To help in prevention of problem recurrence, a systematic approach to problem solving is needed. Root Cause Analysis is one of the tools used in this approach. Root Cause Analysis is a methodology for finding and correcting the most important reasons for performance problems. It is different from troubleshooting and problem-solving as these disciplines typically seek solutions to specific difficulties, whereas RCA is directed at underlying issues.

Course Overview

The basic RCA method is to simply ask “Why” over and over again until you arrive at a Root Cause. The real question then becomes: how do we know when to stop asking “Why”? At what point are we satisfied that we’ve identified a Root Cause? What is a Root Cause? These are questions that constantly spark disagreement among RCA practitioners. This course is designed to introduce the bigger picture of problem solving approaches, geared towards organisations. The course will concentrate on one critical step of the approach, which is finding out “why” the problem occurred and provide links to a subsequent “corrective actions” path. You will learn how to lead investigations and Root Cause Analysis using various techniques such as Causal Factor Charting, Fault Tree Analysis and Root Cause Charts. This is a “How To” course designed to teach these skills. By the end of this course, you will understand the overall problem solving methodology used in successful organisations. You will be able to use different tools and techniques for identifying problems, accurately defining them and finding out the cause/causes that if removed will reduce the likelihood of recurrence.

Day Three Day One

Day One starts building concepts of problem solving. Organisational life is shown as either continuous fire-fighting, or continuous improvement. Organisations that are continually fire-fighting cannot afford the resources to improve or even keep up with market efficiency. Successful organisations are capable of reducing the likelihood of recurring problems, thus, once a problem is solved, there is no need to re-consume resources in solving it again and again. • Introduction to Root Cause Analysis (RCA) • Benefits of RCA and disadvantages of not using it • The need for RCA - Predictive event based analysis • Relationship between RCA, Corrective Action Programmes (CAP) and Quality

Day Two Day Two takes delegates into the mechanics of defining a problem correctly and investigating the events that led to its occurrence. Also, during this day, delegates are introduced to the concept of efficiency in problem solving. The principle of “You don’t get a million dollar hammer to hammer a two cent nail” is discussed and applied. Control of solution expected is initiated right from the initial definition. • RCA generic cycle: - Problem identification • Problems vs. symptoms • Problem statement structure and use as scope limiter • Problem statements as function of risk - Fact finding/investigation • Principles of investigation • Sources of information • Common investigation pitfalls • De-personalisation principles • Investigation tools • Data precision and reliability

This day is the heart of the course. Delegates get to live the process applied by leading organisations to determine the cause that if removed, will reduce the likelihood of problem recurrence. Rules for “how deep” to investigate vs. problem significance are introduced.
