Dr. Martinez is the only surgeon in SW Florida who utilizes patient specific instrumentation (PSI) while performing a reverse total shoulder replacement.
HIGH TECH SHOULDER REPLACEMENT W hen it comes to shoulder surgeries millimeters count, and surgeons have one shot to get it right. That’s why Dr. Robert Martinez spends countless hours on the computer, tweaking measurements and practicing certain procedures before going into the operating room.

Dr. Martinez specializes in treating and repairing shoulders at Gardner Orthopedics. He is the only surgeon in SW Florida who performs the “reverse total shoulder” operation using patient specific instrumentation or PSI. The modern reverse total shoulder originated in France as a way to treat patients with a broken or fractured humeral head, or shoulder. Dr. Martinez began doing the same locally, then gradually expanded the procedure for use in shoulders with torn rotator cuffs where arthritis has also set in. The surgeon explains, “The major reason that regular artificial shoulders don’t work in patients as they get older, is that the rotator cuff wears out allowing the ball to come up and hit the collar bone.” Dr. Martinez is one of only a few orthopedists who have not only embraced, but also regularly performs the reverse
shoulder saying in amazement, “It looks crazy, but it works.” In this surgery, he places a socket where the ball used to be and a ball where the socket used to be. The geometry of this backwards-looking apparatus allows the shoulder to pivot and use the deltoid alone to lift the arm. The surgery is quite unique and requires a lot of time, ef for t, and planning before going into the operating room. The process begins with a CT Scan that is then sent to a company in Europe, which takes anywhere from 4-6 weeks to generate a 3D model of the patient’s shoulder. They also provide a 3D guide that directs Dr. Martinez in the exact placement of the guide pin. Success of the surgery is dependent upon getting this pin in the right spot. If the prosthesis is not aligned properly the shoulder still won’t work properly after surgery. Dr. Martinez says, “I sit at my computer and go into the 3D model electronically to tweak the position of the prosthesis. In the operating room I put in the guide pin and all my instrumentation revolves around it. It gives me so much peace of mind when I do the surgery.” Use of the 3D model also allows Dr. Martinez to see and correct other deformities. He explains, “Arthritis in the shoulder acts like sand paper, with the two bones rubbing together. Because the shoulder blade is smaller and thinner it wears out. If you put a prosthesis where bone is worn out, the shoulder is unstable.” In this instance, the surgeon has to perform a bone graft. He says, “I create real estate, adding bone so I can put the prosthesis where it should be for best function.” The bone used for the graft comes from the ball of the shoulder. It has to be removed for the surgery, so Dr. Martinez says, “Instead of getting bone from the bone bank or throwing this piece of bone away, I have what I need for the graft right in front of me. It’s the patient’s own bone, it’s alive, and the healing rate from this is 94%.”
Rehab is a little slower for patients who have bone grafts, as it takes time for the graft to attach to both the existing bone and the metal prosthesis. However, within 5-6 months after reverse shoulder surgery, patients can resume most of their normal activities. They have relief from their shoulder pain and wonder why they waited so long to get help. If you are suffering from shoulder pain and have limited mobility, it’s time to get some relief. Dr. Martinez was born in Cuba where his father was a professor of orthopedic surgery at the University of Havana. As a teenager, the budding doctor would help his father in surgery as a scrub tech. Dr. Martinez went to medical school at Southwestern Medical School in Dallas and received his orthopedic training at the University of New Mexico. Could Vein Management BE THE ANSWER TO YOUR

enous disease affects about 20% of the U.S. population, about 40 million adults according to Dr. Joseph Magnant, founder of Vein Specialists. He says, “Venous disease is a medical condition like hypertension and high cholesterol.” But the problem is, the majority of people suffering from venous disease don’t realize they have it. Those who are experiencing symptoms of leg discomfort often seek help from pain management professionals, dermatologists, and orthopedic surgeons rather than consider the possibility of vein disease as the cause. Take for instance John Ganos. He was undergoing orthopedic treatment for a number of problems with his legs, knees, and feet. He was in a lot of pain and thought things couldn’t get much worse. “Then I developed a burning in my right leg that did not seem to go away. It was new and different from all of the other pain,” says Ganos.

Looking for answers, Ganos went to a neurologist who was unable to identify any neurologic problems. Then his orthopedist suggested a visit to Dr. Magnant. “I had not even considered the fact that there might be an issue with my veins.” When Dr. Magnant met Ganos, he remembers, “I wasn’t really sure what our chances were of helping him. His symptoms were not classic for vein problems.”

Ganos says, “I really didn’t see my veins protruding much, but apparently there was a problem.” He tried everything to get relief; medications, herbal remedies, but nothing worked. “My legs were heavy, they were dead, they were burning, and I was worried I would lose all use of them before much longer.” An ultrasound per formed by Vein Specialists revealed Ganos had leaky veins. Dr. Magnant later performed an endovenous ablation in which heat is used to seal the veins and stop the leaking. It worked. “The effect was immediate,” says Ganos. “The pain from the burning went away. And after my veins were fixed, there was an improvement in circulation that I think has really helped to relieve many of my other symptoms.” So many of Dr. Magnant’s patients come from orthopedists that he has coined the term “orthovenous disease.” He explains, “Many patients with venous disease are being assessed for knee or hip arthritis. In the process their physician often may notice varicose veins, swelling, or discoloration of the skin. If the x-rays don’t suggest an orthopedic cause of their leg symptoms, or even if they do have arthritis and vein disease, it may be appropriate to evaluate the venous side of the equation.” Taking care of venous issues, before or simultaneously to orthopedic treatments, can enhance healing, improve compliance with physical therapy and reduce perioperative risks such as wound complications or deep vein thrombosis (DVT).
He also recommends therapies such as compression stockings. “If a compression hose trial significantly improves their pain, it is a predictor of a good response to vein ablation therapy.” Venous disease is part of the total health and healing puzzle according to Dr. Magnant. He says, “Have an open mind and consider alternatives to the management of your leg pain. The opioid epidemic is real and if there is something we can do to fix the underlying problem without resorting to prescription pain meds, then it’s a win-win.” Ganos says, “My legs feel so much better. I can get up easier and climb stairs. Dr. Magnant made a huge contribution in my attempt to regain functionality in my legs. Modern treatments are so advanced and it is so easy to fix venous disease. I recommend to anyone having problems they can’t fix, to look into it.”

Of his seven siblings, all but one is a medical professional and all graduated from the same medical school: Medical College of Virginia. o your eyes burn, itch, or water excessi vel y? You may be suffering from dry eye. This is a condition that is more than just irritating; it can damage the eye if left untreated according to Dr. Nicole E. Alessi of Collins Vision. “Dry eyes are a really big deal in Southwest Florida,” says the Florida native. She was born in Fort Lauderdale and was initially inspired to help people with their vision after watching her grandmother slowly loose hers. But she’s extremely passionate about helping people with dry eye because it’s a problem Dr. Alessi lived with for years. “I don’t want my patients to suffer like I was suffering.” Her vision changed while she was attending optometry school at Nova Southeastern University. able to work with exceptional people and it stemmed my passion.” Dry eye is a chronic disease process. “Not addressing it will slowly deteriorate the surface of the eye,” explains Dr. Alessi. “The top layer of the cornea will become irregular and distort the quality of vision.” It occurs when there is a problem with the tear film, a multi layer structure that protects your eyes. To help maintain the integrity of your tears, small glands secrete an oily substance. Those glands can become obstructed and in need of expression so they can work properly. But that is just one cause. There is a not a one size fits all treatment for dry eye. Dr. Alessi does a comprehensive analysis that includes looking at a patient’s lifestyle and when the symptoms tend to occur. She says, “The solution could be as easy as redirecting the vents in your car. If you

are waking up with irritation it might be due to dust mites, so you would just need a humidifier.” Small mites, or an overgrow th of bacteria, can also live along the lash line. In this case BlephEx is a common treatment, a procedure that could be described as an exfoliation of the eyelids and lashes using a micro-sponge. Patients with this see an improvement in symptoms almost immediately. Allergies are another dry eye culprit. At Collins Vision Dr. Alessi is able to do a surface test on the forearm of a patient, exposing them to environmental allergens that specifically affect the eyes. Anti-inflammatory drops are often used to protect the surface of the eye so the doctor can then turn her attention to healing the tear film. If you have cataracts or an interest in laser vision correction and have dry eye, treating the dry eye should be done prior to surgery. She explains, “We don’t want to do vision correcting procedures on an eye that is fluctuating.” Dry eye can be exacerbated or induced by Lasik surgery, but can also be managed after the fact. W h e n D r. J a m e s Wo rd e n , a n anesthesiologist and pain management doctor, was having trouble managing his own pain, he knew it was time to seek help. The Naples doctor was suffering from arthritis in his left hip. It progressed over the course of a couple of years to the point where he couldn’t stand too long or walk too far without hurting. Like many people with hip pain, Dr. Worden looked for alternative treatments such as injections before pursuing surgery. He also used anti-inflammatory medications saying, “I reached the point where I was taking them every day. They started bothering my stomach and raising my blood pressure and I was still in pain. I was having trouble putting on my shoes and socks, so I knew it was time to get my hip replaced.” Well connected to medical professionals all over Southwest Florida, Dr. Worden began investigating doctors and various hip replacement procedures. After much research he remembers, “I went to Dr. Gardner (owner of Gardner Orthopedics), an old friend, and asked if he would recommend I do the surgery in the posterior or anterior approach. He explained that there was actually a third way of doing hip surgeries and introduced me to Dr. Ameglio.”
Dr. Peter Ameglio per forms the SuperPATH® approach at Gardner Orthopedics. This procedure isn’t new. It was originally introduced in 2003 and Dr. Ameglio is the only surgeon in Southwes t Florida per forming SuperPATH. Its key lies in not dislocating the hip joint during surgery. In traditional hip replacements a dislocation of the hip is required. This results in injury to the joint capsule and small muscle attachments of the hip. Following the surgery done with traditional approaches, the patient is instructed in hip precautions or positions they should not place the hip to prevent joint dislocation. The SuperPATH approach does not carry these restrictions as the joint is never dislocated. The capsule and muscles are not injured during surgery making for a quicker recovery. Dr. Ameglio explains, “In this approach you don’t have to dislocate the hip joint and therefore it’s less traumatic.” The surgery is easier on the patient, the recovery time is shortened. When choosing to go the SuperPATH ro u te , D r. Wo r d e n s a y s , “ I w a s concerned about all of the possible complications with the other methods; dislocations, nerve damage, infection. You don’t have as much cutting or have to dislocate the hip during SuperPATH, so the hip is in there nice and tight.” In addition he says, “There are no restrictions post op. I can bend over and I can cross my legs without any risk of injury. That was important to me, along with getting back to work. I have a full time practice and didn’t want to be out any longer than necessary.” Dr. Worden spent one night in the hospital and in two weeks had a follow up with Dr. Ameglio saying, “I was very pleased with his expertise. He put a two-week bandage on the incision that was waterproof and had silver to combat bacteria. When he took it off, the wound looked clean as a whistle. It was a nice bonus.”
Dr. Worden’s recovery was quick. He needed very little pain medication. The doctor had his golf clubs on the driving range four weeks later; in six weeks he was playing 18 holes. He was back to work in 4 weeks. About the SuperPATH approach, Dr. Ameglio says, “We are excited to offer this procedure at Gardner Orthopedics. It just makes sense as far as injuries to the soft tissue. By moving the incision a little farther up and not dislocating the hip joint, it makes all the difference in recovery, and patients say the hip feels more natural.” Dr. Worden thought having the surgery in Fort Myers, rather than Naples, was easy and well worth the effort. He says, “I could not be happier with Dr. Ameglio. He is a well-trained surgeon, a good technician, and a thoughtful doctor who listens. I would recommend anyone in need of hip surgery to get a second opinion, but I think the SuperPATH is an awfully nice way to do it.”

most common acute upper extremity injury is a fracture or break of the wrist. This typically occurs when trying to run backwards to return a ball and landing onto an outstretched arm. If you happen to injure your wrist playing Pickleball or any sport, it is recommended you apply ice to the area, however not directly on your skin, and seek an evaluation with a medical doctor. Often wrist injuries are ignored as simple sprains, only to discover weeks later when the pain persists, that there is a more severe injury.
Despite the wonderful attributes inherent to Pickleball, there are unfortunately many injuries that can occur in the upper extremity from both overuse and acute injury. The

About Dr. Fernandez
Dr. Fernandez was born and raised in New Jersey, and is the youngest of two siblings. She was a nationally ranked springboard diver and gymnast, and was awarded a gymnastics scholarship to the University of Illinois. Following graduation, her studies led her to pursue a career in medicine. Dr. Fernandez was a pioneer in becoming the first graduate of the combined MD, MBA program from Robert Wood Johnson Medical School and Rutgers-Camden business school. She went on to complete a five year orthopedic surgery residency at LSU, followed by two additional years of fellowship, one at Yale University in Hand and Microsurgery followed by another year at the University of Pittsburgh in Upper Extremity Surgery. She and her husband Chris moved to Naples in 2016 and are the proud parents of two Chihuahuas, Alex and Phoebe. Dr. Fernandez founded the Fernandez Upper Extremity Institute in 2018 to help serve the people of Southwest Florida.

our boys – Lucio, Sergio, Alex and Daniel – watched their father work long hours, oftentimes 12 hours a day, in the tomato fields of Immokalee while their mother worked as a school custodian. José and Maria Martinez earned enough to put a roof over their heads and food on the table, but with limited English skills and no high school diploma, their career options were also limited.
“They always said that their job was to work hard and make money for the family, and our job was to get an education,” Daniel said. “They wouldn’t settle for any thing less than straight A’s.”
Tutor Corps changed the boys’ trajectory in life. Lucio earned bachelor’s and master’s degrees from the University of Florida (UF) and is a civil engineer in Tampa. Sergio also earned a bachelor’s degree from UF and teaches English to s tudent s in Hong Kong. A lex graduated from Bowdoin College and works in Philadelphia for an organization helping people with disabilities establish their independence.

In fact, 100 percent of Tutor Corps students have matriculated to college over the past decade, and more than 92 percent have earned a degree. By comparison, the national graduation rate for low-income students is just 16 percent, according to the National Center for Education Statistics. “Tutor Corps sets ex tremely high expectations for our students, but it also provides them with opportunities to meet those expectations – mentoring, employment, college success workshops and help applying for financial aid and scholarship money,” said Jorge Perez, Tutor Corps college coordinator. “If they are willing to work hard and apply their talents, these students can attend college, earn a degree and work in the profession of their choice.”
