Build a Better Birth It’s All About Options in Maternity
By Katie Saltz
T
his past year I went from being the mother of one, to the mother of two. Even though both my children were born in Lexington, the two experiences were as different as night and day. I used different care providers, different hospitals, and had many changes in my birth plan. Changing things up allowed me to have a much more positive birth this time around. What I love about producing Baby Bump is that it empowers moms to discover their options, even if it means changing your mind. Whether you are a first, second, or seventh-time mom, you have choices at your disposal. This year, we’ve written about what acupuncture can do for pregnancy, how placenta encapsulation works, recipes for those “super-foods,” and quirky baby names straight from Hollywood. Check out the Resource Guide in the back of this issue. We give you contact information for care providers, baby stores, fitness classes, support groups, and much more. At Baby Bump, we celebrate how every pregnancy is unique and encourage you to create a birth plan tailor-made to you. Because pregnancy is not a “one size fits all” experience. Y Katie Saltz is the editor of Baby Bump and mother to CeCe, 5, and Ezra, born in August. She writes a monthly column for Lexington Family Magazine about the exploits of her children, and the mommy blog mynewheartbeat.com.

Acupuncture & Pregnancy
An Alternative Therapy for Prenatal Health By Katie Saltz
F
rom fertility issues to decreasing morning sickness, acupuncture may be the key to many pregnancy woes. Acupuncture is a 3,000-year-old holistic form of medicine where solid stainless steel needles are inserted into certain areas of the body to address various physical conditions. Jacqui Kinzig, L.Ac., Dipl.Ac. (NCCAOM), said because acupuncture uses the body’s own healing system it is considered safe during pregnancy. Sessions involve a lot of communication and assessment before the needles are even placed. “During a treatment, the practitioner will do a thorough intake before checking the mom’s tongue and pulse to help gauge what imbalances are present in her body,” Kinzig said. “The needles are then inserted and the patient lies comfortably for 15-30 minutes.” Needles are a common fear, but Kinzig said the process is not painful like many people might think. “Most people do not feel the needles go in and describe acupuncture as very relaxing,” she said. “In fact, many people find that they 8
actually fall asleep during a treatment.” Treatable conditions related to pregnancy include fertility issues and decreasing the change of miscarriage, Kinzig said. “Acupuncture helps bring balance to the endocrine system, regulate menstrual cycles, increase circulation to the reproductive tract, and improve sperm quality in men,” she said. “Recent studies show that regular acupuncture can boost IVF and IUI success rates by up to 60%.” For patients who are currently pregnant, morning sickness, constipation and various body pains are all addressed by acupuncture, Kinzig said. She also reports it can help turn a breech baby, induce labor, and increase milk production and energy levels postpartum. Caitlin Clark of Georgetown began

acupuncture treatment before her first pregnancy to treat back pain. She also suffered from severe migraines, which were treated with Botox injections. When she became pregnant, the Botox was no longer a safe option so she discovered how acupuncture could help her headaches instead. Clark goes weekly for her treatments for relief from the back pain, headaches, and even for helping with anemia. Any first-time jitters were quickly calmed. “I was anxious the first time,” she said. “After the first experience I had no idea why I was so nervous in the first place.” The most common side effect of acupuncture is bruising, due to increased blood volume in pregnancy. Kinzig said eating prior to treatment is also important since it can lower
blood sugar. “Certain points on the body should be avoided during pregnancy, including points on the low back and abdomen,” Kinzig said. “A welltrained professional will be aware of all contraindications and will keep the mom and baby’s safety in mind at all times.” Many medications or therapies can affect an unborn baby, which is why Clark was happy to learn that acupuncture is available to pregnant women. “When you are pregnant, you have options and this is a great one,” she said. “You don’t have to worry about putting your baby in danger. That’s what every mom wants.” Jacqui Kinzig has a Master’s degree in Acupuncture and is Board Certified by NCCAOM. Y

Placenta Pills
Why Moms Are Saving Their Placenta After Childbirth By Katie Saltz
Y
ou learn in any childbirth class or book about the placentathe organ that provides nutrients to an unborn baby. But many women are holding on to this organ after childbirth for their own health. After the baby is born, the placenta is also delivered and usually treated as medical waste. But some believe that ingesting the placenta can help with breastmilk production, increase energy levels and decrease the risk of postpartum depression. A popular way to ingest the placenta is in pill form- it is prepared, dehydrated and placed into capsules for consumption. Julie Six, owner of The Birth Haven in Lexington, provides encapsulation services and said the process is very safe when performed properly. 10
The placenta specialist should provide a cooler, transport kit and spill kit before birth. “Other than the placenta being placed in the provided bags, there is nothing more that the doctor, nurse or midwife needs to do,” she said. Preparing the placenta can be done in the mother’s own home, so the placenta never has to leave the mother’s sight. Once the two-day process is complete, Six said the pills are similar to any other a mother might take for health reasons. “Placenta capsules truly mimic the look of an herbal supplement and each capsule is wiped individually to lessen any taste or smell that may come through,” Six said. Julie Roberts-Solon of Richmond said she experienced postpartum depression after the birth of her first child. When she became pregnant again, she wanted
baby bump 2016
“...instead of feeling like a zombie, I felt refreshed and able to take care of my newborn...” to try something besides traditional medication to prevent the PPD from coming back. She had heard about placenta encapsulation during her first pregnancy and gathered more information from her doula. “When I became pregnant with my second child I knew I wanted to try something different and natural to help prevent the postpartum depression,” Roberts-Solon said. “(Placenta encapsulation) was affordable and easy to set up so I knew it was worth a try.” Roberts-Solon noticed a big difference in her mood after taking the capsules. “The first thing I noticed was that I had an unexplained amount of energy to get me through each day,” she said. “On days that I forgot to take my capsule I was noticeably sluggish. “Even though I didn’t get a full night’s rest for at least six months after my son was born, instead of feeling like a zombie, I felt refreshed and well able to take care of my newborn and my 18-month-old daughter.”
T
he idea of ingesting an organ from your own body keeps plenty of women from trying placenta encapsulation. Six believes women who are made queasy by the thought would probably not feel that way if they knew the
difficulty of postpartum depression. “The ick factor is real and I completely understand it. Those who have struggled with depression or a rough recovery move past the ick factor really fast though,” she said. Some circumstances would make placenta consumption unsafe for the mother, such as uterine infections or improper handling of the placenta. A care provider should help rule out any health reasons a placenta would not be safe for the mother to take with her. The cost for encapsulation services is $300 at The Birth Haven, and the number of capsules created depends on the size of the placenta. Roberts-Solon said she recommends that expectant moms at least take the time to research encapsulation. When comparing her postpartum experiences, she called the difference with taking placenta capsules “astounding.” “I think the best thing was that I just felt like a normal person, which I recognized as a miracle because of the terrible experience I had with my first baby,” she said. Julie Six is a trained Postpartum Placenta Specialist through Placenta Prep, certified in blood borne pathogens and has completed a course on OSHA guidelines for placenta encapsulation. Y

Advice From the Crowd on Etiquette for the Expecting Gift buying is one way your friends and family might celebrate a new baby on the way. But what is the polite protocol for baby gifts and showers? We asked our Facebook friends and here is what they said:
How Long Do I Have to Write Thank-You Notes? Survey said: Until the end of your maternity leave. If you can get started before the baby arrives, that is great. But moms say most people are understanding that life is crazy once a new baby arrives. It’s not impolite to let thank-you notes slip to the back-burner for at least 6 weeks. Mom tip: Keep a notepad with a running list of all the gifts you receive. Jot down the gift and sender as soon as you open it, then cross it off once the note is mailed. Don’t rely on your memory for baby gifts!
What If I Want to Return a Gift? Survey said: In an ideal world, everyone would stick to the registry or attach a gift receipt. But returns are inevitable. Most stores will offer store credit for returns without the original receipt, so you can at least get a gift card to spend on diapers. But you should never ask for a receipt from the gift-giver. Mom tip: If the gift is clothing or a toy, take a picture of the baby with it before returning it. You can show the photo to the gift-giver and they are none the wiser you returned it!
What About “Re-Gifting”? Survey said: Who is going to know? You can always say “I got one of these as a baby gift and loved it, so I wanted to give one to you!” Mom tip: Only re-gift if the item is still in it’s original, unopened packaging.
Can I Host My Own Baby Shower? Survey said: You can, but it’s not rude to check in with your family and friends first. Talk to your loved ones before you send your own invitations. If no one has offered, they may just have assumed someone else was on top of it. Mom tip: Get one-on-one with a female relative or friend and say “I would love a baby shower to celebrate this pregnancy. Do you know if anyone is interested in planning one with me?” Y 12

ny food that is high in soluble fiber can help support a mom’s milk supply and overall health. In this recipe, these are the oats, dried fruit, nuts, ground flax seeds and chia seeds. These cookies make a good substitute for other snack foods. Starting with a basic oatmeal cookie recipe, you can add extra sources of fiber and vary other ingredients to tempt your tastebuds or accommodate dietary restrictions like dairy or gluten! Make a double batch of dough, freeze half of it in 2” wide rolls, and then just cut and bake whenever you need more! Oatmeal Cookie Variations for Breastfeeding Moms 2 c. flour – whole wheat pastry or any combination of flours (if you’re using gluten-free flours, add 1 tsp of xanthan gum) 1 teaspoon baking soda 1 teaspoon salt 1 cup fat – butter, coconut oil, margarine, liquid oil, almond butter, whatever you prefer 1 cup brown sugar 2 eggs (or 2 TBSP chia seeds or

In a large mixing bowl, mix together the fat and sugar until creamy. Mix in the eggs or soaked chia seeds and then the vanilla. Completely mix in the dry ingredients from the small bowl, then mix in the oats, flax, chocolate chips, and fruits and nuts. Refrigerate for a couple of hours, or make into 2” wide rolls of dough on plastic wrap and freeze until ready to bake. When ready to bake, drop walnutsized balls on to cookie sheets about 2” apart. These cookies will not spread much. Bake at 350 for 15 minutes or until golden brown. Remove from the baking sheet and let cool on the rack. After baking, you can also freeze the cookies for later. ‘Hybrid’ Practice Welcomes High- and Low-Risk Patients
K
ristine Lain, MD, derives great joy from accompanying pregnant women on their journey toward childbirth. “It’s a special time in their family’s life. I am honored that we can play a part in that, and hopefully make it better,” says Dr. Lain, a maternalfetal medicine (MFM) specialist at KentuckyOne Health High Risk Obstetrics. She joined the practice Nov. 1. Along with Amy Carrillo, DO, an OB/GYN generalist, Dr. Lain leads what she calls a full-service, “hybrid” obstetrics practice that specializes in high-risk pregnancies, while also screening and caring for low-risk patients. The busy office also includes a nurse practitioner, five clinical nurses, four full-time ultrasonographers and support staff. While women come to the practice for three main reasons — health complications in the mother, in the baby, or in pregnancy/delivery — the practice also features a fetal diagnostic unit, which performs routine prenatal
16
tests for low-risk patients, such as genetic screening and amniocentesis, anatomy scans and third-trimester testing. In addition, Dr. Lain’s office provides ongoing blood pressure and blood sugar management for patients with hypertension and diabetes. Drs. Lain and Carrillo also deliver babies, which is unusual for high-risk practices, Dr. Lain says. About half of KentuckyOne Health High Risk Obstetrics’ patients come from the Lexington area. The other half comes from Eastern Kentucky. Some patients will travel three hours for their appointment because the MFM specialty is not available in rural areas of the state, says Dr. Lain. Dr. Lain graduated from the University of Chicago Pritzker School of Medicine. She completed her residency, fellowship and early faculty work in maternal-fetal medicine at the University of Pittsburgh. For more about KentuckyOne Health High Risk Obstetrics, call (859) 2630141. Breastfeeding
A Guide to Getting Started By Doraine Bailey, MA, IBCLC
M
any moms want to breastfeed, but get intimidated with learning how. Here are some tips to get you started! Before your baby is born, he is practicing sucking and swallowing amniotic fluid. At birth, your baby is ready to put this practice into action. Place your new baby directly on your bare chest to keep him warm and relaxed, and near the nipple so he can latch on. A baby’s stomach is very small at birth; it can only hold 1 – 2 teaspoons of fluid (about ¼ ounce). This small stomach usually empties in about an hour. Babies usually don’t need any water or formula – they get everything they need by breastfeeding frequently. Your baby also has a sleep cycle that lasts about 1 hour. It’s normal for your baby to wake up every hour to eat, have a diaper change, or need other help. Over time, their days and nights will re-synchronize and they’ll gradually have longer times between feedings. Even before your baby is born, your body is making colostrum: breastmilk without the cream, sugar, and water. It is yellowish, thick, and mostly
20
protein and antibodies, just the thing to protect the newborn’s immune system and reduce the risk for jaundice. The amount of milk for the first few days is just right to fill your baby’s small stomach. Around day 3 or 4, your body begins to add in the cream, sugar and water, so your breasts will feel fuller. Your baby is also becoming more alert and proficient with feeding. Your supply will level out over the first few weeks, so you will make the right amount to meet your baby’s needs. Keeping up your supply and keeping baby growing and content means that you and baby will be feeding together at least 8 – 12 times over 24 hours. There will be days where you may feel that all you did was lay around with the baby and breastfeed. This is GOOD because the more you feed and the emptier the breast, the stronger the signals to your body to keep making milk. The more your baby feeds, the faster and more efficient he will become at breastfeeding. Bottom line – if you both don’t do the work, you won’t get the paycheck. If the baby gets a meal from a bottle, that reduces baby’s time at the breast and can change how well he feeds there. If you pump rather than breastfeed, you may get less milk. Separately or together, these can decrease your milk supply. Let your priority be feeding the baby and establishing a strong milk supply. Y

Breastfeeding:
How’s it Going? Here are some signs that breastfeeding is going well:
My baby is over 4 days old and: • Breastfeeds at least 8 times in 24 hours. • Makes swallowing sounds during most of the feeding. • Makes at least 4 dirty diapers in 24 hours. • Has dirty diapers that are lighter in color and not so dark and thick. • Only breastfeeds and does not take formula or other liquids.
Are you expecting? Make sure these two items are on your baby shopping list: • Safe car seat. In Kentucky, it’s the law that children younger than 8 use car seat. Make sure you know how to use and install a child safety seat before your baby arrives. • Safe crib. The crib is the one place your baby will be left unsupervised so be sure to create a safe sleep environment. For more information, visit ukhealthcare.uky.edu/safekids. 6-6784
I delivered more than 4 days ago: • I enjoy breastfeeding my baby. • Latching baby on is getting easier. • After my baby latches on and I count to 10, my nipples do not hurt. • My breasts were larger and heavier after delivery. • My breasts feel softer after I breastfeed. • I believe that it is going well. Here are some signs that you and your baby may need help with breastfeeding. Call your doctor or lactation consultant if you notice any of these things:

Safety
Baby Proof Your Nursery By Chanté Perryman
B
ringing home a new baby is exciting and a memory you will always cherish. But what happens once baby gets a little bigger and more mobile? Is the nursery baby proofed to keep them safe or does it need some attention? Here are a few quick tips to help you keep your little one unharmed! l Cribs should have fixed rails and
no more than 2 3/8 inches of space between slats. Older cribs with drop rails are now banned. To correct this, you can get an immobilizer for it.
l Secure all heavy furniture and
televisions with furniture straps. Do not put a television on top of a dresser – the drawers can be used for climbing. Use furniture corners and edge bumpers on all sharp edges.
l Soft items like pillows, blankets,
stuffed toys and bumpers should never be in the crib or any place where baby is sleeping. Once baby is able to get up on their hands and knees, remove all mobiles and hanging toys. Also, once baby pulls up, move the mattress down. l Keep one hand on baby at all times
when using a changing table. Make sure your changing table has safety straps and a buckle. Don’t ever leave a baby on the changing table alone. 22
l Furniture and cribs should be kept
away from windows. Do not rely on window screens to keep your baby safe. Screens are used to keep bugs and insects out and not children in. Childproof screens and window guards can be installed and have been proven to prevent falls. Keep all blind cords tied and out of reach or cut the ends and attach breakaway safety tassles. l Place outlet covers on all exposed

electrical sockets. Some small covers can be a choking hazard. Get the ones that require two hands to remove, sliding covers, or the cover plates that screw on. And you can always place furniture in front of outlets for more protection!
wound and always away from the crib or choose a cordless monitor. l The best way to see anything that
l Toys should be appropriate for
baby’s age and physical skill. If a toy can fit down a toilet paper roll, it is too small and can potentially be a choking hazard. l Avoid toy boxes with hinged lids
that could slam down. Choose a toy box with a removable lid, ventilation holes or space beneath the lid. l Use cord holders to keep long cords
against the wall. If using a baby monitor, keep the cords tightly
needs to be baby proofed is to get on the floor – on baby’s level. Get down on your hands and knees and crawl around! What is in reach on their level? You will be amazed with what you find close to the ground!
