A. Definitions
Proof of medical exemption:
means an original, signed written confirmation by an authorizing health professional which verifies there is a medical contraindication to vaccination that prevents the person from being vaccinated or verifies the person is a participant in a COVID-19 vaccine clinical trial. The written confirmation must meet all of the requirements of CMOH 45-2021 (as amended, replaced, or superseded).
Proof of negative test:
refers to documentation provided by a privately paid organization that shows a negative result for COVID-19 PCR or rapid test no longer than 72-hours at time of presentation. This cannot be from Alberta Health Services or Precision Laboratories.
Proof of Vaccination:
Documentation showing proof of Vaccination. The documentation must meet all of the requirements set out in the CMOH order 45-2021 (as amended, replaced or superseded).

Vaccination:
As defined by CMOH order 45-2021 (as amended, replaced or superseded) means a person who is eligible for vaccination who has received a World Health Organization approved COVID-19 vaccine. Fully vaccinated:
means a person who has received two doses of a World Health Organization approved COVID-19 vaccine in a two dose vaccine series (has had 14 days or more since receiving the second dose vaccine) or one dose in a one dose vaccine series (has had 14 days or more since receiving the one dose of vaccine).
Partially vaccinated:
means a person who is eligible for vaccination who has received one dose of a World Health Organization approved COVID-19 vaccine in a two dose vaccine series (has had 14 days or more since receiving the first dose vaccine).
B. Procedure 1. Employee: a. In accordance with the Proof of Vaccine Policy Restrictions Program policy, employees will provide one of the following: i. Proof of vaccination through the online form, ii. Proof of medical exemption through the online form, or iii. Proof of negative test. b. A designated Health Services Representative will notify senior leaders and direct reports of staff cleared to be present on campus. c. Human Resources will ensure that reminders are communicated to staff who have not submitted Proof of vaccination, Proof of medical exemption, or confirmation that they will provide Proof of negative test. d. As of October 4, 2021, to be on campus employees must provide Proof of vaccination (Partially Vaccinated), Proof of medical Proof of Vaccination/Exemption/Negative Test Procedure OCTOBER 21, 2021 (updated Nov 19, 2021)

exemption, or confirmation that they will provide Proof of negative test. e. Employees who do not provide Proof of vaccination (Partially Vaccinated), Proof of medical exemption, or Proof of negative test as of October 4, 2021, will not be permitted on campus and may be put on unpaid leave. f. Employees who do not provide Proof of vaccination (Fully Vaccinated), Proof of medical exemption, or Proof of negative test as of November 1, 2021, will not be permitted on campus and may be put on unpaid leave. g. Contract managers, or delegate, will notify contractors of this policy and implications for access to Lakeland College Campuses. 2. Students: a. In accordance with the Proof of Vaccine Policy Restrictions Program, as of October 4, 2021, students must provide one of the following: i. Proof of vaccination (Partially vaccinated) through the online form, ii. Proof of medical exemption through the online form or, iii. Proof of negative test. b. In accordance with the Proof of Vaccine Policy Restrictions Program, as of November 1, 2021, students must provide one of the following: i. Proof of vaccination (Fully vaccinated) through the online form, ii. Proof of medical exemption through the online form, or iii. Proof of negative test. c. A designated Health Services Representative will notify the assigned school and/or departmental representative of students cleared to be on campus. d. Schools and/or departments will follow-up with students who have not submitted Proof of vaccination, Proof of medical exemption or Proof of negative test. These students are not permitted on campus until they comply with the Policy/Procedure. 3. Residence Tenants: a. In accordance with the Proof of Vaccine Policy Restrictions Program, as of October 4, 2021, residents must provide one of the following: i. Proof of first vaccination through the online form, ii. Proof of medical exemption through the online form, or iii. Proof of negative test. Proof of Vaccination/Exemption/Negative Test Procedure OCTOBER 21, 2021 (updated Nov 19, 2021)

b. In accordance with the Proof of Vaccine Policy Restrictions Program, as of November 1, 2021, students must provide one of the following: i. Proof of vaccination through the online form, ii. Proof of medical exemption through the online form, or iii. Proof of negative test. c. It is the residence tenants' responsibility to ensure their visitors are in alignment with this policy and will be held responsible. d. Failure to provide Proof of vaccination (Partially vaccinated), Proof of medical exemption, or Proof of negative test by October 4, 2021, may result in eviction. e. Failure to provide Proof of vaccination (Fully vaccinated), Proof of medical exemption, or Proof of negative test by November 1, 2021, may result in eviction. 4. Community Members: a. In accordance with the Proof of Vaccine Policy Restrictions Program, as of October 4, 2021, Community Members, 12 years and older, must provide one of the following: i. Show Proof of vaccination (Partially vaccinated) to the designated representative of the school or department that they are visiting, ii. Show Proof of medical exemption to the designated representative of the school or department that they are visiting, or iii. Show Proof of negative test to the designated school/department. b. In accordance with the Proof of Vaccine Policy Restrictions Program, as of November 1, 2021, Community Members, 12 years and older, must provide one of the following: i. Show Proof of vaccination (Fully vaccinated) to the designated representative of the school or department that they are visiting, ii. Show Proof of medical exemption to the designated representative of the school or department that they are visiting, or iii. Show Proof of negative test to the designated school or department. c. Students in Kindergarten to Grade 12 who are on campus to participate in a course or program as part of their Alberta school curriculum are not required to show proof of vaccination. Specific

guidelines are in place for these instances. Please contact the COVID-19 Committee for instructions. d. The designated representative of the school or department must notify renters/users of Lakeland College facilities of their responsibilities to comply with all CMOH orders. e. Failure to provide Proof of vaccination, Proof of exemption, or Proof of negative test will result in cancelation of the visit/service.
