haveallmorphed(orbeenacquired)into largeinternationalconglomerates,often ownedbytheshareholdersofmammoth, multinationalcorporations,withleadershipthatoftenknowsnothingaboutthe historyofthenametheyrepresent.To WallStreet,thesefamousnamesaresimply“brands.”

Regularreadersofthiswriter’seditorialramblingshaveprobablynotedthat pastsubjectmaterialhasbeenprimarily limitedtoflyingfunandsafety.Butthathas changedinrecentmonthsastheNational AirspaceSystemfacesaseriesofchallengesuniquetothecurrentpoliticalenvironment.Havingbeenemployedbytwo majorairlines,thelargestairspacecompanyinthecountry,havingalonghistoryin GA,charterflyingandaircraftmanufacturing,thiswriterhascertifiedaircraftand trainedflightcrewsinmorethan30countries.Theendlessfunincludedfouryearsin WashingtonD.C.representingtheairline industrywithamajorlobbyorganization. Onelearnsalotaboutpoliticsandhow“the systemreallyworks.”Thiswriterseesa significantandunwelcomechangetaking place.Moreandmore,thelegislative branchofourgovernmentissubcontractingtheirauthoritytobureaucraticagencies thatdonotreporttoCongress,buttothe Executivebranch.Whilepoliticiansargue aboutgasstovesandbathrooms,thedayto-dayrunningofthecountrynowbelongs
tobureaucratswhomwedonotknowand didnotelect.Thepowerofthesebureaucraciesisnearlylimitless,especiallythose thathaveregulatoryandenforcement responsibilities.TheFAAisaprime,but certainlynottheonly,example.
Giventhiswriter’s65yearsinprofessionalaviation,Ihaveworkedwith somegreatbusinessmanagersandindustryleaders.Earlyoninthislongaddiction toaviation,amemorablemanager offeredmesomeadviceIhaveneverforgotten.Theadvicewentsomethinglike, “Ed,asyoubecomeamanager,youwill needtohirepeoplewiththebesttalents thatcanbefound.Alltoooftenanew managerwhomaybea‘10,’hiresa‘9’. A‘9’hiresan‘8,’andsoon.Don’tletthat downslideinskillhappen.Alwayslook forthe‘11.’Theywillmakeyoulook good,andyoumightactuallylearnsomething.”Goodadvice,eh?Isthatwhatwe havegoingonintoday’sgovernment bureaucracies?
Americaisslippingintoapatternof ruleheadedupbytheExecutivebranch throughcabinetlevelbureaucracies. Remember,thevariousbureaucracies,like theDepartmentofTransportationandits sub-department,theFAA,aregovernedat thecabinetlevel,notbyCongress.Mostof whatwenowdoinlife,especiallyaviation-related,isgovernedbytheDOTand FAA,ledbypoliticalappointees.Allof whichbringsustoadisturbingquestion.Is theDOT,parentoftheFAA,beingledby a“10?”Anddoesthat“10”haveareputationforhiring“11’s?”Obviously,arhetoricalquestion.
Increasingairline-relatedrunway incursions,faileddatasystems,massive airlinedelaysandcancellations,supportof adictatorialandpoorlyqualifiedmedical division,ATCstaffshortagesandtherecommendationofapermanentFAA Administratorsopoorlyqualifiedthathe tookhimselfoutoftherunning,tendtosay alotaboutDOTleadership.Andweare lookingatonlytheaviationsideofthe DOT.Howisourtransportationinfrastructuredoing,especiallyrailroadsafety?This clearlyindicatesthatDOTmanagementis alongwayfrom“10”status.Itistherefore notsurprisingthatafulltimeFAAadministratorhasnotbeenselectedandthelatest ActingAdministratormaybeoneofthe leastqualifiedtoholdthatpositionin manyyears.Tobefair,thiswriterhasseen someprettybadchoicesforFAAleadership,butthisoneunderscoresthetrend towardbureaucraciesrunningthegovernment,ledbyfavoredpoliticaloperatives,

nottechnicallyqualifiedindividualswith experienceinthefieldbeinggoverned. Indeed,bureaucraciestendtobecome “self-running”governmentsofthenonelected.Forexample,givenaten-year study,denialsofthird-classmedicalcertificateshaveincreasedby57percentin justthelastsixyears.Isthisatrueneed baseduponsafetyissues,orsimplya bureaucracyproving“itisgettingthejob done?”Itisnotlostonthiswriterthataviationisnottheonlyhighly-regulated endeavorcaughtupinthisraceforgovernancebyappointee.Iamsurereaderswill recognizethesamesymptomsofdecayin theirownprofessionsorspecialareasof interest.
So,wheredoesthatleaveuswiththe MeettheAdministratorForum?Simply skipit?WithActingAdministrator TrottenbergandtheFAAFederalAir SurgeonNorthrupscheduledtobein attendance,youneedtobethererepresentingtherealworldofcivilianaviation. ActingAdministratorPollyTrottenberg hasabsolutelynoexperiencedealing withtheNationalAirspaceSystemand needstoseethatthe“rankandfile”give ahoot.Thisisnotacomplaintbecause Ms.Trottenbergisnotapilot,butthatan FAAadministrator,actingorpermanent, needstohavetheknowledgetoproperly assessifwhatinternaldepartmentheads aresayingistrue.Thisisexactlythetype of“boss”lovedbythe“deepstate.”Dr. NorthrupMDMPH,andherleadmanagerrunningtheAerospaceMedical CertificationDivision(classmedical issuance,denials,andrevocations),Dr. DavidM.O’BrienMDMPH,havemuch incommon.BothareretiredAirForce professionals,havelimited,ifany,experienceinprivatepractice,withFAR91 flyingandtrainingortheworldofFAR 121andFAR135Operations Specifications.Andwhatdoesthe “MPH”standforwhenaddedtothetitle MD?AquickruntoWikipediacameup with“anadvancedskillsetrequiredto developculturallyawarepublichealth plansandpoliciesthathelpdiverseindividualsandpopulationsthrive.”Justthe leadershipyouwanttotalktoaboutyour personalflightmedicalissues…right?
Whatshouldweexpectatthe“Meet theAdministrator?”Wearesuretohear carefullyscriptedstatementsthatallude tonewprogramsthatwillprobablynever cometopass,followedbyalimitedquestionsandanswersession.Industryrepresentativeswillpresentawelcoming handshakeandpromisetoworkproactivelywiththenewFAAleadership.That istheirjob,andtheyaregoodatit.Butas privateaviatorswhoareoftenvictimsof
thisdownwardslideinskillsets,wedo nothavetobesopolite.Wedeservebetterandneedtomakethatfeelingknown. Hereiswhereyou,thepersonenjoyinga greatAirVentureexperience,canpolitely, andinasoftspoken,factualmanner, voiceorparaphrasethefollowing thoughts.
ToActingFAAAdministrator Trottenberg–Ascertifiedaviators,both privateandcommercial,wearetaughtand testedontheprinciplesofAeronautical DecisionMaking(ADM).Acornerstone oftheADMprocessisanhonestevaluationofourownattitudes,knowledge,and abilitiespriortoeachandeveryflight. Modelsareprovidedtohelpusconduct thisself-evaluation.Riskanalysistechniquesareusedtocancelaflightifwedo notpassourownevaluation.Weareaskingthatyoudothesame.Honestlyevaluateyourskillsandknowledgeasitapplies totheoverhalfmillionpilotsandpassengersthatareairborneatanyparticular momentandtime.Doyoutrulybelieve thatyourbackgroundandunderstanding oftheNationalAirspaceSystemareupto thetask?Shouldyou,inallgoodconscience,withdrawfromyournewly appointedpositionandaidinthesearchfor anFAAAdministratorthatcantrulylead insupportingoneofthemostcomplexsystemseverdesignedbyourspecies?
ToFAAFederalAirSurgeon Northrup–Asaviators,wearecompelled tofollowtheFAR’s.Itisclearlyspelled outinFAR91.3and91.7thatwe,aspilot incommand,havesignificantauthority andresponsibility,uptoandincluding, “maydeviatefromanyruleinthispartto theextentrequiredtomeetanemergency.” ThisPICauthorityrequiresthatwemeet well-documentedstandardsofperformanceandknowledge,maintainedthrough on-goingcurrencystandards.Thebottom line,weareultimatelyresponsiblefor safety,withoutanFAAbureaucratconstantlylookingoverourshoulderandsecond-guessingoureverymove.
ButthatPICauthorityisnotextendedtoourpersonalhealth.The AeromedicalDivisioncan,withlittle morethana“narrative,”acttooverride bothapilot’sheathdecisionsandthe medicalopinionsofpracticingphysicians weengageas“partnersinhealth,”who areexpertinavarietyofspecializeddisciplines.TheAeromedicalDivisionacts ina“topdown”military-basedmanner withnopathforcommunication,discussionoralternativesexploredotherthana legalappealtotheNTSB.Ithasrecently beendisclosedthatalargepercentageof pilotsnowroutinelyomitdata,lieorselfmedicatetoavoidconflicts with

Thereisnothingliketheweekat AirVenture.ItmeansIgettoseelifelong friendsandthousandsofourmembers,but alsotheopportunitytomeetmanynew peoplefromalloverthisgreatworldof aviation.ItalsoneverfailsthatIgettosee excitingnewaircraftandgear.Evenfor someonewhohasbeenflyingasmuchasI have,Ialwaysseesomethingnewat Oshkoshandthat’soneofthebigreasons whyIlovegoingbackeveryyear.
There’snothinglikeputtingtheairplanedownontheyellowdot–orwhen theytellyouit’snowthepurpledot–that IfeelIhavetrulyarrived.Taxiingand

seeingthefolksliningtherunwayand wavingmeansIhavecomehome.
Iamalwaysamazedaswetakeour placeamongthethousandsofairplanes fortheweekatWittmanField,toseethe incrediblerangeofaircraftthatrepresent allcornersofthisgreatpassionweshare. Walkingupanddowntherowsislikea livinghistory,amuseumthatrepresents thelast100yearsofflight–fromthepioneersofthe1920’stotheverynewest experimentalorturbine.Itshowsthatwe havecomealongwayandthatwehave manymoremilestofly.Theevolutionof aviationisonfulldisplayandIencourage everyoneinOshkoshtotakethetimeto soakupallofthatamazinghistory.
Oshkoshwillmarkanotherchapter inagreataviationeventyearforussofar in2023.Wehadawonderfultimewith theAOPAteamatourfly-inatthe BuckeyeAirFairand,ofcourse,Sun‘n FuninFlorida.Butwhiletherearehundredsofaviationeventsacrossthecoun-
try,thereisoneAirVenture.Thousands andthousandsofaircraftaroundtheairportisasighttobehold.ButasEAA founderandgeneralaviationpioneer PaulPobereznyoncesaidsopowerfully, “it’stheairplanesthatgetustogether,but it’sthepeoplethatbringusback.”

that was fast and easy to build, but one that presented the fewest safety risks (chemicals, materials, processes) while building it. We needed an airplane that would be durable, easy to inspect and repair. It had to be easy and safe to fly, especially for eventual flight training use.
The Zenith Aircraft all-aluminum aircraft met all of our parameters, with thousands built around the world. They’re American made, using the latest CNC technology to simplify construction and guarantee a safe outcome. Their CH750 model was able to fly slower and land shorter than the majority of aircraft, a safety feature we insisted on.
Zenith Aircraft CEO Sebastien Heintz was impressed with the potential of our plan, and his generous support is what allowed us to launch the project. A decision had been made to begin with the largest components of the aircraft, the wings, rather than the traditional rudder or tail “starter kit.” This would generate more visually impressive results as soon as possible, create more excitement in the youth (and the parents), and hopefully get the entire community excited.
One of the program’s unique core values was that we wanted the parents to be there as equal participants from the start, learning to build an airplane alongside their kids in real time. It’s a shared educational opportunity that proved to be a big win. Parents are there in person to watch as their child becomes a confident achiever, and reaches for the sky. More importantly, it makes aviation a family activity, with a much higher probability of long-term participation.
We knew that our Young Eagles program would be the ideal venue to get the word out about the new aircraft build program. In early 2021, we finally started promoting the upcoming launch… EAA Chapter 40’s Project: JACKPOT youth aircraft building program was on!
Ideas and intentions are all well and good, but it was an outpouring of community support that made JACKPOT actually happen. Our success was built upon the generosity of forward-thinking people who saw the chance to create a life-changing opportunity for youth.
Zenith Aircraft, the top manufacturer of ‘light sport’ aircraft construction kits, was the founding donor and continues to provide financial and logistical support.
Vista Aviation, a leading flight school and avionics dealer, donated an entire aircraft that we could sell to raise critical funds.
Aircraft Spruce and Specialty, whose catalog is in virtually every aircraft shop worldwide, is a major sponsor of the program. Galpin Motors Group, a consistent
supporter of community programs, also made an early donation.
Valley Nonprofit Resources (VNR), a think tank and resource center that helps small nonprofits build solid infrastructure and capacity, made three highly experienced consultants (Jan McElwee, Susan Dunn, and Dr. Thomas Backer) available, thanks to funding from Dwight Stuart Youth Foundation. VNR’s involvement then helped facilitate grant funding from the Mark Hughes Foundation, and secured invitations to submit funding requests to other foundations.
And Project: JACKPOT itself? The first build session took place Oct. 23, 2021. Thirty people showed up for the first three-hour session on a Saturday afternoon. We started with a very brief introduction, getting right to the fun part of assembling airplane parts as soon as possible. A balance was struck between the excitement of putting an airplane wing together, explaining the purpose of each part, and the fundamental imperative of safety and quality control.
All instruction was hands-on workshop based; anything resembling a “classroom” would take away a lot of the magic we all knew was being created. By the end of the very first build session, the group had assembled the main skeleton of an entire wing, young and old cheering with pride as they held it in the air.

CUBCRAFTER RECENT MILESTONES DRIVE INVESTORS,’ TRIGGERINGA 9 PERCENT RISEIN STOCK PRICE
CubCrafters, a leading designer and manufacturer of Light-Sport and Part 23 Certified aircraft, recently announced that the price of shares in their Regulation A+ public offering was raised to $5.45 due to a number of positive developments.
In November of 2022, for the first time in its history, CubCrafters began accepting investments at $5 per share with a minimum investment of $400 per investor. Since that time, investors have enthusiastically responded and subscribed to the stock offering.

NAHF NAMES AIMEE MARUYAMA PRESIDENTAND CEO
The National Aviation Hall of Fame (NAHF) has announced the appointment of Aimee Maruyama as its President and CEO. Ms. Maruyama emerged as the top candidate after a competitive and inclusive national search conducted by the NAHF’ s Board of Trustees and facilitated by Korn Ferry, a leading global executive research and recruitment firm. She assumed her new role on June 5, 2023.

The island of Saint Barthelemy, or St. Barth (sometimes spelled St. Bart), has a public use airport, Gustaf III Airport (SBH), more commonly known as Remy de Haenen Airport, located in the village of St. Jean. However, this airport is best left to very experienced pilots in that the History Channel’s documentary titled Most Extreme Airports ranked it the third most dangerous airport in the world. It earned it’s ranking due to a very short runway, close proximity to nearby roadways and the necessity for a license to land at the airport.
The concrete runway is 2,119 feet. It is at the base of a gentle slope ending directly on the beach. The arrival descent is extremely steep over a hilltop traffic circle; departing planes fly right over the heads of sun-

A memorandum of understanding between Aura Aero and Embry-Riddle was signed at the Florida Activation Event, hosted by Florida Lieutenant Governor Jeanette Nuñez and Enterprise Florida on June 20 at the Business France Headquarters in Paris.

ARMYAND AIR FORCE AVIATION GUARD RESERVE FIRE TRAINING

On April 15, 1942, an Army Air Forces Site Selection Board recommended an area northeast of Walnut Ridge, AR for a Basic Flying School. The War Department approved the project May 12th. Construction started June 20th, and Flight Training began October 12th. 5,310 students entered training. By May 1, 1944, 110 marriages had been performed in the base chapel; more followed. Eighty-two babies were born in the base hospital – 40 girls and 42 boys. Sadly, 42 young men were killed in training. The last Army Air Forces class graduated June 27, 1944.
By John YountIn a tradition dating back to the 1960’s, in support of California’s private and government fire agencies, the annual National Guard fire preparedness training was conducted April 18 through 23. The training was held at three locations: classroom at the Mather Army Air Guard Rancho Cordova, Calif., Water rescue (crew egress) in case of an unplanned water touch down incident at Dolphin Scuba Club in Sacramento, Calif., and bucket drop accuracy at Pardee Reservoir near Sutter Hill, Calif.
The third class at Pardee was flight training, which consists of deploying fire buckets, field refueling and water drops in mountainous terrain. With each bucket full of water, the flight crew must show target accuracy in uphill drops, terrain contour drops and the most difficult drops downhill. The Amador Sheriff Boat Patrol provided the lake security. Coordinated communication units of the Guard and Cal Fire near the lake served to track aircraft locations and spacing.
During water bucket operations you will hear on the radio the term “at the dip” referring to the location of the water source where a helicopter fire bucket can be refilled. Radio communication between helicopters on a discrete frequency would sound like this: “entering the dip” (bucket full of water) and “leaving the dip.” On fires, this communication goes on constantly during helicopter flight operations. That way, other helicopters in the area know who is where. It isn’t unusual to have as many as eight or more helicopters (agency, private, military) “dipping” from the same water source.
A helicopter aerial coordinator (HLCO) was also utilized, during the
flight training. HLCO’S are the airborne traffic managers over the fire who keep an eye on the helicopter flight operations. They fly above the water dropping copter, usually in a smaller type helicopter, Bell Ranger. HLCO’S also communicate with ground fire fighters on changes in fire behavior or identifying a target (places to drop water) that needs to be cooled down.
The water dropping helicopters provided by the Guard are Blackhawk UH 60s, which carry under the ship, a Bambi Bucket of 660-gallon capacity. The Chinook carries a 2000-gallon bucket. Both are attached to the center cargo hook. In California both Blackhawk and Chinooks buckets are connected directly to the center hook. Other Guard units out of state may require or prefer a nylon sling of various lengths (20 to 80 feet) connected to the hook and that end of the sling is connected to the Bambi factory steel sling cables. Regardless of the bucket size or Guard aircraft utilized, if a problem develops, the entire sling and bucket can be jettisons.
Both the Army and Air Force Reserve companies are part of a state’s militia; not typically in the main stream of military operations. The units can be called to active duty or are at the demand of state government. The governor of each state, based on the incident needs –fire or disaster – activates the Guard. The Guard now “works” for the requesting agency until released from duty by that agency.

helicopter pilots. Carding is conducted by Cal Fire helicopter instructors stationed at McClellan Field, North Highlands, Calif.
When a forest fire reaches a certain level, one or more factors may require a Guard air rescue helicopter be assigned to the incident. A factor or factors activating a Guard air rescue copter could be: number of personnel on the fire line, terrain features that could require a winch operation, inaccessible to ground transportation or a hospital outside an area for immediate care, sometimes called the “golden hour.” The Guard air rescue helicopters, both Air and Army are HH 60 Pavehawk or a UH 60 Blackhawk respectively. These air rescue copters are winch capable, have a 24-hour response window and are equipped with night vision goggles (NVG). The flight group assigned to the air rescue copter consists of one Cal Fire helicopter manager, two military medics and the flight crew of three. There are two air rescue companies in California. Mather Army Guard, Sacramento, County, and Moffett Air Guard, Santa Clara County. These air rescue copters move up or down the state as required.
This annual training has expanded
each year. The disastrous drought and fires in California in the past few years have seen Guard helicopters coming to California to assist from as far away as Wisconsin, This year’s attendees were Nevada, Utah and Wisconsin. Next years attendees could be any Air National Guard or Army Guard unit out of state.
Have you ever wondered how much water a Guard helicopter can drop in an hour? If the water source is within two air miles, the Blackhawk with its 660-gallon bucket can deliver approximately 7,200 gallons of water per hour. The Chinook, a much larger copter, can deliver approximately 16,000 gallons of water per hour utilizing its 2,000-gallon bucket. Both copters can drop water on the fire for more than two and half hours before having to refuel.
Each Guard copter has assigned a Cal Fire military helicopter qualified manager (MHEM) who flies on all missions. The MHEM arranges for food, lodging and ground transportation regardless of fire size – all logistical support. If the Guard copter is working as a single resource on individual fires, like a series of lightning strikes, it may drop water on three or four separate lightning fires within a few hours and have to land at a remote airport for fuel and remain
overnight. The MHEM also directs the guard helicopter on initial attack (new fire starting) fires as numerous airplanes (tankers) and other copters will be arriving within the hour. The work load is hectic but organized.
The Blackhawks have a Guard crew of three, the Chinooks of four. Of course, the operation would be of limited value if not for the support staff on the ground –the aircraft fuelers, mechanics, and agency liaison representatives.
SEI Bambi Buckets provide all the Guard helicopter buckets. Part of the training was a representative from this company gave a refresher class on repair and maintenance. Each Guard ship carries a tool box of spare parts for the buckets, in addition “spares” are kept on board the copters for limited copter field repairs. As mentioned, a crew of three (Blackhawks, Pavehawks) or four (Chinook) Guard personnel keep the aircraft flying. The flight engineers (Chinook) and crew chief (Blackhawk or Pavehawk) assigned to these aircraft are usually part of the aircraft’s maintenance team even when not on fires. In other words, these copters have assigned crew chiefs and flight engineers who know the aircraft.

The story of how this particular paint was selected is quite interesting. Putting any paint on a military aircraft is a project that has to be approved at all levels of the Guard, including Washington. In order to paint high visibility paint on a Guard Copters, a research program was initiated. There were six panels two feet by four feet of aluminum radar reflective camouflaged paint provided by the Guard at Stockton, Calif. Six selected paints were applied. Then baked in 105 degree temperature outside at Cal Fire Baseline for a month. The test revealed that some panels faded, and others wouldn’t wash off using cold water and a wash brush with very little down pressure. The product could not leave a stain or outline and could be used in different areas of the copter for high visibility, like the nose or transmission housing. The paint had to be applied by a hardware “paint roller and brush.” The entire process of testing was videoed by a video team at Baseline Camp, near Jamestown, Calif. Since 2012, the only
approved paint is Crayola Tempera. All other brands of temperas failed the test. Pink was selected after painting some of the Guard copters flying on fires with yellow, orange, green and blue tempera; pink was the most visible. A paint kit with the Crayola pink tempera paint with rollers and brushes are on all Guard copters fighting fire. If a Guard ship is flying on lightning fires it will encounter rain. This will probably require a quick touch up of the paint at the next refueling. After numerous water bucket pickups the water mist from the rotor blades will also require a “touch up” after a few days.

But the word “legacy” does not apply to the aircraft being offered by Tecnam. Other major GA manufacturers will be at AirVenture, proudly showing their new offerings. After all, while a visitor to EAA AirVenture may think of this “fly-in” as a week-long party, punctuated by some of the most unpredictable weather on the planet, venders at AirVenture see this as a “trade show” where new products and offerings abound.
The question is, are the other airframe manufactures (Textron, Cirrus, Piper, and others) really showing “new” aircraft, or are their products “legacy” designs, some of which were originally certified in the 1940s? Sure, new paint schemes, interior feature improvements, new avionics and perhaps a different engine make them seem “new,” but the reality is these are not new designs, certified to the most recent FAA and international standards. This is where Tecnam is proudly different. Every airplane on display was certified in the 21st century.

It provides shared charter in the Northeast and the Caribbean, and in the case of St. Barth’s, scheduled air charter service from Puerto Rico to St. Barths. Other providers include Air Antilles, St. Barth Community, West Indies Helicopters and WinAir.
Most visiting aircraft carry less than 20 passengers in aircraft such as the Twin Otter. The de Havilland Dash 7 is the largest aircraft allowed to operate at this airport. The most common aircraft flying in for commercial service are the Pilatus PC-12, Cessna 208B Grand Caravan, de Havilland DHC-6 Twin Otter and the Britten-Norman BN-2 Islander.
Flying to the island from the states is a two-step (at least) process. Travelers must land at a neighboring island and

TheFederalAviationAdministration (FAA),onJune21,proposeda$1,290,780 civilpenaltyagainsttheCityofAlamosa andCountyofAlamosa,Colorado,for allegedlyfailingtoensureaircraftrescue andfirefightingpersonnelwereavailable duringscheduledaircarrierflightsatSan
LuisValleyRegionalAirport.
TheFAA-approvedAirportCertificationManualforSanLuisValley RegionalAirportrequiresittohavequalifiedfirefighterspresentatleastanhour beforethefirstscheduledairlineoperationofthedayand45minutesbeforeall
subsequentcarrieroperations.Theyalso mustremainonsitefor15minutesaftera scheduledairlineoperation.
TheFAAallegesthatbetween November2020andSeptember2021, approximately138scheduledaircarrier flightstookplacewhenfirefighterswere
noton-airport.

TheFairchildC-123Providerservedas acargoandtrooptransportaircraftthroughoutthelate1950stomid-1970s.About300 werebuilt,butonlyonesurvivesinflying condition.Thatairplane,affectionately knownas Thunderpig,willbecomingfrom theAirHeritageMuseuminBeaverCounty, Pennsylvania,tobepartofthe“Vietnam Remembered–50YearsLater”commemorationatEAAAirVentureOshkosh2023.
Thisparticularairplanewasbuiltin1956
andservedatanumberofbasesintheU.S.It wasresurrectedfromanaircraftboneyardin Arizonainthe1990sanddeliveredtotheAir HeritageMuseum’srestorationfacility.The name Thunderpig wasthenicknameusedby the911thAirliftWingbasedatGreater PittsburghAirportwhenitflewtheC-123there.
Theaircraftwasbasedonapost-World WarIIgliderdesigncreatedbyChase Aircraft,withthecontracteventually assumedbyFairchild.Inthe1960s,manyof
theaircraftweremodifiedwithjetengines addedtosupplementtheirtwinradials.They wereknowninSoutheastAsiafortheirabilitytouseroughjungleairstripstodeliver troopsandsupplies.Followingtheendofthe VietnamWar,theC-123wasprimarilyused bytheAirForceReserve,theU.S.Coast Guard,andanumberofforeignairforces.

AirEducationandTraining Command,withheadquartersatJoint BaseSanAntonio-Randolph,Texas,was establishedandactivatedinJanuary 1942,makingittheoldestmajorcommandintheAirForce.AETC’straining missionmakesitthefirstcommandto touchthelivesofnearlyeveryAirForce member.Overtheyears,morethan25 millionstudentshavegraduatedfrom AETCtrainingandeducationprograms.

In1974,NASAcreatedtheAviationSafetyReportingSystem(ASRS)toallowaviationprofessionalstoshareexperiencesinafrank,non-punitivemanner.TheASRSstructureallowspilotsandotheraviationprofessionalstofileananonymousreportofanincident,errororoccurrencethatthecontributorfeelsmightbeofvaluetoothers.These reportsaregathered,analyzedanddatabasedbyNASAexpertsandmadeavailabletoallinterestedpartiesasatoolforcreatingproactiveaviationsafetyprograms.Additionally,NASAdistributes anelectronicpublication, CALLBACK,whichcontainsselected,de-identified,reportsonafreesubscriptionbasis. WALKAROUND WISDOMAND PREFLIGHT GEMS
Athoroughwalkaroundinspection andassociatedactivitiescouldmitigatea hostofproblemsthatmightdevelopduringflightorgroundoperations.Thecharge toconductagoodwalkaroundflowsfrom 14CFR91.3(a),whichassignsdirect responsibilityandgrantsfinalauthorityas totheoperationofanaircrafttothePilotin Command(PIC)ofthataircraft.
Thewalkaroundinspectionand associatedactivitiesnecessarilycovera largenumberofitemstypicallydescribed inanAirplaneFlightManual(AFM)or AircraftOperatingManual(AOM).In thespiritoftheCFRandfromapractical pointofview,mosteverythingthatis possibletoobserveisalsocovered. Accordingly,thewalkaroundinspection andpreflightactivitiesareimportantto thesafetyofanyflight.
ThisissueofCALLBACKshowcasesreportedincidentsthatoccurred duringwalkaroundinspectionsandassociatedactivities.Notethevarietyofincidenttypes,thereporters’actions,andthe pearlsofwisdomineachnarrative.

AnobservantDA42TwinStarstudentpilotmadesomeinterestingdiscoverieswhileperformingthesewalkarounds.
• Ibeganpreflightingtheaircraft…andnoticedtherightmaingear hadcordshowing,andtheleftmainwas missingslipmarks.Iswitchedaircraft andbeganpreflighting[again]. Inspectingtheleftmainfueltank,Idiscoveredafuelnozzlecoverinthetank.[It was]thesmallrubbercoverthatgoes overthenozzlewhenfueling.Iremoved thecover,and[my]instructordeliveredit tothe[FBO]employeefuelinganother aircraft.Theflight…continuedwithout furtherincident.Ensurethevisualcheckingoffueltanksbeforeeveryflight.

Followingrelatedmaintenance work,thisAirbusambulancehelicopter pilotponderedhowacriticalitemwas
missedduringthepreflightwalkaround inspection.
• Maintenancedidatailrotorinspectionthatmorningandneededmetodothe run-uptocheckthebalanceafterward.They haddoneaRequiredInspectionItem(RII) inspectiononthework.Ididmypreflight walkaroundandlookedatthecowlings, latches,tape,wires,andtrackandbalance equipment.Iclimbedinandstarteditup, runningEngineNumber1uptoidle.Since itwasatrackandbalance,Iexpecteditto notbeanormalfeelingtailrotor,butthis wasmorevibrationsthanIwouldhave expected.Themaintainersaidtoshutit downandnotevenstarttheotherengine. Weshutthehelicopterdownandusedthe braketostoptheblades.Uponinspection, oneofthetailrotorbladeshadbeen installedfacingthewrongdirection.Inthe future,Iwilldomorethananormalpreflight walkaroundfollowingmaintenanceevenifit hadanRII.Iknewthebladeshadcomeoff; Iheldthebladewhile[themaintainer]was ontheladder.WhyIdidn’tlookatthedirectionoftherotorswhenIlookedatthem,I don’tknow,asidefromcomplacencyand relianceontheRII.Toensurethiswon’thappenagain,Iwillbesuretoverifyallaspects oftheworkthatwasdonewithverbalconfirmationandthemaintainerpresent.
UnverifiedAssumptions
Althoughallendedwell,thisFlight Instructoroverlookedadetailthatquickly placedtheaircraftandcrewinjeopardy.
• Thatmorning,Imet[mystudent],a privatepilottowhomIamprovidinginstrumentinstruction,atZZZ1airport.WhenI arrived,[thestudent]hadalreadyconductedapreflightinspectionoftheaircraft.… Unfortunately,Ididnotverifythefuelquantityintheaircraftandreliedonthestudent todoso.Theplanforthisflightwastodo theRNAV…approachintoZZZ2,gomissed [approach],andthenflyVFRbacktoZZZ1. AfterthemissedapproachatZZZ2and uponreaching3,500feet,theenginequit withoutwarning.Itookcontroloftheairplane,requestedprioritywithZZZ2 Approach,andturnedtowardZZZ.Iwas clearedfortherunwayandland-
ed…withoutfurtherincident.Wecouldnot restarttheplaneintheairorontheground.
Thereissomebackgroundneededat thispoint.Priortothisflight,Iwasthelast persontoflythisplane.Aspartofour standardpractice,afterthelastflight,I requestedfuelservicefromtheFBOand askedthattheplanebeputawayforthe night.AssumingtheFBOwouldhonormy request,Ileftfortheevening.WhenI arrivedattheairport,mystudenthadcompletedthepreflightandconfirmedthere wasfuelintheaircraft.I’veflownwiththis studentmanytimesandfoundhimtobea verycompetentpilot.…Accordingly,Idid nothavereasontothinktherewasnot enoughfuelintheaircraft.Whenwestartedtheplane,weresetthefueltotalizerfor fullfuel,sowhentheenginequit,Iexpectedtheretobenearly40gallonsoffuelin theplanewhenitwas,indeed,empty.
Thereareseverallessonstolearn here.Asaninstructor,youcanneverfully trustyourstudent,evenifheorsheisa certificatedpilot,andit’simportanttoverifyallinformation.Anotherfactorwas expectationbias.Aspartofourstandard practice,theaircraftisalwaysputaway withfullfuel,andtheFBOhasneverfailed tohonorthisrequest.WhenIarrivedatthe airport,Iexpectedtheaircrafttohavefull fuel,andIexpectedmystudenttoverify thisforme.Anotherfactoristhatitis impossibletoviewthefuelgaugesfromthe rightseatontheCessnaCardinalasthey areontheleftofthepilotsideyoke,soI wasunabletousethemtoverifythefuel state.Inordertopreventarecurrence,I amgoingtosuggesttotheclubthatwe standardizeawaytomeasurethefueland tomakeitarequirementtomeasurefuel beforeeveryflight.Theaircraft’sfuel quantityisdifficulttocheckvisuallysince thereisaspring-loadedcoveroverthefuel port.Attheendoftheday,however,Iwas theFlightInstructorandshouldhaveverifiedthefuelquantitybeforedeparting.
HazardousMaterials Compromised
Duringpreflightinspectionactivities, thisB767FirstOfficer(FO)discovered
andmitigatedadangerousflighthazard.
• DuringtheHAZMAT[preflight] inspection,Idiscoveredaflammableliquidboxthatwascrushedonthebottomof thestack.Therewerenofumesor spillage,buttheboxhad“DoNotStack” ontheside.Wedetermineditwasnotsafe totransportinitsconditionafterhaving adiscussionwithDangerousGoods (DG)personnel.Itwasremoved,andwe continuedtheflightaftera35-minute delay.Cause:Improperloadingofhazardouscargo.Suggestions:Properly loadcargo.Payattentiontohazardous goodsdocumentation.

ThisFlightInstructorpondersa humblingthoughtspawnedbyadiscoverymadeduringastudentpreflight inspection.
• [Iwasconducting]alocalseaplanetrainingflightinaPiperPA-18150Supercubonamphibiousfloats.The studentwasgettinginstruction…toaddonasingle-engineseaplaneratingtohis commercialpilotcertificate.Itwasour thirdtrainingflight.Theflightwasmostlyonwateroperationswithmultipletakeoffsandlandings.Waterconditionswere fairlycalmbutnotglassy,withabout5to 7knotsofwind.…Theflightwas uneventful,andI…didnothavetotouch thecontrolsformostoftheflight. Takeoffsandlandingsweresmooth,and noanomalieswerenotedbymyselforthe student.Theflightended…withnoincidents.Thepreflightinspectionoftheaircraftbythenextstudentdiscoveredafailureofthefurthestoutboardsectionofthe rightelevatorjustpastthehingepoint. Thatstudentfoundthisfailureapproximately30minutesaftertheconclusionof ourflight.Wecannotbecertainthatthe failureexistedoroccurredduringour trainingflight,asnoabnormalaircraft handlingcharacteristicswerenotedby meormystudent.However,basedonthe timeline,itisprobablethatthefailure couldhaveexistedoroccurredduring ourflight.Thecauseofthefailureis unknownatthistime.

Supplyingdoorsformilitarybasesis nothingnewforSchweissDoors.The companyhasbeendoingitforyears. AlthoughCanadianForcesBaseBorden (CFBBorden)didn’thaveaneedforspecialtyblast-proofdoorslikeothermilitaryestablishments,arecentbuilding expansionprojectforitstrainingschool calledfortheuseofSchweisshydraulic andbifoldliftstrapdoors.
CFBBordenislocatedapproximately62milesnorthofToronto,intheheart ofSimcoeCounty,Ontario.Thebaseis ideallylocatedtoservicepersonnelfrom allacrossCanada.CFBBordentrains 15,000militarypersonnelannuallyand employsapproximately3,250military membersand1,500civilians.Itsmission includessupportingitscustomersinthe mostcost-effectivemannertoenable themtoaccomplishmissions.Thesecustomersincludeseveralmilitarytraining establishmentsandavarietyofothermilitaryandcivilianorganizationslocated onthebase.
