Dr. Calcagni’s parents began vacationing in the area in the early 90’s. He laughs, “I always wanted to move here, but they thought no one lived here year around. In medical school, I remember being told to move where you want to live, then figure the rest out.” That’s what he did. Of Italian decent, Dr. Calcagni’s surname means “heel” when translated, and his mother’s maiden name is Naples. Add to the fact that there are nine doctors on her side of the family and it’s easy to see how his stars aligned.

In Casale’s case, he had been suffering for months with a non healing foot ulcer on his toe due to his diabetes. In addition his legs were discolored and leaking. Dr. Calgni recalls, “His doctors up north were having trouble successfully treating his toe ulcer. We were able to get things healed in three weeks.” The process involved using proper bandages, compression stockings, a lymphedema pump for his legs, and skin grafts.

About Dr. Calcagni

However, Dr. Calcagni says, “I perform a lot of ankle repairs using scopes and special implants that can have patients moving normally in two weeks rather than six to eight weeks.” Instead of sacrificing a ligament or tendon, he is able to use a special fiber wire that speeds recovery quite remarkably. Stem cell therapy also aids in healing and recovery from injuries and surgery; Dr. Calcagni is one of the few podiatrists in Southwest Florida to use it. Stem cell therapy also eases pain from chronic problems such as arthritis of the foot and ankle, Achilles tendonitis, plantar fasciitis and more.

Dr. Calcagni received is Doctor of Podiatric Medicine degree from Kent State University College of Podiatric Medicine in Independence, Ohio. He is board certified by the American Board of Lower Extremity Surgery and the American Board of Podiatric Medicine. Married for six years he met wife Michelle on a blind date. They have two boys ages two and four. Although he enjoys exercising and travelling, he says “My hobby is really spending time with my family. They are the most important things. I love spending time and hanging out with them.” Orthotics are not just for adults with foot pain but they’re also a part of his growing pediatrics practice. “Most of the children I see with foot pain have bio mechanical problems that can be addressed with the help of inserts,” says Dr. Calcagni.

Plantar fasciitis, another common problem for Dr. Calcagni’s patients, can often be solved in just a couple of visits through any combination of stretching, injections, and professionally fitted shoe inserts called orthotics.

If a parent has f lat feet, bunions, hammertoes, or arthritis, it’s likely that their children will develop these problems as well, according to the podiatrist. “Having children properly fitted for orthotics can help prevent them from developing more serious problems as they get older,” he says.

“Problems with feet start with the biomechanics of how you walk and how the foot is shaped. Custom molded orthotics help correct problems and keep conditions such as plantar fasciitis from coming back.”

E R C Stability The phrase “Core Stability” is used a lot by all disciplines involved in health and wellness. In our twenty years of experience as physical therapists, we have seen that the definition of “the core” is highly variable but most often includes the abdominals. The core muscles attach to, and stabilize, the spine, the rib cage and the pelvis. The muscles combined are thought to construct a cylinder that allows for pressure to be built in the abdominal region that provides stability. The definition of the core from our perspective is as follows: Comprised of the following muscles: Transverse Abdominus: The deepest abdominal muscle. The outside wall of the cylinder. Diaphragm: The top of the core’s Pelvic Floor Muscles: The bottom of the cylinder. Iliopsoas (often referred to as the psoas) Multfidii: Small muscles along the spine. Provides a solid foundation (when efficient) on which the arms, legs and trunk move Requires that the framework (skeleton) is efficiently aligned, that is to say: “posture is KING!” Injury, pain and habitual posture all affect the speed, strength and endurance of core muscles, so it is

important to take care of and be aware of anything that may affect them. The theory of instability of the lumbar spine and pelvis rests, in some part, on the inefficient contraction of core muscles that allow the vertebrae to move excessively and in a relatively uncontrolled fashion causing shearing, compression or tearing forces on the tissues of the spine including the ligaments, muscles, fascia, nerves and blood vessels that provide support and nourishment to the spine. If spinal instabilities are left unaddressed for even short periods of time, degeneration of the spine may occur. Pain and weakness often follow but it may take weeks, months or years to manifest. Nagging, persistent little aches and pains in the back, hips and legs are often associated with these dysfunctions. The theories of stabilizing the spine focus on strengthening the core muscles so that they can prevent the excessive movements of the spine and the “wear and tear” associated with it. Across the spectrum of physical therapy and personal training, one can find a wide variety of exercises directed at strengthening the core. The difficulty is choosing the appropriate exercises to be done at any given time in a training program. Research is now demonstrating that the core is effectively contracted

when our parts (head, neck, rib cage and pelvis) are stacked on top of each other...again, posture is King! When muscles are either stretched or shortened from posture habits such as standing with the back hyperextended and, most commonly seen, sitting like “a boiled shrimp”, they are unable to contract at the correct time, with enough force or long enough to stabilize the spine when we sit, stand or perform daily tasks, much less when we play a sport. Standing straight, as many of us were told to do by our parents, may actually place the rib cage in a slight backward tilt rendering the core less responsive. Learning to stack the rib cage appropriately over the pelvis is one small step to improving the core response. Remember that habits are often deeply ingrained and require frequent corrections. Depending on the severity of the habit, those corrections may be every few minutes, hours, days and weeks. Even when we think that the old habits are broken, we may find ourselves lapsing into the old habitual postures, especially when tired or under stress. When training ef ficient posture for sitting, standing or during sport or exercise, building strength and endurance of the muscles is important. This is the reason that exercises are

per formed. Exercises should be performed with appropriate form and to the point of fatigue of the target muscles. However, when proper form falters, the exercise should be stopped. Depending on the severity of the habit, those corrections may have to be performed every few minutes, hours, days and weeks to implement. A well trained Functional Manual Physical Therapist can help guide you through the morass of posture changes and exercises that you will need to make your core muscles responsive and strong so that you are able to do what you want with better strength and endurance and reduce the potential for injury.

In Minor’s case, Dr. Campbell was able to use a combination of veneers and crowns to restore his entire mouth. He is also able to utilize more technique sensitive preparations such as inlays and onlays when able, saying, “I try for a conservative approach. I just don’t go straight for a crown. Following her firs t appointment she remembers, “I always thought my veins were just a cosmetic problem. I never really knew about vein disease.” Dr. Magnant explains, “Venous disease is a medical condition like hypertension and high cholesterol and deserves investigation and potential treatment before complications occur.” Venous disease affects about 20% of the U.S. population, or roughly 40-50 million adults. Venous insufficiency occurs when valves in the leg veins fail to close, making it difficult for blood to return to the heart. When the blood can’t return, it pools in the legs resulting in increased venous pressure which causes varicose veins, swelling, inflammation, and skin conditions such as stasis dermatitis and venous ulcers. A majority of people who are suffering from venous disease don’t even realize they have it; not all signs or symptoms are as obvious as bulging varicose veins. Some patients may experience nighttime leg cramps, Charlie horses, restless legs or frequent urination, what Dr. Magnant refers to as the “nighttime triad.” Other patients may have skin pigmentation and thickening around the ankle for which they have sought the attention of a dermatologist. While the most advanced stage, venous ulceration might land other patients in a wound care center for many months of wraps and even hyperbaric oxygen tank treatments. What all these patients may share is the underlying condition of venous insufficiency. Venous insufficiency is an easily diagnosed and treatable condition in the hands of a Board-

Certified Vascular Surgeon focused on vein disease management. Spang had an ultrasound of both legs by a Registered Vascular Sonographers, which revealed leaking veins or venous insufficiency. Most of the time the superficial veins are the problem veins which is good news for patients since after treating the superficial veins, blood can still return to the heart through the normally functioning deep veins. The abnormal veins can be sealed by either heat or medical adhesive techniques. These are procedures that Dr. Magnant has performed on more than 28,000 patients in Southwest Florida. He performs this procedure in the office, under local anesthesia and in a sterile environment, and it takes approximately 30 minutes to complete. Most patients can return to normal activity within 24-48 hours. Two weeks after Spang’s first treatment, the same procedure was done on the leaky veins in her right leg. She says, “It was not painful at all. Dr. Magnant explained everything thoroughly and talked to me through the entire procedure. We actually had fun. It was such a positive experience.” After her first vein procedure nearly 20 years ago, Spang’s veins did not completely disappear. Today, thanks to Dr. Magnant, they are completely gone. “It’s a miracle,” she says. arbara Bukousk y was born with hip dysplasia, a congenital hip disorder affecting 1 out of every 1,000 babies born. This deformity occurs when the hip fails to form normally. Barbara was unaware that she had this disorder until her early 30’s. That’s when she took a fall and shortly thereafter started to experi-

ence severe groin pain.“In 1988 I began visiting every doctor I could find in Southwest Florida and around the state, and no one could tell me what was wrong.” Some physicians even dismissed her, suggesting the pain was in her head. In 1991, she traveled to Boston to find a solution and found herself under the care of Dr. Stephen Murphy. Barbara underwent extensive reconstruction surgery to preserve her hip joint, the result of the hip dysplasia. Since total hip replacements typically last from 10-30+ years, this surgery was designed to delay the need for a total hip replacement - which Dr. Murphy informed Barbarba she eventually would need. When that time came little did Barbara know Dr. Murphy, co-inventor of SuperPATH® total hip replacement, would be the surgeon who personally introduced Dr. Ameglio to this cutting-edge technique. Although developed in 2003, Dr. Ameglio is the pioneer of this procedure in SW Florida and is the most experienced SuperPATH surgeon in our area. With a traditional hip replacement, even after the hip heals the potential exists for it to dislocate during normal activities. Therefore, patients are sent home with a list of movement precautions that they must adhere to for 2 months. SuperPATH carries none of these restrictions. This is the only hip replacement where both the hip is never dislocated, and no muscles or tendons are cut. According to Dr Ameglio, “The SuperPATH

It just makes sense and anatomical studies confirm it.” It made sense to Bukousky as well. Over the summer she had her right hip replaced by Dr. Ameglio via SuperPATH and was driving two weeks after the surgery. “I experienced little to no pain,” she marvels. Her hip was back in working order however, about 4 months later Bukousky began experiencing back and groin pain on the opposite side. “I would be up for two weeks and in bed for three, and thought it was time for a hip replacement on the left side,” she recalls. Dr. Ameglio began to investigate the cause of her pain. Although Barbara’s X-ray clearly indicated arthritis in her hip, this time her symptoms were slightly different from the right side. This raised the suspicion that Barbara may be suffering from sacroiliac joint dysfunction. “The symptoms of hip arthritis and SI dysfunction many times overlap making it challenging to diagnose the source of a patient’s pain.” The sacroiliac joint functions as a shock absorber, supporting the entire spine. It dissipates the shock to the sacrum and pelvis; however, with age, it’s less able to absorb shocks and becomes less mobile. SI dysfunction can also present itself after an accident, fall, childbirth, or simply picking up a suitcase.

Problems with the SI joint can result in pain in the buttocks, thigh, groin, and can radiate down the leg. SI dysfunction is not identifiable on X-rays or any imaging study, adding to the complexity of the diagnosis. “This is why understanding the difference is critical in avoiding an unnecessary hip replacement or revision hip replacement surgery”, adds Dr. Ameglio. Dr Ameglio is the only Orthopedic Joint Surgeon in SW Florida treating both the hip and the SI Joint. This gives him a unique perspective in understanding the source of patients’ pain. “15-30% of people with low back pain have sacroiliitis and in patients who have a lumbar spine fusion, up to 45% have sacroiliitis,” says the surgeon. “It’s a sizable number and it is critical to consider this diagnosis.” Dr. Ameglio performed an SI fusion on Barbara’s left side in November. Barbara is now living pain free for the first time in decades. “30 years is a long time to live the way I lived,” says Bukousky. “To be able to get up and walk and not be in a wheelchair is amazing AND I’m not in pain.” Convinced by the success of her surgeries performed by Dr. Ameglio, her husband has scheduled SuperPATH hip replacement with the surgeon. “There’s not too many doctors I trust anymore because of what I’ve gone through,” says Bukousky. “I love Dr. Ameglio, I believe in him, and most importantly, I trust him.” Dr. Ameglio opened his solo orthopedic practice in July with the help of his wife. Patients have embraced his move and the welcoming atmosphere. The surgeon is known for treating the whole person, for being a good listener, and a physician with empathy. “I love caring for others, I believe it’s what God created me to do,” expresses Dr. Ameglio. He and Audrey enjoy the outdoors and just celebrated their one-year wedding anniversary.

Pets & People At Gulf Coast Humane Society

Pets are more than just animals; they are family members for many people. But what happens to them when the family breaks up? When there is divorce or sickness? A financial set back, job loss or relocation? Many of them end up at the Gulf Coast Humane Society (GCHS,) a not-for-profit, no-kill shelter. In 20 1 9 t he shel ter rescued a whopping 2532 animals and adopted 2309 of them. “Many people are surprised to know we’ve been around since 1947 and at our current location off of MLK in Fort Myers since the early 70’s,” says Executive Director, Jennifer Galloway. Formerly known as Lee Count y H umane S ociet y, t he name was changed in the late 90’s; people often confused the shelter with the county’s animal services department. In addition, the organization’s reach goes far beyond Lee County. The shelter partners with smaller towns all over Southwest Florida that have limited resources. Animals are often euthanized in smaller communi-

ties because there is no medical care or even worse, no space for them. Galloway says, “Florida is one of top states that still euthanizes animals. By helping rural communities we help reduce those numbers.” When it comes to adoptions, “One of the biggest myths is that because an animal is in a shelter, they must have done something bad when that is just not the case.” Galloway says most animals end up there because owners can no longer take care of them. Once in the shelter, it is her mission to ease the fears of the animals, make sure they feel loved and leave with new skills. She likens a shelter stay to summer camp saying, “We do enrichment. We have certified trainers. We have play dates so they can go in the yard with other dogs. We want them to keep their social skills.” The same holds true for cats. GCHS has a free roaming cat room complete with a lanai for felines to lounge in the sun. Galloway explains, “It makes an easier transition into a home if the cats aren’t stuck in cages while they are here.”

“For A Greater Cause” is a monthly initiative of livinglocal, spotlighting a local nonprofit that is making a difference in our community. livinglocal also makes a monetary donation to the nonprofit on behalf of its advertising partners. Not all cats are adoptable; there is a large feral (free roaming cats) population in Southwest Florida. GCHS partners with Fortunate Ferals, another local non-prof it organization that teaches people how to colonize feral cats with an emphasis on spaying and neutering. “It’s our goal to reduce the cat population problem in the state,” says Galloway. The influence of GCHS is indeed much greater than the walls of the shelter. “We go into the community with animals to ease tensions and stress,” says Galloway, adding, “Even 911 call centers because it’s such a stressful job. The animals bring smiles to their faces.” The same holds true for children who are experiencing trauma. Shelter dogs make regular cameos at places such the Children’s Advocacy Center of Southwest Florida and Valerie’s House. Animals are used for humane education programs in local schools and at summer camps, and volunteers take them to nursing homes and adult day care programs throughout Southwest Florida.

GCHS was founded before the National Humane Society, and while a member of that organization, receives no funding from it. GCHS is privately funded, relying on donations and volunteers. Volunteers walk dogs, take them on car rides and even home for observation. If any behavior issues are noticed, trainers will work with the animals. Dog-training classes are offered to the public as well. “The goal is to get them in a home and keep them in a home,” says Galloway, “We do all we can to give our animals good skills.” Perhaps one of the biggest needs, besides funding, is that for foster parents. This is especially true during kitten season when babies whose eyes aren’t even open, are abandoned and have to be bottled fed. Ditto for puppies and injured animals. “There is only so much we can do in the rehabilitation center,” says Galloway. “It’s just like with people. We get better faster in a home, with fresh air, love and sunlight. I’m a true believer that heals us all.”
