UNDERGO HEART SURGERY WHILE PREGNANT
Sonia Jamin of Huntington Beach had a stroke and discovered she had a hole in her heart that required an operation.
t was December 15, 2022. I was visiting my 92-year-old grandmother at a nursing home. My family was taking shifts sitting with her because she had broken her hip. I had just found out that I was pregnant two or three days before. I had a 10-year-old daughter at the time, so this was a surprise baby. I went to get a nurse for my nonna, and as I walked out into the hallway, I lost control of the left side of my body. I held on to a rail with my right hand, and I just kind of twirled down to the floor. The sta could immediately see that I was showing signs of a stroke, and they immediately called the paramedics, who were able to get me in the ambulance and over to Hoag in Newport Beach. They rushed me into the ER, and the neurologist there, Dr. Jason Muir, evaluated me, got me into interventional radiology (IR) right away. I remember the nurses telling me that I was so lucky because I got to have both Dr. Christopher Baker and Dr. Alexander Misono on my case. They don’t frequently work together, but they’re both amazing. And the nurses said they listen to really good music in the IR. The anesthesiologist explained that there was a risk to my baby because I was only seven weeks pregnant, but, you know, we had to do this. And next thing I knew, I was awake, and I’m pretty sure it was Dr. Misono standing over me. And I said, “So what did we listen to?” And he laughed. He was like, “We didn’t have time for music!” It was 45 minutes from the time I got through that door until they removed the clot in my brain. It was a massive stroke on the right side of my brain, in the middle cerebral artery. It’s just absolutely certain that if it had been in there longer, I wouldn’t be here, or I would be significantly impaired. Immediately after the surgery, I had no deficits whatsoever. They, of course, were running other tests while I was recovering in the ICU, and they were watching my heart, and that’s how they detected that I actually had a PFO, a patent foramen ovale, in my heart. Everyone is born
with these little flaps that open and close and allow the blood to pass through when you’re in utero. For most people, it closes within something like an hour of birth. But mine never closed.
I had never had any cardiologic issues whatsoever. No doctor ever detected a murmur. No irregular heartbeat. I’ve always been super active: hiking, yoga, Pilates, swimming. So this was wildly shocking. It put me at risk of another stroke. And the cardiologist, Dr. Subbarao Myla, was so wonderful. He immediately wanted to schedule the PFO closure, but my OB was able to weigh in and say, you know, it really would be best if we waited until the beginning of the second trimester. Knowing that this little baby had already been through anesthesia, they decided to hold o . And my neurologist, Dr. David Brown, also said he would prefer we wait too, because my brain had a significant amount of swelling from the clot and the removal of the clot.
So, at 16 weeks pregnant, on February 22, I was able to get the PFO closed. It was a super-simple process where they went in through a vein, and they put in this little 25-millimeter device that’s like an umbrella. From incision to closure in 10 minutes.
I had my son Rhodes, and he was perfectly healthy. I completely believe that this baby came to save my life.

HAVE ANKLE RECONSTRUCTION SURGERY AS
A PRO ATHLETE
Professional rugby player Ty Lewis of Los Alamitos had his ankle completely reconstructed after a debilitating injury.
n high school, I played football, wrestled, ran track. I wrestled all the way through college, then got into MMA. I got to meet Chuck Liddell, train with Tito Ortiz, Rampage Jackson, all these legends. But my wife wasn’t a fan of watching me get punched. So I transitioned to rugby. I’m on two teams, Belmont Shore Rugby Club, a union team, and the L.A. Banditos, a league team.
ILast season, I was getting these injuries, and I didn’t really pay attention to them. I just thought, you know, you play sports, you’re gonna have bumps and bruises. But I just kept getting more and more. And I was like, OK, let me finish out the year and see how that goes. Had a great year. And then coming into this season, our first league match, I got tackled the wrong way, and three guys landed on my ankle, and I felt a pop. I was thinking, all right, well, I don’t think that’s very good, but we still have, like, 60 minutes left in this game. I’ve got to finish it out. So I kept playing on it, and it kept swelling. I got tackled again. I felt another pop. I was like, Jesus, it’s not my day.
By halftime, the swelling was so bad, I had to tape my shoe on. I kept playing. We won by a lot. I got to the car, and as soon as I took the tape o —you know those Pillsbury dough things? It was like that. I got home, my wife saw it, and was just like, “Hey, we should go get this checked.” I was like, “Ah, give it a day.” The next morning, it was changing colors, still numb, and like four or five times the size.
So I went to Kaiser, got X-rays and an MRI. When the results came back, I got a phone call. “Hey, um, you free for surgery?” I was like, “Sure, yeah. What’s going on?” They were like, “You need an emergency surgery.”
They said there was an old injury there. As soon as they said it, I knew exactly what they were talking about. Apparently, I broke my ankle twice and just kept playing through it and kind of made it worse. There were
floating bone chips, bone spurs, and bone fragments. I had torn the ligament that’s in between the two bones of your ankle.
I was sent to the surgeon, Dr. Wesley Tran. We had a lengthy conversation about biomechanics and kinesiology and the function of certain ligaments and joints in the ankle. It was awesome, because he was just very laid back, understanding, and he could recognize I wasn’t just a normal person who just blindly says OK to everything. We talked about what the options for healing were, what the surgery was going to be—very nuanced discussions about everything. He showed me a lot of respect doing that, because I’ve been to other doctors where they are like, “I’m the one with the degree, you’re going to shut up, and I’m going to do what I’m going to do,” that kind of attitude, you know? He wasn’t like that at all. And none of the people in the orthopedics department were like that.
I had the surgery over the summer and another one in the fall. It’s been a really good process. I don’t really have any complaints. The only thing that was really frustrating was not being able to walk for a period of time. And my son’s young, he wants to play, and now he’s looking at me like, “Why isn’t Daddy getting up? Why isn’t he playing ball with me?” So it was a hard process, but luckily, I had physical therapy, and I had Dr. Tran to call if I had issues. I can probably start playing again soon. What I miss most is just being around my teammates and the camaraderie. That’s what I’m looking forward to the most.

FIGHT BREAST CANCER AT 38
Juliette Landgrave of Mission Viejo was diagnosed with triple-negative breast cancer. She discusses the journey with her physician, Dr. Amanda Schwer at City of Hope.
uliette Landgrave: It began in the shower. It was April 2022. I was doing my self-exam, and I felt a tiny lump by my chest wall. I thought that it could be a cyst. I went to the doctor, got a referral, got my first mammogram and a few other tests. It was found that I had two malignant masses in my right breast. It was just shocking. I was diagnosed with triple-negative breast cancer. It turned my world upside down. It was something that I was not expecting at 38. It was challenging—emotionally, mentally, and obviously physically. But thankfully I found City of Hope, and once I was there, to be honest, I felt a sense of relief because I was with the experts. I’m someone who’s always very positive. I just said that I was going to be the poster child of every single drug that I was given, and I was going to beat it.
JDr. Amanda Schwer : She got teamed up with just the best doctors you could imagine, and by the time she saw me, she had already been through surgery and chemo and so much. I will agree that she is a positive person, because she had been through the ringer by the time she got to me, and she was still bubbly and adorable and positive and just willing to take whatever we needed to throw at her to get rid of this disease. She was amazing.
J.L.: I went through 16 rounds of chemo. I did multiple surgeries, a year of immunotherapy, and also 25 radiations. I just had to put my brave face on and go for it.
A.S.: It used to be, you’d occasionally see a couple people in their 40s, maybe 30s, but honestly, I think I see as many young people now as I do older people. It’s a really disturbing trend. I have two 20-somethings on treatment right now, who both have triple-negative breast cancer. I’ve been an oncologist for a while, and I’m usually pretty good at getting through my day, but there’s been so many times lately where I’ve had to just kind of take a break. The challenges that come with being a young person with cancer are really emotional but also just so incredibly
inspiring. I feel so lucky to be taking care of people like Juliet.
J.L.: When they told me that it was triple-negative breast cancer and I would have to go through chemo, I just knew my fertility would be compromised. So before starting treatment, I decided to do fertility preservation. I was able to save some eggs, and that gave me a little peace of mind, because chemo did put me through medical menopause. Also, breast conservation was very important for me as a young woman. I knew I was going to live because I had decided that that was my goal. But I wanted to make sure that in the aftermath, I felt like myself. A lot of people just go with a mastectomy, thinking that that’s going to be the safest. But there’s been studies that a lumpectomy with radiation is as good as a mastectomy. When I told that to the doctors, they were just so happy, and they made my desires happen.
A.S.: We’re seeing a noticeable rise in young people getting cancer. But City of Hope is doing everything we can to turn this trend around. There are a number of research studies we have ongoing prioritizing early diagnosis. Triple-negative used to be a really challenging diagnosis, but now we have drugs that are working so much better. But regardless of age, stay vigilant. If you feel symptoms, get checked out, speak to your physician, advocate for yourself, and know that there are a lot of doctors out there who are here for you.
J.L.: I’m Hispanic, and you know, in my family we don’t talk about cancer; it’s a taboo. When I started my journey, I made a point to share my experience with my family and my social media friends, and just to really tell them that they have to advocate for themselves. It’s really important for everybody to know their history, to do their self-exams, because they know their body best.

What It’s Like to … CREATE AN ANXIETY PODCAST FOR TEENS
Breann Durham is host and producer of NAMI Orange County’s “Anxiety Sucks” podcast, now in its fourth season.
Istarted at National Alliance on Mental Illness (NAMI) doing peer mentoring. Then I was a program coordinator. And I started the podcast for the youth program that we have. At first, it was called “NAMI’s Declassified Anxiety Survival Guide” which was a huge mouthful, but it was based on the TV show “Ned’s Declassified School Survival Guide.” Eventually it became “Anxiety Sucks.”
I usually describe it as a podcast that’s made by young adults, for young adults. We’re really just trying to create a space that is destigmatizing mental health, and we want people to feel less alone, especially students.
My co-host Madi Morrison and I have been best friends since freshman year of high school, so it’s been a lot of fun working with her. Initially, we started o with me and two of my teammates, and then they kind of got too busy, so we asked Madi to join me.
We’ve had guests like authors, clinicians, people who work at NAMI. We’ve had students come on and share their mental health stories. Madi’s mom came on, and she talked about grief, which was one of our more heavy episodes, but it was a really good conversation. Probably one of my favorite episodes we’ve ever done. We talk about our own mental
health struggles. Because I also edit the podcast, it’s really hard for me to listen back and hear myself. I have to stop myself so many times from just cutting out all of my parts. It’s hard to open up sometimes. But I think as I’ve done it more and more, it’s gotten easier.
When I was in high school, I just never talked about mental health. It just wasn’t really talked about. And then when I went to college, I kind of had a tough time moving. I moved from Southern California to New York City. So it was a huge culture shock, and I struggled a lot. And it was the first time that I felt this overwhelming sense of anxiety. Talking about it more with my friends, and just seeing how many other people were going through the exact same thing I was, I was like, why don’t we talk about this more? We need to share and be open about it. So that’s what led me to be so passionate about it. Of course, COVID had a huge e ect on me. I was 19 in 2020, and my school year got cut short. I was just so isolated and anxious and overwhelmed.
It’s so nice to hear the feedback I get when I do outreach events. When young people say, “I’m so happy that you open up, because now I feel like I can open up.” And we also get emails that come in saying they loved a certain episode.
I’m still pretty young, so I’m still trying to figure out what I want to do, but I’ve found that talking about mental health has become my passion. So, in the future, I hope to make this podcast into something bigger, and maybe even cover more than just Orange County. I’m really excited to keep growing this podcast.
