Young Person Housing Support Worker

ACCRUED HOLIDAY PAY: CONTRACT ______________________________________________________________________________________

MAIN PURPOSE To provide day-to-day direct support to young people (aged 16-25) living in Manchester Settlements Accommodation in order that they may gain the life skills needed to maintain their own tenancy, live independently and secure a positive and sustainable move on option. To support the management and delivery of an innovative supported housing project that meets the needs and requirements of the young people, Manchester Settlement and Manchester city councils children’s services and housing related support teams. MAIN TASKS 1. Produce and update individual support plans that will support young people to meet their agreed outcomes and aspirations.

2. To develop and maintain effective relationships with young people that enables them to meet their agreed outcomes and aspirations. 3. Help young people to make decisions about their own lives by working within a person centred approach. 4. Assist young people to and to develop the skills to enable positive and address barriers to independent living allowing a smooth move on from our service. 5. Create opportunities and enable young people to develop and widen their social, educational and work experience and their leisure networks. 6. To work with other agencies both statutory and voluntary to support young people to meet their agreed outcomes, aspirations and live independently.


7. Enable young people to be signposted to receive appropriate advice when necessary. (e.g. health care, benefits, housing etc). 8. To support each young person to manage their own behaviour to ensure a safe environment for tenants and staff 9. To effectively liaise with each young person in our service on matters relating to the conduct of their tenancy/licence agreement 10. To effectively monitor the Health and Safety of properties in line with Manchester settlements policy and procedures. 11. To be proactive in promoting young people participation and involvement. 12. Follow agreed risk assessment and contribute to risk management planning highlighting any changes in risk to more senior staff and other professionals. 13. To undertake lone working in line with Manchester Lone working Policies and procedures 14. To ensure that all funders targets and milestones are met 15. To support the completion of monitoring and evaluation in a timely and effective manner and in accordance to funders specifications. 16. To make contact and share relevant information with colleagues and line managers 17. Assist Chief Executive with Funding Applications. 18. To support volunteers and work placements and assist them in developing their skills. 19. To provide assistance with the delivery of other projects when short staffed

OTHER TASKS 1.

To work within all Settlement polices at all times and complete all appropriate proformas in line with current policies.

To ensure that all work reflects the projects commitment to challenging discrimination, in line with the Settlements equal opportunities policy.

3.

To work unsociable hours which may include evenings, weekends, and bank holidays to ensure that adequate staff cover is maintained at all times.

4.

To attend Settlement team meetings, management committee and other project meetings as required through consultation with your Line manager.

5.

To undertake any other duties in connection with the work of the project as requested from time to time by Manchester Settlements Management team.
