ealthcare service providers have always been at the forefront of any crises, be it natural or manmade. It is of no surprise that during peak periods of the COVID-19 pandemic, healthcare professionals and hospitals across India strived day in and day out and made immense sacrifices to provide the best of care to all those affected and impacted.

Medical practice is a service to mankind. It is both a skill and an art

08 | March 2021

On the same note, through our edition titled India’s Best of 5 Recommended Hospitals, we wish to exhibit those hospitals and healthcare service providers that have been actively contributing towards India’s recovery from the pandemic. And one prominent name among those hospitals is Bangalore Baptist Hospital (BBH). BBH started as an 80-bed general hospital on January 15, 1973 and has today grown into a 340bed tertiary care hospital providing state-of-the-art health care facilities to all strata of society. BBH is a nonprofit organization registered under Karnataka Societies Act and is being governed by Christian

Medical College (CMC), Vellore since 1989. It is the first mission hospital to be National Accreditation Board for Hospitals (NABH) and National Accreditation Board for Laboratories (NABL) accredited. BBH is on a mission to provide high quality holistic care, train and develop committed professionals through quality academic and research programmes, to serve the community. The hospital is also a recipient of many awards and accolades. Through an educational motive, post graduate residency programmes (Diplomate of National Board) in 13 Specialties and multiple B.Sc., and Diploma programmes (in Nursing and in Allied Health

BBH’s super specialty services include Renal transplant services, Bone Marrow transplant services, Arthroscopy and Arthroplasty – Knee, Hip and Joint Replacement, Bone Densitometry, Bariatric Surgery, Cochlear Implants, Cardiac Science – Interventional, Cardiology with Cath Lab and Cardiothoracic Surgery, Dental Surgery with Dental Implants, Orthodontics and Pedodontics, Dermatology and Cosmetic Surgery, Endocrinology and Diabetes, Gastroenterology – Medical and Surgical, Geriatrics, Interventional Radiology, Infertility Clinic and Reproductive Medicine

The hospital also offers one-stopfull-body health check-ups where in every patient is able to do a series of lab tests and investigations based on their age and requirement and complete with doctor consultation.

Best of the equipments are available in the hospital. A state of the art Linear Accelerator for radiotherapy treatment in cancer, a 128 slice high-end CT scan machine, MRI, PCR Lab and a digital mammography machine with facility for stereotactic biopsies, critical care, anaesthesiology, operation theatres, state of the art bone marrow transplant unit etc have seen much investment and importance. Not only what is seen, but what is at the ‘back end’ – that is unseen is also given much importance at BBH. In addition to high tech care, a large community health initiative covers a population of one million in the urban slums and another million in Bangalore rural district. BBH is also a model Palliative Care Centre. It is also the implementing agency for District Disability Rehabilitation Centre of Bangalore rural district. “Quality with Compassion is the BBH motto. There is a misunderstanding in some quarters that compassionate care can be of a lesser quality. It could not have been further from the truth. True compassionate care is also of the best quality possible. A conjunction of the two has been a success story of BBH,” says Dr Naveen Thomas, the Director and CEO of BBH. Dr Thomas has been in a leadership position at BBH since 2014 and has successfully led the hospital to zenith of success since then. He joined the hospital initially as a Junior Medical Officer in 1988 and later did his specialty in General Surgery, super specialty in Paediatric Surgery and an MPhil in Hospital and Health Systems Management. “We invest in people. ‘Personnel’ 10 | March 2021

have always been our greatest asset. Our commitment to teaching has resulted in further improvement in quality care,” says Dr Thomas. Commenting on the challenges that BBH had to face during the peak periods of COVID-19 pandemic, Dr Thomas says, “BBH has faced with the challenging task of providing care/treatment to the COVID patients. Additional infrastructure was made. Patient beds were purchased. Partnership and support of many well-wishers helped us to sail through this. COVID did not stop our services in the hospital and in

the villages. Apart from acute care for COVID patients in the hospital, our community outreach team continued reaching out to disadvantaged communities as well.” Bringing his leadership experience into the spotlight, we asked Dr Thomas on what advice would he give to aspiring doctors and healthcare enthusiasts who are stepping into the healthcare sector, to which he replied, “Medical practice is a service to mankind. It is

both a skill and an art. You must train yourself to become the best in your area of expertise. You must selflessly give yourself into this. Your reward is the satisfaction that your patients experience when healing occurs in their body and mind.” BBH’s blueprint for a successful and ceaseless future includes achieving more milestones, addition of more healthcare facilities, and other provisions as follows: • Be a beacon of healthcare innovation and quality.

• Continue to impact many more lives, communities, villages, and slums. • Continue to grow to a 500 bed credible tertiary care centre of ‘Quality with Compassion’. • To influence and partner in policy development and the quality journey of fellow travellers and health care organisations. • A new mother and child block, addition of brachytherapy centre, liver transplant services, Assistive Technology services and oxygen generator are all taking shape. • Another one of our aims is to grow as an educational institution – imparting quality medical education at all levels.

Dynamics of

HEALTHCARE POST COVID-19 Future Scenario

I

n the past century when World War I was still going on which precisely started in January 1918, a deadly influenza virus was identified for the first time in Kansas. It lasted 36 months, that is till December 1920. The virus infected over 500 million people which was around one third of the world’s population at the time. The death toll has had, and still has, a rough estimate of at least 25 million people, but possibly even up to 50 or more.

There is reason to believe that virus attacks may become more common in the near future. In any region, but especially in the health sector, prevention and preparedness are critical. This article uses India’s dramatic experience with COVID-19 to address the core aspects of the role of health technicians. The health technicians that were in partnership with management and medical staff, have responded to the ever-changing demands by ensuring appropriate care facilities for contaminated patients while also ensuring the safety of all patients and operators. With the rapid dissemination of COVID-19, hospitals have been forced to reorganise their network and assistance activities in order to

www.insightssuccess.in

March 2021 | 15

respond to the health emergency in a matter of days, if not hours. To accommodate the ever-increasing number of patients, hospitals started to convert many of their wards into acute, sub-intensive, and inpatient sections. The healthcare sector should expect a slowdown in growth as the planet grapples with a global epidemic triggered by the COVID-19 pandemic. Governments will re-allocate funds to fund healthcare programmes and determine the viability of immunity cards, mass vaccines, and scaling up touch tracing as the shortterm need for COVID research and the search to discover a vaccine intensifies. The rapid spread of the infection, as well as the unexpected influx of people at high risk of infection, necessitated an urgent increase in bed capacity, especially in intensive care units, which were housed in specially equipped wards with separate routes and dedicated teams. Inside a hospital, enabling a section or pavilion for the management of positive COVID-19 patients involves transferring whole units, creating multi-professional teams, ensuring the availability of Personal Protective Equipment (PPE), even a hundred times more than normal, educating the dressing and undressing teams, and recognising various dirty/clean content paths. Reminding all personnel, urgently upgrading the electrical grid, oxygen system, and room ventilation, obtaining negative pressure, if possible, installing all equipment, and in parallel separating the pathways and finding locations for the other patients (transplants, oncology, etc.) to ensure the continuity of treatments will be necessary. Key health administrators, teams of professionals, directors and coordinators of the divisions and programmes concerned, nursing care, technical services, pharmaceutical engineering, risk control, pharmacy, and others is fundamental and must function in harmony. The cumulative effect must be close to that of an orchestra, resulting in complex “machine control” in motion in the sector.

Informatics and artificial intelligence (AI) applications for process automation and organisational analytics will expand at a rate of 100 percent. For example, GE Healthcare’s FDA-approved Critical Care Suite AI tool can assist radiologists in prioritising critical cases. The resumption in imaging for the backlog of elective procedures in Q3 and Q4 will result in teleradiology and AI-based solutions gaining from new investments. Scale-up in capacity, flexible payment options, and redistribution of the workload will accelerate partnerships. The United States will have an excess of 100,000 ventilators, while Western Europe will buy another 30,000 to 50,000 ventilators. This geographic gap could reshape non-hospital and home critical care models, while embedded analytics solutions will resurrect the mature tracking device segment post-pandemic. Resmed’s cloudbased AirView app for home treatment has introduced a remote control solution for ventilators in the United States and Europe. In-vitro diagnostics (IVD) research in a healthcare environment is currently unable to satisfy unparalleled demand. The $5 billion point-of-care monitoring industry for infectious diseases will accelerate the imminent revolution in service models by the end of 2020, with alternative testing sites such as pharmacy being permanent ecosystem participants. The cultural transformation was not as fast as the technological one. And in order to benefit from the technological growth we experienced, healthcare participants must leapfrog cultural transformation.

-Ashish Chandgude

There will be many changes in the future due to the pandemic. Some of them forecasted are mentioned below: With more than 200 percent growth since the pandemic, India’s telehealth markets have reached a tipping stage. Following the pandemic, automated meetings with healthcare providers will become the normal form of treatment delivery. Reimbursement, medical preparation, and network scalability, on the other hand, would be crucial in recalibrating telehealth. 16 | March 2021

Embracing Technological Innovations At Fortis Escorts, it is not just about being better, but making each day better. The institute has been at the forefront of innovation. Critinext, an E-ICU model, is Asia’s first tele-ICU, which provides ‘Intensive Care beyond Boundaries’. It is a fusion of critical care skills, technology, and innovation to provide compassionate 24*7 intensive care, to address critical care, staff shortage in the peripheries, and to enable remote to manage critically ill patients at site instead of transferring them to metros. The institute employs advanced technology to maintain medical records, audio visuals, and smart alert system as rapid response tools and provide 24*7*365 monitoring assessment, support, and interventional services. This also enables them to bridge the knowledge and educational gap by providing teaching and training to doctors, nurses, and paramedical staff.

After a tough year managing the pandemic, FEHI is moving forward as a stronger entity with its core values, focusing on its work as a responsible and reliable healthcare service provider, dedicatedly committed to patient care. FEHI is excited about the cutting-edge clinical work being done at its hospitals. The institute has joined forces with IHH-Berhad. This will allow it to expand the brand. It is a partnership which has great potential to leverage clinical and corporate expertise. Advice to the Young Healthcare Leaders Team FEHI believes that the healthcare landscape is a rapidly changing and demanding one, driven by innovation, research, growing patient needs, technology, and globalization is also providing a great platform for Indian professionals.

Battling the Pandemic Effectively The hospital believes that the pandemic has certainly affected its operations. “We treat a lot of international patients, those who are rushed to the hospital with endstage cardiac ailments, and patients who are brought in emergency and because of COVID, our ability to do the same was affected”, states team FEHI. Supply chains were disrupted, medical travel for global patients was put on hold, a separate designated area had to be built for COVID patients, and all resources were dedicated to ensuring the pandemic was controlled. The OPD footfall was affected as it dipped to 15-20%, the infrastructure was redesigned with the end objective of ensuring the virus did not spread, elective surgeries were postponed, and many heart patients did not get the care they needed at the right time.

It states, “We must nurture and develop leadership and excellence within ourselves to adapt to emerging situations and crisis. In a growing sector such as healthcare, there is a need for good doctors, allied staff, and caregivers, in specialties such as oncology, organ transplant, neurology, paediatrics, and mental health, where there is a disproportionate shortage of clinicians”. FEHI believes that there will always be a demand for people who are not only highly qualified but are also caring, compassionate, and courageous.

The hospital staff had to be regularly quarantined, making the workforce sparse, and some hospitals had to give monetary bonuses to staff to encourage them to come to work despite the risk. This led to a marked deterioration in their health. “However, from the second quarter of 2020, we started to witness improvement in the results. We did

T

he Internet of Things in healthcare has emerged as an inescapable power in diverse structures. As a way to interface virtually with any individual along with any gadget at a distinct location, IoT is especially a resounding idea for medication and wellness programs. With the advancement in medical care, IoT is nearly inseparable from Remote Patient Monitoring (RPM) and constantly changing the idea of existence in healthcare. Capturing and observing information on diverse devices that are associated with patient engagement, these remote patient monitoring gadgets are intended to provide care and assistance with the help of home-based devices that are interconnected with smart sensor technology. Furthermore, because of such savvy innovation, medical facilities are now competent enough to not just access a wide assortment of information, but also use that information immediately to administer the most ideal consideration for patients by diminishing inefficient expenses. A promising growth has been seen in this imaging technology, although the concept was already widespread and is making its mark with innovative devices in the healthcare sector. From motion sensors to pacemakers – Healthcare gadgets are already advanced in utilizing the IoT platform extending its imprint in equipping with application and machine connectivity. Remote Patient Monitoring is redesigning the healthcare space and has improved the convenience of care delivery at the key areas of efficiency, protection, and satisfactory assistance. The level of comfort that patients are receiving is one of the key factor for the maximization of IoT healthcare. Especially for more serious and critical medical conditions, the extra time frame offered by remote monitoring can be a huge difference between life and death. Precision, which can support both effectiveness and early mediation, is another significant result of IoT-equipped medical solutions. Coupling all smart devices and alarms into a simpler GPS network, that not only avails the possibility of early intervention for hailed patient conditions but also guides other help groups to react quickly and with better transparency in times of crisis. The smart innovation enables with the internet of things is especially valuable for patients who live in provincial or distant zones. Acknowledged security and safety of patients are also one of the reasons that transformed the practice of patient engagement in the healthcare sector. With improved virtual security and upgraded tools, the accessed confidential data of patient’s health information remains secure within the network.

26 | March 2021

Some more modest emergency clinics have utilized RPM joining as a way to help update in-house information security endeavors, just as to more readily coordinate with electronic health records and meet the obligations. Portability is another factor that is actively taking part in the model of patient management through the developing IT technology. It improves patient fulfillment with the launch of smart wearable devices – Fitbit, Google Glass, Chest Strap, and many more that monitors heartbeat and make calls instantly at the time of emergencies. Such advanced gadgets not only save time but are also effective in communication. It reduces lots of stress and confusion for both patients and providers. Smart watches, phones, and other wearable devices provide accurate waiting times for emergency treatment and assist patients in finding their way to the nearest hospitals along with tracking the nearest parking space. The self-driven applications directly connect the provider and equip them with the information of location, required services, and time of appointment. The automated technology with sensor badge can track and monitor the real-time surgery and post-recovery of a patient. Keeping updated with every procedure in real-time, these smart devices have proved to be an improved technology for patient’s satisfaction. Subsequently, IoT is in fact an aid for medical care as well as pretty much for every industry that is fit for empowering its administrations through IoT. Its present speed of development in medical care is ideally best for this area. IoT medical care implies an effective and more open patient consideration experience. It permits staff to tackle their job in a superior manner, and this is the motivation behind why 78 % of medical services specialists estimate that IoT will change the business in a superior manner and have increased their usage of IoT gadgets dramatically. With worldwide connectivity and developed infrastructure, the opportunity is already demanding healthcare sectors to adopt IoT-enabled techniques for a better experience and patient satisfaction. As patient engagement is the top priority of the healthcare sector, therefore revolutionizing the world with the concept of the right information at right time could proactively provide valuable treatment and engagement.

India’s Best of 5 Recommended Hospitals

1. Kindly talk about different care packages and services offered by your hospital. The mother’s womb is the first institute to personality and intelligence development. Tatva Health & Care a.k.a. THC offers unique healthy progeny antenatal services based on Yoga, Ayurveda, Diet and Meditation. Tatva Health & Care is a place where authentic Panchakarma treatment is given for weight loss and other health related issues. Along with physical fitness, THC also handles mental health with psychological counseling. 2. What is the vision and mission of your hospital? Vision: To provide healthy progeny to our patients and always prefer to implement Health Beyond Medicine aspect. Mission: Awakening of Ayurveda and promotion of yoga practices including benefits of a diet for the betterment of the healthy individual. 3. What kind of challenges has your hospital and its team faced at the time of its inception? In this modern world, people are more oriented towards modern medicines with the quick relief phenomena. We are not yet convinced of traditional medicine and therapies and this was our toughest challenge. Eventually, as we started implementing our concept of “Health Beyond Medicine”, slowly the vision of individuals changed, and they understood the true benefits of traditional treatment. THC has eventually handled most of the patients which have a history of several modern medicinal treatments. In today’s world, as new diseases like Covid are emerging, we need to be ready for these kinds of pandemic/epidemic situations. Unidentified variations in diseases and identifying the treatment for these is the biggest challenge. 4. How the technological advancements and innovation in the healthcare space has influenced your hospital’s daily operations? Innovations in traditional medicines are providing simple options to patients including the prepared form of formulas, churna, etc., and a systematic approach towards Panchakarma procedures. Highly modified instruments like steam therapy equipment, the shirodhara arrangement has provided great relief and confidence to the patients. This eases the implementation of procedures daily.

As we all are aware the Covid Pandemic compelled us to explore alternatives to modern medicines. This helped us to think beyond the present medicinal world and we discovered few untouched alternatives, Indian Yoga and Ayurveda is one of them. Indian Yoga and Ayurveda with changing world vision is slowly building a platform to resolve future endures of mankind. And hence THC visualizes a better tomorrow. 6. We all know the Covid-19 pandemic has created a lot of workload for caregivers and ancillary healthcare service providers. But what problems your hospital has faced specifically in this ongoing pandemic? As the pandemic situation spread lots of fear in the people’s minds, it was an uncertain situation in the clinic. The treatment protocol was not in place and hence the managing patients were difficult. THC took an initiative and trained the staff to handle the covid patients. “Awareness” was the most critical aspect, and we handle it well. Still, the hidden fear of Covid and lack of knowledge kept the normal patients away from the clinics. 7. According to you, why is patient engagement necessary when it comes to enabling prompt healthcare? How IoT is changing the healthcare paradigm by leveraging patient engagement and delivery of carerelated services? Yes, THC believes patient engagement is necessary preferably we count on those who are proactive about their health. We built their trust by educating them with factual knowledge of Yoga and Ayurveda to get a cure. so patient faithfulness drives them to schedule more and more necessary appointments. (e.g., Preventive care appointments and those needed to manage chronic conditions.) When it comes to the healthcare industry, IoT has opened a myriad of possibilities to improve accessibility, safety, and affordability of services through better analytics and techniques. The healthcare industry is facing various challenges today, from rising healthcare costs and increasing aging population to care management and redefining the way healthcare is delivered. There are numerous uses and applications of IoT in the healthcare industry. E.g., telemedicine and remote health monitoring. IoT technologies’ have extended patients care beyond the four walls of the clinic. Professionals from THC can remotely monitor patients and keep track of their vital signs and respond quickly to their medical conditions especially in our Antenatal Mother care Patients.
