f I had my way, this magazine would have been 200 pages long. That’s because the Bluegrass is experiencing so many exciting events and developments in maternity care. And we’ve kept up with it for you. In addition to our usual articles about pregnancy health and baby gear, we’ve been building relationships with local maternity resources. Weight Gain & Exercise In Pregnancy By Ramon Thomas, MD

E

at up. Remember, you’re eating for two now!” Many people think it’s reasonable for women to gain larger amounts of weight in pregnancy for the sake of the baby. In reality, excessive weight in pregnancy increases the risk for large infants, C-section, gestational diabetes and pre-eclampsia. Gaining appropriate weight is important for your health during pregnancy and beyond. The American Congress of Obstetricians and Gynecologists recommends appropriate weight gain based on your pre-pregnancy body mass index (BMI): underweight (BMI <18.5) 28-40 lbs normal (18.5-24.9) 25-35 lbs overweight (25-29.9) 15-25 lbs obese (>30) 11-20 lbs For extremely obese women (BMI >35), new recommendations show slight weight gain (<10 lbs) and some weight loss (up to 11 lbs) can improve risks from pre-pregnancy obesity.

Pregnancy is the perfect excuse to start exercising. Physical activity during pregnancy should focus on aerobic exercise (walking, swimming, and bicycling). Light strength training, core and flexibility exercises can also be included. In the absence of certain complications, the ACOG recommends all pregnant women engage in moderate exercise for roughly 150 minutes per week, which can help avoid excessive weight gain. Weight management and exercise routines should be developed with physician supervision. Talk to your physician to determine the best routines for your unique needs. Y

New Group Helps Women Know Their Birthing Options

I

mprovingBirth.org Lexington started out this summer as a small group of local moms wanting to improve maternity care. We knew families were interested in safer birth options and better care, but I think we were as surprised as anyone when 400 people came to our first rally and maternity fair this past Labor Day.

That kind of turnout confirmed what we already had been thinking: There is a real need and desire for change. The decisions families make about their care are only as good as the information they have. Too often, consumers are shopping blind when looking for providers. Most of us get our information about our doctor from that doctor – and that’s not the best way to make a decision about any kind of service. Or, we go on a recommendation from a family member or a friend who can speak to that provider’s bedside manner. Still most friends lack the insight on how evidencebased his or her practices are. For the average woman, it can be difficult to get independent, factual information on how individual providers practice.

For example, if you are seeking a birth based on best evidence, it’s important to know if your provider places time limits on your labor or “requires” continuous monitoring even for low-risk women. We at ImprovingBirth.org Lexington are addressing this issue head-on in an exciting new project meant to bring transparency to Lexington maternity care. We will continue to talk to moms and compile data about birth practices and policies at local facilities, with the goal of making that summary public in mid-2014. Currently, at our web site, you can find information about C-section, VBAC, episiotomy and artificial breaking of water rates for area providers. It will be a “by moms, for moms” tool for women to make informed decisions. This year, we had wonderful responses

Water Birth Coming to Lexington?

mprovingBirth.org Lexington is working on bringing water birth to Lexington. Although a couple of hospitals in the area have tubs, they are for labor only – but actually giving birth in water is supported by evidence as safe and beneficial. We have already had positive feedback from one local hospital about offering this option, and we’ll be asking all of the hospitals to consider updating their policies to make water birth a possibility for moms who want it.

Taking Your Pregnancy To Full Term

D

ates bear striking significance. We anticipate the fourth Thursday of November, the 25th of December, and of course, the day that marks the 40th week of pregnancy. But how significant is that date, really? The latest research indicates that the average length of pregnancy for a firsttime mother is 41 weeks and one day. For births after the first one, the average pregnancy is a few days less. But the key word here is “average.” Average is just a middle spot in a range of “normal.” The latest research shows that this range is as wide as five weeks. Unfortunately, many women don’t know this. A 2009 survey published in “Obstetrics and Gynecology” found that many women had no idea what full-term actually meant. When asked when a pregnancy was “full-term,” only 25% responded 3940 weeks; more than half thought the answer was 37-38 weeks and 24% said 34-36 weeks. This is a far cry from 41 weeks. Here are the current standards, which are endorsed by the American Congress of Obstetrics and Gynecologists and the

Society for Maternal-Fetal Medicine: Early term: 37-38 weeks & 6 days Full term: 39-40 weeks & 6 days Late term: 41-41 weeks & 6 days Post term: 42 weeks & beyond Why does this matter? Research shows that 15% of NICU admissions in the U.S. are caused by early delivery of a baby by induction or C-Section. Babies born before their bodies are matured enough to function optimally outside of the womb are at a heightened risk for breathing difficulties, feeding problems and an inability to maintain body temperature. In other words, going into labor spontaneously is your best chance at keeping your baby out of the NICU and in your arms after birth. Will you allow your baby to determine his or her own birthday? Remember, in the absence of complications, research supports spontaneous labor as safest and best. Y

Level Three NICU at the Women’s Hospital at Saint Joseph East Provides Specialized Care While Keeping Infants Close to Parents

W

hile the majority of infants are born healthy everyday, there are times when a baby is born prematurely or with an unexpected medical concern. It is during these times when the care of a level three neonatal intensive care unit (NICU) is critical. The Women’s Hospital at Saint Joseph East, part of KentuckyOne Health, is home to a level three NICU that provides specialized care to premature and ill newborns. The expertise of a level three NICU within the hospital allows ill or premature infants to remain close to their parents, while still receiving the care they need to grow and thrive. Neonatologists, nurses, physicians, respiratory therapists and other medical professionals who staff the level three NICU at the Women’s Hospital are specially trained to ensure infants are stabilized and all of their medical needs are met.

In addition, the staff is trained to help parents to bond with their babies despite the circumstances. “The bond between a newborn and its parents is very important to the child’s wellbeing,” said Khaldon Jundi, M.D., a neonatologist at the Women’s Hospital. “Specialized care that keeps premature and ill infants close to their mothers is ideal for both the parents and the child.” When an infant is so ill that touching or holding her is a challenge, bonding can be promoted in other ways. For example, a baby recognizes its mother’s smell and knowing its mother is close can help promote healing. Bringing the mother’s hands into the infant’s isolette and encouraging other interaction promotes mother-baby bonding even during a difficult situation. exington moms may not be able to hire Bravo TV’s maternity guru Rosie Pope as their own baby planner, but luckily, Chante Perryman is here to help. Perryman recently launched Baby Dreams Maternity Concierge, a fullservice baby planning business located in Lexington. As the mother of 2-yearold Zoe, Perryman feels that she knows what an expectant mother needs. Perryman formerly worked in the hospitality industry with a focus on event planning. After she watched a few episodes of the Rosie Pope show, “Pregnant in Heels,” Perryman realized her skills could help parents during pregnancy. “The ‘a-ha’ moment was when I was sitting on my mom’s couch while she was Nana-sitting,” she said. “I remember thinking I could have really used some professional assistance during my pregnancy with resources and information. “I really wanted to pursue it as a career to comfort and prevent women

from going through the ‘I don’t knows,’ ‘Who can help me?’ and ‘Where do I go for this?’ moments.” As a maternity concierge, Perryman offers services such as car seat installation and education, nursery organization, hospital preparation, product research and demonstrations, registry creation and more. The average cost of a baby planner ranges from $30 an hour to a flat fee of hundreds of dollars, Perryman said. Baby Dreams offers a free first-time consultation. “In the end, using a maternity concierge can save parents money from purchasing items they may hardly ever use,” she said. Special circumstances such as bed rest often raise the need for a baby planner, but all parents could benefit from a helping hand, Perryman said. “Parents don’t have to feel overwhelmed,” she said. “With more couples working fulltime careers, sometimes planning for a baby can be stressful.” Y

Shopping for Baby

One blanket statement is true for all babies: they are expensive. Cost-conscious parents might seek secondhand stores to find baby products, but babies are fragile and sensitive, so a few careful purchasing guidelines are essential. Some products can be bought pre-loved while others must come fresh from a store shelf.

Buy New:

Buy Used:

Car Seats. A used car seat will never come with an accident report to detail its history, which could compromise your child’s safety.

Clothes. After a careful inspection for stains, tears and signs of excessive gnawing, a thorough washing will make them ready for baby.

Cribs. The U.S. Consumer Product Safety Commission has seen a strong decline in crib-related injuries, but many of the accidents that still occur are due to outdated safety standards. The bar for product security is always rising, so the best thing you can do for your baby and your own peace of mind is to stay current.

Bedding and Nursery Décor. Precious as they may seem, these items will be short-lived in your house. Wash or sanitize them and save your money for other necessary things.

Infant Carriers, High Chairs, Swings. These items come with a number of essential pieces. To ensure you have them all, buy new.

Toys, Puzzles, Books. Check battery compartments for signs of erosion before buying anything electronic.

If you’re not sure about an item you have purchased secondhand, always look it up for recalls and a diagram of the necessary pieces.

Recycling your own stuff also works. Redeem your used paperbacks for board books at a resell book store. Baby outgrew his or her summer clothes? Trade them in for a nextsize-up wardrobe. The Birth Haven Is Kentucky’s First 24/7 On-Call Doula Service

F

rom the first pregnancy test to the first week home with a newborn, every mother needs support in her pregnancy journey. Giving that support is the mission of The Birth Haven, Kentucky’s first 24/7 on-call doula group. A doula is a professional who provides emotional, physical and informational support to the entire family during pregnancy, birth and postpartum. Doulas are not medical professionals but rather act as a resource liaison and personal continuous support system. Research shows less unneeded intervention, lowered surgical rates and higher rates of satisfaction when doulas are part of the childbirth experience. Julie Six, owner of The Birth Haven and a doula for eight years, knows the importance of a childbirth advocate. In 2008, Six and her family moved to Lexington. She met other doulas through Baby Moon and began to teach Hypnobabies childbirth classes.

ut as her business grew, she knew she needed her own space to pursue her vision. “I wanted to bring a group of experienced, well-respected doulas together to provide back-up services for each other,” Six said. The group currently consists of six doulas, and once a month, The Birth

Haven hosts a “Meet the Doulas” speeddating event at 2891 Richmond Rd., # 206. Families can attend for free and take a few minutes to meet each doula. Families can either choose to hire a particular doula or stay within the oncall rotation of a certain group. Instead of at-home visits, families attend “Centering Visits” at The Birth Haven. The visits are held twice a month, and families can attend as many as they want throughout pregnancy. “We’ll talk about the creation of a birth plan, how to choose the right childbirth education class, fears in pregnancy, along with hosting occasional guest speakers,” Six said. A common misconception of doulas is that they are meant only for women seeking natural, unmedicated childbirth. Not so, said Six. “Our doulas believe, ‘It’s not our birth. It’s yours.’ Every woman needs support in her birth journey no matter what choices she makes,” she said. The fees for the group range from $400-$900, depending on the doulas experience level. Learning to Make Breastfeeding Work By Doraine Bailey

A

s more and more moms start out breastfeeding, it’s important to note that breastfeeding, like any new skill, takes practice. It also takes support from knowledgeable people. Too often, moms blame themselves when breastfeeding hits a snag. Moms receive confusing information or they don’t understand their baby’s cues. Rather than seeking help, too many moms quit. Getting help doesn’t mean a mom has failed – it means she is getting the answers and support she needs to succeed. Choosing where to go for help is important. Surround yourself with people who understand breastfeeding and have had positive breastfeeding experiences. The Resource Directory at the back of this magazine has many places to go for breastfeeding help. Plus, you can also call the Breastfeeding Warm Line and speak with a Lactation Consultant at (859) 288-2348. Sometimes getting help also means connecting with others who have nothing to do with breastfeeding but can help out with other day-to-day tasks. In the early days, a mom’s job is to recuperate and feed

the baby. When you start stressing out about the laundry, dishes, meals, thank-you notes and Facebook statuses, it can interfere with the job of mothering a new baby. You will lose sleep, miss meals, and can become grumpy and impatient just taking care of yourself and Baby. Get help with household tasks, meals and care for other children so you can rest and breastfeed. As your baby grows, your breastfeeding questions may change. That’s normal. Continue to make connections with breastfeeding moms, lactation consultants and on-line forums to help you navigate returning to work, pumping, keeping up your supply and starting solids. If you feel breastfeeding is not going well, don’t delay in asking for help. The longer you wait, the harder it can be to make things better.
