A

s 2018 approaches, Nanhall Pet Spa will celebrate the golden anniversary of 50 years serving the Triad for boarding, grooming, and training. During that time, we have shared in the lives of your pets. It is very emotional for us to have grown old together. As the years go by, how can one not get attached when we have developed a relationship with them? At Nanhall we specialize in Elder Care, providing aids to assist in the grooming and bathing process. Our grooming tables are padded for additional comfort and a quiet room gives the utmost care and attention for an enjoyable experience. Because pets age faster, sometimes we forget that our pets are getting older and need assistance. A pet that is 12 years old is about 75 in human years. They develop aches and pains as do the rest of us. Knee, hip, and back problems are worsened by being overweight or eating an improper diet. They sleep more, so a soft fleece bed should be provided. Note that many have a favorite side to sleep on causing hair to mat more in that area. As they age the skin becomes fragile and more sensitive to brushing out knots. Temperature changes cause additional stresses such as coughing, running eyes, dehydration, and wind-chill. A lightweight jacket or sweater is comforting for them indoors and out. As your pet becomes older other issues such as pet dementia and incontinence can occur. Keeping them confined to specific rooms and not allowing them to navigate stairs, will keep a disaster from happening. Talk to your vet about your pet’s incontinence, it could be an infection or you may need a pet diaper. Remember changes and new situations can stress them, so try to keep routines and placement of furniture the same. This is especially important for pets that are losing their eyesight. Their immune system becomes weaker to fight off pests and disease, so proper nutrition and vitamins can help them live a more active life. We all need daily exercise and continuing some moderate walks on a soft turf, not hot concrete, is beneficial. Take in account that with long nails, arthritis can develop in the foot making it harder for your pet to walk. Staying healthy also means regular grooming and pedicures. Stimulating the skin and keeping nails clipped will allow them to trot along easily beside you. Have your groomer keep your pet in a maintenance trim—something that is easy for you and your pet. Using a soft cloth or cotton ball daily is wonderful for removing discharge from the eyes. Check for loose teeth and food that may become lodged in their mouth and teeth. Remember your biggest concern should be keeping them happy and comfortable. According to the American Association of Feline Practitioners, “Mature cats: 7–10 years (44–56 years for humans), Senior: 11–14

years (60-72 years for humans), Geriatric: 15+ years (76+ years for humans)”. Having this in mind, consider keeping your cat inside as he ages. Older cats tend to be less active and playful. They may sleep more, gain or lose weight, or have trouble reaching their favorite places. Since cats tend to be more independent, they tend to hide warning signs of aging. Some of the common changes associated with aging include: altered sleep-wake cycle, changes in vision, decreased sense of smell, brittle nails, loose and less-elastic skin, or reduced ability to clean themselves. Let’s not forget about going to the vet and getting wellness exams. The elderly pet requires more attention, so be prepared for agerelated health issues. In the end, the love and companionship you have with your pet is like none other. They stand by you, love you, and many would even die for you. Man’s best friend has proven himself to be there for us, now we need to be there for them.

She decided to specialize in feline medicine two years into private practice as she found feline medicine to be more challenging and rewarding. She currently shares her home with several cats- all of which have her trained to their beck and call.

HYPERTHYROIDISM IN CATS

H

yperthyroidism is a condition where the thyroid gland enlarges and begins producing too much thyroid hormone. The gland can enlarge either by increasing the size of its individual cells or by increasing the number of cells within the gland. Most enlarged glands end up being benign in nature. However, there is a small percentage of these enlarged glands that end up being malignant. Hyperthyroidism is a metabolic disease of older cats, usually cats older than 10. Because the thyroid gland helps control the metabolism of the body, most of the clinical signs seen are related to an increased metabolic rate in the body. The most common presentation of a hyperthyroid cat is one where the cat presents for a weight loss problem despite having a very good appetite. Another common symptom is vomiting, followed by diarrhea. These cats are usually very active and seem to revert to a second “kittenhood” again in their play behavior. Some hyperthyroid cats can also present with excessive lethargy, although this symptom is not as common. Some clients will also notice that their hyperthyroid cats seem to have very elevated heart rates. This is because the higher thyroid levels push the heart to work harder and to beat faster. In severe cases, some hyperthyroid cats will develop secondary congestive heart problems which do not resolve even after the hyperthyroid condition has been addressed. Diagnosis can be done with a simple blood test that checks the level of the thyroid hormone. In some cats, it is also possible to palpate the enlarged thyroid gland. The gland is situated near the middle of the throat and when enlarged will feel like a large pea when palpated. Some doctors call this the “thyroid slip”. However, there are some cats that will be hyperthyroid without concurrent enlargement of the thyroid gland. These cats are believed to have extra thyroid tissue located alongside the trachea making it difficult to palpate as it enters the chest cavity. In these cases, bloodwork result would confirm the hyperthyroidism. Once the diagnosis is made, treatment can be started. Failure to treat will ultimately lead to death as the hyperthyroid state causes massive metabolic derangements which finally lead to organ failure such as myocardial failure.

There are several treatment options and they are listed as follows: Methimazole tablets: This medication helps control the amount of thyroid hormone that is being produced by the thyroid gland. Most cats tolerate this medication well and can stay on this medication long term. This medication can also be compounded into a transdermal ointment which is then applied to the skin on the inside surface of the ear. Pros: Medication is relatively inexpensive and it’s easily given. It can be crushed and mixed with canned food. Cats also tolerate this medication well. Cons: The medication will not stop the thyroid gland from growing and the dosage will have to be adjusted multiple times as long as the cat stays on the

medication. This requires the cat to have its thyroid levels checked by doing blood work every 4 to 5 months. Some cats will not tolerate this medication and will either vomit or develop a severe facial rash. It can also cause bone marrow suppression and or liver disease in certain cats. Thyroidectomy: This is surgery to remove the affected, enlarged thyroid glands. This procedure can be very effective in resolving the hyperthyroid condition if only the two accessible thyroid glands are involved. Pros: Removing the affected glands is curative. Cats do well if no additional complications arise from the surgery. Cons: Cost of the surgery can be expensive. There may also be additional thyroid glandular tissue hiding in the chest cavity that may be missed during surgery. Risk of complications from the surgery can be high. Y/D diet: This is a prescription diet that is formulated specifically for hyperthyroid cats. It is very restricted in iodine which helps lower the thyroid hormone levels of cats that eat this diet. Pros: This is an easy fix without having to resort to medications or surgery. Cons: This is an expensive prescription diet. Some cats may not like it and refuse to eat it. In multicat households, feeding the Y/D diet means having to supplement the diet of the normal, healthy cats with regular cat food that contain normal levels of iodine while trying to keep the hyperthyroid cat away from the regular cat food. This translates into a hassle for most owners. Unfortunately, the diet is also just a temporary fix and the cat will have to undergo further treatments in the future once the thyroid gland has outgrown the suppressing effects of the diet. Radioiodide treatment: This is the gold standard for hyperthyroidism and is how most hyperthyroid human patients are treated. The treatment for cats consists of a single injection of radioactive iodine subcutaneously. The radioactive iodine is then taken up by the thyroid gland to be used to produce more thyroid hormone. The radioactive iodine then kills all the abnormal, overactive thyroid cells while sparing the normal, quiescent cells. Pros: One injection is curative for the life of most cats. There is a small percentage of cats that may need a second injection but that is rare. If the thyroid gland is cancerous, this injection is also curative since it’s radioactive. There are no side effects from the injection and it’s very well tolerated by cats. Cons: The most prohibitive reason against this treatment option is the cost. Treatment prices range from $1250 - $1600. High cost of treatment include the cost of purchasing the radioactive iodine and housing the treated cats in a special facility as the cats are now radioactive. Urine and feces remain radioactive for several weeks and have to be disposed of separately in accordance with state health regulations. Most cats are hospitalized for about 3-4 days post-injection where they have to be monitored for any adverse reactions to the injection. This may not be the best treatment option for a feline patient that is showing early signs of kidney disease. If these cats undergo the radioiodide treatment, the drastic lowering of the thyroid hormone levels may cause overt kidney failure.

Cover Story

It Takes a Village By: Laura Michel, Program Coordinator Burlington Animal Services

Y

ou have likely heard the proverb, “It takes a village to raise a child”. It means a child has the best chance at being a healthy, productive adult when the community takes an active role in rearing that child. When applied to the animal welfare world, “It takes a village” holds true as well. Without the support of community partners, rescue organizations, volunteers and fosters, we could not save as many lives and provide positive outcomes for as many animals. We simply could not do this work without our village.

COMMUNITY PARTNERS & RESCUE ORGANIZATIONS A few years ago, Burlington Animal Services (BAS) started working more closely with local community partners and rescue organizations in North Carolina and beyond. These relationships are vital and play a critical role in assisting us by transferring animals from our facility, placing them into their foster or adoption programs and using their networks and resources to find adopters for final placement. In 2013, only 68 animals were transferred from BAS to other agencies. Since January 2014, well over 2,000 animals have been transferred to numerous rescue organizations locally, regionally and in neighboring states. By working with other non-profit animal groups, we have made great strides in increasing our live release (adoption, return-to-owner and rescue/transfer) and reducing euthanasia. Working with and supporting organizations that are dedicated to helping pet owners keep their pets has also proved beneficial. These groups often act as a safety net for pet owners and are a valuable resource in our community. Animal Resource Friends (formerly Friends of Mebane Animals) assists pet owners who demonstrate financial need with pet food, building fences and shelter to untether their dogs, and some medical needs. Amber’s Animal Advocacy networks shelter pets to help find homes or rescues. Amber’s also works to raise money to assist with rescue transport costs and has a heartworm treatment fund to assist adopters of heartworm positive pets. Animal Kind’s $20 Fix program assists low-income households with getting their pets spayed or neutered. There are many wonderful, hard-working, local organizations that are devoted to helping individuals care for and keep their pets at home versus surrendering them to a shelter. They also work 12

Duke tirelessly to help find loving homes for homeless pets. These nonprofit, 501c(3) groups depend solely on donations and financial support to carry out their mission and work. Consider making a donation and becoming a supporter.

FOSTERS & VOLUNTEERS Two other very important and valuable components of our village are our foster and volunteer programs. Not everyone can own and care for one, two or multiple pets even though the desire is in their hearts. What’s the next best thing? Fostering, of course! Short-term foster homes are needed for pets all year long and are vital to our ability to ensure positive outcomes for pets admitted at our shelter. Foster volunteers are asked to care for an adoptable pet in their home in order to free up kennel capacity at Animal Services. Pet food and any necessary medical care are provided for the foster pet at no cost to the foster family. Animals most in need of foster care include those with medical needs, such as dogs with heartworm disease or demodectic mange; animals with behavioral needs, such as those who are shy, fearful, or stressed by the shelter environment; and animals who are too young to be adopted, such as kittens and puppies as well as moms with nursing young. The foster program allows these pets to be placed into a foster home where they receive medical treatment,


work on socialization skills, or simply wait until they are old enough to be adopted. Other pets without any medical or behavioral condition may also be placed into a foster home while waiting to find a home to free up limited kennel space at the shelter. Foster homes work to find the pet an adoptive home through the BAS adoption program. If you can’t adopt or foster, but still want to help, consider volunteering your time. Most shelters have staff that clean, feed and care for the animals, but there is much they don’t have the time or resources to do. At BAS, volunteers are an integral extension of our organization and we depend on these unsung heroes in so many ways. Most of our volunteers simply enjoy spending time with our adoptable pets, taking them out of their kennels for exercise, playtime or enrichment activities, such as obedience training or learning to use our agility equipment. This provides much needed mental stimulation and a physical outlet for the stress many pets experience while in the shelter environment. Plus, there is nothing cooler than a shelter dog that knows basic commands! Some volunteers will occasionally take a dog out for a hike at a local park or for a day-long field trip, showing them off around town and treating them to a burger or chicken nuggets from a drive-thru restaurant! Cats can enjoy quiet time with a volunteer in our playroom or go for a stroll outside in a pet stroller, giving them a nice change of scenery and fresh air. Volunteers also help us with numerous special events we attend in the community throughout the year; at our rabies/microchip clinics; they transport pets to rescues or other places as needed; or, they take pets to visit nursing home residents. Volunteers take photographs of pets for our website and social media pages that are usually much better than our intake “mug” shots! Our wonderful volunteers even help with the endless dirty laundry, and weekend/ holiday cleaning and feeding duties, providing much needed and appreciated assistance to our animal care staff.

Dr. John Wehe is a Greensboro native. He is owner and veterinarian of Downtown Greensboro Animal Hospital, an animal hospital in downtown that opened in March 2013. Dr. Wehe attended North Carolina State University College of Veterinary Medicine and received his Doctorate of Veterinary Medicine in 2007. His professional interests include small animal medicine, surgery and ultrasonography. He values serving his patients and educating his clients, as well as getting to spend time downtown. Dr. Wehe shares his life with his amazing wife and four children.

PET LOSS... A Time for Building

f you have ever had to euthanize a pet, you know it can be a painful process. Whether it is an older pet that lived a full life or a young animal whose life was too short, the loss hurts. Euthanasia is intended to heal animal suffering and it is one way in which veterinarians give back to the pets that have given us so much. It may not seem natural, but it is one way in which we serve our clients and help to strengthen the human-animal bond. I often hear the statement, from pet owners prior to a euthanasia, that this must be “the hardest part of my job”. It can be difficult and, at times, very upsetting. There are times it takes my staff and me a while to regroup before we can move on with our workday. One week, this past September, was a particularly tough week for our hospital. We experienced six scheduled euthanasias. Three of them occurred in one day. This is something I hope to never repeat. That week became increasingly difficult for myself and my staff. We shared in the loss of pets we had grown close to over several years, as well as pets we had tried desperately to treat through chronic illness but, ultimately, lost the fight. There were also new clients/pets to our hospital. Throughout the week, I became weary and frustrated with the number of euthanasia appointments. This was something I had not experienced before and lead me to wonder if this was compassion fatigue, or dissatisfaction/frustration with my own limitations as a veterinarian, or possibly this was just a natural human reaction to a difficult time.

I don’t know what it was completely, but ultimately time and reflection allowed me to remember that, simply, death can be hard. Preparing for this article has lead me back to two resources. First, as a new graduate from veterinary school, I made a pledge to the profession called the Hippocratic Oath. Part of the oath reads “… I solemnly swear to use my scientific knowledge and skills for the benefit of society through the protection of animal health and the welfare, the prevention and relief of animal suffering…” The second is The Argus Institute, a grief counseling program at Colorado State University Veterinary School. Their mission reads “honoring the human-animal bond by providing compassionate support and advocacy for people who care for animals.” By the Hippocratic Oath, euthanasia is one duty as a veterinarian we could perform to help to relieve animal suffering. As noted by the Argus Institute, euthanasia is a form of compassion through which we strengthen the powerful human-animal bond. It is out of caring for our clients and their pets that we take on this heavy responsibility. It is through honest discussion about the quality of life of a pet, assessed by our medical expertise, that we help guide owners through the difficult and, often, painful decision to end the suffering of their pet through euthanasia. It is with great care and respect that we perform this service for our clients. Finally, it is with compassion and sorrow for the loss that we all experience that we engage each other. We acknowledge the pain and remember the unconditional love that we received from our pets, and in doing so build relationships. Relationships are one of life’s greatest gifts. Something we were created for. I have found that, being a small animal veterinarian, I am blessed with many wonderful relationships with my clients and their pets. My hope is that these are long term relationships. It is easy to build relationships through sharing the joy that comes with new pet ownership or the pride in seeing someone’s pet’s health flourish. Other times, that joy may be replaced by stress and sadness in caring for a pet with an illness, or caring for a client through the pain in saying goodbye to a beloved pet. Relationship building at these times can be much more challenging, but equally or even more rewarding, for both the veterinary team and the pet owner, as well as, strengthening the human-animal bond. Euthanasia is one of the hardest parts of my job. Loss of life is painful and it affects everyone involved. However, I have found that, in these tender moments, sometimes we do the most to create lasting relationships and truly foster the human-animal bond.

The name Zara means “seed” and our gorgeous Zara is ready to grow and bloom in a new, loving home. Three year old Zara (Boxer-Lab mix) came to our shelter with her four puppies, but Zara did not qualify for adoptions due to testing positive for heartworm disease (she was also emaciated and very sick). Although all our foster homes were full, we knew she truly deserved a second chance and by no surprise, she Project Pearl easily blossomed into loving house dog. Lucy was born 7/19/08 and weighs 6.25 lbs. Lucy came from a bad situation where she was being kept in a garage with no A/C. An update from her recent foster: Lucy is a very timid dog when you first meet her. When given the chance, however, she is very sweet and loves spending time with her caretakers. She is notably excited to spend time with Chihuahua Rescue children and bonded first with them in her last foster environment. She was very fearful of men at first, but grew to love them after spending time with them. Lucy was also scared of staying in her crate, but is now crate-trained and will even go in her crate herself if given a treat. After getting used to her surroundings, Lucy is a very playful and loving dog. She is great with other dogs and can even do a few tricks like standing on her hind legs. She loves to cuddle and is making great progress on her house training. So far, Lucy is not happy to walk on a leash – but could improve with proper training. hen trying to improve personal heart health, diet and exercise are normally near the top of the list. But owning a dog may also help you reach your health goals. That’s right! A recent study has shown links between people with good heart health and owning a dog. Owning a dog tends to increase the owner’s level of physical activity, which is wonderful for both doggy and human hearts. Just by taking your dog for a daily walk, you can reach the recommended level of physical activity, which is 30 minutes a day most days a week or 150 minutes a week for adults. Even short walks help throughout the day. A short walk in the morning, and a longer walk after dinner with your best friend at your side can get you up to the goal of 30 minutes of physical activity each day. Along with the physical benefits, comes love and emotional support. Playing with your pets can help to manage stress of daily life and petting your dogs or cats has been shown to lower blood pressure. Pets, especially dogs, also provide a social support that encourages their owners to stick with new healthy habits, such as walking. Dog owners are acutely aware that even if you accidentally mention the word “walk” in a conversation, your dog or motivational “speaker” will get you up off the couch and out the door for a walk through the neighborhood. Pets can also be the motivation you need to recover when health issues arise. Another study showed that people at risk for stroke were
