Dobutamine Stress Echo
Definition: Is a graphic outline of the heart’s movement, valves and chambers. The Dobutamine stress echo test involves infusing a medication through an intravenous (IV) line, called Dobutamine, while you are closely monitored. The medication stimulates your heart and makes it «think» it is exercising. The test is used to evaluate your heart and valve function when you are unable to exercise on a treadmill or stationary cycle. Your doctor uses the Dobutamine stress echo to: •• Determine how well your heart tolerates stress or exertion. •• Evaluate the function of your heart and valves. •• Evaluate the effectiveness of your cardiac treatment plan. •• Determine the likelihood of having coronary artery disease and the need for further testing.
To prepare: •• DO NOT eat or drink anything except water for four hours before the test. DO NOT drink or eat caffeine products for 24 hours before the test. •• Ask your doctor what medications should be taken before and on the day of your test. DO NOT take the following heart medications on the day of the test unless your physician tells you otherwise or if the medication is needed emergency, it’s as follow (Beta blockers/ Sorbide dinitrate / Isosorbide mononitrate / Nitroglycerin). •• If you have diabetes and take medications to manage your blood sugar, ask your physician how to adjust your medications the day of your test. Do not take your diabetes medication and skip a meal before the test. Bring glucose monitor to check your blood sugar levels before and after then tell the result to the lab personnel immediately passed on that you will eat and take your blood sugar medication after your test. •• You can wear whatever you like. You will need to change into a hospital gown to wear during the procedure. Do not bring valuables. What to expect: •• Before the test the nurse or physician will explain the procedure in detail, including possible complications and side effects. They will be available to answer any questions you may have.
•• First a Sonographer will gently rub ten small areas of your chest, and
place small sticky electrode patches to these areas. The electrodes are attached to an electrocardiograph (ECG) monitor, which charts your heart’s electrical activity during the test. •• An intravenous (IV) line will be inserted into a vein in your arm so the Dobutamine medication can be delivered directly into your bloodstream. The cardiac sonographer will perform a resting ECG, measure your heart rate and take your blood pressure. •• The sonographer will ask you to lie on your left side on an exam table so he or she can perform a resting echo. The sonographer will place a wand on several areas of your chest. The wand will have a small amount of cool gel on the end, which will not harm your skin. This gel helps get clearer pictures. This should not hurt. •• The physician or nurse will administer the Dobutamine medication into the IV while the cardiac sonographer continues to obtain echo images. •• The medication will cause your heart to react as if your were exercising: your heart rate will rise and you may feel it beating more strongly. It may cause a warm, flushing feeling and, in some cases, a mild headache. •• At regular intervals, the lab personnel will ask how you are feeling, and will check your EKG, blood pressure and heart rate. Please tell them if you feel chest, arm, or jaw pain or discomfort; shortness of breath, dizziness;
lightheadedness; or irregular heartbeats immediately. This procedure is usually well tolerated, but occasionally there are complications. Please consult the health care provider performing the test about the associated risks. •• As soon as your heart rate is adequately high, echo pictures are taken. The IV will be removed from your arm once all of the medication has entered your bloodstream. •• The Dobutamine stress echo takes about 60 minutes. The actual infusion time is usually 15 minutes. You should also plan to stay in the waiting room for at least 30 minutes after completing the procedure, or until all of the symptoms you have experienced are resolved. •• After the cardiologist reviews your test, the results will go into your electronic medical record. Your physician will have access to the results and will discuss them with you. Ask your doctor if you have any questions about the Dobutamine stress echo.

اختبار القلب بالصدى أثناء اإلجهاد بإستخدام الدبوتامني: عب��ارة ع��ن تش��كيل ص��وري حلرك��ة القل��ب والصمام��ات واحلج��رات ،.وهو يتضمن اعطاء عقار خالل الوريد يدعى (دوبوتامني) أثناء مالحظتك عن قرب. هذا العقار يحف��ز القلب ويجعله في حالة مش��ابهة حلالة اجمله��ود البدني، كما يس��تعمل هذا االختب��ار لتقييم أداء عضلة القلب والصمامات القلبية ف��ي حال��ة ع��دم قدرتك على املش��ي باستخدام السير الكهربائي. يستخدم طبيبك هذا االختبار لـ: • •حتدي��د مدى اس��تجابة القلب لإلجهاد أو التمرين. • •تقييم أداء القلب والصمامات. • •تقيي��م مدى جناع��ة العقاقير التي تتناولها حاليا ً. • •حتدي��د احتمالي��ة اإلصاب��ة بأمراض الشرايني التاجية ومدى احلاجة إلجراء اختبارات إضافية.
لالستعداد: • •الرجاء االمتناع أكل وش��رب أي ش��ئ ما ع��دا املاء وذلك ملدة 4س��اعات على األقل قبل االختبار ،وجتنب تناول املنبهات مثل املش��روبات الغازية ،منتجات الش��وكوالتة ،الش��اي والقهوة وذلك ملدة 24ساعة قبل االختبار. • •استفسر من طبيبك عن األدوية املسموح تناولها قبل وفي يوم االختبار. • •ال تتن��اول األدوية التالي��ة في يوم االختبار وي��وم قبله إال إذا أخبرك طبيبك أن بإمكانك استعمالها أو إذا استدعت احلاجة الطارئة ،وهي (مضادات البيتا ،موسعات الشرايني ،مضادات قنوات الكالسيوم). • •إذا كنت تتناول أدوية للسكري ،فاستفسر من طبيبك عن الطريقة املثلى الستخدامها يوم اإلختبار .ال تس��تخدم دواء الس��كري بدون تناول وجبات ي��وم االختبار .ويرجى إحضار جهازك لقياس مس��توى الس��كر في الدم قبل وبعد الفحص وإخب��ار املمرضة بالنتائج فعلى أساسه يتم تناول األدوية والوجبات. ً • •ميكن أن تلبس ما يحلو لك ،س��وف يطلب منك أن تلبس ردا ًء خاصا باملستشفى وعدم إحضار األشياء الثمينة معك.
ماذا سيجرى في اإلختبار
قبل االختبار ،س��يتم ش��رح خطواته عن طريق الفني اخملتص أو املمرضة أو الطبيب مبا في ذلك املضاعفات احملتملة الس��مح اهلل واآلثار اجلانبية ،س��وف يك��ون اخملتصون حاضرون ألي استفسارات.
• •س��وف يقوم الفن��ي اخملت��ص بتعقيم عش��رة مواضع صغي��رة على الص��در لتثبيت حساسات الكترونية الصقة وذلك ملراقبة تخطيط القلب خالل االختبار. • •سوف يتم يتم تثبيت مغذية بالوريد لكي يتم إعطائك (الدوبيوتامني) مباشرة. • •س��يقوم الفني خالل هذا الوقت إجراء اختبار تخطي��ط القلب ،وقياس معدل نبضات القلب وضغط الدم قبل بدء االختبار. • •سيطلب منك الفني اخملتص االستلقاء على اجلانب األيس��ر على السرير وذلك لعمل دراسة مبدئية بالصدى باستخدام جهاز صغير يوضع على مواضع محددة على الصدر بعد وضع مادة هالمية ب��اردة غير مؤذية ،هذه امل��ادة الهالمية تساعد في إيضاح صورة أنسجة القلب. • •س��يقوم الطبي��ب أو املمرض��ة بالب��دء في اعطائ��ك العقار تدريجيا فيما يس��تمر الفني اخملتص بتصوير القلب بشكل مستمر. • •سيس��بب العقار تسارعا ً في نبضات القلب كما لو كنت متارس مترينا رياضيا ومن الطبيعي أن حتس بنبضات القلب في هذه املرحلة .من املمكن أن تشعر بحرارة في اجلسم أو صداع خفيف. • •خالل فترات منتظمة س��وف يتم التأك��د من ارتياحك أثناء االختبار وس��تتم معاينة تخطي��ط القلب ،ضغط الدم ومعدل النبضات .الرجاء إبالغ الطبيب في حالة ش��عورك بأل��م ف��ي الصدر أو ال��ذراع أو دوخة أو صعوبة ف��ي التنفس .في الع��ادة ال يواجه املريض صعوبات أثناء االختبار .في حاالت نادرة قد حتدث مضاعفات. • •الرجاء التحدث إلى طبيبك ملعرفة طبيعة هذه املضاعفات. • •عندم��ا يصل القلب إلى معدل النبضات املطلوب س��وف يتم تصوير القلب مرة أخرى بواس��طة أش��عة الصدى .س��يتم إزالة املغذية حاملا ينتهي طبيبك من اعطائك كافة العقار من خاللها. • •يس��تغرق االختبار عادة مدة 60دقيقة ولكن الوقت الذي يتم اعطاءك العقار خالله ال يتع��دى غالبا ً 15دقيقة .الرجاء االنتظار بع��د االختبار ملدة 30دقيقة على األقل بعد االنتهاء من االختبار أو حتى تزول أي أعراض تشعر بها. بع��د مراجعة دقيقة لنتيجة االختبار س��وف يت��م اعداد تقرير مفصل بالنتيجة وس��يتم إيداعها إلى ملفك حتى يتسنى لطبيبك مناقشتها معك.
