W
ith the name ‘family’ in its title, you might expect Park Family and Cosmetic Dentistry to serve the needs of children as well as adults. However, they do more than just serve needs, they pay special attention to their smallest patients, thanks in part to the pediatric specialties of Dr. Leanet Ascunce and Dr. Ana Scopu. According to the Academy of Pediatric Dentistry, a child should have their first dental visit by age one. Dr. Ascunce says, “It’s important to see your pediatric dentist by this age so that parents can be educated on how

to keep their child’s oral health in tip-top shape. We teach techniques which can make brushing a baby and toddler so much easier for both the parent and the child.” Dr. Ascunce’s aunt was a dental assistant and she would go to her office during school breaks rather than go home. She says, “At Park Family, our goal is to have our patients feel like they are family. Pediatric dentistry is the first exposure to good oral health. We want to make this experience as good as possible to shape a child’s view of the dentist as a positive one.”

She remembers hers saying, “I had 14 cavities, a root canal, AND I left with a scar on my lip.” She is dedicated to making sure that doesn’t happen to patients at Park Family. Focusing in orthodontics, Dr. Scopu can see skeletal patterns forming in children very early from 2 years old and up. Some of these patterns can predict the eventual need for braces. Keeping an eye on the development of young patients during regular checkups
allows her to plan ahead with parents on treatment plans. She explains, “When children are eight and nine, their permanent premolars are coming in under the primar y (baby) molar s . There is a lot of hidden space underneath these baby molars. I always try to utilize this time to preserve as much space as possible, especially for young patients already having signs of crowding. This is why we star t orthodontic consults as early as 8-9 years of age.”

Applying them when bones are still growing allows Dr. Scopu to subtly change the skeletal structure of the mouth and help prevent more serious problems from forming as an adult. Park Family is the only dental practice in Naples to use Insignia Custom Braces, according to Dr. Scopu. Made individually for each patient, she says, “The system allows me to do a comprehensive analysis in the beginning. This will allow me to virtually visualize the final treatment outcome before we even place the braces. By doing this we can work out any expected problems before they even happen. This system also lets me to use custom arch form wires with prebends, which gives us the best final visual result.” In turn, the combination of custom braces and wires allows for a more predictable outcome and can shorten
the amount of time a child wears braces by approximately six months. These braces are also smaller and more comfortable for the patient to wear. When planning a child’s first dental appointment, Dr. Ascunce advices parent s to, “Hype up the visit as lots of fun, with new and exciting adventures. Keep the dental terms friendly. Tell them we are going to brush their teeth and maybe take some pictures with our special dental camera.” She says the best part is, “If they are good listeners, they will be rewarded with a prize from the treasure chest.” It’s important for a parent be positive and conscious of not transmitting their own fears to their children. Born in Serbia, Dr. Scopu graduated from Bingham University in New York and completed her Doctor of Dental Surgery Degree from NYU College of Dentistry. Her list of specialty training is impressive and includes a Masters in Specialized Orthodontics from the prestigious Medical College at Munster University in Germany, a degree earned by fewer than 20 general dentists in the U.S. Her husband is a ceramist at Park Family, and they have a toddler, Stefan, who takes up most of their free time. Dr. Scopu is tri-lingual and enjoys spending time on the beach and on personal growth when not working.

Born in Havana, Cuba, Dr. Ascunce grew up in Miami. Completing her undergrad at Florida International University she obtained her DMD degree from the University of Florida College of Dentistry. After practicing general dentistry for four years, she was exposed to the pediatric side and fell in love with the specialty. Dr. Ascunce went back to school to specialize in pediatrics at the prestigious Boston University’s Henry M. Goldman School of Dental Medicine. Dr. Ascunce is a Tampa Bay Buccaneers fan and often goes to the games with her husband. They have two four-legged children named Sophie and Paris.

When Preparing for Knee Replacement Surgery

One of the most common medical diagnoses associated with advancing age is osteoarthritis, also known as OA. This disease occurs over time due to years of “wear and tear” on our joints. Although OA can damage any joint in the body, the knee is among the top locations affected in the majority of the population. For example, consider a 60-yearold woman who has been working the majority of their lives on their feet. Over time, they begin to have a gradual worsening knee pain and stiffness. After several weeks or months of over the counter medications, home remedies, or ice and heat, they present to an orthopedic clinic for further evaluation. The orthopedist then performs testing and imaging, which often includes x-rays, and diagnoses the patient with OA in the knee joint. Over time, after conservative therapies have been exhausted, a mutual decision between the physician and patient has been made to proceed with knee replacement surgery. What now? You’ve just been told you will need a knee replacement and you are looking for answers as to how you may achieve the best possible surgical outcome with the least amount

of complications. Vein Specialists has been working with a number of Orthopedic Surgeons to optimize their patients’ vascular status prior to joint replacement, including total knee replacement. Patients over the age of 50 often have both orthopedic disease and venous disease. We have coined the term “orthovenous” disease for this group of patients. What is venous insufficiency? Everyone has arteries, which pump the blood away from the heart. Veins bring the blood back to the heart. Leg veins have numerous one-way valves within them that carry blood back to the heart. Venous insufficiency occurs when the valves within the veins no longer open and close properly, which leads to the leaking of blood backwards toward the feet. This leaking of blood results in increased venous pressure in the lower part of the legs, due to the force of gravity. This may lead to signs and symptoms of vein disease such as bulging varicose veins, spider veins, swelling, leg cramps, restless legs, increased urination throughout the night, heaviness, achiness, and even skin discoloration or ulceration. These signs and symptoms may occur in one or both legs and may not necessarily be accompanied by pain.

Refractive Cataract Surgery Success at
Some 24.5 million Americans have cataracts, according to the American Academy of Ophthalmology, and Roland Martin was one of them. The renowned professional sport fisherman and television host coincidentally noticed changes in his vision when he started having trouble seeing the fish. “Fishing involves quick decisions that need to be made in an instant, and it’s so important to have perfectly accurate vision at all distances,” says Roland. bout three years ago, Bob Zekanoski began experiencing pain in his hip that would not go away. A supervisor for Lee County Parks and Rec, Zekanoski led an active lifestyle both in and out of work, enjoying biking, swimming, and lifting weights. Then somet hing changed . H e remembers, “I had a hard time putting on my socks. I didn’t have the same flexibility I was used to.” But that wasn’t all. He adds, “I didn’t really enjoy walking anymore. It was painful. I hated to walk even 50 yards.” Seeking medical help, his first course of treatment involved physical therapy and anti-inflammatory medications. The medication helped for a time, but when the pain persisted Zekanoski says, “It was bad. I can tolerate a lot of pain. I’ve passed 27 kidney stones in my life, so when I say I’m in pain I’m not kidding.” When it became clear that he was running out of options, Zekanoski began researching the inevitable, hip surgery. He had just been promoted to his super-

visory roll and was concerned about being out of work for several months while he recovered from conventional surgery. That ’s w hen he discovered SuperPATH® hip replacement. After doing more digging, he learned there was a physician in the area that was proficient in this unique surgical technique and had great success with it, Dr. Peter Ameglio. Although SuperPATH was originally introduced in 2003, not many doctors implement it. She said ‘I could not do any of this.’” A speedy recovery was not only important to Zekanoski because of returning to work, but also because he did not have the help he would need for an extended period of rehabilitation and recovery at home.
After an overnight stay at the hospital he went home. His son stayed with him for a few days to make sure he was OK, and then Zekanoski was able to complete the recovery on his own. The parks and rec supervisor did his physical therapy religiously, only using his walker for nine days. Two weeks after his surgery he was able to ride his stationary bicycle again. He says, “My progression after having major surgery was just amazing.” About the SuperPATH approach to hip surger y, Dr. Ameglio says, “SuperPATH approach reduces the risk of hip dislocation, decreases post-operative pain, and shortens the patient’s recovery time.” Zekanoski says, “I give the entire experience five stars. TREATING LYMPHEDEMA IN THE COMFORT OF YOUR HOME
“P
eople would see the swelling in my hand and arm and say ‘Oh my God,’” says Naples resident Paul Borsari. Borsari is living with chronic lymphatic leukemia, which caused severe swelling in his arm, and lymphedema. Lymphedema is a condition in which excess lymph fluid collects in tissue, most often in the legs and/or arms. The fluid causes swelling and if left untreated often causes changes in the skin, even wounds. There are different causes of lymphedema, one can be hereditary or genetic where the individual is born with a compromised lymphatic system. The other is the result of a surgically invasive procedure that may obstruct the lymphatics, and it may result secondary to venous insufficiency.
The severe swelling makes it hard for patients to bend their limbs, complete regular tasks, and can be painful. Compression stockings are often used to aid circulation and help reduce swelling, but they don’t always solve the problem, according to Alyssa Parker, Clinical Administrator at Acute Wound Care. The company provides patients with in-home compression pumps that have shown great success in boosting circulation and reducing swelling in both the upper and lower extremities. In Borsari’s case, he first tried a compression sleeve, then 15 rounds of therapy involving massage and compression wraps. Following a hospital stay, he was sent home with physical therapy. It was the Lymphedema therapist who connected

him with Acute Wound Care. Before long Borsari had an in home, hospital grade compression pump and was able to successfully manage his lymphedema on his own schedule. Parker says, “Compression pumps have been around for decades. Many doc tors know about their use in hospitals to help with blood clots, but they are unaware of the availability of the treatment for in-home use.” The pumps sleeves fit from the foot to the groin or the hand to the shoulder. They contain multiple chambers that fill with air in a sequential motion, replicating the circulatory system and assisting blood flow from the extremities back to the heart. “I recommend this treatment to everybody and anyone who has this disease. I’m back to playing golf again. I’m back to normal. I feel great!” – Paul Borsari The excess fluid in the limbs is worked back through the circulatory system where it can be naturally eliminated by the body. Compression pumps are not only used to treat lymphedema but are also helpful for patients who have venous insufficiency, chronic edema, poor circulation and ongoing open wounds. Parker became interested in lymphedema through a research project conducted on the predisposition of the condition in college, later working with patients directly in the treatment of the disease. She says, “A lot of people are misdiagnosed and are not being treated effectively.” Then there are the wounds. She says, “In some cases we get patients that haven’t had success healing a wound in months to a year. With the right
treatment and specialty dressings, our goal is to get the wound healed within 45 days.” It’s hard for wounds to heal when the body has poor circulation. Parker says, “It’s important to use proper dressings along with compression when needed. Individuals trying home remedies to heal a wound without proper dressings are at a higher risk of infection which may lead hospitalization.” Once Borsari got a compression pump to aid in his circulation, he saw results almost immediately. He says, “It feels like a massage, like someone is hugging your arm.” He uses it at least once a day for an hour explaining, “I’m gonna watch Wheel of Fortune and Jeopardy anyway, so why not do something else while I’m sitting there.” After suffering with the unsightly swelling for years, he says, “My arm looks great, my wrist is a normal size, and I can see the veins in the back of my hand again.” About his experience with Acute Wound Care, Borsari says, “My therapist was unbelievable. She was so thorough and taught me techniques for how to use the machine. I was so impressed by her professionalism.” The compression pumps are portable so patients can travel with them. However, Parker says, “A prescription is needed for the machine. Many insurance carriers cover the treatment. FOR KIDS IN NEED

ith so much noise in our daily lives, we tend to lose sight of what is truly important in the grand scheme of things. The most treasured possession for some can be considered menial by others. Something we take for granted each day, like a simple pair of shoes, can make all the difference in someone’s life, especially children in need.
It seems obvious, but there is an overwhelming lack of resources for underprivileged youth across the country. Over 60% of kids, roughly 26,500 students, in Collier County public schools are designated economically needy – that’s over three times the number of seats inside of Germain Arena! Luckily for the Southwest Florida community, Jeanne Nealon is tackling this problem head on. Laces of Love is a non-profit organization that provides new shoes to low income and disadvantages children in both Lee and Collier Counties, while educating the community about their needs. As Co-Founder and President, Jeanne is doing all that she can to find a solution to this issue. “It started when I

took an 8th grade student of mine home, and he had been wearing some of the worst shoes I had ever seen! So I gave him a new pair, and noticed it made such a difference. Then I started looking at my migrant students and the ones living just above the poverty line, and really saw the need was so great.” With the help of a volunteer Board of Directors, including two additional Co-Founders, Susan Warren and Mary Myrmo, Jeanne was able to bring her cause to life, growing little by little each year. In many schools, she is known simply as “The Shoe Lady” because she usually shows up pushing a rolling bin full of stylish, brand new sneakers. The kids light up at the sight of her, while patiently waiting to see what pair of shoes she may have for them. “We try to have an assortment of colors and styles for them to choose from, since it’s important for us to give them something that’ll boost their confidence,” Jeanne adds.
Now working out of two separate storage facilities, the team keeps thousands of shoes bagged up, labeled, and ready to be distributed throughout Lee and Collier counties. Julie McLeod,
Board Member and volunteer, says, “It would be really great if someone could donate some warehouse space to us, but for now we will continue doing whatever we can, even if that means storing some shoes in our own homes!” Over the years, Laces of Love has partnered with various organizations in Southwest Florida, including PACE Center for Girls, Dress for Success, Our Mother’s Home, Salvation Army, and the Red Cross, as well as all Collier and Lee County schools. In fact, over 150,000 children have received new shoes from Laces of Love since 2004! This isn’t something that happens overnight; Jeanne has dedicated countless hours to her cause, educating our community about the overwhelming need for support of underprivileged youth. Thanks to her passion and drive, she has started a movement that shows no signs of stopping.

Wrist Factures Demystified

F
ractures of the wrist are one of the most common injuries treated by orthopedic surgeons. We typically see wrist fractures in one of two settings and in a bimodal age distribution. Younger patients with normal bone density, typically sustain high energy injuries such as a fall off a ladder, where as an older individual can sustain a fracture from a low energy injury such as a fall while playing tennis or pickleball. The difference between the two is very important, as a low energy injury typically indicates that the bone was not strong enough to withstand the impact. This can indicate a more serious underlying bone deficiency such as osteoporosis. The lifetime risk of sustaining an osteoporotic fracture is 40% in women and 30% in men. Quite often an individual has never had a bone density test or was told they had a normal bone density at the time of presentation to an orthopedic surgeon with a fracture. Below is a list of the most common questions I receive from my patients at the time of their initial evaluation for a wrist fracture. I went to the ER and was told I have a wrist facture. What bones are involved in a wrist facture? The radius carries approximately 80% of the axial load at the joint surface, and typically is the bone injured in a wrist fracture.

How quickly should I see an orthopedic u p p e r ex t r e m i t y s p eci a l i s t a f t e r my injury? You should ideally be evaluated by a specialist within one week of your injury. If by chance surgical intervention is indicated for your fracture, it does become more difficult as the time passes, and the bones begin to heal in a poor position. What should I do while I wait for my specialist appointment? The most important thing you can do is refrain from any lifting, pulling, or pushing activities. These activities will increase your discomfort and can allow the bones to shift into a position that may require future surgical intervention. In addition, if there was no injury to your fingers, elbow, or shoulder, you will want to move these joints to prevent stiffness. I always tell my patients that their goal should be to make a full fist and have full extension of their fingers within a few days of their injury. I was told my wrist is “shattered”, does that mean I need surgery? No, it does not. There are certain radiographic criteria that are utilized to help decide if surgical intervention will improve someone’s ultimate functional outcome. In addition to these criteria, I also keep in mind the activity level of my patients in conjunction with their concurrent medical conditions. I always tell my patients that I don’t just treat an X-ray, I treat them as a whole individual and the X-ray is only one component of this decision tree. I enjoy
reviewing the X-ray findings and helping my patients come to a decision regarding their treatment plan. If I don’t have surgery, what are my other options? Conservative treatment options for a wrist fracture include casting, splinting, or a combination of the two. I typically follow my patients weekly for three weeks following initiation of conservative treatment. This is to assure there is no change in alignment of the fracture. One can expect to be immobilized in a cast and/or splint for a total of six weeks prior to initiating wrist range of motion. Sometimes at the end of the immobilization period, individuals may be referred to an occupational therapy to improve their range of motion and further down the line to improve their grip strength. What does surgery involve? Surgical intervention involves making an incision to access the fracture fragment(s), in order to improve the alignment and restore stability to the wrist so early range of motion can be instituted. There are a variety of implants that can be utilized to achieve the desired outcome. Often these plates and screws are retained indefinitely, however depending on the fracture pattern and bone quality there is a possibility that certain types of plates must be removed once the bone is healed. What should I expect after surgery? The first office appointment will occur two weeks after the date of surgery, at which time the sutures are removed, and a removable splint is applied. There is an 8-10 week period post-operatively that you must refrain from any lifting, pulling, or pushing with the operative hand. At 12 weeks I typically allow my patients to return to all activities gradually with no restrictions.

t might be summer and thoughts of the next school year far away, however, if you are a parent of a high school student now is the time to plan for your child’s college education. The evening news has been punctuated as of late with stories of scandal, cheating, fraud and bribery in exchange for guaranteed admission into some of the nations most popular and respected colleges and universities. The fields are competitive and spaces limited, making it increasingly difficult to get into some schools. Getting admission is step one; step two is paying the sometimes-exorbitant fees required. Parents often plan for those fees by enrolling in savings plans including the Florida Pre-Paid Program, designed to help Florida families save money by locking in the future cost of attending the state’s public institutions.
However, some families struggle to contribute to pre-paid plans while others can’t afford it at all. Parents–don’t throw in the towel just yet. Take heart in the fact that if your child does a little preparation, the money might just be there, in the form of a Bright Futures Scholarship. Bright Futures helps Florida students pay for college and vocational schools in the state. There is a list of requirements that include things such as ACT/ SAT scores, GPA and service hours. Liv ing Local publisher Audrey Ameglio got a late start in the Florida Pre-Paid Program saying, “I didn’t begin contributing until Timothy was in middle school. I knew I should have started earlier. It was definitely a financial stretch starting so late, but knew it was worth securing the opportunity for a college education, realizing he still had to do his part to gain admission.”

When Timothy entered high school, ACT and SAT testing were mentioned, but she thought the scores only mattered when it came to college admission. She didn’t realize that high scores could also help pay for his education until she heard about Bright Futures. Timothy was a good student with a high GPA yet like many, he struggled with standardized tests. Unfortunately for many, it can also be filled with anxiety and fear with the thought of college debt looming. That financial fear may keep some parents and students from even considering higher education and trade schools. Even for parents who’ve invested in the Florida Prepaid College Plan, supplemental funds may still be needed and they are not aware of the financial assistance that is available in the form of scholarships, free money for education. To get financial assistance, parents and students need to start preparations EARLY - in their junior year of high school. Statistics show those who do, have a much better chance of achieving the Bright Futures score requirements, and tutoring is an excellent way to start.
