Systembeschreibung Die Modulare Intrakondyläre Totale Kniegelenkprothese Endo-Modell®-M ist eine Ergänzung zu dem herkömmlichen LINK® Endo-Modell® Rotations- und Scharnierkniegelenk Prothesensystem. Unter Einhaltung des low-friction-Prinzips ist der physiologische Bewegungsablauf dieser Prothese durch einen, im physiologischen Bereich gelagerten,Drehpunkt optimal gestaltet. Die Rotations-Kniegelenkprothese Endo-Modell®M erlaubt eine Beugung des Gelenkes bis 165°. Die Gelenkkinematik beinhaltet darüber hinaus eine physiologische Rotation, die durch die besondere Formgebung der tibialen Lauffläche eine elastische Kraftübertragung ermöglicht. Die Scharnierknieprothese erlaubt ausschließlich eine Beugung des Gelenks bis 165°ohne Rotation. Bei jedem Schritt, besonders bei Stürzen, treten Drehmomente auf, die sich auf die Prothesenverankerung fortsetzen und die Lebensdauer der Verankerung nachteilig beeinflussen. Die konstruktiv eingebrachte elastische Kraftübertragung sorgt für eine Schonung in den Grenzschichtbereichen Prothese / Knochenzement und Knochenzement / Knochen. Die erforderliche Resektion beim Einsatz der Kniegelenkprothese Endo-Modell®-M ist durch die günstige Dimensionierung denkbar gering und beträgt in der Gelenkebene Tibia-Femur nur 14 mm. Der intrakondylär zu versenkende Anteil der mittleren Grösse ist lediglich 30 mm breit. In der Regel ist damit die Resektion geringer als beim Kniegelenk-Oberflächenersatz. Das ist im Hinblick auf die Rückzugsmöglichkeit ein wesentlicher positiver Faktor. Durch die vorteilhafte Dimensionierung und Formgestaltung der Kniegelenkprothese ist eine günstige Übersicht im Operationsfeld gegeben. Die Montage der femoralen und tibialen Komponenten erfolgt einfach durch Zusammenstecken, wobei das UHMWPETibiaplateau mit einem Spezialinstrument eingesetzt wird. Beide Komponenten werden durch das Plateau luxationshemmend gekoppelt, ohne die Bewegungs- und Rotationsabläufe zu vermindern. Die Scharnierknieprothese wird durch den Achsmechanismus gekoppelt. Erleichtert wird die Implantation durch eine geringe Anzahl einfach zu handhabender Instrumente. Die Beuge- und Rotationsbewegung der Rotations-Kniegelenkprothese erfolgt in einem Kreuzgelenk. Die Überstreckung beträgt 3°. Die Kompromissachse liegt im Bereich des physiologischen Drehpunktes. Die Beugung ist bis zu 165° möglich. Bei endoprothetischem Ersatz des Kniegelenkes wird häufig ein Vorschub der Patella bzw. des patellaren Gleitlagers beobachtet. Durch den Versatz der Femurkomponente nach dorsal, gegenüber der Tibiaachse wird auch im FemoroPatellar-Gelenk ein physiologischer Bewegungsablauf erhalten. Das schützt vor Progredienz einer retropatellaren Arthrose. Die Rotation der Prothese endet in Streckstellung durch Formschluss und gewährleistet einen sicheren Stand. Mit zunehmender Beugung nimmt auch die Rotationsmöglichkeit kontinuierlich zu. Diese Rotation wird zunächst durch den Kapsel-Bandapparat begrenzt. Bedingt durch die Form der sich berührenden Laufflächen, dämpft die auf dem Gelenk lastende Körpermasse weitergehende Rotation elastisch. Die femorale Komponente der Totalen Kniegelenkprothese EndoModell®-M hat eine physiologische Valgusstellung von 6°. Beide Prothesenkomponenten stützen sich großflächig auf den jeweiligen Kniegelenkflächen ab, so dass die Druckfestigkeit der Spongiosa gegenüber Femur und Tibia nicht überschritten wird. Die Kufenform der femoralen Komponente ist der Anatomie angepasst. Ventral ergibt sich durch eine Mulde ein stufenloser Übergang in das knöcherne patellare Gleitlager. Die modularen Prothesenschäfte sind sowohl in zementierbarer Version ohne Strukturierung als auch mit Längsstrukturierung für die zementlose Implantation lieferbar. Um eine zentrale Position der zementierbaren Schäfte in der Markhöhle zu erreichen, enden sie in sternförmigen UHMWPE-Aufsätzen. Damit wird ein direkter Knocheninnenwandkontakt der Metallschäfte verhindert. Die Schäfte sind in den Längen 50 bis 280 mm verfügbar. Für Revisionen von Oberflächen-Kniegelenkendoprothesen sind zur Wiederherstellung der Kondylen, als auch für Tumorfälle (Resektionen) spezielle, femorale und tibiale Segmente erhältlich. Hierbei muss unbedingt beachtet werden, dass diese Segmente nur in Verbindung mit entsprechend längeren Schäften verwendet werden. 02 |

With every step, and especially when falling, torsional stresses are transmitted to the prosthetic anchorage which adversely affects the longevity of the cement interface. The cushioned transmission of forces, made possible by the design features, provides a dampened impact upon the boundary layer of the cement interface. The resection required during implantation of the Endo-Model®-M Knee Joint Prosthesis amounts to only 14 mm in the tibia-femur joint plane. With the medium sized intracondylar component only 30 mm wide, there is usually ample bone mass left in the event a revision is necessary. Normally, the resection is smaller compared with a Total Knee implant. Design and dimensioning of the Rotation Knee Joint Prosthesis significantly simplify the surgical procedure. Mounting of the femoral and tibial components is a simple task, requiring only one special introducer instrument for the UHMWPE Plateau. Both components are linked by the special anti-luxation device of the plateau without reducing the motional and rotational sequences. Implantation is facilitated by a small number of easily manipulated instruments. The Hinge Knee Prosthesis is linked by an axis mechanism. Flexion and rotation of the knee prosthesis occurs in a cross joint. Hyperextension amounts to 3°. The compromise axis lies in the region of the physiological pivot point. Flexion of up to 165° is possible. Often during endoprosthetic replacement of the knee joint, an advancement of the patella or of the patella bearing surface is observed. By displacing the femoral component dorsally relative to the tibial axis, the natural range of motion is also preserved in the patellofemoral joint. This protects against progression of retropatellar arthrosis. Rotation of the prosthesis terminates in the extended position by form closure and assures a secure posture. The rotational option increases continuously with flexion. This rotation is limited primarily by the capsule-ligament apparatus. The body weight, bearing on the joint, elastically dampens further rotation. The femoral component of the EndoModel®-M Total Knee Joint Prosthesis has a normal valgus position of 6°.

Montageanleitung Assembly Instruction

Based on the same low friction principle, the rotational movement of this prosthesis passes smoothly through a pivot point in the physiological region. The Endo-Model®-M Rotation Knee Joint Prosthesis permits flexion of the joint up to 165°. Due to the special shape of the tibial contact surfaces and physiological rotation, the kinematics of this prosthesis allow for a cushioned transmission of forces. The Hinge Knee Prosthesis permit only flexion of the joint up to 165°, without rotation.

The stems are supplied in lengths of 50 up to 280 mm. Special femoral segments for revision surgery of resurfacing knee implants (reconstruction of condyles) and for tumor cases are also available. It is absolutely necessary to use these segments only in combination with longer stems.

The modular prosthesis stems are available in cemented and cementless versions (with smooth surface or longitudinal ribs respectively). To achieve a central position within the medullary canal, the tips of the cemented stems are fitted with star shaped UHMWPE caps. Direct contact of the metal stem with the inner wall of the bone is thereby prevented.

Both prosthesis components are broadly supported on the respective knee joint surfaces so that the compres-sion strength of the cancellous bone in the femur and tibia, is not exceeded. The flanges of the femoral component are anatomically shaped. Its ventral depression provides a smooth transition from the implant to the trochlear groove.

With the knee in flexion both components are assembled.

The Modular Stems are joined to the Femoral and Tibial Components by a cone assembly. To secure the fixation two opposite tongues at the stems are inserted into the medial and lateral grooves at both the Tibial and Femoral Components.

By tightening the Locking Screw (2) located in the Taper (3) of the Tibial respectively Femoral Component its pointed tip presses the stem (1) firmly onto the taper. A Counter Screw (4) secures the Stem Locking Screw against loosening. The screw fixation is performed medially.

Indications for the various types of knee joint prosthesis systems depend on the state of passive and active stabilizers (ligaments and capsule, muscles and tendons), the extent of the prevailing joint deformity, the basic illness, the type of patient and the experience of the surgeon. If the application of an unicondylar or total resurfacing knee prosthesis is not justified due to the present deformities and joint instability the advantages of the stabilizing Endo-Model® Total Knee Prostheses can be used.
