In the Spotlight WAGD Membership Spotlight D. Andrew Lewis, DDS, FAGD
Dr. Lewis graduated from the University of Michigan School of Dentistry in 1998 as a recipient of a Naval Academic Scholarship and a commission as a Lieutenant in the United States Navy. He was accepted for an Advanced Education in General Dentistry (AEGD) residency at the Marine Corps Recruit Depot at Parris Island, South Carolina. After graduating at the top of his class, he was awarded the Navy Achievement Medal and assigned as Dental Department Head aboard the USS Sacramento while on deployment to the Western Pacific and Persian (Arabian) Gulf. While on deployment, Dr. Lewis was awarded his Surface Warfare pin for Dental Officers (SWMDO) after displaying significant knowledge of the ship’s systems. Upon his honorable discharge from the United States Navy in 2001, Dr. Lewis pursued private practice in Seattle, Washington. During the past twelve and a half years in private practice Dr. Lewis has earned his Fellowship in the Academy of General Dentistry (FAGD), published in a leading peerreviewed journal, and served as a peer-review editor for the Journal of the Academy of General Dentistry as well as the Compendium of Continuing Education in General
Dentistry In addition to these professional achievements, Dr. Lewis has consistently taken opportunity to volunteer his expertise for the education and oral health of children and battered women in his community. He has provided volunteer education to grade-schoolers and free dental care to school-age children in dental pain through Seattle-King County Dental Society’s “Adopt-ASchool” program. As a volunteer with the American Academy of Cosmetic Dentistry’s “Give Back A Smile” program, Dr. Lewis has provided no-cost cosmetic and restorative dental care for victims of domestic violence. Most recently, after several years of study, Dr. Lewis has achieved the status of “Mentor” at the worldrenowned Kois Center in Seattle, Washington where he will assist in teaching other dentists. The Kois Center is one of the premier advanced post-graduate teaching facilities for dentists around the world. Dr. Lewis has been married for 17 years to his wife and mother of their two children. He is an elder at his church and has served in multiple leadership and teaching positions there over the last 9 years.

The Academy of General Dentistry has awarded the 2014 Senior Student Dental Award on Friday, June 6, 2014 to Allen Tucker, DDS. This award has been established to recognize outstanding senior dental students from each dental school in the United States, Canada, and Puerto Rico who exhibit potential for becoming exceptional general dentists. This AGD award is given to student members in their senior year of dental school who have indicated a desire to practice general dentistry, have maintained an academic scholastic balance between his/her clinical and didactic basis courses of study and are in good academic standing with his/her graduating class. The Washington Academy of General Dentistry was selected to honor Allen for his demonstrated outstanding attributes conducive to exceptional doctor-patient relationships, on Friday, June 6, 2014 during the University of Washington, Honors & Award Ceremonies. Dental Wealth To Roth or Not to Roth
Roth IRAs are attractive vehicles and can provide income tax diversification as well as makes an outstanding asset to pass on to younger generations. The main attraction of the Roth IRA (or Roth 401k option) is the tax free build-up of earnings. In a traditional IRA (or 401k) the invested funds grow and compound on a tax deferred basis. When withdrawn in a qualified manner, the funds are taxed at your then current marginal income tax bracket. At age 70.5, the government requires you to begin taking Required Minimum Distributions or RMD’s for short. The RMD is a percentage (starting at about 3.6%) of the prior year principal balance and is increased each and every year, creating more and more taxable income as you age. Roth IRAs are not subject to RMD requirements during your lifetime. This represents another advantage of Roth IRAs.
Roth IRA Funding: You can directly fund a Roth IRA only if your Modified Adjusted Gross Income is below certain levels. For 2014, the ability to fund a Roth IRA is phased out between $181,000 and $191,000 on joint return and between $114,000 and $129,000 on a single return. For many dentists, this precludes a direct Roth contribution.
Roth Conversions:
In the past, converting traditional (taxable) IRAs to Roth were also limited by income. However, a few years ago, this 10

restriction was removed, so now, all taxpayers qualify for conversion – even those with very high earnings.
Conversion Math: The computations on whether Roth conversion makes sense for you can be quite complex; however, we can generally say the following: If you are in a high income bracket while actively practicing dentistry and you would expect to be in a similar or (more likely) lower bracket in retirement then as a general rule, Roth Conversion would not make sense while you are actively practicing. Why? Well, who wants to pay tax sooner and at a higher rate – versus years later and at a lower (perhaps much lower) rate. For most dentists, conversion won’t make sense unless an exception applies. One exception might be an unusually low income year (e.g. new office build-out or going back to specialty school).
Indirect Roth Contribution: If your income level precludes you from directly funding a Roth IRA and you have no taxable IRAs, you have an opportunity to make what is called an “indirect” or “back door” Roth IRA contribution. On Day One you make a non-deductible contribution of $5,500 ($6,500 if 50 or better) to a traditional IRA and on Day Two you make a non-taxable conversion to Roth IRA. This works wonderfully, but only if you don’t have (significant) taxable IRAs. The reason is that when you convert, you must treat the conversion as if you are converting a percentage of all traditional IRAs regardless of whether the taxable and non-taxable traditional IRA’s are in separate accounts – therefore picking up taxable income.
If you have taxable traditional IRA accounts and you have a 401k through your practice, you may have the option of rolling the taxable IRA funds into the 401k – and voilà you are now set to make indirect Roth IRA contributions (however, you should review the costs and investment options of the 401k before making this decision). Another fact to know is that spouses IRAs are treated separately under federal tax law. So perhaps one spouse has significant taxable IRA funds and the other has none. That latter spouse could exploit the indirect Roth IRA. The world of Roth versus Traditional in both IRA’s and in 401k Plans can be quite complicated. It is best to evaluate these decisions in the context of your comprehensive (written) financial plan. It is also a very sound idea to work with a tax and investment professional (not just one or the other – both) to make sure you are making the smartest decisions.

Cunnington, DDS, FAGD When things are tough, or frustrating, or miserable, then I propose that you’ve just bumped into a little clue that here is an opportunity to learn. That’s it, nothing more. Well now, if you don’t buck-up & learn, that’s a whole different kettle of fish. So why ARE you so pig headed at being behaviorally ossified? At this point maybe a psychologist would ask you to check out your relationship with your parents. I admit most of us are stuck with wondering around in life in a virtual fog, & can see only a short way down the road. That’s certainly true for me anyway. But that doesn’t mean it’s not easy. I didn’t postulate however that it’s not hard work. It IS hard work. It’s

just that this hard work is really, really easy. Think of it like gardening. You get on your hands & knees out in the sultry glare. You pack that grime & slime under your fingernails. And then voila, you ever so slowly transform your mundane weed patch into this glorious maze of color. Easy! Yes, your back will be sore, your muscles will ache all over, you’ll sleep at night like a baby, but what a delight you’ve accomplished. Are you uncomfortable with endo, pedo, ortho, extractions, implants, botox, interpersonal skills, time management, anger management, or anxiety etc? This only means you’re in need of a tool. The more competitive dentistry becomes, the more important it is to implement communications and systems to attract and retain patients
that drives desire and motivates purchasing behaviors. Nordstrom is well aware that earning your loyalty far outweighs the dollars they may lose in returned goods.
Today’s savvy patient has more dental products and dentists to choose from than ever before. This increase in offerings has shifted people from thinking like a “typical” patient to acting like a consumer.
Whether obvious or not, the same applies in dentistry. I realize that consumers cannot return a crown, but certainly you could guarantee fit, look, comfort and against breakage for a reasonable period of time. And I would guess that many of you already do. However, from a consumer perspective we generally have no idea this type of guarantee is offered until it is revealed at the time of the case presentation. This is much too late. It should be promised much earlier in the communications process.
This shift to consumerism requires today’s dentist to place equal value on the business side of dentistry as they do clinical training. Developing a strategy that fosters patient loyalty is the most proactive method to surviving in today’s consumer driven economy. However, while recognizing the need for equal efforts is the first step, implementation is not without significant time and effort. The good news is that many industries have already developed successful road maps for developing unique value propositions and measurable customer experiences. Consider Nordstrom for example. Because we can return items at any time, we are genuinely more confident when we purchase an item regardless of price. We know we can always return it if it doesn’t work out the way we intended. This simple yet powerful value proposition that Nordstrom has implemented is aimed not only at building consumer confidence but also building loyalty and return visits. On a psychological level it goes even deeper, aiming to motivate the place in the consumers’ brain 12

WAGD Student Member Testimony William Howard Memorial Award Competition
This case taught me the importance of working closely with different specialists and forming a dental team in order to achieve the desired results.
I was fortunate to be able to participate in the William Howard Memorial Award competition at University of Washington School of Dentistry. Throughout my dental education I have learned the importance of treatment planning in order to attain the best results for the patient; the case I presented was no exception and proved to be a great learning experience for me.
As dental students we do not get many opportunities to present our work in a venue other than the dental school, in fact this was my first time presenting work outside the class room. Being able to present to accomplished dentists did make me slightly nervous as the people I was presenting to had significantly more experience than me. However once the presentation was over I felt as though I was being viewed as a peer and no longer a student. It was an honor to have the opportunity to present my work and an even greater honor to be recognized for
my accomplishments while in dental school. I wish to thank the Washington AGD and its board members for organizing this event as well as all of my advisors who provided insight during the treatment of this case.

WAGD Mastertrack Member Testimony Puneet S. Aulakh, DDS, FAGD
In June 2007, I was two years out of dental school, when for reasons unbeknownst to me now, I decided to open a scratch-start office in Bellevue. I went to dental school in Detroit, had no local connection, and best of all, I had no prior business experience. But, this was dentistry, how hard can it be? The economy was going great, I asked US Bank for a $100,000 loan, they offered me $300,000. After my build-out and setting up my office, I was half a million in debt. I asked what they wanted for collateral, they said don’t worry about it, dentistry is a great field, and everyone does great. Good luck. About a year later I got married, a big Indian wedding with 500 guests. A month after that, Lehman Brothers collapsed. My timing was impeccable. I made sure that the 3-4 patients I had booked every day at least saw each other, so that I would look busy. In retrospect, I should’ve been bankrupt by the end of 2008. As I look back I have a pretty good idea why I survived with all of my naiveté and inexperience. I owe a bid debt to the Washington AGD and to the Mastertrack. More than the lectures, the hands on participation, and the camaraderie between the participants, it was the ability to lean on people in the Mastertrack for advice and guidance. You learn pretty quickly after dental school that not only do you not know a lot, but that there is SO much to learn. Mastertrack 14

itself, and the people in Mastertrack are the main reason I am here today. They are the reason I am getting my MAGD in Detroit this summer, they are the reason I am involved with the WAGD and the AGD as board member, delegate, and the WAGD president-elect. In order to be a successful dentist you have to have a support structure, people you can ask when you have questions, need guidance, and need advice. Hey, what do you think is the best implant system, or should I buy the Wave 1, or what composite do you use and why. I remember I had a patient come to me with an implant he had placed somewhere in Vietnam. I had no idea what type of implant it was. I sent out an email to the group and I had 5 answers by the end of the day. Dentaltown is great, but its even better when you can ask your colleagues. I always people there are 3 things that made me a better dentist, the WAGD, Dr. Tucker, and Mastertrack. In Aug 2007, I got a flier for Mastertrack and called Valarie Bartoli. I asked her if this would be a good program for a new graduate, my words were, “I don’t want to be the newbie with a bunch of old guys.” She laughed and assured me that I would be just fine and that they have a great group of people in the class and it would be good to learn from all of them. She told me I wouldn’t regret it. Boy, was she right. The first person I met was Steve Thomas (our current WAGD
President). I asked him if he was Tim Hess (currently the head of Tucker Institute, lecturing nationwide on Implant failure and teaching Botox to dentists). There was Linda Edgar (current AGD national president), her husband, Bryan Edgar (please see attached 10 pages of CV). Gary Heyamoto (see 15 pages of CV), Scott Martin (best Endodontist ever), Binh Tran (currently teaching Ortho for the WAGD). And other great people such as Joe Sepe, Doug Smith, Esther Minh, Gary Pape, Wendy Crisafulli, Teresa Kang, Lisa Buttaro, Randy McLeary, Tammy Eckart, and Ted Bear. I had these great people as my colleagues and mentors. They helped guide me to be who I am today. If you are thinking about taking Mastertrack this fall and are not really sure if you can take the necessary time off, or are unsure about the lectures, or unsure if it is a worthwhile. I can tell you without a doubt you will not regret it. Feel free to call or email me anytime if you want to know more. I could keep writing, but I know the 2 of you that are still reading this are tired, and I apologize for rambling. I was in Mastertrack for 5 years and look forward to rejoining the group this fall. Volunteers Needed for a Massive Free Health Clinic at Seattle Center Save the Dates: October 23 & 26, 2014 Washington Academy of General Dentistry is joining with more than 25 other organizations from around the State of Washington to host a massive free health clinic to treat underserved and vulnerable populations, as well as connect them to area resources that can enable continuity of care. In partnership with the non-profit, humanitarian organization Remote Area Medical (RAM), and under the guidance of local colleagues Dr. Jeffrey Parrish, Dr. Michael Karr and Dr. Carl Youngquist, the clinic aims to fully-staff 60 dental operatories per day. Although this is the first time RAM has been invited to our state, they are not new to operating large-scale, volunteer-driven clinics. RAM was founded in 1985 and initially focused on providing healthcare access to people in remote areas of the world such as Haiti and Guyana. In 1992, they were invited to hold a clinic in Appalachia and from there other rural and urban areas began requesting their support. RAM has since held over 700 clinics in the United States and served over 500,000 patients.
A clinic such as this is a true collaboration between RAM and a Host Community. RAM provides a tremendous amount of infrastructure, especially in dental and vision areas. Then they follow the lead of the local professionals, selected by the Host Community, who determine most of the healthcare offerings and protocols, as well as manage the on-site delivery of care. Add to that the generosity and dedication of over 300 dental, vision, medical and support volunteers each day, and a clinic can make an incredible contribution to the lives of many people in need, as well as to our community. We encourage you to be a part of the October clinic! RAM provides everything the volunteer dental professional needs including the dental equipment, tools, medicines, and chairs. We’ll also provide meals and parking. All you do is show up and help and we take care of the rest. The clinic needs Dentists, Oral Surgeons, Hygienists, Dental Assistants and Dental X-Ray Techs, so feel free to bring your staff. Volunteer for one day or all four, we’d greatly appreciate your participation. Don’t forget to encourage other dental colleagues to join you.

Program Approval for Continuing Education (PACE) AGD Credit for Study Clubs, existing or new, Dr. Linda Edgar wants to hear from You. Each year, thousands of continuing education courses are presented by hundreds of program providers—dental schools, dental societies, and companies that specialize in course presentations. Most provide dentists with valuable information that can be successfully integrated into the dental practice. The Academy of General Dentistry (AGD) Program Approval for Continuing Education (PACE) was created to assist members of the AGD and the dental profession in identifying and participating in quality continuing dental education. PACE is an evaluation of the educational processes used in designing, planning, and implementing continuing education. Approval by the AGD does not imply endorsement of course content, products or therapies presented, nor does this approval imply that a state or provincial board of dentistry will accept courses for re-licensure. Approved program providers are expected to comply with all relevant state and federal laws. Continuing education offered by approved program providers will be accepted by the Academy for Fellowship/Mastership credit. PACE reviews and approves program providers, not individual speakers or specific courses content. The applicant may be a major unit or department within an institution. To be eligible for Program Approval for Continuing Education the following criteria must be met: 1. The program provider must have been offered a planned program of continuing dental education activities for at least 12 months. 2. The program provider must ensure that all courses offered have a sound scientific basis in order to adequately protect the public. PACE reserves the right to require acceptable substantiation from providers that their courses have a sound scientific basis, proven efficacy, and ensure public safety. 3. The program provider must ensure that the educational methods and the facilities selected are appropriate to accomplishing the stated objectives of each activity. 4. You MUST NOT be representing a company. 5. You cannot provide these courses out of the state of Washington

Dr. David Dean is an Acting Assistant Professor in the department of Oral Medicine at the University Of Washington School Of Dentistry and a clinical attending at UW and Seattle Cancer Care Alliance. A lifelong Husky, he received his BS, DDS, and MSD at Washington. During his oral medicine training he completed his master’s thesis and a clinical fellowship at SCCA. As a student researcher, Dr. Dean was involved in several projects under the direction of Dr. Edmond Truelove which focused on the utility of adjunctive diagnostic techniques in the assessment of oral mucosal lesions. Their study on narrow-band fluorescent imaging of the oral mucosa in routine dental patients was published in General Dentistry in 2011. Dr. Dean’s current research focuses on the relationship of oral health to post-treatment outcomes in hematopoietic cell transplantation. Dr. Dean previously practiced as a general dentist in Tukwila, WA and is an active member in the AGD.

Course Outline: Attend the two-day Fellowship Review Course on Friday, September 26 & Saturday, September 27, 2014 from 8:00 a.m. to 5 p.m. The Fellowship Review Course is a review of currently accepted knowledge, techniques, and practices in general dentistry. Members may sit for this course as part of their preparation for the Fellowship Exam; however, taking this course does not guarantee that a member will pass the exam. Information on the content of the Fellowship Exam is held in strictest confidence, and as such, information regarding the overall content of the exam will not be released during the review course or to the course speakers. Candidates may refresh their knowledge of basic clinical information in the 19 areas of general dentistry covered in the exam by attending the review course. Candidates are expected to draw on their own practical experiences as they take the Fellowship Exam. The Exam is not offered during this course. Participants are encouraged to register with a SMT Testing Center near their home following the course. The outline for the fellowship exam is regularly updated. This keeps the content current and pertinent as possible, and enables it to reflect a more thorough portrait of dentistry today.
