He resided in La Verne, CA at the time of his passing. Patrick lived a healthy, beautiful life who loved his family and friends. Patrick is survived by his wife Sharon, his three children, Patrick Jr., Michael, and Shauna, his two sisters Kathleen and Bernadette, ten Grandchildren and seven greatgrandchildren. The old order has passed away: welcome them into paradise, where there will be no sorrow, no weeping ____________________________________________________________ or pain, but fullness of peace and joy with your Son and the Holy Spirit forever and ever. Amen.
Chapter President’s Message (continued)
I want to ‘thank’ Sylvia and Deacon Sergio for making our appointment arrangement possible with the Bishop and for their hospitality. During the visit, Rev. Fr. Thomas Asia, Pastor of St. Martha, was officially introduced as the new incoming Chapter Chaplain. Despite the Bishop’s busy schedule, on 1/11/2020, Saturday we will have a retreat along with a Holy mass at 5:00pm. This is a wonderful chance to meet His Excellency Bishop David O’ Connell at St. Martha Church, McNamara Hall for welcoming all our new brother knights with its families who joined our order from July 2019 through January 2020 for the Consecration to the Holy Family. This is more reason why we must invite all persons that are 18 years of ages that are in communion with the Holy See. We are all fishers of Men. We want to share that love…”God’s Loves Embraces Us All.” Every Member is a recruiter! Reminder to mail or send your Form 365 - Service Program Personnel which is due on 8/1/2019. Form is available for download at http://www.kofc.org/un/en/forms/council/revised_forms/cn_365_en_print_rev.pdf
The Semi-Annual Audit (for period January 1, 2019 through June 30, 2019) Form 1295 is due on 8/15/2019 and can be accessed through www.kofc.org/un/en/forms/council/audit1_1295_p.pdf. Why do we need this audit? It is a requirement by the Supreme because the Financial Secretary is automatically bonded at no-charge to the council for the amount of $5,000.00. Additional bonding is available at a cost of $7.50/thousand. The same applies to the treasurer upon receiving Form 185, Report of Officers Chosen for the Term 2019 - 2020, due on 6/30/2019. Two (2) previous audits must be on file with the Supreme in order for bonding to be in effect. Failure to submit your semiannual audit can cause a council to forfeit the bond coverage of the FS and the Treasurer. Other requirement is our responsibility under the law to file your annual income tax with the IRS and with the California Franchise Tax Board for your Tax- exempt status. Failure to file three (3) consecutive years will result in a loss of council tax exempt status under IRS Sec. 501(c)(8). Depending on your council accounting period, i. e. Columbian Year /Fiscal Year or Calendar Year, the dateline to file your income tax is by the 15th of the 5th month after the end of your annual accounting period. The two (2) dates to file are either on or by 11/15/19 for accounting period ending 6/30/19; on or by 5/15/2020 for accounting period ending in 12/31/2019. Who are persons involved in the Semi-Annual Audit? The Grand Knight, FS, Treasurer and the Board of Trustees, because the GK and the three (3) trustees need to sign-off this form.
