Still Using Windows XP? Better check your bank accounts.
Imagine this scenario. Like any other day, you head to work, stopping by for a Starbucks on the way. You sit down to check your email and tidy up some loose ends around the office. You’re feeling pretty good about things lately. New patients are coming in and existing patients are singing your praises. Life is looking up for you. Right then your cell phone rings and a staff member informs you that her paycheck didn’t clear. You tell her you’ll look into the situation and get back to her. You quickly log into your bank account. The color drains from your face as you read your balances...all zeros, in all accounts. All your money is gone; transferred into a far off account in a far off country; unretrievable by authorities.

Sound far-fetched? It isn’t. For one small town medical facility in Washington State this nightmare came true in a big way...2 million big ways. Hackers gained access to the clinic’s computers and drained their accounts of 2 million dollars. How, you may ask? Through a security hole that allowed access to their computer and ultimately their bank
So what happens when one of the world’s largest technology companies retires one of the most popular operating systems to ever land on a hard drive? Historically, these, “End of Life” scenarios turn into a large scale, technological game of chicken. Big corporations are typically unable to make the necessary changes quickly enough to meet the deadline. In the past Microsoft has been the one to swerve, pushing back the end-oflife timelines to accommodate users clinging to the technology they know and love. But in the case of Windows XP, Microsoft didn’t swerve.
How does a security hole like the one in this scenario happen? One of the most prevalent culprits of security breaches is the use of outdated s o f t w a r e . Software becomes outdated when companies like Microsoft or Apple stop development on tools and operating systems that have been replaced by newer versions. This is the case for Windows XP.
On April 8th 2014 Microsoft officially stopped development on the widely

used Windows XP operating system. XP reached the ripe old age of 10 years before it was finally retired. Halting development on one of the most popular platforms ever has left many users out in the cold, wandering dangerously through cyberspace without any protection in the form of security updates and patches. What does this mean for users who are still lingering on Windows XP? It means their playing Russian roulette each time they boot up their computer. Malicious hackers have a new playground, attacking and plunder unsuspecting, vulnerable users who have yet to make the switch from XP. It’s open season for internet pirates using new exploits to gain access to personal and business computers. Hackers increased their efforts early in 2014 knowing that any security holes they found would not be plugged by new updates. Once a computer is compromised...the sky is the limit on the damage that can occur. Bank information, passwords, personal info, pictures and more can all be obtained. But, like so many things in life, such as cigarettes or seat belts, people choose to ignore the obvious dangers thinking it won’t happen to them. News flash...it can and it will.

to provide the Secretary (and affected individuals) with a notice that a breach has occurred. These occurrences would be subject to investigation and could result in penalties if negligence is discovered.
So how does this affect your dental practice? In more ways than you can imagine. Patient health information, financial data and your personal identity are just the tip of the iceberg. Hackers can place tools such as “key-loggers” on computers and begin recording every keystroke you make on your computer to collect passwords and personal information. When that happens, your money can be transferred to Chelyabinsk, Russia quicker than you can say “why didn’t I upgrade my computer”. Beyond the business and personal damage, practices need to consider
HIPAA related issues. While HIPAA does not have specific, minimum requirement for an entity’s operating systems they do use language that addresses the responsibility of dealing with known risks from vulnerable software that reside on your computers. As well, HIPAA requires that a remediation plan be in place that displays the entity’s intentions for upgrading any vulnerable software that contains or has access to personal health information. If a breach of health information were to occur in your practice it would be subject to the breach notification rule which requires covered entities
What now? Like any good doc worth his or her salt will tell you...an ounce of prevention is worth a pound of cure. Like anyone who owns a business, you don’t need one more thing to do. But it’s critical that steps are taken to begin the process of getting your network out of the cyber wilderness it’s currently in. Many practices that I work with aren’t unwilling or unable to make the switch, they simply don’t understand the possible consequences. For others it can be challenging financially to gut their network and replace it with new technology. My firm (Firefly) has helped replace hundreds and hundreds of Windows XP computers and it isn’t a “one size fits all” scenario. What’s most important is to develop a plan. Even a phased approach is a step in the right direction. It may seem daunting but with the right help the transition can go very smoothly. Ask yourself this; can you afford not to?

Washington AGD Hosts Black Tie Masquerade Gala Benefiting the Oral Cancer Foundation Teresa K. Kang, DDS
On Friday, October 3rd 2014 the Washington AGD will host its second annual gala to support the Oral Cancer Foundation. Join us at the Westin Bellevue for this wonderful event and meet other Washington AGD members. Dress up, have fun and support great cause! Like last year, net proceeds will be donated to the Oral Cancer Foundation. The Washington AGD decided to support the Oral Cancer Foundation because like our organization, it advocates oral health, patient care and lifesaving early detection.
The Oral Cancer Foundation is a public service charity that helps with advocacy, information, research and support. Funding to the Oral Cancer Foundation helps maintain web-based support for patients.

My Cancer Personal Experience Dzon M. Nguyen, DDS, FAGD
Nothing prepares you for hearing the words “you have cancer.” March, 2013, I was diagnosed with Thyroid cancer and a month later I had my cancer successfully removed. I am one of the lucky ones as my cancer was caught early and I was spared. How my cancer was caught is an interesting story. My thyroid has always had a 1cm nodule. My physician and I have monitored it over the last ten years. Yearly blood works and bi-yearly ultrasounds showed no changes, always normal. It was only when my sister had her thyroid removed because of it’s enlargement that alerted me. During her surgery, her thyroid was biopsied and to the surgeon’s surprise, it was cancerous. This prompted me to have mine

biopsied. My doctor felt a biopsy was not necessary because the lack of any changes over the years. I was insistent and had the needle biopsied done. This was the one decision that changed my life forever. The biopsied had 19 specimen and they all came back positive for cancer. The clear message here is when in doubt, biopsy! As my friend Eric Statler has said, it’s peoples lives and doing our part will save someone. It only takes one person to make a difference. I was thankful that I had caught it early. My practice has always done head and neck exams. We were an early adaptor of the Velscope. We routinely biopsy. Being personally

A Personal Experience Eric Statler
I was 33 years old, a husband and father with two young boys when I was diagnosed. I’ll always remember the day, it was January 23rd, 2008 when I heard the words “You have Oral Cancer”. Shock and disbelief were the first emotions I felt having never been a tobacco user, nor heavy drinker, I didn’t fit the typical demographic of an Oral Cancer patient. Those emotions quickly turned to horror. From the look on the face of the Oral Surgeon who I’d been referred to, I could tell it didn’t look good. A quick search with Google didn’t help the situation, actually adding to the sense that I wouldn’t live through this experience after reading the overall survival rates of the disease. As my case was so advanced due to late detection, I had to be referred to the head of the University of WA’s Otolaryngology program, Dr. Neal Futran, MD, DMD. I was told he was the best option in the NorthWest for my diagnosis, and the UWMC was one of the top programs in the country for cancer. My case was put in front of a tumor board, which is a multidisciplinary panel of experts that decide the best course of treatment for those advanced cases that fall outside of any standard protocols. Their recommendation was that I go through concurrent radiation, and high dose chemotherapy treatments for seven weeks, followed by major surgery to remove lymph nodes from my neck, as well as the right half of my jaw and tissue in my face, replaced by my fibula and tissue from the left leg. 14

The treatment experience is an extended stay vacation in hell basically. Radiation to the head and neck can only be described as “barbaric”, and the side effects of the chemotherapy drug hospitalized me after the second treatment. There were many touch and go moments in the seven weeks of chemoradiation, I had to have a PEG tube placed as the pain of swallowing was so great I had to be placed on fentanyl, the strongest opiate pain medication prescribed to humans. I was on a high dose of that drug for the better part of two years. I was told my tumors had responded well to the treatment, however my jawbone had developed OsteoRadioNecrosis, or ORN, due to radiation and the infection could be fatal if not surgically repaired. I had lost 35 pounds through that portion of treatment, and was given six weeks to rest and try to put on weight before the surgery. My family and I much needed the reprieve, as they go through just as much as the patient does, if not more so in some aspects. The follow up mandiblectomy with fibular flap, and neck dissection were
performed in June of 2008 at the UWMC in Seattle by Dr. Futran and his team. I was in ICU for several days, waking up to see all of the tubes sticking out of me is a vivid memory I have, I wasn’t breathing so well and they were contemplating placing a trachea tube. After stabilizing I was transferred to the 4th floor, pain and discomfort are what I remember most, and shock at seeing all of the swelling and disfigurement. Due to the size and location of my primary tumor, my facial nerve had been compromised during surgery leaving the right half of my face paralyzed. As vanity is a word that could best describe me pre cancer, the emotional trauma of not recognizing your reflection in the mirror was devastating. I had made a living in the Hotel Industry where appearance and your smile are everything, seeing my face in the mirror for the first time was a realization that that life was gone for me. My jaw was wired shut, my head looked to be about the size of a watermelon due to the swelling, I had to have all food through a tube, and my leg was devastated by the graft, leaving me with a condition known as “Foot Drop”. At this point of the treatment I had lost close to 70 pounds total. Eighteen months after this surgery the swelling had gone down enough for Dr. Kristin Moe, MD, F.A.C.S. with the University of WA, to perform a cranial nerve graft (CNVII to CNXII) in the hopes of restoring nerve function
ARTICLES and reinvervating the paralyzed side of my face. Last year, about four years after this surgery, I began noticing nerve function in the right side of my face. I have spent the last year in intensive physical therapy developing the atrophied muscles in my face and neck, which includes facial, and swallowing exercises, even running electrical current through my face with a TENS Unit. Working atrophied muscles is a lesson is pain and frustration, but much easier than the other aspects of treatment I’ve faced to be sure. I’m really encouraged by my progress and seeing my face change almost daily, and I’ve been able to gain forty pounds of the weight I lost during treatment back. I’ve recently been notified that Nobel Biocare is willing to donate dental implants for me to finish my facial reconstruction.
With a full set of teeth and continued physical therapy I’m hopeful to be able to enjoy more solid food. My team and I believe that being able to chew more will continue to improve my swallow function, and improve the facial musculature and symmetry, all important to Quality of Life. So six and a half years later I’m still in recovery, I suffer from numerous side effects from radiation, the drugs used in treatment, as well as surgery. Residual Pain, chronic fatigue, hearing loss, xerostomia, tooth decay, dysphagia, and somedays even walking is difficult due to the tissue and bone taken from my leg are issues I deal with, however I’m thankful to be alive and with my family. I am left to wonder if I would have had as hard of road if my cancer had been diagnosed months earlier

My wife and I have since focused our efforts to raise public and professional awareness about Oral Cancer through helping with Awareness Events like the Washington Academy of General Dentistry’s Gala/Masquerade Ball “An Evening to Conquer Oral Cancer”, on Oct. 3rd, and the Seattle Oral Cancer Walk on Oct. 11th in Magnuson Park. We hope that we can help save other families from having to go through an experience like ours with the help of our friends in Dentistry.

Volunteers Needed for a Massive Free Health Clinic at Seattle Center Save the Dates: October 23 & 26, 2014 Washington Academy of General Dentistry is joining with more than 25 other organizations from around the State of Washington to host a massive free health clinic to treat underserved and vulnerable populations, as well as connect them to area resources that can enable continuity of care. In partnership with the non-profit, humanitarian organization Remote Area Medical (RAM), and under the guidance of local colleagues Dr. Jeffrey Parrish, Dr. Michael Karr and Dr. Carl Youngquist, the clinic aims to fully-staff 60 dental operatories per day. Although this is the first time RAM has been invited to our state, they are not new to operating large-scale, volunteer-driven clinics. RAM was founded in 1985 and initially focused on providing healthcare access to people in remote areas of the world such as Haiti and Guyana. In 1992, they were invited to hold a clinic in Appalachia and from there other rural and urban areas began requesting their support. RAM has since held over 700 clinics in the United States and served over 500,000 patients.
A clinic such as this is a true collaboration between RAM and a Host Community. RAM provides a tremendous amount of infrastructure, especially in dental and vision areas. Then they follow the lead of the local professionals, selected by the Host Community, who determine most of the healthcare offerings and protocols, as well as manage the on-site delivery of care. Add to that the generosity and dedication of over 300 dental, vision, medical and support volunteers each day, and a clinic can make an incredible contribution to the lives of many people in need, as well as to our community. We encourage you to be a part of the October clinic! RAM provides everything the volunteer dental professional needs including the dental equipment, tools, medicines, and chairs. We’ll also provide meals and parking. All you do is show up and help and we take care of the rest. The clinic needs Dentists, Oral Surgeons, Hygienists, Dental Assistants and Dental X-Ray Techs, so feel free to bring your staff. Volunteer for one day or all four, we’d greatly appreciate your participation. Don’t forget to encourage other dental colleagues to join you.

Program Approval for Continuing Education (PACE) AGD Credit for Study Clubs, existing or new, Dr. Linda Edgar wants to hear from You. Each year, thousands of continuing education courses are presented by hundreds of program providers—dental schools, dental societies, and companies that specialize in course presentations. Most provide dentists with valuable information that can be successfully integrated into the dental practice. The Academy of General Dentistry (AGD) Program Approval for Continuing Education (PACE) was created to assist members of the AGD and the dental profession in identifying and participating in quality continuing dental education. PACE is an evaluation of the educational processes used in designing, planning, and implementing continuing education. Approval by the AGD does not imply endorsement of course content, products or therapies presented, nor does this approval imply that a state or provincial board of dentistry will accept courses for re-licensure. Approved program providers are expected to comply with all relevant state and federal laws. Continuing education offered by approved program providers will be accepted by the Academy for Fellowship/Mastership credit. PACE reviews and approves program providers, not individual speakers or specific courses content. The applicant may be a major unit or department within an institution. To be eligible for Program Approval for Continuing Education the following criteria must be met: 1. The program provider must have been offered a planned program of continuing dental education activities for at least 12 months. 2. The program provider must ensure that all courses offered have a sound scientific basis in order to adequately protect the public. PACE reserves the right to require acceptable substantiation from providers that their courses have a sound scientific basis, proven efficacy, and ensure public safety. 3. The program provider must ensure that the educational methods and the facilities selected are appropriate to accomplishing the stated objectives of each activity. 4. You MUST NOT be representing a company. 5. You cannot provide these courses out of the state of Washington

Korey R. Korfiatis Editor: Korey, tell us about how marketing has changed for dentists over the past several years. Korey: Most of us remember that just a few short years ago, marketing
your dental practice was considered a sign by your peers that your practice was failing. Those days are long gone. In fact, it’s now the opposite. Our company is excited to see practices adopt marketing as a viable business strategy. Unfortunately, many practices are unsure where to start and jump on the generic dental marketing bandwagon yielding less than desirable results.
Editor: So, are you saying that dentists need to use new media versus older tactics? Korey: Yes, and no. It isn’t just the media landscape that has changed but more importantly the patient decision making behavior. Consumers who are looking to find a new dentist have access to more information and options than ever before in part due to social media and the vast internet information. Simply put, todays patient is way more savvy and has more options.
Editor: Like online reviews for example? Korey: Well, sure. Online reviews are one of the components, but I am still talking about the overall picture.
Dentists tend to get very granular in their decisions and get hung up on media choice early on in the process. They will often choose the media before ever considering the message or the tactical strategy. This type of decision making often results in multiple vendors working on a practices marketing each with their own individual agenda. What is left is a fragmented program that lacks consistency. And an in-consistent message leaves doubt in the consumers mind. This is a problem because the consumer is less trusting of marketing than ever before.
Editor: So are you advocating a single source to handle the marketing? Korey: In a perfect world yes. But that isn’t always feasible for smaller businesses like dental practices. It can
be more cost effective to outsource to more than one company. But what I am advocating is a solid strategic plan and proper management of the marketing. The investment that is made in the strategy will inevitably outweigh the lost revenue that results in a program with no plan. It’s vital to understand that the media mix you choose is only a small fraction of the overall marketing puzzle. The entire patient life-cycle and their experiences along the way must be strategized in order to make a marketing plan effective.

Korey: We start with brainstorming with the dentist in addition to a unique questionnaire to get to know the
practice. We then share real world example scenarios to demonstrate why we need them to think and market differently.
Editor: Do you have an example of a scenario you would use? Korey: Sure. This is a common external marketing scenario I use: The scenario: A well-established dental practice enjoys high praise from their patients. Naturally, they want to convert customer satisfaction into referrals. They understand internal marketing is the key to achieving this. The problem: Why aren’t they executing? The simple fact is, most teams don’t know how to properly ask for referrals, track them, reward them, and build “buzz” in the community. For this team, although they were motivated, other duties came first leaving marketing last in line.
