con medicina nuclear, se transforma en una imagen que permite identificar el sitio donde se acumula el trazador, si fuera el caso, o bien, si falta la acumulación del mismo”, documentó.

La doctora Erika Barragán Pineda consulta en Servicios Oncológicos de Reynosa y es precisamente ella la encargada de la interpretación de los estudios, pues mencionó que este tipo de procedimientos los debe interpretar un especialista en medicina nuclear que tenga el aval del Consejo de Médicos Nucleares.

“Nos apoyamos de un técnico y una enfermera parra realizar el estudio. La enfermera

inyecta el material radiactivo y el técnico adquiere las imágenes en el equipo y yo las interpreto y al paciente se le da un informe médico además de unas imágenes”, indicó.

Reseñó que la medicina nuclear en un área muy completa, ya que cuenta con dos ramas, la diagnóstica y la que se utiliza como tratamiento en su rama terapéutica.

Misma que involucra todo el espectro de una patología desde el momento en que se diagnóstica y en algunos casos específicos ayuda a dar tratamiento.

“Es una herramienta útil, antes se solía decir que era la medicina del futuro, porque basamos nuestros hallazgos en la biología molecular, pero hoy en día sabemos que es medicina actual, porque es algo que nos va a permitir hacer un diagnóstico muy temprano en las enfermedades de nuestros pacientes”, añadió.

Y es que antes se tenía que visitar grandes ciudades para acceder a ella hoy está al alcance de la mano; con médicos entrenados, especialistas en ello y que cuentan con cursos internacionales que avalan su conocimiento, con lo que se tiene una opción importante para el diagnóstico o tratamiento de cualquier enfermedad.

Atopic dermatitis is a skin disorder, which causes swelling, itching and redness.

Dermatologist, Pedro Martínez Amador, mentioned that this disease is more frequent in children and adolescents, and can occur after three months of age.

“The cause in seven percent of people is genetic, is inherited from parents, or grandparents who have had or had atopic dermatitis, bronchial asthma, rhinitis or allergic conjunctivitis,” said the doctor.

He added that other factors that favor that a child presents this disease are the lack of a protein in the skin that works as a skin barrier, heat or cold, clothing of synthetic fibers or very long baths.

OTHER CAUSES

CLASSIFICATION

The specialist indicated that there are three phases of atopic dermatitis, depending on the stage of life in which it occurs.

The first is the infant phase, which goes approximately three months to two years of age.

Later there is the preschool, which is from two years until 10 or 12 and finally the adolescent, who is located at this stage of life.

However, dermatologically it is classified into two categories: according to the topography, which is the site where the lesions are and the type that is presented.

“In infants it is common to appear on the cheeks, respecting the center of the face and we will also find folds and on the type of lesions, in an acute phase we will find scales, vesicles, erythemas and inflammations of the skin, but If it becomes chronic we will notice a thickening that is medically called lichenification,” he explained.

RECOMMENDATIONS

La llegada del invierno, la aplicación de cremas y pomadas, el tipo de ropa y hasta la alimentación pueden provocar la aparición de esta enfermedad en personas propensas a padecerla.

En esta época del año cuando la temperatura desciende y llega el invierno, se incrementan los casos de dermatitis atópica, debido a que la piel sufre una deshidratación.

Esta afección se incrementa en las personas que ya tienen una predisposición a padecerla, la cual puede deberse a varias causas.

La dermatitis atópica es un trastorno cutáneo, es decir, de la piel, que provoca hinchazón, picazón y enrojecimiento.

El dermatólogo, Pedro Martínez Amador, mencionó que esta enfermedad es más frecuente en niños y adolescentes, y puede presentarse a partir de los tres meses de edad.

“La causa en el siete por ciento de las personas es genética, se hereda de padres a abuelos que han tenido o tuvieron dermatitis atópica, asma bronquial, rinitis o conjuntivitis alérgica”, expuso el doctor.

Agregó que otros factores que favorecen que un niño presente esta enfermedad son la falta de una proteína en la piel que funciona como una barrera cutánea, el calor o el frío, ropa de fibras sintéticas o baños muy prolongados.

“Estar mucho estar tiempo expuesto al agua también pueden provocarla, los baños muy frecuentes ayudan a secar mucho la piel y también algunos alimentos están relacionados con este tipo de alergias”, manifestó.

OTRAS CAUSAS

Algunas sustancias aplicadas en la piel de los bebés pueden provocar irritación al remover una sustancia protectora natural.

“La aplicación de cremas y lociones o el tallar la piel del niño con esponjas o estropajos durante el baño la deja desprotegida y la vuelve más susceptible a padecer esta patología”, apuntó.

Respecto a la ropa comentó que se debe elegir de algodón y que sea holgada y evitar las elaboradas con fibras sintéticas y utilizar para su lavado jabón en lugar del detergente, pues podrían quedar algunas partículas que afectan la piel.

CLASIFICACIÓN

El especialista indicó que existen tres fases de la dermatitis atópica, según la etapa de la vida en la que se presente.

La primera es la fase de lactante, la cual va aproximadamente de los tres meses hasta los dos años de edad.

Posteriormente está la preescolar, que es de los dos años hasta los 10 o 12 y finalmente la adolescente, que se ubica en esta etapa de la vida.

Sin embargo, dermatológicamente se clasifica en dos rubros: según la topografía, que es el sitio en donde están las lesiones y el tipo que se presenta.

“En los lactantes es frecuente que aparez-

can en las mejillas, respetando el centro de la cara y también vamos a encontrar pliegues y sobre el tipo de lesiones, en una fase aguda vamos a encontrar escamas, vesículas, eritemas e inflamaciones de la piel, pero si se vuelve crónico notaremos un engrosamiento que médicamente se llama liquenificación”, detalló.

RECOMENDACIONES

Tanto niños como adultos al presentar lesiones en la piel no reciben la atención y el tratamiento adecuado, lo que desencadena en complicaciones cutáneas.

“Cuando vienen los niños con dermatitis atópica lo hacen con algunas lesiones sobreinfectadas y algunas dermatitis por contacto que están más inflamadas y si hubieran venido antes se podría haber evitado”, sentenció el doctor.

Por eso destacó la importancia de no automedicar al niño con pomadas que no son indicadas por el especialista o con remedios caseros.

Es común que las personas utilicen medicamentos triconjugados, es decir, aquellos que tienen cortisona, antibióticos y antimicóticos, sin embargo, será el médico quien

indique si son los adecuados, pues deben utilizarse por períodos cortos porque ocasionan efectos secundarios.

“Cuando se utilizan se quita la lesión pero se debe ir eliminando su administración poco a poco porque la cortisona ocasiona disminución de la piel, atrofia, o sea se va adelgazando y se produce un fenómeno de rebote, que se refiere a que la piel se hace adicta al medicamento y al suspenderlo vuelve a salir la lesión”, dijo Martínez Amador.

TRATAMIENTO

La dermatitis atópica se tratará de dos formas: farmacológica y no farmacológica.

El rubro farmacológico se refiere al uso de medicamentos como los antihistamínicos, que son la loratadina y cetirizina, que ayudan a disminuir la comezón e hidratantes y emolientes tópicos aplicados dos o tres veces al día.

También deberá vigilarse la dieta del paciente y observar si al consumir determinados alimentos como semillas, cacahuates, lácteos o huevos manifiesta alguna lesión en la piel.

El tratamiento no farmacológico consiste en las recomendaciones dadas sobre la elección de ropa, cuidados al lavarla, así como la duración y método de baño.

“El área donde duerman debe tener una temperatura ambiente agradable, la ropa de cama debe ser de algodón, evitar la exposición al sol y no usar la alberca continuamente”, finalizó.

Headaches are one of the most common discomforts that affect the human body and is a frequent symptom in half of the world population, but in some cases it may indicate the presence of a more serious disease.

Although a headache is a transient and benign disorder, it can also be related to a life-threatening illness, although it usually has to do with eating disorder, fatigue, or stress.

For the most part, it is enough to relax or ingest a dose of analgesic so that the discomfort stops.

“Some patients associate it with high blood pressure or high sugar labels, but it can also be because they have not rested or fed well.

The doctor said that when common drugs do not remove a headache this could have another cause.

Si bien un dolor de cabeza es un trastorno transitorio y benigno, también puede estar relacionado con una enfermedad que ponga en peligro la vida, aunque por lo general tiene que ver con un desorden alimenticio, fatiga o estrés.

En su mayoría basta con relajarse o ingerir una dosis de analgésico para que el malestar ceda.

“Algunos pacientes lo relacionan con presión alta o azúcar, pero también puede ser porque no han descansado o alimentado bien.

“El problema del estrés para todos desde niños, adolescentes y adultos puede ser

una de las causas de una cefalea o un simple dolor de cabeza”, señaló Mónica Alvarado Sánchez.

La médico cirujano partero por la Universidad Valle de México (UVM), quien durante cuatro años ha estado al frente de la consulta familiar, aseguró que cuando una persona llega con un síntoma de este tipo lo primero que se hace es revisarle la presión arterial.

En algunos casos, especificó, el dolor de cabeza podría indicar que algo no está funcionando bien dentro del organismo.

“Lo que se conoce como la fiebre tifoidea o la salmonela también pueden ser causantes de cefalea, pero convencionalmente la gente lo que hace es tomarse lo que tienen en casa, con una fórmula de paracetamol o un ibuprofeno y se controlan.

“Los pacientes vienen a consulta ya cuando el dolor es más intenso o tienen dos o tres días con malestar permanente”, agregó.

HAY QUE ESTAR INFORMADOS

La doctora manifestó que cuando a una persona los fármacos comunes no le quitan el dolor de cabeza éste podría tener otra causa.

Así como el malestar estomacal, las cefaleas pueden ser comunes en el cuerpo humano, pero hay casos en los que pueden alertar de otros signos más preocupantes.

“Existen quienes las asocian con la hipertensión o la diabetes. Y efectivamente cuando vienen a consulta nos damos cuenta que sí, porque hay personas que ya conocen más sobre el funcionamiento de su cuerpo”, comentó Alvarado Sánchez.

Dijo que es recomendable acudir al médico cuando los episodios son muy repetitivos.

“Dentro de las cefaleas la migraña puede ser un cuadro. Esa la tenemos por las presiones, el estrés, el exceso de trabajo y todo lo que conlleva a provocar dolor de cabeza, como malos hábitos alimenticios o trastornos del sueño.

“Por eso hay que buscar una valoración, principalmente de la presión arterial y si es

necesario con exámenes de laboratorio, para revisar cómo la persona se encuentra de colesterol, triglicéridos, las fiebres; una biometría hemática para descartar una anemia.

“Tenemos que estudiar por completo al paciente para descartar cualquier otra patología que pueda estar ocasionando esos dolores”, subrayó.

La doctora expresó que con normalidad estos malestares son atendidos por el médico general, pero ya cuando el problema es muy consecutivo se envía al paciente con otro especialista.

“Puede participar el neurólogo, hasta un psicólogo, o incluso un psiquiatra, porque hay determinados casos donde es psicológico el dolor. Los neurólogos pueden enviar a los pacientes a hacerse una topografía para detectar algún tipo de alteración craneal”, ilustró.

ENTENDIENDO LAS CAUSAS

Una de las mejores maneras de evitar las cefaleas son prevenirlas y conocer mejor el funcionamiento del cuerpo humano.

–¿Técnicamente, para que el paciente lo entienda, ¿qué es lo que ocurre adentro de la cabeza?, ¿chocan las membranas del cerebro?

“A veces hay una mala oxigenación, puede deberse a eso y en casos más extremos algún bloqueo por un tumor o una inflamación. El líquido cefalorraquídeo, ese debe de circular constantemente y si hay au-

mento o disminuciones se presentan malestares”, explicó.

En el caso de los niños la entrevistada detalló que las causas de una cefalea por lo regular están vinculadas a los hábitos alimenticios, de que no comen adecuadamente o entran en una fase de anemia.

“Ahí lo ideal es acudir con su médico. Sí se les puede dar su Tempra con dosis pediátrica, que se basan en el peso del niño, pero por ser menores hay que tener más cuidado”, recomendó.

Otras patologías que pueden provocar dolores de cabeza, dijo, son el colesterol o los triglicéridos.

“Hay personas que tienen años sin revisarse y esas son las causas y al echarle la culpa al estrés no se atienden y se toman una pastilla para calmar unas horas la molestia, siendo recurrente estarse automedicando, mientras el dolor les está avisando de otra cosas.

“Y existen ocasiones que simplemente con dormir mejor solucionamos el problema”, ponderó.

Y es que tener un dolor de cabeza, por mínimo que sea, provoca que una persona no se sienta a gusto. Algunos echan mano de la medicina tradicional y otros de la natural, ya que hay gente que cuando no tiene medicamentos a la mano utiliza remedios naturales, como ponerse rebanadas de papas frescas en la frente y las sienes.

4Realizar actividad física al menos unas tres o cuatro veces por semana.

4Ayudar a tu cuerpo siendo más organizados, para evitar presionarse.

4Tener en casa un botiquín –con Tempra para niños– y analgésicos para evitar salir a la farmacia en horarios nocturnos.

4Acudir al médico si las molestias persisten.

4Preferencialmente realizarse periódicamente estudios de laboratorio.

“También hay quienes nos llegan con sus ramitas de hierbabuena o que se ponen alcohol. Es medicina alternativa, yo siempre le comento a mis pacientes que preferencialmente se tomen su tratamiento, aunque no estoy en contra de lo natural”, abundó Alvarado Sánchez.

Y en el otro extremo de los casos, cuando una persona se deja los dolores de cabeza puede provocarse un daño mayúsculo, pues hay quienes de alguna manera se adaptan a vivir con esta tortura física.

“Si padece presión alta podría desarrollar un evento cerebrovascular, porque nunca ha sabido que tiene la presión alta y esa es la causa de su dolor de cabeza”, advirtió la doctora.

f you see: Blinking lights suddenly (like seeing stars when you get hit in the eye), flying flies, spots, lines or cobwebs in your eyes and perceiving a gray curtain that covers part of the visual field you may be having a retinal detachment.

What is retinal detachment?

Retinal detachment is a serious problem that must be treated immediately, otherwise, the vision of the eye may be lost.

Retinal detachment occurs when the retina separates from the back of the eye. The retina does not work when it is detached and this makes the vision blurred.

How is a retinal detachment produced?

As we age, the vitreous in our eyes be-

gin to shrink and become thinner, when the eye moves the vitreous moves around the retina without causing problems.

But on many occasions, the vitreous can stick with the retina and pull hard enough to detach it.

When this happens the fluid can pass through the tear and can detach the retina.

Dado que va en aumento la lista de enfermedades comunes que sufren las personas en el corazón, el cardiólogo Edgar Noel Sosa del Ángel alertó al respecto.

Enlistó la presión arterial alta o hipertensión arterial; afectaciones de las arterias del corazón, es decir, cardiopatía isquémica o obstrucción de las arterias de corazón por placas de grasa; arritmias cardiacas, princi-

palmente las taquicardias palpitaciones, las bradiarritmias y los trastornos donde el corazón se vuelve lento y tiene menos de 60 pulsaciones por minuto.

“En éste último padecimiento, hay que colocar a los pacientes marcapasos para ayudarles a me-

jorar su frecuencia cardiaca; otros enfermedades cardiacas importantes son la insuficiencia, que es un padecimiento grave, consecuencia de la presión arterial alta, que a su vez, es consecuencia de las arritmias o de la cardiopatía isquémica”, agregó.

Explicó que si bien los síntomas de las enfermedades cardiacas son muy similares en los hombres y mujeres, básicamente en los pacientes que son diabéticos, es el grupo en donde puede variar, porque éstos pa-

cientes pueden sufrir un ataque al corazón o un infarto y no sentir el dolor u opresión en el pecho; señal de un infarto.

“Un diabético puede tener la sensación de falta de aire, sudoración, cansancio; los mismos que un infarto, pero los diabéticos tienen síntomas que nosotros les llamamos atípicos, o sea que no son comunes de un padecimiento cardiaco”, añadió.

Detalló que los enfermos de diabetes tienen una afectación en el sistema nervioso autónomo, es decir, lesión en las terminales nerviosas que provocan que no tengan el dolor, denominada neuropatía periférica.

“Al tener entumidos brazos, pies y manos y no tienen dolor; a veces llegan a tener lesiones, úlceras y hasta perder dedos de los pies sin sentirlo; dado que hay una afectación una enervación del área cardiovascular y por eso no tienen dolor”, advirtió.

EL PELIGRO EN LA MUJER

El cardiólogo destacó que las enfermedades más comunes en las mujeres y principal motivo de consulta, son hipertensión arterial, presión arterial alta, cardiopatía isquémica, que son arterias tapadas en el corazón; sobre todo en quienes superan los 70 años.

Así mismo agregó que las taquicardias o palpitaciones que son un motivo común de consulta, sobre todo en mujeres jóvenes de 30 a 40 años; y en general todas en femínas con factores de riesgo, es decir, diabéticas, o que padecen presión alta, colesterol, triglicéridos, fumadoras o con obesidad.

Por otro lado advirtió sobre la más peligrosa de las enfermedades cardiacas en la mujer denominada cardiopatía en el embarazo; ya que no sólo afec-

ta a la madre sino también al producto en gestación.

“En el embarazo el volumen de sangre de la mujer aumenta un 50 por ciento y eso va a ocasionar una sobre carga de presión y si hay una cardiopatía preexistente, es decir, algo que la paciente tenía antes de embarazarse o puede ser congénita y presentar síntomas y puede traer complicaciones”, aseveró.

Precisó que la incidencia en que una mujer embarazada presente una cardiopatía a nivel mundial, es de 0.5 hasta un 3.7 por ciento, una enfermedad incluso que puede contraindicar un embarazo.

Ejemplificó que a quienes padecen o se les detecta una insuficiencia cardiaca, hipertensión arterial pulmonar severa; el embarazo esta contraindicado y se lo tiene que hacer saber su médico ginecólogo o cardiólogo, dado que las complicaciones e incluso de muerte pueden ser muy altas.

“Es de suma importancia que la mujer siempre le diga a su ginecólogo si se le ha detectado alguna enfermedad cardiaca desde la niñez o si tenían un soplo, para que cuando esté en su primer semana su ginecólogo la refiera con el cardiólogo y le practique una evaluación integral, un buen diagnóstico y evitar las complicaciones”, opinó.

UN BUEN DIAGNÓSTICO

Sosa del Ángel recalcó que los exámenes necesarios para su diagnóstico, inicialmente son el físico, que incluye la toma de presión arterial, revisión de su frecuencia cardiaca; además la historia clínica, un interrogatorio para buscar antecedentes y completar con la búsqueda y detección de síntomas.

“Con el electrocardiograma podemos observar si tienen arritmias, sobre todo si sospechamos que existe una cardiopatía y si la paciente está embarazada, porque

por ese medio revisamos la anatomía cardiaca y detectamos alteraciones vasculares o congénitas para emitir un pronóstico”, precisó.

Sentenció que un ecocardiograma es una herramienta de diagnóstico muy importante, que permite saber qué tan grande es el corazón y la fuerza que tiene dicho órgano para trabajar, además que se realiza de una forma rápida y de fácil acceso, ya sea en un consultorio u hospital.

CONFUSIÓN DE ENFERMEDADES

Externó que existen otros padecimientos ligados a sufrir una enfermedad cardiaca, ya que cuando el paciente padece presión arterial alta y no hay control sobre ella, puede afectar y hacer que el corazón crezca; eso puede generar una insuficiencia cardiaca.

“Esto significa que el órgano no bombea bien la sangre, y si hay una insuficiencia puede afectar la función de los riñones y ésta a su vez dañar el hígado o el estado nutricional del paciente, lo que

conlleva a una afectación de todos los órganos”, aconsejó.

Subrayó que puede ser mortal confundir una enfermedad cardiaca con otro padecimiento, aunque aclaró que si se retrasa diagnóstico, puede a su vez retrasar un tratamiento y ello puede tener como consecuencia final catastrófica, la muerte.

Añadió que los padecimientos que más se confunden con enfermedades cardiacas son los digestivos; pues a veces el paciente supone que padece gastritis o úlcera, cuando en realidad es una arteria tapada en el corazón.

“Las molestias pueden generarse en el centro del pecho como una opresión o un ardor, son los que más se pueden llegar a confundir, en ocasiones ya pasó por el gastroenterólogo e hizo su endoscopia la persona y se siguen teniendo síntomas de opresión”, detalló.

Así mismo indicó que una gran cantidad de personas, ya sea, hombres o mujeres lo visitan por estrés, una situación actual que puede ocasionar muchas de las

lestias y síntomas que pueden confundirse con problemas del corazón.

“Es muy común que el paciente tenga tensión, estrés y ello desencadene arritmias cardiacas, taquicardias, palpitaciones, dolor de pecho, todos ellos atípicos.

Pero precisamente es en ellos en quienes se les realiza un diagnóstico de exclusión antes de decirle al paciente que lo que tiene es puro estrés”, afirmó.

RECOMENDACIONES

El cardiólogo recomendó que para evitar un padecimiento cardiaco, se debe llevar una dieta sana, evitar toda la comida chatarra; comer frutas verduras; consumir menos carnes rojas y con grasa; ingerir más pollo y pescado; además de hacer caminata o bicicleta, 30 a 40 minutos, cuatro o cinco veces a la semana.

“Se debe también regular el peso, controlar la ingesta de sal y sobre todo si ya sabes que padeces presión alta, diabetes, colesterol, o que fumas, es tratar de controlar esas enfermedades, para evitar que ésto nos pueda llevar a una enfermedad cardiovascular”, finalizó.

Luego de tres décadas e igual número de generaciones de médicos, el Hospital Santander celebra el trigésimo aniversario de su fundación convertido en referente de la atención médica de excelencia en Reynosa y la región.

Ante la necesidad que tenía la ciudad de contar con un centro hospitalario donde pudieran confluir las diferentes especialidades, a finales de los años setenta nació la inquietud de abrir un hospital de este tipo por parte de un grupo de doctores (en su mayoría ginecólogos).

Con la intención de mejorar la calidad en la atención a sus pacientes se inició con su construcción sobre un terreno baldío ubicado en la colonia Prado Sur. Presidente del Consejo de Administración del Hospital de Especialidades Santander Gabriel Rosado Triay, comentó que en 1988 inicia operaciones con la disponibilidad de sólo once camas para atender a igual número de pacientes, dos quirófanos, dos salas de parto y un total de 75 personas laborando en el nosocomio, mientras que en la actualidad se cuenta con 46 habitaciones.

“Estaba muy limitada la infraestructura hospitalaria en Reynosa, cuando un paciente se ponía grave o se iba a McAllen o a Monterrey, no teníamos forma de atenderlo porque no contábamos con los elementos necesarios y eso fue lo que hizo que nosotros pensásemos en proveer un sitio idóneo para que se pudiesen resolver los problemas graves”, recordó el Dr. Rosado.

Relató que han sido pioneros en instalaciones hospitalarias, así como en cirugía de corazón abierto; pues la primera intervención quirúrgica de ese tipo en la zona norte de Tamaulipas se realizó en el Hospital Santander.

“Para mis compañeros y para mí es muy satisfactorio junto con el que hayamos podido lograr un hospital que sigue estando a la vanguardia de la medicina privada en Tamaulipas y queremos seguir avanzando; no pensamos detenernos” agregó el también pediatra.

En voz de su presidente detalló que los objetivos del Hospital Santander son el refrendar la confianza de sus pacientes, mantener personal altamente capacitado y tecnología de punta para seguir atendiendo a la ciudadanía.

Por su parte el director médico del Hospital Santander, el doctor Víctor Humberto Salas Mejía, agregó que la misión principal en la comunidad y en la región ha sido ofrecer servicios médicos de calidad y ser referente regional de lo que es la salud así mismo de cómo mejorarla entre la población.

“Mi función dentro del hospital es ver la parte de la dirección médica, así como todo lo necesario para encontrar las mejores opciones de servicio para los pacientes, darles una atención que sea de calidad, eficaz, eficiente y cálida”, mencionó Salas Mejía.

LA ADMINISTRACIÓN

El Director General de Hospital Santander, el contador Juan José Villarreal Rosales, detalló que la administración de un hospital es muy compleja, pues explicó que el sistema es basado en operaciones, pues se tiene que estar preparados para lo inesperado, es decir para la urgencia, para la emergencia.

“Nuestro principal compromiso es la ética, la mejor atención y ésta centrada en el paciente, creo que esa es la parte más importante de nuestra misión, que es brindar una atención medica integral; se puede decir fácil, pero significa que lo más importante es que el paciente tenga la oportunidad de tener el diagnóstico y el tratamiento oportuno en el momento indicado para que recupere su salud y pueda tener calidad de vida”, resaltó el director.

Villarreal Rosales aseguró que el Hospital Santander cuenta con las personas correctas en el momento indicado, en el área médica los doctores, en-

fermeras, técnicos o paramédicos, imagenología, laboratorio, personal de cuidados intensivos o de urgencias quienes cuentan con la preparación médica y clínica adecuada para atender las enfermedades que vienen.

Por otra parte, apuntó la continua preparación y entrenamiento con la que cuenta el personal y finalmente el área de soporte; por ejemplo, los farmacobiólogos haciendo la conciliación de los medicamentos, mantenimiento, ingenieros biomédicos que están al pendiente de los aparatos y su funcionamiento y poder obtener de ellos todas las atribuciones y ventajas.

Villarreal Rosales agregó que una de las funciones y compromisos más importantes del Hospital Santander con la comunidad es buscar y fomentar más la prevención de la salud e incorporar más y nuevas tecnologías médicas, de tal manera que ayuden al diagnóstico oportuno y al tratamiento de los pacientes.

LOS QUIRÓFANOS

El Hospital Santander cuenta con media docena de quirófanos altamente equipados para realizar todo tipo de cirugías, cada sala cuenta con una máquina de anestesia, monitor de signos vitales, el equipo de laparoscopía y su monitor para visualizar las imágenes, dicho equipo funciona para realizar cirugías de mínima invasión.

CUIDADOS INTENSIVOS

Tiene además la Unidad de Cuidados Intensivos (UCI) más avanzada de la región; así como tratamiento en hemodinámica para pacientes con padecimientos cardiacos y de enfermedades vasculares.

CONSULTA EXTERNA

En el área de la consulta externa se atiende a los pacientes ambulatorios y se canalizan a consulta, a exámenes de laboratorio, rayos equis, tomografías, ultrasonidos o médicos de especialidad.

Para mejorar el servicio de drenaje en el fraccionamiento Las Palmas, cuadrillas de la Comisión de Agua Potable y Alcantarillado trabajaron en el saneamiento en una sección de su sistema sanitario.

Siguiendo las instrucciones de la presidente municipal, la doctora Maki Esther Ortiz Domínguez, la Comapa de Reynosa continúa con el mantenimiento preventivo y correctivo de los sistemas de drenaje sanitario en colonias y fraccionamientos.

En este sentido, cuadrillas de Limpieza 1016 y el Vactor 001 de la Comapa de Reynosa acudieron a la Avenida Las

Se implementó un operativo especial para otorgar seguridad a los asistentes a los panteones.

Los panteones de la localidad han recibido durante dos días más de 85 mil visitantes que fueron resguardados mediante el Operativo Vial de Días de Difuntos implementado por instrucciones de la alcaldesa Maki Esther Ortiz Domínguez.

El día 1 de noviembre acudieron aproximadamente 10 mil 840 personas a siete panteones locales, siendo uno de los de mayor afluencia el Sagrado Corazón que recibió a tres mil 300 visitantes, sin registrarse ningún accidente ni incidentes que afectaran la integridad de ningún ciudadano.

Más de 75 mil visitantes fueron contabilizados el viernes 2 de noviembre, con cerca de 21 mil personas en el panteón municipal Sagrado Corazón superado con siete mil asistentes por uno de los panteones privados del bulevar Hidalgo.

La administración de cada panteón privado determinó sus cierres entre las 18:00 y 20:00 horas mientras que los panteones municipales cerraron a las 18:00 y 19:00 horas.

Israel Ramos Sánchez is a dental surgeon out of the Autonomous University of San Luis Potosí (UASLP) and a specialist in Dental Implantology.

For several years he has performed hundreds of extractions and modern grafts to patients who come from the United States and Canada.

He has his office very close to the international bridge of Nuevo Progreso, a small town on the border of Tamaulipas, which is a medical tourism center regularly visited by the ‘Winter Texans’.

In fact, this town has prestigious specialists in various fields, but in the dental area Ramos Sánchez is one of the few that performs a procedure known as “immediate post-extraction implant”, which affirmed that it is increasingly in demand.

Israel Ramos Sánchez es médico cirujano dentista por la Universidad Autónoma de San Luis Potosí (UASLP) y especialista en Implantología Dental.

Desde hace varios años ha realizado centenares de extracciones y modernos injertos a pacientes que provienen de Estados Unidos y Canadá.

Tiene su consultorio muy cerca del puente internacional de Nuevo Progreso, un pequeño poblado en la frontera de Tamaulipas, el cual es un centro de turismo médico asiduamente visitado por los ‘Winter Texans’.

De hecho, esta localidad cuenta con prestigiados especialistas en diversas rubros, pero en el área dental Ramos Sánchez es uno de los pocos que realiza un procedimiento conocido como “implante inmediato post extracción”, el cual afirmó que cada vez presenta una mayor demanda.

Usualmente los vacacionistas se instalan en los municipios urbanos del sur de Texas como Mission y McAllen, donde tienen sus remolques o contratan hospedaje de tiempo compartido.

En la frontera de Estados Unidos ellos transcurren la temporada más cruenta del invierno, donde además de realizar algunas compras, aprovechan para atenderse diversas áreas de la salud.

Pero como en algunos casos su estancia es corta, requieren someterse a tratamientos prácticos y al mismo tiempo seguros. Además necesitan hacer rendir su dinero y en sus lugares de origen algunos tratamientos son muy costosos, por ello se trasladan a México.

Pero la gran mayoría no es partidaria de permanecer mucho tiempo en el médico durante su etapa vacacional, es por ello que los especialistas han tenido que encontrar la forma de sofisticar sus procedimientos y optimizar el tiempo disponible.

tiquen los estudios correspondientes, que reciba un buen diagnóstico y que pueda ser atendido rápidamente y de una forma profesional.

ALGUNOS PORMENORES

Es por ello que el doctor Ramos Sánchez habló del procedimiento que le está permitiendo hacer un abordaje breve a quienes requieren la sustitución de piezas dentarias, de tal manera que las visitas al médico puedan programarse solamente de uno a tres días, siempre y cuando no exista un daño de mayor importancia.

“La técnica es conocida como implantes inmediatos post extracción. Para esto debe haber varios lineamientos que, dependiendo del caso, nos van a dar la posibilidad de realizarla. “¿Qué se necesita?, una combinación de factores como que los tejidos –alrededor del área que se va a implantar– estén libres de infección; que exista disponibilidad ósea, es decir, que no se hayan perdido las tablas que afecten el transcurso post operatorio, lo cual puede llegar a exponer el implante durante el tiempo en que se realice o el periodo en el cual cicatrice y regeneren los tejidos”, explicó.

Por ende no todas las personas son candidatas a entrar y salir el mismo día del consultorio con el trabajo terminado, sino solamente aquellas en las cuales existen las condiciones, ya que los escenarios más comunes que dificultan la colocación de un implante son desde contaminación e inflamaciones, hasta la transformación de la plataforma dentaria en la encía y el hueso.

Y si falta un diente, una muela o un colmillo sin una pronta sustitución se presenta un desgaste natural al masticar.

“Eso pasa al no haber un órgano dentario que mantenga la estabilidad del hueso, porque éste tiende a reabsorberse en la zona donde falta la pieza dental.

“El implante proporciona esa capacidad de mantener en su mismo estado el hueso y aparte de que resuelve un problema de edentulismo conserva la estabilidad ósea en la misma proporción como si fuera un diente natural”, agregó.

EN PALABRAS MÁS SENCILLAS

Para que la gente lo entienda, suponiendo que a alguien se le pudrió una muela, que finalmente terminó por caérsele… si deja pasar mucho tiempo puede modificarse la estructura dentaria y no podría ser candidato a un implante inmediato post extracción.

“Obviamente puede haber con el tiempo reabsorción ósea y también pasar que el diente antagonista avance más allá del plano de oclusión que tienen los otros dientes y se salga, por así decirlo”, advirtió el médico.

Exclamó que para colocar un implante en una zona donde no hubo un diente

durante un tiempo va a depender de qué tanto se haya absorbido el hueso.

“Es posible que si el paciente se espera mucho tiempo o el área edéntula, que en este caso es la zona que no tiene el diente, pasó mucho tiempo para colocar el implante, en esa área se haya reducido tanto el hueso que no sea posible en ese momento colocar el implante y sea necesario realizar otro tipo de procedimientos como la regeneración ósea, en la cual se trata de reconstruir tridimensionalmente la cantidad de hueso para que nos permita ahora sí colocar el implante”, detalló el especialista (cuyo consultorio se ubica en la calle Coahuila número 12, local nueve).

La regeneración, refirió, se realiza por medio de biomateriales que pueden ser membranas biocompatibles y tejido hueso.

RELACIÓN DOLOR–COSTO–TIEMPO

Una de las mayores causas de no acudir al dentista es el temor a sentir malestar físico. Es casi una fobia generalizada; no obstante, este dentista hizo hincapié que sus tratamientos los practica de una forma muy amigable.

“Nuestros procedimientos son completamente indoloros, pues todo se hace bajo anestesia local y el tiempo post operatorio tampoco se cursa con dolor, con una mínima inflamación, aunque generalmente no la hay y es un método muy seguro”.

En el caso de los “implantes inmediatos post extracción”, como en todos los demás, indicó que se realiza un proceso de diagnóstico y posteriormente se lleva a cabo una planeación del caso.

“Es necesario contar con un tipo de tomografía que se llama ‘cone beam’, la cual no es una simple radiografía, sino que nos permite obtener el modelo tridimensional del hueso y verlo en todas sus dimensiones, ancho, alto, largo, y revisar las relaciones con otras estructuras, otros dientes, el nervio inclusive, que esté cerca de la zona que vayamos a trabajar y eso nos permite mejorar aún más y dar una mayor precisión en el diagnóstico”, comentó.

Con base a ello el dentista argumentó que entonces ya se define un tratamiento que consiste en colocar solamente el implante o en realizar otro tipo de procedimiento.

“En cuanto a tiempo normalmente pue-

de ser en un mismo día en casos relativamente sencillos hasta dos o tres días en cuestiones más complejas, en las que tenemos que realizar procedimientos o planeaciones más complicadas”, agregó Ramos Sánchez.

Por último, el médico cirujano dentista e implantólogo expresó que antes de realizarse un implante inmediato post extracción se necesita una planificación del caso.

“Y cada paciente debe de estar consciente de eso, de que para un buen tratamiento lo principal es una buena planeación y para ello es necesario contar con todos los estudios previos que nos permitan a nosotros llevar a un buen término los implantes”, manifestó el dentista entrevistado.

Lo cierto es que hoy en día lucir una buena dentadura sin dolor es posible gracias a las nuevas técnicas y los mejores implantes, que son realizados en Nuevo Progreso, Tamaulipas.
