YOUR BENEFITS | who pays what? Benefit Basics Once you elect your Legacy Supply Chain Services benefit options, your elections remain in effect for the plan year. You may only change coverage due to a qualified "life event" and must do so within 31 days of the event. Legacy Supply Chain Services encourages you to review all your benefits and make your selections wisely. You decide: ●How much to save - up to 100% of your eligible compensation to a maximum contribution limit set forth by the IRS. The Human Resources Department is pleased to announce the launching of MiHR, a web-based tool to facilitate Employee Self-Service. MiHR is now AVAILABLE from ANY Computer and ANYWHERE you have Internet Access. In order to assist you with navigating this system, we have developed Web Tours to demonstrate how to use this tool. Each tour is based upon the steps that either an employee or one who manages an employee will need to complete.
The tours will provide information on how to: • Log into the system • Change your password • Navigate through the system • Understand the basic functions The self service portal will provide employees the ability to: View Pay Statement / Pay Stubs View W2’s Add or Change Personal Information (i.e. Address, Emergency Contacts, Direct Deposits)

Benefits Online Open Enrollment Open enrollment will be administered exclusively online and most benefit forms are now fillable PDFs. You will be able to complete the form online and submit them directly to our benefits mailbox at benefits@legacyscs.com or to your facility HR Representative.
Important: Even if you are satisfied with all your current benefit elections, you must still participate in the Online Enrollment by re-enrolling in all available Benefit selections. Note: If you are experiencing any difficulties when logging into the system, we suggest that you review the System Requirements or email hris-security@legacyscs.com.

Benefit Glossary The following definitions should help you understand your benefit plans. Remember, it is very important for you to seek out services from In-Network providers. Our Medical, Dental and Vision benefit providers have contracted rates that are much lower than Out-of-Network providers. Your out of pocket expenses will be significantly lower by using In-Network providers. Co-Insurance—The percentage of cost-sharing between what you and the insurance company must pay, after any applicable deductible has been met. Co-Pay—The set dollar amount that you must pay to a provider when services are rendered. Calendar Year Deductible (CYD) - The amount you pay before co-insurance is paid. You only need to meet your deductible once per calendar year. Network— The group of providers who are approved for services and are available for treatment under the insurance company’s contract. Outpatient—A patient who receives treatment at a hospital or outpatient facility without being admitted overnight. Out-of-Pocket-Maximum—The maximum dollar amount that you will pay each calendar year. Once the out-of-pocket maximum is reached, your co-insurance is 0% and the insurance company’s co-insurance becomes 100% for the remainder of the calendar year. Co-pays continue even when your out-of-pocket-maximum is met. Provider—Any facility, person, or entity recognized for payment by the insurance company. FSA—Flexible Spending Account DCA—Dependent Care Account
