SAFETY POLICY
It is the policy of RACE INTERNATIONAL CO. Ltd to provide and maintain safe and healthful working conditions, and to follow operating practices that will safeguard all employees and result in secure working conditions and efficient operation. Since the safety and wellbeing of all employees are paramount, RACE Management considers no phase of operation as being of greater importance than Accident Prevention. Hence, Safety must take precedence over expediency. All levels of RACE Management have a primary responsibility for the safety of all employees. RACE Managers and Supervisors shall make the safety of their employees their daily and hourly concern. Their responsibility can be met only by working continuously to promote safe work practices among all employees and maintaining safe working environments.
RACE employees must follow all safety rules and observe established safe work practices. No job shall be considered efficiently completed unless the worker has followed every precaution and safety rule to protect himself and his fellow workers.

RACE INTERNATIONAL RESPONSIBILITIES The specific responsibilities of the following personnel regarding the safety issues are:
1.1
Project Manager The Project Manager shall be responsible for monitoring the effectiveness of the safety program. He shall be directly responsible for compliance with the provisions of the safety manual and for the execution of all items contained in the safety program. He shall issue directives and delegate authority required for the efficient operation of the safety program. He shall direct modification(s) to assure efficient safety performance. He shall establish and maintain coordination and cooperation with all personnel project-wide to assure a safe environment.
1.2
RACE INTERNATIONAL’s HSE Representative 1.2.1
Frequent and regular inspections of the Work site shall be made by a competent onsite HSE representative designated by RACE INTERNATIONAL. Unsafe acts and/or conditions noted during inspections shall be corrected immediately.
1.2.2
RACE INTERNATIONAL shall provide one or more full-time onsite competent HSE representative: when RACE INTERNATIONAL’s on site employee total is less than thirty individuals, the HSE representative may have other responsibilities; if RACE INTERNATIONAL’s onsite total equals or exceed 30 employees, the HSE representative shall be dedicated full time to the responsibilities of the onsite HSE representative and must devote all of his/her time to these responsibilities. Such Representative(s) shall be fully qualified to perform the responsibilities of the position and they, or a fully qualified alternate, shall be present on the Work Site at all times when RACE INTERNATIONAL is onsite and working. Additional full-time competent HSE representatives shall be assigned to the Project by RACE INTERNATIONAL for each increase of 200 employees. Additional competent HSE personnel may be required due to specific hazards, client requirements, or local regulatory requirements.
1.2.3
The HSE representative who, because of training and experience, shall be capable of identifying existing or potential hazards in the job(s) being performed, identifying working conditions that are unsanitary,

hazardous, or dangerous to the safety and health of employees, has the authority to take prompt action in assuring a safe work environment. 1.2.4
1.2.5
RACE INTERNATIONAL's HSE Representative shall be responsible for initiating the HSE Program, Practices and Procedures ensuring that Work site HSE requirements and procedures are being accomplished, conducting HSE inspections of Work being performed, conducting weekly safety meetings with craft employees. The HSE representative will also be responsible for a continuing survey of its operations, to ensure that the probable causes of injury, accident, or incident are controlled and that operating equipment, tools and facilities are used, inspected and maintained as required by applicable HSE regulations.
1.3 Site Engineer Site Engineer shall be assigned by the Project Manager to assist and advise in the successful operation of the safety program. He shall work closely with the HSE representative in the execution of the safety program and report directly to the Project Manager.
1.4
Supervisors Supervisors shall be responsible for safe conduct and work practices of the workers and shall execute provisions of the safety program pertaining to their operations. They shall be provided with safety training as required to perform the work in a safe, efficient manner.
1.5
Workers The success of the Safety Program depends to a great extent on the safety attitude of the workers. This attitude shall be shaped and harnessed by the effectiveness of the safety training they shall receive and follow-up safety supervision of their supervisors. It is of utmost importance that the workers, thru training and supervision, be made continuously aware that he is responsible for his own safe conduct.

The Safety Committee shall be composed of the Project Manager, HSE Representative and Site Engineers who shall meet on a regular basis to formulate and evaluate safety programs.
1.7
RACE INTERNATIONAL’s General Obligations 1.7.1
RACE INTERNATIONAL Safety Inspector’s responsibility shall include the following: 1.7.1.1 He shall work closely with the owner’s Consultant personnel. 1.7.1.2 He shall document all trainings, safety meetings, and/or demonstration on safety awareness. 1.7.1.3 Safety Inspector shall help in the maintenance of an efficient accident record system and coordinates action taken by Site Supervisors to eliminate accident causes.
1.7.2
RACE INTERNATIONAL shall provide to the owner’s Consultant a copy of all reports made to government agencies or insurance companies relating to any Work site accident, injury, or incident during RACE INTERNATIONAL's performance of the Work.
1.7.3
RACE INTERNATIONAL shall be responsible for providing and requiring the use of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions.
1.7.4
RACE INTERNATIONAL shall be responsible for the development of a fire protection and prevention program. RACE INTERNATIONAL shall also comply with all fire and safety rules and regulations established on the project.
1.7.5
Any Work site HSE regulation, which exceeds the minimum standards established by regulation, shall be incorporated in RACE INTERNATIONAL's safety program.
1.7.6
RACE INTERNATIONAL shall conduct a pre-construction meeting to understand the project conditions and HSE requirements before starting work at the Work site. A Work site tour shall be made to confirm RACE INTERNATIONAL's awareness of potential hazards. 5

1.7.7
Appropriate methods, equipment, devices and material shall be provided by RACE INTERNATIONAL to assure a safe work place.
1.7.8
RACE INTERNATIONAL’s HSE representative has the responsibility to translate site or contract specific activities and requirements into specific plans and practices that provide direction for ensuring that environmental protection requirements are met. These practices identify, for each area of environmental concern, the specific actions that are to be taken and responsibility for their implementation. Practices shall be developed and implemented before affected activities commence.
1.8 Work Rules and Regulations RACE INTERNATIONAL shall comply with all work rules and regulations set forth in this program entitled Project Health, Safety and Environment Rules and Regulations. RACE INTERNATIONAL shall be responsible to provide this information to all subcontractors that will be utilized in the Project.
1.9 Hazardous Substances RACE INTERNATIONAL shall comply with the hazardous substance disclosure requirements set forth herein.
1.9.1
RACE INTERNATIONAL is responsible for notifying the owner’s Consultant of any hazardous chemicals or substances that are brought or cause to have been brought on Work site. RACE INTERNATIONAL shall provide the owner’s Consultant with a copy of RACE INTERNATIONAL's Hazard Communication Program and the Material Safety Data Sheet(s) (MSDS) for the chemical(s) or substance(s) intended for use on the site. RACE INTERNATIONAL shall be responsible for keeping this information current. RACE INTERNATIONAL is responsible for maintaining a copy of RACE INTERNATIONAL's Hazard Communication Program and access to Material Safety Data Sheet(s) on site for RACE INTERNATIONAL's own reference and employee training. All records shall be maintained at a location accessible to the owner’s consultant.
1.9.2
The legal storage, use and disposal of wastes of any hazardous chemicals or substances shall be the responsibility of RACE INTERNATIONAL. 6

RACE INTERNATIONAL’s employees shall be protected from inhaling hazardous substances in quantities that exceed national or international “allowable limits/ exposure limits”.
1.10 Regulatory Permits and Programs 1.10.1 RACE INTERNATIONAL shall initiate and maintain such permits and programs as may be necessary to comply with requirements set forth by local government. 1.10.2 A copy of all such permits will be provided to the owner’s Consultant prior to commencement of Work at the Work site. If regulatory permits are not required to perform Work, a letter shall be submitted to the owner’s Consultant prior to commencement of Work at the Work site stating that no permits are required.
2.0
SAFA CO’s SUBCONTRACTOR PRE-QUALIFICATION RACE INTERNATIONAL shall submit to the owner’s Consultant, company profiles of any subcontractor it intended to utilize as subcontractor for review and approval. RACE INTERNATIONAL shall obtain a prior approval from the owner’s Consultant before engaging and making any commitments to any subcontractors.
3.0
JRA – JOB RISK ANALYSIS RACE INTERNATIONAL and all of its subcontractors shall provide, for the owner’s consultants approval, a detailed Job Risk Analysis (JRA). This analysis shall identify the scope of work, known hazards, and corrective measures and controls that will be implemented to abate these hazards.
4.0
STA – SAFETY TASK ASSIGNMENT RACE INTERNATIONAL shall require all the foremen to plan their work each day and review the known hazards and corrective actions to safeguard the work with their employees by utilizing the Safety Task Assignment card. All employees working in the area and the supervisor are required to review and sign-off on the form daily. The completed form shall be posted in the work area. RACE INTERNATIONAL shall be also 7

complete the post-STA portion of the form, and return it to the owner’s Consultant Safety Department at the end of the shift.
5.0
INSTRUCTION AND TRAINING 5.1
RACE INTERNATIONAL shall provide instruction and training of its employees with proper documentation and the owner’s Consultant will be furnished a copy of the attendees. Examples of such documented training to be provided by RACE INTERNATIONAL include, but are not limited to: 5.1.1
Indoctrination / Orientation - Newly employed, promoted and/or transferred personnel shall be verbally instructed in the HSE practices required by their work assignments. RACE INTERNATIONAL’s shall reserve at least 4 hours per person for orientation, issue of entry badges and project HSE training initiatives.
5.1.2
Work Assignments - All work assignments must include specific attention to health, safety, and environmental concerns. "Follow-up" monitoring is required in order to prevent accidents. (Safety Task Assignments / Job Risk Analysis)
5.1.3
Meetings - Properly conducted and recorded weekly, craft toolbox, safety meetings are required. Additionally, staff meetings will have a portion of the agenda devoted to HSE and accident prevention.

RECORDKEEPING OF INCIDENTS AND TRAINING RACE INTERNATIONAL shall be responsible in maintaining all records required by Worker's Compensation Insurance, or similar in-country regulations. This includes the maintenance of an accident log, annual summary and the posting of all prescribed posters. A log of HSE activities, near misses, accident investigations, employee instruction, training, "tool box" meetings, etc., shall be maintained on Work site and copies shall be promptly provided to the owner’s Consultant upon request.
7.0
SUBSTANCE ABUSE PREVENTION The possession, use, manufacture, distribution or dispensation of any illegal drug or controlled substance shall be prohibited inside the project site. In addition, RACE INTERNATIONAL personnel shall be expected to report to work in proper condition and not under the influence of any controlled substance. RACE INTERNATIONAL shall provide for work those personnel who understand the requirement of this Article and who will comply. RACE INTERNATIONAL employees will also be required to submit to (urinalysis) testing based upon reasonable suspicion, including aberrant behavior and post accidents. In addition, depending upon job requirements, RACE INTERNATIONAL employees may be subject to periodic random (urinalysis) testing. RACE INTERNATIONAL employees failing to conform to testing requirements or failing such tests will be removed from the Work site immediately and will be restricted access to the project for an interval to be determined by owner’s Consultant site management. RACE INTERNATIONAL shall submit a copy of its Substance Abuse Prevention Program to the owner’s Consultant for review and approval. Violation of this Article may, at FLOUR ARABIA LTD’s option, be deemed by the owner’s Consultant to be a material breach of this Contract and subject the Contract to termination for default, as well as other remedies under contract, law, or equity.
8.0
HSE INSPECTIONS RACE INTERNATIONAL shall conduct and document routine HSE inspection on a weekly basis for safety-related inspections and provide copies to the owner’s Consultant upon request. 9

RACE INTERNATIONAL is required to correct any hazard it creates immediately upon its discovery by RACE INTERNATIONAL, Consultant, Owner, Regulatory Agency or Insurance representatives.
9.0

FIRE PROTECTION/PREVENTION 9.1
RACE INTERNATIONAL shall be responsible for fire protection in its work and operational areas, including offices, tool rooms, and storage areas 24 hours per day, seven days per week through the duration of this Contract.
9.2
RACE INTERNATIONAL shall provide approved fire-fighting equipment in adequate quantities, and RACE INTERNATIONAL shall conduct training in the usage of such equipment. Any recommendations by the local Fire Marshall, Consultant, or Owner as a result of fire inspections (including fire-watch, fire access, etc.) shall be followed up and adhered to by the RACE INTERNATIONAL.
9.3
RACE INTERNATIONAL shall be responsible for the development of a fire protection and prevention program conforming to OSHA, NFPA, or equivalent incountry standards. RACE INTERNATIONAL shall also comply with all fire and HSE rules and regulations established on the project.
THIRD PARTY HSE INSPECTIONS In addition to visits and HSE inspections by its own corporate or insurance representatives, RACE INTERNATIONAL is advised that the Project may be inspected from time to time by authorized third parties. Among others so authorized are representatives of the Conslutant and Owner, insurance companies and regulatory agencies. Upon their proper identification, they are entitled to access and courteous consideration. The Consultant shall be made aware, if possible upon their arrival, but in any case as soon as possible, of the purpose and results of such visits which relate to health, safety or environmental concerns.
11.0
CONSTRUCTION EMERGENCY PREPAREDNESS PLAN & DRILLS The owner’s Consultant will provide to RACE INTERNATIONAL an Area Emergency Preparedness Plan for evacuation of the work area in the event of a natural disaster and/or project emergency. RACE INTERNATIONAL shall be responsible for advising its employees of the potential hazards, emergency signals and primary evacuation route. RACE INTERNATIONAL is advised that mandatory periodic emergency evacuation drills

will be held. Any time lost by the RACE INTERNATIONAL in complying with these drills will not be reimbursable. 12.0
PROJECT HSE & SECURITY RULES & GUIDELINES RACE INTERNATIONAL shall require all personnel of the project to comply with following guidelines set forth herein. RACE INTERNATIONAL shall ensure and indicate that all its employees have read these guidelines and understood its contents. The employee must sign a declaration, which shall then be retained by RACE INTERNATIONAL with the employee's personnel file.
12.1
Clothing Requirements 12.1.1 Shirts shall be worn at all times. Sleeveless shirts and tank tops are not permitted. 12.1.2 Long pants are required. 12.1.3 Sandals, tennis shoes, or any other street type shoe will not be permitted. Only sturdy, leather, work boots are permitted. Safety toe footwear may be required for certain activities. 12.1.4 Loose fitting clothes or jewelry shall not be worn around moving machinery, grinding operations, etc. 12.1.5 Hair, which could come in contact with, or be caught in machinery, shall be protected by a hard hat. 12.1.6 Contamination or cleanliness protocol may dictate the necessity to wear special Personal Protective Equipment (PPE).
12.2
Personal Protective Equipment 12.2.1 Approved hard hats meeting specifications contained in American National Standards Institute (ANSI), Z89.1-1981, Z89.2-1971, or equivalent in-country standards shall be worn at all times in construction areas, lay-down yards, and other designated areas.

12.2.2 Safety glasses meeting specifications of ANSI Z87, or equivalent incountry standard, with rigid side shields shall be worn in all plant and construction areas at all times. A.
Approved eye and face protection shall be as follows: Goggles, welding hoods and shields, and face shields shall be properly worn at all times when in the area of operations, such as when welding, burning, grinding, chipping, chemical handling, corrosive liquids or molten materials, drilling, sawing, driving nails, power actuated tools, concrete pouring, tampers and gas fuelled hand operated equipment (i.e. chainsaws). Prescription glasses must meet ANSI Z87, or equivalent incountry standard, or approved eye protection shall be worn.
B.
To prevent hand and finger injuries, appropriate hand protection shall be worn for the following activities:
Handling sharp metal materials Cutting with hand-held non-power cutters Handling wood material Concrete operations Pulling wire in electric panels Rigging Operating a grinder Handling chemical & paints Handling glass Handling hot or cold objects Welding / cutting
12.2.3 NIOSH, MSHA, or equivalent in-country approved hearing and respiratory equipment shall be worn when required. The selection, fitting and maintenance requirements shall be met by RACE INTERNATIONAL. 12.2.4 RACE INTERNATIONAL shall provide for all necessary clothing and protective equipment required by their employees in the execution of their contract work. RACE INTERNATIONAL shall also fully comply with the requirements of the "Material Safety Data Sheets" including the use and wearing of additional protective equipment when working on or with chemicals. 12.3

12.3.1 Full Body Safety harnesses equipped with two lanyards shall be worn by all employees when working six feet or more above the ground or when working in a precarious position. One lanyard shall be securely attached to the employee 100% of the time and shall be rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level. Safety harnesses shall also be worn and attached to the tieoff rail when working out of extensible and articulating boom platforms and to vertical drop lines when working from suspended scaffolding. 12.3.2 Safety nets shall be provided when work places are more than 25 feet (4 m) above the ground where the use of other fall protection devices is impractical.
12.3.3 Guards rails consisting of top rail (42” or 1 m H); mid rail (21” or 0.5 m H) and toe boards (4” or 100mm), and capable of supporting 200 pounds (90 Kg), or secured, labeled floor hole covers capable of supporting 2 times the weight of personnel and material, shall be erected by the RACE INTERNATIONAL, or replaced in suitable condition if displaced by the RACE INTERNATIONALs work, for all floor holes or leading edges 6 feet (1.8 m) or more above lower levels. 12.4
Signs, Signals, Barricades and Lights 12.4.1 Signs, signals and barricades shall be visible at all times where a hazard exists. 12.4.2 All streets, roads, highways, and other public thoroughfares, which are closed to traffic, shall be protected by effective barricades on which shall be placed acceptable and highly visible warning signs. Barricades shall be located at the nearest intersecting public highway or street on each side of the blocked section. 12.4.3 All open trenches and other excavations shall be provided with suitable barriers, signs, and lights to the extent that adequate protection is provided to the public. Obstructions, such as material piles and equipment, shall be provided with similar warning signs and lights. 12.4.4 All barricades and obstructions shall be illuminated by means of warning lights from sunset to sunrise. streets and highways shall be so placed, and the work at all times shall be so conducted, as to cause the minimum obstruction and inconvenience to the traveling public. 12.4.5 All barricades, signs, lights and other protective devices shall be installed and maintained in conformity with applicable statutory requirements and, where within railroad and highway right-of-way, as required by the authority having jurisdiction there-over. 12.4.6 When any work is performed at night or where daylight is shut off or obscured, RACE INTERNATIONAL shall, at its expense, provide artificial light sufficient to permit work to be carried on efficiently, satisfactorily and safely, and to permit thorough inspection. During such time periods the access to the place of work shall also be clearly illuminated. All wiring for electric light and power shall be installed and maintained in a firstclass manner, securely fastened in place at all points, and shall be kept as far as possible from telephone wires, signal wires, and wires used for firing blasts. 12.4.7 Signs, signals and barricades shall be removed when the hazard no longer exists. 12.4.8 RACE INTERNATIONAL's employees working in an area of potential traffic hazard shall wear approved reflective type vests.
12.5
Rigging Equipment 12.5.1 All rigging equipment shall be free from defects, in good operating condition and maintained in a safe condition. 12.5.2 Equipment shall be inspected by a designated, competent employee of RACE INTERNATIONAL prior to initial use on the Work site work and monthly thereafter to ensure that it is safe. Records shall be kept on Work site of each of these inspections by RACE INTERNATIONAL and shall be made available to the owner’s Consultant upon request.
12.5.3 Damaged equipment shall be immediately removed from service. 14

12.5.4 RACE INTERNATIONAL shall adhere to Appendix C, Rigging Requirements Overview and Administrative Practice.
12.6
Hand and Power Tools 12.6.1 All hand and power tools, whether furnished by RACE INTERNATIONAL, or by RACE INTERNATIONAL's employee, shall be maintained in a safe condition. Records shall be provided by RACE INTERNATIONAL to the owner’s Consultant upon request. 12.6.2 RACE INTERNATIONAL shall not issue nor permit the use of unsafe hand or power tools. 12.6.3 Electrical power tools shall be grounded or double insulated with proper assured equipment grounding inspections or Ground Fault Interrupter circuit protection provided. 12.6.4 Pneumatic power tools shall be secured to the hose or whip by some positive means. 12.6.5 Only properly trained RACE INTERNATIONAL's employees shall operate powder actuated tools. Certification records, employee certification cards, or equivalent certification documentation shall be maintained by RACE INTERNATIONAL on each of its employees using powder actuated tools during performance of the work. 12.6.6 All grinding machines shall conform to OSHA, ANSI, or equivalent incountry standards and requirements. 12.6.7 Cords, leads and hoses shall be kept at least seven feet off the ground or whatever height is necessary to be protected from traffic and creating tripping hazards.
12.7
Compressed Gas Cylinders 12.7.1 Compressed gas cylinders shall be secured in an upright position at all times. Location of cylinder storage areas must be approved by the owner’s Consultant.

12.7.2 When transporting, moving and storing cylinders, valve protection caps shall be in place and secured. 12.7.3 Cylinders shall not be hoisted by magnets or choker slings. Valve protection caps shall not be used for hoisting cylinders. 12.7.4 Cylinders shall be kept away from sparks, hot slag and flames, or be adequately protected. 12.7.5 Cylinders shall not be placed where they can become part of an electrical circuit. 12.7.6 Cylinders shall be labeled as to the nature of their contents. 12.7.7 Oxygen cylinders in storage shall be separated from fuel gas cylinders or combustible materials by a minimum of 20 feet (6m) or by a noncombustible barrier at least five feet (1.5 m) high having a fire resistant rating of at least one-half hour and empty cylinders shall be separated from full cylinders and stored with like cylinders. 12.7.8 "No Smoking" signs shall be posted at storage areas and signs shall clearly indicate contents of cylinders. 12.7.9 Anti-flash back valves shall be provided on all oxygen and acetylene lines.
12.8
Scaffolds 12.8.1 Scaffold Erection will be in compliance with OSHA 29 CFR 1926.450 or equivalent in-country standard. Workers erecting, dismantling or using scaffolds shall be trained in accordance with applicable regulations. Scaffolding shall be erected, moved or dismantled under the direction of a Competent Person qualified in scaffold erection. 12.8.2 Footings or anchorage for any scaffold shall be sound, rigid and capable of carrying the maximum intended load without settling or displacement. 12.8.3 No unstable objects such as concrete blocks shall be used to support scaffolds or planks. 12.8.4 Any part of a scaffold weakened or damaged shall be repaired or replaced immediately.

12.8.5 All scaffold planking shall be free of knots and cracks (Class A lumber) and shall completely cover the work platform. 12.8.6 Scaffold planks shall be laid tight, cleated at both ends or overlapped a minimum of 12 inches (300 mm) and nailed or bolted to prevent movement, with overlaps occurring directly above scaffold supports. 12.8.7 Safe access shall be provided to the scaffold platform. Specifically, a ladder with a safe means of access to the platform from the ladder. 12.8.8 Scaffolds shall be equipped with a top rail made of lumber not less than 2 x 4 inches (50 mm x 100 mm) (or equivalent strength). Minimum top rail height 42 inches (1 m) with a mid-rail of 21 inches (0.5 m) high made of lumber not less than 1 x 6 inches (25mm x 150 mm) (or equivalent strength); and toe-boards shall be installed on all open sides and ends of scaffold platforms 10 or more feet (3 m) above the ground or floor for scaffolds where the guard rail is the primary means of fall protection. 12.8.9 Scaffolding not adaptable to guard rails shall require the use of safety harness with two lanyards attached to a secure substantial object. 12.8.10 Mobile scaffold casters shall be secured and locked prior to mounting. 12.8.11 No personnel shall be on mobile scaffolding when it is being relocated. 12.8.12 RACE INTERNATIONAL’s employees working swing stages, boatswain chair, floats, suspended scaffolds and needle beam scaffolds, etc., shall wear safety harnesses with two lanyards attached to an independent lifeline. 12.8.13 A proper scaffold inspection and tagging system shall be instituted and maintained by the RACE INTERNATIONAL erecting or using the scaffold.
12.9
Jobsite Transportation Rules 12.9.1 Drivers of motor vehicles shall be instructed to exercise judgment as well as observe posted speed limits. Maximum speed limit is (10) miles per hour (15 Km per hour), or as otherwise posted. However, much slower speeds are necessary in congested areas.

12.9.2 Seat belts shall be worn by all employees operating any motor vehicle and any equipment with rollover protection structures during performance of the work. 12.9.3 All RACE INTERNATIONAL means of ingress and egress routes will be adequately marked as such, since their employees are to travel these routes only. 12.9.4 Pedestrians and bicycles have right of way over motorized traffic. 12.9.5 Use of the car’s horn shall be employed for safety at blind corners, when passing, etc. 12.9.6 Established hand signals or turn signals are to be used. 12.9.7 Ignition key is to be left in vehicles at all times when within the Work site. (RACE INTERNATIONAL's employee’s vehicles may be locked within the designated RACE INTERNATIONAL's perimeter parking area.) 12.9.8 Reckless driving or other non-observance of these instructions will be cause for disciplinary action up to and including removal from Work site. 12.9.9 No two-wheeled motorized vehicles are allowed in the Work site other than in RACE INTERNATIONAL's perimeter parking lot. Any ATV's used on the Work site shall be 4 wheeled, not three wheeled.
12.10 Cranes and Derricks 12.10.1 All cranes and derricks shall be certified by an independent crane certifying company for safe operating condition by RACE INTERNATIONAL prior to using the crane or derrick on Work site. This certification shall be maintained by RACE INTERNATIONAL and made available to the owner’s Consultant upon request. 12.10.2 All crane operators shall be properly licensed and certified by a competent person capable of identifying existing and predictable hazards either in the surroundings or working conditions, which may be unsanitary, hazardous, or dangerous to employees. Such competent person shall have authorization to take prompt corrective measures to eliminate any such hazards. Certification records shall be maintained on

Work site by RACE INTERNATIONAL and made available to the owner’s Consultant upon request. 12.10.3 The swing radius of cranes shall be barricaded. 12.10.4 Hand signals prescribed by ANSI or equivalent in-country standard shall be posted at an operator's station. 12.10.5 RACE INTERNATIONAL shall adhere to applicable Crane Procedures. 12.10.6 Manufacturer specifications shall be observed by RACE INTERNATIONAL. 12.10.7 RACE INTERNATIONAL's employees shall not ride headache ball, hook, or load being handled by the crane. 12.10.8 Equipment shall not be lubricated while in use. 12.10.9 Rated load capabilities, recommended operating speeds, special hazard warning, specific hand signal diagrams and special instructions shall be visible to the operator while he is at the control station. 12.10.10 RACE INTERNATIONAL's employees shall not be allowed to work under the load of cranes. Tag lines shall be used on all loads. 12.10.11 Cranes and derricks shall be inspected prior to use and at monthly intervals. Records shall be maintained on Work site of these inspections by RACE INTERNATIONAL. Copies shall be made available to the owner’s Consultant upon request.
12.11 Equipment and Motor Vehicles 12.11.1 All equipment shall be inspected daily, before use, by operator. Formal inspections must also be made by RACE INTERNATIONAL competent person at 30-day intervals with proper documentation maintained at the Work site by RACE INTERNATIONAL and copies shall be made available to the owner’s Consultant upon request. 12.11.2 Defective equipment shall be repaired or removed from service immediately. 19

12.11.3 All rubber-tired, self-propelled scrapers, rubber-tired, front-end loaders, rubber-tired dozers, wheel-type agricultural and industrial tractors, crawler tractors, crawler-type loaders and motor graders shall be equipped with rollover protective structures and seat harnesses. 12.11.4 All of RACE INTERNATIONAL’s employees, who operate Motorized Equipment including forklifts, lulls, etc., shall be properly licensed and certified by a competent person. Copies of the certifications shall be maintained on Work site by RACE INTERNATIONAL and made available to the owner’s Consultant upon request. Operating requirements and safety procedures of the assigned equipment shall be verified by written examination, by a hands-on test, and by observation of his/her performance during the first month of operation. A competent person shall conduct the hands-on test to determine the applicant’s operating ability. A hands-on test shall be conducted on each equipment type and model to be operated. 12.11.5 All equipment shall have reverse signal alarm. Any equipment with an obstructed view to the rear must have a flagman. 12.11.6 All cracked and broken glass shall be replaced before bringing vehicles on the Work site. If glass is broken or damaged on Work site and if damage is severe enough to cause a potential safety problem, the machine shall be stopped until such damage has been repaired. 12.11.7 Vehicles used to transport employees shall have seats firmly secured and adequate for the number of employees to be carried and all passengers should be properly seated. Standing on the back of moving vehicles is prohibited. 12.11.8 Locations for storage of all fuels, lubricants, starting fluids, etc., shall be reviewed by the owner’s Consultant prior to use by RACE INTERNATIONAL for storage. 12.11.9 Seatbelts shall be worn by all employees operating any motor vehicle and any equipment with rollover protection structures during performance of the work.

12.12 Electrical 12.12.1 All electrical work, installation and wire capacities shall be in accordance with the pertinent provisions of the National Electrical Code, ANSI, OSHA, or equivalent in-country standard. 12.12.2 RACE INTERNATIONAL shall develop a ground fault program in accordance with requirements established by applicable regulation. In the absence of ground fault protection, RACE INTERNATIONAL shall establish a monthly-assured grounding inspection program to include color-coding. The color codes used on this project are: January Yellow April Green February White May Red March Brown June Blue (The cycle of colors is repeated for the next six months). 12.12.3 RACE INTERNATIONAL shall develop and implement a Lockout/Tagout procedure and strictly adhere to the use of this procedure. The owner’s Consultant will monitor adherence to the procedure on a regular basis. 12.12.4 All temporary power panels shall have covers installed at all times. All open or exposed breaker spaces shall be adequately covered.
12.13 Ladders 12.13.1 The use of ladders with broken or missing rungs or steps, broken or split rails or other defective construction is prohibited. 12.13.2 Ladders shall extend no less than 36 inches (1 m) above landing and be secured to prevent displacement. 12.13.3 Portable ladders must be equipped with safety shoes. 12.13.4 Wooden ladders shall not be painted. 12.13.5 Metal or conductive ladders shall not be used on the Work site. 21

12.13.6 Ladders shall be inspected regularly and records provided by the RACE INTERNATIONAL to the owner’s Consultant upon request.
12.14 Floor and Wall Openings and Stairways 12.14.1 Floor and wall openings shall be guarded by a standard guardrail, midrail, and toe-board, or adequately covered. 12.14.2 Guardrails shall be of sufficient strength to support 200 pounds (90 Kg) of pressure when applied at mid-span of the guardrail parallel with the floor and perpendicular to the guardrail with a minimum deflection of 3 inches (75 mm). 12.14.3 Covers shall be adequately secured to prevent displacement and have "Danger" signs attached identifying the hazard. 12.14.4 Every flight of stairs having four or more risers shall be equipped with standard stair railings. Stairs are not to be used until risers and railings are securely installed. Treads will be poured as soon as possible where poured treads apply. 12.14.5 Debris and other loose materials shall not be allowed on stairways or at access point to stairway. Debris shall not be allowed to accumulate in stairwells. 12.14.6 RACE INTERNATIONAL shall replace any floor and wall opening or protective systems removed during the course of their work.
12.15 Excavations and Trenching 12.15.1 Prior to opening any excavation or trench, RACE INTERNATIONAL shall notify the owner’s Consultant. In addition, RACE INTERNATIONAL shall contact any other necessary personnel to determine whether underground hazards, i.e., contaminated soil, containers or equipment, or installations, i.e. sewer, telephone, fuel, electric lines, etc., may be encountered and where they are located. 12.15.2 Excavations and trenches shall be inspected by a competent person daily and after every rainfall to determine if they are safe. 22

12.15.3 All banks 4 feet (1.2 m) high or more shall be sloped to the angle of repose (the greatest angle above the horizontal plane at which a material will lie without sliding), or shall be adequately shored. 12.15.4 Ladders or steps shall be provided in all trenches 4 feet (1.2 m) or more in depth. Ladders or steps shall be located to require no more than 25 feet (7.5 m) of lateral travel before having access or egress. 12.15.5 Material excavated shall be stored at least 3 feet (1 m) from the edge of the excavation or trench shall be shored to prevent material from falling into the excavation. 12.15.6 All trenches and excavations shall be properly barricaded to prevent persons from walking into them. 12.15.7 All walkways or ramps crossing over excavations shall be securely fastened and equipped with standard guardrails. 12.15.8 All excavations and trenches 4 feet (1.2 m) in depth and greater shall be air monitored as needed by a competent person to determine safe air quality limits. Before RACE INTERNATIONAL's employees are permitted entry into any excavations and trenches the atmosphere within the space shall be tested to determine the oxygen level and concentrations of flammable vapors, gases, and toxic contaminants. 12.15.9 Potentially contaminated soils or contaminated materials excavated from a trench will be assessed to determine proper disposition of the soils or materials. The owner’s Consultant shall be immediately notified if contaminated soils or materials are discovered. Disposition of contaminated soils or materials will be done in accordance with applicable regulations. Disposal plans will be coordinated with the owner’s Consultant.
12.16 Steel Erection 12.16.1 RACE INTERNATIONAL shall develop a site-specific Steel Erection Plan and submit to the owner’s Consultant. 12.16.2 Steel erection shall not begin until concrete in footings and piers is certified to be sufficient to support imposed loads and anchor bolts 23

requiring repair or replacement meet OSHA or equivalent in-country regulation requirements. 12.16.3 Cranes shall be inspected for adequate condition of control mechanisms, rigging, hydraulics, pneumatics, hooks and latches and ground conditions prior to hoisting. 12.16.4 Multiple lift rigging (Christmas Treeing) shall be permitted only in accordance with OSHA Standards and crane manufacturer’s approval, or equivalent in-country standards. As applicable, RACE INTERNATIONAL shall develop a site-specific multiple lift rigging (Christmas Treeing) plan and submit to the owner’s Consultant competent person for approval. 12.16.5 Workers shall not walk top flange of coated steel unless a minimum slip resistance of 0.5 is achieved in the coating. 12.16.6 A minimum of 4 anchor bolts shall be installed per column. 12.16.7 Bridging shall be installed and anchored on long span steel joists. 12.16.8 Connectors shall be protected from fall hazards of more than 6 feet. 12.16.9 Deckers, working in a controlled, decking zone (CDZ) at 6 feet, shall wear fall arrest equipment. 12.16.10 RACE INTERNATIONAL shall not allow any of RACE INTERNATIONAL employees to walk the steel. RACE INTERNATIONAL employees must be tied off and “coon” the beam until safety cables are provided to which employees shall tie off. Two lanyards are needed to ensure 100% tie-off. 12.16.11 At no time shall there be more than four floors or 48 feet (15 m) of unfinished bolting or welding above the foundation or uppermost permanently secured floors. 12.16.12 A temporary and/or permanent floor shall be maintained within 2 stories or 30 feet (9 m), whichever is less, below and directly under that portion of each tier of beams on which any work is being performed. 12.16.13 Planking or metal decking in temporary floors shall be of proper strength and thickness to carry the working load. Decking shall be secured to prevent movement.

12.16.14 Standard guardrails and toe-boards shall be installed around open sides of permanent floors. During structural steel assembly, a safety railing (cable) of one-half inch (1/2) (12 mm) cable diameter shall be installed approximately 42 inches (1 m) high, recessed into interior of floor around all temporary floors. 12.16.15 Where fall distance exceeds 6 feet (1.8 m), scaffolds, ladders, catch platforms or a full body safety harnesses with two lanyards attached to lifelines or other substantial objects shall be used. If the use of these is impractical, safety nets shall be provided. 12.16.16 Tag lines shall be used to control all loads. 12.16.17 Containers shall be provided for storing or carrying bolts or rivets. When bolts drift pins, or rivet heads are being removed, a means shall be provided to prevent accidental displacement. Tools shall be provided lanyards to prevent falling. 12.16.18 During the final placing of solid web structural members, the load shall not be released from the hoisting line until the members are secured with not less than two bolts, or equivalent, at each connection. 12.16.19 A safe means of access to the level being worked shall be maintained. Climbing and sliding columns are not considered safe access.
12.17 Confined Areas or Spaces 12.17.1 RACE INTERNATIONAL shall develop an entry procedure to be used when RACE INTERNATIONAL's employees are required to enter confined areas or spaces. Such areas include, but are not limited to, storage tanks, process vessels, bins, boilers, ventilation or exhaust ducts, sewers, underground utility vaults, tunnels, pipelines, open-topped pits, basements and temporary wood framing covered with plastic. 12.17.2 All of RACE INTERNATIONAL's employees who are required to enter confined areas or spaces shall be instructed as to the nature of hazards involved, necessary precautions to be taken and in the use of protective and emergency equipment required. 12.17.3 Before RACE INTERNATIONAL's employees are permitted entry into any confined areas or spaces, the atmosphere within the space shall be tested to determine the oxygen level and concentrations of flammable 25

vapors, gases, and toxic contaminants. RACE INTERNATIONAL needing access to the confined area shall furnish the testing equipment and a person competent in the use of the testing equipment. 12.17.4 When welding, cutting or heating in confined areas or spaces, ventilation shall be provided. When sufficient ventilation cannot be provided without blocking the means of access, RACE INTERNATIONAL's employees shall be protected by airline respirators. 12.17.5 An employee of RACE INTERNATIONAL shall be stationed outside the confined area, whenever required, to maintain communication with those working within and to aid them in an emergency. 12.17.6 Hazardous atmosphere means an atmosphere that may expose employees to the risk of death, incapacitation, impairment of ability to self-rescue, injury, or acute illness from: A.
Flammable gas, vapor, or mist in excess of 10% of its lower flammable limit.
B.
Airborne combustible dust at a concentration that meets or exceeds its LFL (which may be defined as a condition in which the dust obscures vision at a distance of 5 feet (1.52 m) or less)
C.
Atmospheric oxygen concentration below 19.5 percent or above 23.5 percent.
D.
Atmospheric condition or concentration of any substance that is immediately dangerous to life or health as published in 1910.146 (Z) or (G) or which may be capable of causing one of the above reactions.
12.18 Housekeeping and Waste 12.18.1 RACE INTERNATIONAL shall at all times keep its work area in a neat, clean and safe condition and remove from the Owner's premises and the vicinity thereof and properly dispose of all debris and rubbish caused by RACE INTERNATIONAL's operations. RACE INTERNATIONAL’s cleanup responsibilities shall be continuous and RACE INTERNATIONAL shall provide sufficient workers dedicated to clean up to assure that it’s clean up obligations are fully complied with. 26

12.18.2 During the course of construction, all debris and scrap material shall be kept away from the work area. 12.18.3 Containers shall be provided by RACE INTERNATIONAL for the collection and separation of waste, trash, oily and used rags and other refuse. Metal (dumpster type) containers must be used and emptied promptly. 12.18.4 Garbage and other waste shall be disposed of at frequent and regular intervals in a manner approved by the owner’s Consultant. 12.18.5 RACE INTERNATIONAL shall not pour, bury, burn, nor in any way dispose of a chemical on the Work site without the permission of the owner’s Consultant or Owner. 12.18.6 RACE INTERNATIONAL shall clear all combustible debris to an authorized solid waste disposal site. NO OPEN BURNING OF DEBRIS OR RUBBISH WILL BE PERMITTED AT THE PROJECT WORK SITE. 12.18.7 Materials and supplies shall be stored in locations that will not block access ways, and shall be arranged to permit easy cleaning of the area. 12.18.8 All hoses, cables, extension cords, and similar materials shall be located, arranged, and grouped so that they will not block any access way and will permit easy cleaning and maintenance.
12.19 Environmental Protection 12.19.1 RACE INTERNATIONAL shall determine the relevance of the environmental protection requirements contained in national and local statutes, regulations, and requirements as applicable to the scope of this Contract. RACE INTERNATIONAL shall comply with all applicable environmental protection requirements. 12.19.2 RACE INTERNATIONAL shall notify the owner’s Consultant of any hazardous waste it will generate during performance of the work. RACE INTERNATIONAL has the direct responsibility of maintaining proper management of these wastes while on the Work Site and will verify to the owner’s Consultant in writing that the wastes have been managed and disposed of in a legally compliant manner.

12.19.4 RACE INTERNATIONAL shall develop a Pollution Prevention Plan / Waste Minimization Plan. 12.19.5 RACE INTERNATIONAL shall develop a Waste Management Plan that identifies the quantity and characteristics of projected waste streams, and the methods used to manage, store, transport, and dispose of the wastes. The Waste Management Plan shall address solid waste, recycled waste, hazardous waste, and any other projected waste stream.
12.19.6 RACE INTERNATIONAL shall identify and document:
Erosion control measures to be used at the Work Site. Air emission sources and control measures associated with the Work. Storm-water Controls. Liquid effluent sources and control measures, associated with the Work.
12.19.7 RACE INTERNATIONAL shall ensure personnel receive appropriate environmental training and that such training is documented. 12.19.8 RACE INTERNATIONAL shall take appropriate actions at the beginning of an activity to minimize potential impacts by effectively managing preventive and proactive environmental programs. 12.19.9 RACE INTERNATIONAL shall effectively integrate compliance activities with new and ongoing actions.

12.20 Spill Prevention and Control All personnel associated with a project are responsible for immediately notifying the owner’s Consultant of any observed spill, hazardous substance release, unauthorized water discharge, wildlife incident, or other nonconformance with permits or regulations.

12.20.1 RACE INTERNATIONAL shall develop a Spill Prevention Control and Countermeasures Plan that addresses management and response activities for materials used under the Contract. 12.20.2 In areas where equipment might drip oil or cause other damage to the floor surface, a protective cover of heavy gauge, flame resistant, oil proof sheeting shall be provided and maintained by RACE INTERNATIONAL between the equipment and the floor surface sheeting so that no oil or grease contacts the concrete. This requirement is applicable to both finished and unfinished floors. 12.20.3 Fueling and other fuel transfers shall be performed only in approved locations and by approved methods. 12.20.4 Temporary fuel oil or petroleum storage tanks shall be surrounded with temporary barriers of sufficient size and strength to contain the contents of tanks in the event of leakage or spillage. 12.20.5 Absorbent materials and drip pans are to be used to contain potential spills from fueling or bulk transfers of fuels including over-filling, malfunctioning valves, flange leakage, and flow-back. 12.20.6 RACE INTERNATIONAL’s personnel shall be appropriately trained to respond to spills and this training shall be documented.
12.21
Resource Protection Resources, to the extent practical, shall be preserved in their existing condition. Construction activities are confined to areas necessary to conduct the work as required by the Contract schedule, plans, and specifications. Environmental protective measures and procedures are provided to limit habitat disruption and mitigate environmental damages that may occur during construction. Site features subject to environment protection include, but are not limited to:
Existing roads and access routes Cultural and archeological sites Threatened and endangered wildlife and habitat.
12.21.1 RACE INTERNATIONAL’s personnel shall not deface, injure, or destroy shrubs, grass, or other landscaping in the project or surrounding area, or remove or cut same without permission from the ownr’s consultant. 29

12.21.2 Impacts to existing landscape shall be minimized when selecting sites for field offices and storage areas. 12.21.3 Storage of materials shall be confined to the project area or as approved by the owner’s Consultant. 12.21.4 Construction activities shall be confined to areas defined by the Contract.
12.22
Landscape Protection Maintaining good field practices in the planning and management of traffic and personnel will assist in the preservation of established vegetation. 12.22.1 No shrubs shall be used for the purpose of anchorage. 12.22.2 Precaution shall be used against injury to vegetation by controlling the use of site equipment, equipment traffic, and personnel and by using protective equipment when necessary. 12.22.3 Soil erosion pathways shall be protected with sandbags or comparable materials to mitigate silt transport. 12.22.4 Structures, equipment and vegetation on or adjacent to the Work site, which are not to be removed and which do not unreasonably interfere with the work, shall be preserved and protected. 12.22.5 Trees are only removed when specifically authorized and special care shall be taken to preserve vegetation that remains in place. 12.22.6 Only established roadways or temporary roadways will be used unless otherwise authorized by the owner’s Consultant. 12.22.7 When it is necessary to cross curbs or sidewalks, these structures shall be protected from damage. Damaged curbs, sidewalks, or roads shall be repaired.

The following project aspects shall be controlled to prevent adverse impacts to the waters in and around construction areas. 12.23.1 Wastewaters shall be managed according to applicable requirements. 12.23.2 Non-potable water sources shall be used where possible. 12.23.3 Ground water discharge shall be limited and controlled. 12.23.4 Storm Water discharge shall be controlled. 12.23.5 Fueling and hazardous materials shall be properly managed.
12.24
Sanitary Waste Sewage is normally disposed of through connections to a sanitary sewage system. Where such systems are not available, sanitary conveniences shall be provided. 12.24.1 Sanitary conveniences shall be properly secluded form public observation and maintained. 12.24.2 Upon completion of the work, sanitary conveniences shall be removed from the premises, leaving the premises clean and free from nuisance.
12.25
Temporary Facilities and Work Areas Field offices, staging areas, stockpile storage areas, and temporary buildings shall be placed in areas designated on the drawings. Temporary movement or relocation of facilities shall be made only when approved by the owner’s Consultant. 12.25.1 Spoil areas shall be managed and controlled to limit spoil intrusion into areas designated on the drawings and to prevent erosion of soil or sediment from entering nearby waters. 12.25.2 Spoil areas shall be developed in accordance with the grading plans. 12.25.3 Temporary excavation and embankments for work areas shall be controlled to protect adjacent areas from despoilment.

12.25.4 Signs of temporary construction facilities such as haul roads, work areas, structures, foundations of temporary structures, stockpiles of excess or waste materials, or other vestiges of construction shall be obliterated prior to final acceptance of the Work.
12.26
Historical and Archaeological Resources If historical or archaeological resources such as artifacts, features, deposits, human bones, or other cultural remains are encountered, work is to be stopped in that area and the owner’s Consultant shall be immediately notified.
12.27
Noise 12.27.1 The noise standard shall not be exceeded unless a variance is granted pursuant to the appropriate local regulation. 12.27.2 Low-noise-emission products shall be used to the maximum extent practical. 12.27.3 Hazardous noise signs will be posted wherever equipment and work procedures produce sound levels greater than allowable noise levels, as measured at the project limits. 12.27.4 No blasting or use of explosives is permitted without authorization by the owner’s Consultant.
12.28
Erosion and Sediment Control 12.28.1 Erosion and sediment control measures shall be provided in accordance with applicable requirements. 12.28.2 Reasonable precautions shall be employed to prevent run-on/run-off from construction areas, especially where the water may be exposed to soils and excavated areas. 12.28.3 Precautions shall be employed to prevent storm water and other run-off from leaving the site during activities such as excavation and trenching.

12.28.4 Areas of bare soils exposed by construction activities shall be held to a minimum. 12.28.5 Earthwork brought to final grade, as indicated or specified, shall be finished in a timely manner. 12.28.6 Side slopes and back slopes shall be protected upon completion of rough grading. 12.28.7 Earthwork shall be planned and conducted to minimize the duration of exposure of unprotected soils.

Dust Control 12.29.1 Dust control shall be performed as the work proceeds and whenever a dust nuisance or hazard occurs. 12.29.2 Dust shall be kept down at all times, including nonworking hours, weekends, and holidays. 12.29.3 Excavations, embankments, stockpiles, access roads, plant sites, waste areas, and other work areas shall be maintained free from excess dust that could cause a hazard or nuisance.

Construction and Demolition Debris Construction and demolition wastes include solid wastes such as building materials, packaging, and rubble resulting from construction, demolition, paving, and infrastructure. This non-hazardous, solid waste is the primary waste stream generated by construction activities. 12.31.1 Non-hazardous, solid waste shall be collected in designated containers established at or near the point of generation. 12.31.2 Debris in work areas shall be cleaned up as frequently as necessary. 12.31.3 All solid waste shall be removed from the construction site as needed to keep the worksite orderly and to prevent the dispersion of debris in high winds. 12.31.4 Burning of any debris or trash shall be prohibited. 12.31.5 All construction and demolition debris shall be disposed of in accordance with regulatory requirements.
12.32
General 12.32.1 The owner’s Consultant reserves the right to regulate smoking, open fires, carrying matches, and welding permits when and where deemed necessary in the interest of safety. RACE INTERNATIONAL shall be responsible for the enforcement to RACE INTERNATIONAL's personnel and visitors to comply with the regulations and restrictions as directed. These smoking, open fires and similar regulations may vary during the progress of construction, or during the testing, or initial operation of any portion of the plant. 12.32.2 Within certain areas designated as "OPEN AREAS" the owner’s Consultant will issue a "HOT WORK PERMIT" on a weekly basis which will authorize open flames, burning and welding activities during regularly scheduled working hours. 12.32.3 Within certain areas designated as "RESTRICTIVE AREAS" the owner’s Consultant will issue a "HOT WORK PERMIT" daily which will authorize open flames, burning and welding activities during regularly scheduled working hours. "OPEN AREAS" shall be reclassified as "RESTRICTED AREAS" whenever designated by the owner’s Consultant and upon completion. For “RESTRICTED AREAS” a trained individual, referred to as a “Fire Watch”, shall be assigned and equipped with personal protective gear, provided by RACE INTERNATIONAL. 12.32.4 The fire watch shall be maintained during lunchtime and other breaks and shall continue for 30 minutes after welding operations have been completed. This individual shall keep the area clear of unauthorized personnel during welding operations. 12.32.5 No matches except safety matches are permitted on the Work site. 12.32.6 No firearms or ammunition are allowed on the Work site. 12.32.7 Use of photographic equipment inside the Work site is strictly forbidden without specific approval of the owner’s Consultant and Owner. 12.32.8 No lewd or offensive material of a sexual or racist nature is permitted on the Work site. 13.0 FIRST AID & INJURY REPORTING REQUIREMENTS 13.1
RACE INTERNATIONAL’s Responsibilities A.
Ensure the availability of medical personnel for advice and consultation.
B.
Prior to commencement of work, make provisions for prompt medical attention in case of serious injury.
C.
Ensure that at least one of RACE INTERNATIONAL employees for each 15 employees performing work at the Work site has a valid certificate in first aid training from the U.S. Bureau of Mines, the American Red Cross, or equivalent in-country source that can be verified by documentary evidence and is available at the Work site to render first aid.
D.
Ensure that first aid supplies approved by the consulting physician are easily accessible when required. A First Aid Box shall be available and maintained on site and should contain medicine (list of medicine)
E.

in a weatherproof container with individual sealed packages for each type of item. Contents of the first aid kits shall be checked by RACE INTERNATIONAL before being sent out to the Work site and at least weekly during performance of the Work to ensure the expended items are replaced.

Provide proper equipment for prompt transportation of injured person to a physician or hospital or a communication system to contact necessary ambulance service.
G.
Telephone numbers and addresses of the physicians, hospital and ambulances shall be conspicuously posted.
H.
RACE INTERNATIONAL must orally notify the owner’s Consultant Construction Manager, Superintendent or HSE Manager IMMEDIATELY after any injury to RACE INTERNATIONAL’s employee(s) or employee(s) of subcontractor; equipment or motor vehicle damage; or fire or property damage.
I.
A daily record shall be kept on all employees requiring first aid treatment.
J.
RACE INTERNATIONAL shall ensure that each of the subcontractor meet these medical requirements.
K.
RACE INTERNATIONAL shall complete and retain on file all “Employee’s First Report of Injury”, and Incident Investigations and provide the owner’s Consultant a copy within 24 hours of event. Complete and retain on file the OSHA form 300 (or equivalent in-country form) and provide FLOUR ARABIA LTD a copy upon request. Reports must be made within 24 hours of notice of injury or illness to the RACE INTERNATIONAL.
L.

Each occupational illness or injury shall be reported immediately by RACE INTERNATIONAL's employee to RACE INTERNATIONAL's first aid attendant.
C.
All personal injuries and illnesses (job related) treated or reported (actual or alleged) shall be recorded on a daily First Aid log. This log should contain such information as employee's name, badge number, foreman, nature and cause of injury, treatment administered, date, time and first aid attendant's name. RACE INTERNATIONAL shall maintain such data for the owner’s Consultant review upon request.
D.
RACE INTERNATIONAL’s first aid attendant should treat the injured employee as often as necessary to ensure complete recovery, or the decision is made to seek medical treatment.
13.2.3 Medical Cases Not Requiring Ambulance Service A.
Medical cases not normally requiring ambulance services are injuries such as minor lacerations, embedded foreign bodies in eye, minor sprains, etc.
B.
RACE INTERNATIONAL must provide proper equipment for prompt transportation of the injured person to a physician or hospital.
C.
A representative of RACE INTERNATIONAL should always drive the injured employee to the medical facility and remain at the facility until the employee is ready to return. RACE INTERNATIONAL's representative should also carry necessary forms, i.e., authorization slips, return to work notices, etc.
D.
If it is necessary for the RACE INTERNATIONAL's first aid attendant to accompany the injured employee, provisions must be made by RACE INTERNATIONAL to have another employee, properly trained and certified in first aid, to render first aid during the absence of the regular first aid attendant.

If it is necessary to call the medical facility to be utilized, this call should be made by a designated RACE INTERNATIONAL representative while the injured employee is being transported.
F.
If the employee is able to return to the Work site the same day, he should bring with him a statement from the doctor containing such information as date, employee's name, date he is able to return to work, regular or light duty, date he is to return to doctor (if applicable), diagnosis, signature and address of doctor. If the injured employee is unable to return to the Work site the same day, the employee who transported him should bring this information back to the Work site and report it to the owner’s Consultant.
G.
RACE INTERNATIONAL should designate one employee to keep files on all medical cases and retain all medical statements in this file. The owner’s Consultant will periodically check to ensure the RACE INTERNATIONAL's medical files are being maintained.
13.2.4 Emergency Cases Requiring Ambulance Service A.
Medical cases requiring ambulance services would be such cases as severe head injuries, amputations, heart attacks, etc.
B.
Should ambulance service be necessary, the following procedures should be taken immediately. 1.
Contact RACE INTERNATIONAL first aid attendant or nearest employee properly trained and certified in first aid.
2.
While first aid is being administered, contact necessary ambulance service.
3.
While the injured employee is being transported, RACE INTERNATIONAL should contact the medical facility to be utilized.

One designated RACE INTERNATIONAL representative should accompany the injured employee to the medical facility and remain at the facility until final diagnosis and other relevant information is obtained. 39

A Serious HSE Violation is an infraction of the Project HSE Rules that involves a substantial probability that death, serious physical harm; major equipment damage, major property damage, or work stoppage could result as a result of the infraction. An Imminent Danger to Life or Health (IDLH) is a situation that places an employee at risk to their life by the actions that are being performed by an employee. B. A RACE INTERNATIONAL’ employee who commits a Serious HSE Violation or is in an IDLH situation shall be given a written reprimand by RACE 40

INTERNATIONAL and / or shall be removed from the Project Site and not returned to duty at the Project Site if such violation is determined by the owner’s Consultant in its sole judgment to be a flagrant violation. A documented oral reprimand cannot be issued for this type of safety violation. C. A second violation categorized as serious shall result in the automatic removal of the employee from the Project Site by RACE INTERNATIONAL, provided the second violation occurs within one (1) year of the first. 14.2

Progressive HSE Violation A.
A Progressive HSE Violation is an infraction of the Project Safety Rules that occurs when the infraction is not likely to cause death, serious physical harm, major equipment damage, or work stoppage.
B.
A RACE INTERNATIONAL employee who commits a Progressive HSE Violation shall be given a documented oral reprimand by RACE INTERNATIONAL. A second Progressive HSE Violation to the same employee requires that the employee be given a written reprimand by RACE INTERNATIONAL. The employee shall be removed from the Project Site by RACE INTERNATIONAL should the employee be guilty of a third Progressive HSE Violation, provided that the third Progressive HSE Violation occurs within one (1) year of the first. It is not necessary for the Progressive HSE Violations be identical for the progressive steps of discipline to apply.
