Maria Andrea Santillana Optometrist Argentina I want to thank you in advance for the excellent care provided for you at The Vision Care InstituteTM Bogota, enjoy this beautiful city and to enable us, gives us a new horizon in our professional work. My most sincere appreciation to the entire group of teachers, for their work and dedication, I hope to come back and continue your support

During this time you should be prepared to provide the educational needs of your clients regarding the cultural changes of adoption of ocular and visual hygiene behaviors. This requires you to provide clear presentations concerning the use and periodic replacement of disposable lenses, as well as information about the use of sun block creams and UV filters. The fitter and his team should be able to clearly communicate information about controls and monitoring of the refractive defects, be capable of creating in the patient a positive attitude regarding importance of his loyalty and the professional service being offered. In the same way, the beginning of the year is a moment to renew and complete your stock of contact lenses. Now is the time to obtain the latest technologies, change up your advertisements and materials, and ensure that the new year and return from vacation generates a permanent renewal and change in your customer’s attitude. Not only is it necessary to change the price of products, but also show a substantial improvement in service, quality, sales and benefits of the products. Diligent work at this time of the year is crucial for the success of your service for the rest of the year. Take advantage of it now!

Dr. Guillermo Carrillo, from Perú.

Thank you very much.

Fitting of soft contact lenses in teenagers has been determined to be safe worldwide, as well as a proactive service. This also represents a good opportunity to create long term loyalty. In Japan, it is reported that the users average age is around 13 years old, but many Latin American fitters are afraid of addressing potential users in this age range. They are influenced by myths regarding use, care and maintenance, motivational barriers in parents, fear of presenting adverse events and, in general, have uncertainty regarding possible problems that might occur during the fitting process for patients of this young age. According to a general survey, many Latin Americans are serving this age segment, and even patients of a younger age with fitting of soft disposable contact lenses. There appears to be a special increase in the correction of astigmatic defects, both myopic and hyperopic, and surprisingly, in the 0.75 cylinders, with a great number of successful fittings. The practice of contactology states that the test of the diagnostic lens is important and decisive for the user and the people around them to understand that the use of the soft lens, nowadays, is a common and safe practice, and that it corrects defects that some years ago were considered difficult to overcome. The information this new user receives is vital so that he, through proper information and perceptions, can spread the benefits of contact lenses. During this time, when people are returning to school or work, it is important that teenagers and new users experience the novelty that they are not wearing glasses, and are happy users of soft contact lenses.

When we talk to them about contact lenses, it is important we give them information about the category and dual use culture (glasses and contact lenses), without undervaluing their capability to easily fit to a new life style given by the frequent replacement of the contact lenses and take responsibility of their own care of visual and ocular hygiene. They only need to be told and explained the change proposed, and ensured that you will watch over their safety and comfort.

Due to the fact that in the south of the American continent they are enjoying an intense summer time, and that in the septentrional areas there are very high levels of these rays, with rates between 8 and 11 (the highest in the international scoring), the visual and ocular health professionals receive a wake-up call. Through communication media and campaigns, and especially through their example, they have a responsibility to prescribe products for UV protection. They should also do this through permanent and effective blocks, which users who are more interested in the topic, and especially who are more conscious of the current problems that require UV protection. The UV indicators are shown on the product boxes and sometimes there is no big difference between one and another. In the specific case of contact lenses, there is no way to compare any value which is higher than 90% of effectiveness to any other value that lays under that same value because they will never be as safe in terms of protection, especially in this summer time and holidays. Each country shows UV indicators on their principal pages of the national meteorological institutes. However, spreading the UV daily levels to the community and having them take real actions should be a social and civil responsibility of our profession, and performed in a manner where sincerity and good faith should prevail.

Doctor Guillermo Carrillo Orejuela, an outstanding professional in contactology in the region, coordinator of the International Association of Contact Lens Educators for Latin America (IACLE), is our guest in this section in this monthly issue. . ¿What is your best advice for contactology professionals during this season following the holidays where students are returning to school? First of all, we should remember that vision is fundamental during the school stage because the first six years present a great plasticity for stimulation and learning. With this in mind, the question should be what to do regarding visual defects and its negative consequences on learning. The obvious answer is to correct vision in this group of patients who will also obtain the maximum benefit from this visual correction. So, ¿why not consider fittings for this group of patients in a more proactive manner? Parents may be concerned that they might lose them, but we can inform them that we have Frequent Replacement to eliminate this fear. They may be concerned about infections. We have daily disposable lenses that minimize these risks, including bad lens treatment or lens maintenance. We, contactology professionals, “sell” a health service that should consider the proper management of the patient, not as an isolated sale of a product, but the visual benefits that will be provided to this group of patients during their daily life and their capacity to learn through proper vision. ¿Are professionals and parents conscious about the UV protection offered by contact lenses? Among professionals and patients we could say that nowadays there is a higher tendency toward getting this information year after yea, especially during the summer time. This information is about the need of sun protection with sun blocks or creams with a proper Sun Protection Factor (SPF). However, visual protection has not been completely understood yet because in the case of glasses, visual protection is reached by cutting the spectrum in 400 nm (UV400) and hindering the possibility of the UV radiation to go through the eye refractive media. The patients’ perception of the fact of the filtration of the UV radiation in the eye and retina through the contact lenses has been less understood. ¿Does the dual use (glasses for optical correction and contact lenses) constitute a prescription routine and sale in Latin America? Why? I personally believe that it is not the case, at least my knowledge of the region lets me state that the majority of your patients substitute the use of a way of visual correction with another one, and it does not mean that the contact lens users do not wear their glasses. There are no demographic studies that could show concrete reasons, but a great part of the problem is our people’s idiosyncrasies. The opinions expressed in articles reflect solely the viewpoint of their authors, and not necessarily the views and policies of The Vision Care InstituteTM.

¿What are the recommendations the visual health specialist should instill in his patients before their next holiday which should be included in their educational package? • If you present visual reduction, get an appointment before going on vacation in order to have a new medical test and fitting of new contact lenses. • Before going on vacation have new contact lenses made so have one or two spare pairs in order. • In case you have any inconvenience with your contact lenses, do not wait until you come back from your holiday to ask for an appointment and go to the office to have it corrected. ¿How can we regulate the contactology visits to the office so that there will not be an excess of demand when people return from their holidays? It is a difficult question to answer because in order to optimize our professional service we should offer our patients the greatest benefits of a convenient visit. This is one of the most important factors for patients and professionals. The excess of demand can be reduced by expanding our work hours during this time of the year, allowing enough time to be able to offer optimal services to our patients of contact lenses. We should also check our patient records to ensure we have enough contact lenses in stock, as well as contracts, types of lenses, solutions, etc., for timely replacements. Although these options do not change the situation of excess of demand of patients, it can serve to mitigate the problem and allow for the best professional service. Please state three rules of a good contact lens user which a teenage student must follow during the holidays. • Replace your contact lenses according to the recommendations of your visual health professional. • If during your holidays you will not be able to follow the proper care and maintenance of the contact lenses, ask for daily disposable lenses. • Protect your eyes against ultraviolet radiation.
