Ben Stevens has led many successful maintenance conferences and seminars with IIR ME throughout the Gulf region over the past decade. He has been running his own company, DataTrak Systems Inc., since 1984 with a concentration on maintenance management and reliability. From 1995 to 2002 Ben was on a full time contract with PricewaterhouseCoopers International Centre of Excellence in Physical Asset Management, working closely with CMMS vendors. During 2003 to 2011, he was President of OMDEC Inc – a Canadian company in the forefront of reliability and maintenance innovation. Prior to his work in the maintenance field, he was trained as an economist and practiced as a financial manager – including CFO and CAO in several manufacturing companies. Ben has delivered hundreds of training programmes and delivered maintenance consulting projects in a wide range of companies in 25 countries. His sessions are focused around his own experience on which he bases the mini-workshops and case studies, which consequently relate directly to the practical workplace. He has developed numerous innovative approaches linking reliability and maintenance management to finance – focusing on the definition, and delivery of risk and value in maintenance.

Course Requirements And Certificates Delegates must meet two criteria to be eligible for an IIRME/GW Certificate of Completion for a course: 1. Satisfactory attendance – delegates must attend all sessions of the course. Delegates who miss more than 2 hours of the course sessions will not be eligible to sit the course assessment. 2. Course Timings: Registration will be at 07:30 on Day One. Course sessions will start promptly at 08:00 and end at 14:30. There will be two short breaks for refreshments and lunch will be served at the end of each day’s sessions.
Course Overview Root Cause Failure Analysis (RCFA) is a reliability tool used to analyse operational and maintenance failures. It determines the root causes, and the actions required to eliminate them; it is an essential ingredient in continuous maintenance cost effectiveness. If done right, RCFA will greatly aid in determining how businesses can identify and eliminate expensive failures. The course covers the fundamentals of RCFA – where it came from, and how it fits with other aspects of Maintenance and Reliability. Delegates will explore the advantages and disadvantages; learn when to use the technique and when not to use it. The core of the course deals with how RCFA actually works, what are the prerequisites on which to build the success and how to increase the probability of success. Delegates will cover the RCFA process in all aspects – focusing on the practical application of the techniques. This section is always very exciting as delegates explore all the causes of the common and not-so-common failures of familiar equipment. Yet RCFA is not focused only on equipment – it is equally applicable to processes, quality operation and management; so examples of these will also be part of the many workshops. One key question will be addressed in depth – does every problem have a single root cause? Delegates will also learn how to successfully implement RCFA in a business; what to do if the analysis does not seem to work and there is a repeat failure; how to keep their knowledge up-to-date by integrating feedback related to the latest equipment and process information.
Personal Development Plan The Personal Development Plan (PDP) is often described as the most important lasting feature of the course. After each session, delegates browse their materials, select and prioritise key elements that they want to implement or research. At the end of the course, the selections are consolidated, and a task list is built for the top priority items. In this way, the benefits of the course can last a lifetime, and form an important part of your career development.
Course Methodology The course is intensely practical and each session is a blend of presentations, discussions, workshops, practice and case study; active participation is expected from the delegates. Delegates are encouraged to bring examples of problems and failures to address on the course.
Post-Course Assistance All delegates are encouraged to remain in e-mail contact with Course Director Ben Stevens for as long as they wish – to ask further questions, solve implementation problems, share successes etc.
Day One Origins And Scope Of RCFA The introduction to RCFA will define its objectives and scope, and orient it in relation to other maintenance and reliability tools. It will not work in isolation; so the prerequisites need to be well understood. RCFA is most frequently used as part of RCM (Reliability Centered Maintenance); yet it can still be very effective when separated from that technique. But of course it has its disadvantages as well as advantages – exploring these will provide a solid grounding for the delegates to decide on how best to use
RCFA. Included in the first session will be: • • • •
Use and application of RCFA to equipment failures and to other forms of failure Advantages and disadvantages of RCFA How RCFA fits within the toolset of maintenance and reliability Comparison to other forms of analysis such as Fault Tree, Cause and Effect
Days Two And Three The RCFA Process This session details the essential steps in understanding and using the techniques of RCFA. It is the core of the course and will concentrate on providing knowledge in a way that is easily transferable to your daily work environment. Accordingly, delegates will examine, discuss and practice each element as follows: • • • • • • • • •
A rigorous definition of the problem plus the preliminary assessment Data gathering – a critical step in capturing the knowledge on which the RCFA is based The analysis process – including exercises using fishbone and other techniques Categories of equipment failure, including standards to be applied to specific types of equipment Other types of failure, including failures of processes, communications and supervision Selecting the right remedy and relating it to the alternative maintenance work tactics Understanding the pluses and minuses of the different work tactics Evaluating the costs and benefits of the remedies and tactics Preparation and content of the final report and recommendations
This will be an extended session as it contains the practical exercises that will ensure that RCFA becomes a useful and continuing practice in your workplace.
Day Four Making RCFA Work For You The final session will focus on the implementation of the technique – an important step if RCFA is to become a standard tool in the implementation tool kit. The key steps are: • • • •
Defining the prerequisites for implementing RCFA Establishing the management and training and personnel requirements to make it a success Defining the procedure to ensure adoption as a regular tool Applying KPIs that will measure success
Once implemented successfully, attention should shift to how the analysis process can continue to deliver the results demanded. The key here is to understand that failures will still occur, but that they do not mean that the process is flawed – merely that like all tools, keeping it sharp is vital. This final session will focus on how the feedback loop can be established and maintained. This will ensure that the analysis is developed and extended, and that knowledge is continually upgraded as equipments age.
