A. Policy Statement Lakeland College is committed to protecting the health and safety of all students, staff, and community members who access our facilities. Lakeland College and all students, staff, and community members have obligations to ensure the health and safety of everyone who participates in on-campus activities at Lakeland College. Our current general understanding in Alberta is that immunization against COVID-19 is an effective means to prevent the spread of COVID-19. To that end, Lakeland College adopts this Policy to protect the health and well-being of our students, staff, community members, and communities during the COVID-19 pandemic. B. Purpose As of September 20, 2021, Lakeland College will move to implement the COVID-19 Proof of Vaccine Policy Restrictions Program according to the Government of Alberta’s COVID-19 Public Health regulations in alignment with the post-secondary education sector. This policy responds to Alberta’s CMOH public health orders for post-secondary institutions who are seeking exemption, in order to continue operations without physical distancing requirements. Along with following the public health orders, this policy is to help ensure the safety and well-being of the College through testing and vaccination, existing infection prevention and control. C. Scope Statement This policy applies to all Lakeland College community members who attend In-person activities at the Lloydminster and Vermilion Lakeland College campuses and locations leased by the College. Students participating in placement, co-op, or other work-integrated learning programs and who do not attend Lakeland College campuses are subject to the health and safety policies of the host employer. If, however, these students are required to attend In-person activities at the College, this policy applies.

D. Definitions College Community Member: includes all employees, students, students living in Residence, Board of Governors members, contractors and visitors who are attending inperson activities. For clarity, College Community Member, for the purposes of this Policy, does not include individuals who do not attend In-person activities. In-Person activities:
in-person activities means on-campus activity, academic instruction in any setting, attendance or participation in events or activities and working in either public or business areas of the College. This definition is not intended to include brief or transient access to college campuses, such as deliveries or provision of a service that requires only outdoor access or minimal indoor access otherwise fully compliant with Alberta Health Services Public health orders.
Proof of medical exemption:
means an original, signed written confirmation by an authorizing health professional which verifies there is a medical contraindication to vaccination that prevents the person from being vaccinated or verifies the person is a participant in a COVID-19 vaccine clinical trial. The written confirmation must meet all of the requirements of CMOH 452021 (as amended, replaced, or superseded).
Proof of negative test:
refers to documentation provided by a privately paid organization that shows a negative result for a Health Canada approved COVID-19 rapid screening test or a lab-based PCR test approved by Health Canada or the lab accreditation body of the jurisdiction in which the test is performed. The test must be no longer than 72-hours at the time of presentation. This cannot be sourced from the Alberta Health Services public COVID19 testing system.
Proof of Vaccination:
documentation showing proof of Vaccination. The documentation must meet all of the requirements set out in the CMOH order 45-2021 (as amended, replaced or superseded).
Vaccination:
as defined by CMOH order 45-2021 (as amended, replaced or superseded) means a person who is eligible for vaccination who has received a World Health Organization approved COVID-19 vaccine. Fully vaccinated:
means a person who has received two doses of a World Health Organization approved COVID19 vaccine in a two dose vaccine series (has had 14 days or more since receiving the second dose vaccine) or one dose in a one dose vaccine series (has had 14 days or more since receiving the one dose of vaccine).
Partially vaccinated:
means a person who is eligible for vaccination who has received one dose of a World Health

Organization approved COVID-19 vaccine in a two dose vaccine series (has had 14 days or more since receiving the first dose vaccine).
E. Guidelines 1. This policy shall be reviewed regularly, at least every four (4) months, to ensure alignment with public health measures and regulations, and to confirm it adequately covers the health and safety risks it addresses. 2. Lakeland College will require all students, staff, and community members to be Partially vaccinated, provide Proof of Medical Exemption, or continue to provide Proof of negative test against COVID-19 in order to attend any Lakeland College campuses or activities after October 4, 2021. 3. Lakeland College will require all students, staff, and community members to be Fully vaccinated, submit Proof of medical exemption, or continue to provide Proof of negative test against COVID19 in order to attend any Lakeland College campuses or activities as of November 1, 2021. 4. Lakeland College will require all students, staff, and community members to provide Government issued Proof of vaccination, Proof of medical exemption or Proof of negative test in accordance with Alberta Government Public Health Orders. 5. Proof of vaccination, Proof of medical exemption or Proof of negative test are being collected to protect the health and safety of Lakeland College students, staff, and community members. Proof of vaccination will be securely and permanently destroyed once verified. 6. Access to any personal information collected under this policy will be limited to staff that require this information for the performance of their job duties. All staff are required to have up to date FOIP Training. 7. Proof of vaccination, Proof of medical exemption or Proof of negative test is collected in response to the COVID-19 pandemic and under this Policy shall be used only as necessary to determine fit for work/fit to learn status of students, staff, and community members required or requesting to attend Lakeland College's campuses or activities, to manage and administer employment and other working relationships, to address accommodation requests, and to comply with all applicable laws. 8. The personal Information collected under this policy is collected under the authority of Section 33(c) of the Freedom of Information and Protection of Privacy Act (Alberta) and shall be used, accessed, and disclosed in accordance with the legislation and relevant Lakeland College records management policies. 9. Students failing to comply with this Policy and Procedure may be disciplined in accordance with the Student Code of Conduct. 10. Employees failing to comply with this Policy and the Procedure may be placed on an unpaid leave of absence or otherwise removed from service.

F. Exceptions Lakeland values that vaccination is a personal choice or belief for all individuals. However, if you don't have Proof of vaccination or Proof of a medical exemption, then you must provide Proof of a negative COVID-19 test (within 72 hours) of coming to campus. First responder responding to an emergency situation.
