I

n 2004, Dr. Michael J. Collins started with a vision to create a unique medical experience where patients were always greeted with enthusiasm, taken care of in a timely fashion, and received personalized care throughout their visit. Not only did Dr. Michael Collins, Founder and Medical Director of Collins Vision, survive those odds, but he thrived in spite of them. “It’s not easy to start a new business regardless of what that business is, where you come from, what you know or who you know,” says the board-certified ophthalmologist. He continues, “I started my own practice 14 years ago with big dreams, very little cash flow, and a defiant desire to succeed, but even I did not anticipate the kind of growth we’ve had in recent years.”

He began by leasing a small office space in Southwest Florida and within five years was able to design and move into his “dream office.” Dr. Collins says, “I still remember walking into the relatively huge office and it seemed too big. I thought we had made a mistake.” Even with his consistent growth, Dr. Collins thought many of the rooms would sit empty for years, so he asked the staff to keep the doors closed. He didn’t want to “spook the patients” or himself. Fast forward another five years and not only are those rooms full, but the practice once again needs more space. Growing up, Dr. Collins excelled in math and science. Fascinated and amazed by the human body, he originally planned to follow in his uncle’s footsteps and become a physical therapist. Instead, he ended up in medical school at Vanderbilt University and completed his eye surgery residency at Emory University. VENOUS It’s More Than Just Varicose Veins

e have all seen someone walking W down the street and noticed large bulging varicose veins and thought

– they should really see a doctor about that. Though varicose veins or prominent spider veins are the most obvious sign of venous disease, they represent a medical condition called venous insufficiency that is much more than just varicose veins. Venous insufficiency can manifest as a variety of signs and symptoms, many of which may not immediately come to mind when thinking about venous disease. Venous insufficiency is a condition where the valves in the superficial venous system in the legs fail and, over time, lead to impaired blood return to the heart. Blood pools in the leg veins, especially when in the standing or sitting position, leading to abnormally high pressure in the veins or venous hypertension. It is this venous hypertension which leads to the many signs and symptoms of venous insufficiency. Venous insufficiency is a very common and underdiagnosed condition affecting millions of Americans. It is estimated to be present in as much as 40% of the population and becomes more prevalent after age 50. Heredity is the major risk factor, as are a history of blood clots or DVT, obesity, standing or sedentary occupation, female gender and prior pregnancies, though the condition afflicts many men as well.

Most people are aware of the most common sign of venous insufficiency which is varicose veins, but many people without varicose veins also have venous insufficiency. Legs swelling as well as achy, heavy or tired legs at the end of the day are also common symptoms. In severe cases, the skin around the ankles can become discolored, thickened or even ulcerated over time. As the pressure in the venous system builds throughout the day when patients are on their feet, symptoms progress and are worse in the evening and at bedtime. This is when the lesser known symptoms of venous insufficiency such as restless legs, leg cramps and excessive urination become problematic and can rob a patient of a good night’s sleep. All of these symptoms can exist in the absence of venous disease but many patients’ symptoms improve with treatment for venous insufficiency. A trial of compression hose or socks can be the best predictor of which symptoms will most likely improve or respond to venous interventions. Modern vein evaluation and intervention is much different than it was years ago. Prior to 2000, procedures such as vein stripping were very invasive, required general anesthesia, and had unacceptably high complication rates. For this reason, most patients with venous insufficiency were forced to suffer as there were no effective treatment options available other than compression hose and leg elevation.

you’ve seen? He told us that most major repairs could be avoided if homeowners perform their own monthly plumbing checks. Look for leaking, dripping, discoloration, wet spots in the grass, and for any sinks or tubs that appear to be draining slowly. He stresses that when you encounter something that seems off- don’t wait. Have a licensed plumber come out and inspect immediately. The key to avoiding expensive repairs is catching the issue at the beginning. Mike gave us an example of a large issue

that could have been avoided by paying closer attention to the piping in the home. A customer had a leaking pipe in their ceiling - they had copper piping that had corroded, resulting in a small pinhole leak. This small leak caused some discoloration in the ceiling, but the owners did not notice. The leak worsened and waterlogged the insulation in the ceiling. Suddenly, the ceiling began to sag. When the attic was inspected, the plumber found a large leak, a lot of water damage, and rampant mold growth due to the warm, wet environment. What could’ve been one small pipe replacement was now a pipe replacement, water damage restoration, mold removal, new insulation, and drywall work on the ceiling. Moral of the story: inspect your home, and if you find something off, take it seriously. It is better to be safe than sorry!

CARING FOR

ANKLE SPRAINS By Gina Birch

A

nkle sprains are the most common injury that keeps athletes from returning to the game. Non-athletes also suffer from sprains, most commonly from walking on uneven ground or even wearing improper footwear. There are three different grades of ankle sprains according Dr. Brad Castellano, a podiatrist and foot and ankle surgeon at Gardner Orthopedics. Grade one is a strain, grade two is a partial tear, and grade three is a full tear. What’s often surprising according to Dr. Castellano is, “With a grade three injury, when the ligament is torn completely, people often have less pain because there are no little fibers left to be pulled on. It’s the most severe injury but often the most undertreated because when patients aren’t in a lot of pain, they don’t seek help.” These are the people that will have more frequent ankle injuries and sprains. Castellano explains, “Because they were never treated properly they often sprain the ankle again and again. It hurts, but many times it doesn’t even swell anymore.” livinglocalfl.com | JUNE 2018

In the past MRI’s were not commonly preformed on ankle sprains, mainly because of the expense according to Dr. Castellano. But today he says, “The price of MRI’s have come down quite a bit, and we utilize them more frequently. A lot of associated injuries such as fractures and tendon tears are missed in lateral ankle sprains. An MRI helps us find those and that’s something that is especially important for a competitive athlete.” For the most severe tears, surgery may be required to repair the ligaments. Dr. Castellano says there is hope for someone who has let their injury go for years or has injured their ankle so many times that there are little or no ligaments left to repair. “We now have a technique called an Internal Brace from Arthrex® that has been working really well. It’s very strong suture material that augments the patient’s own tissue. It’s a fairly simple procedure and most can return to their sport in 6 to 8 weeks.” Treatment for an ankle sprain depends on the grade but can involve the use of a boot, crutches and/or a brace. Physical therapy is another important piece of the

healing puzzle for strengthening muscles around the ankle. Before resuming sports after an ankle sprain, Dr. Castellano puts his patients through a “hop test.” He has them hop forward, backward and sideways, first with both feet, to see if there is any pain. Then it’s one foot at a time, starting with the uninjured to set the standard. If a patient can’t hop as far or fast on the injured foot, more healing time is needed. If you are a non-athlete but have instability in the ankle, Dr. Castellano says it might be time for new shoes, especially if you are a man. He laughs, “Women have like 50 pair of shoes while most men have a black pair and a brown pair and will wear them until they are worn out.” He advises, “Look at the heel. The lateral part (the pinky toe side) is the first to wear out. Then you’re walking on the side of the foot. If the shoe is worn it is letting you tip.” Proper treatment early on is key to healing ankle sprains. If you suffer from ankle instability, you can still be helped. Born in New Orleans, Dr. Castellano grew up outside of Atlanta. He graduated from Georgia Southern University with a degree in Biology. After obtaining a four year degree in Podiatric Medicine from Temple University, he returned to Georgia to complete a threeyear residency program at Northlake Regional Medical Center (now an Emory University hospital). Dr. Castellano is a Fellow in the American College of Foot and Ankle Surgeons. He has been in practice for almost 30 years, with the majority of that time spent here in the Fort Myers area. He joined Gardner Orthopedics more than a year ago but also travels around the world lecturing and teaching with the Podiatry Institute. He has published numerous articles in his field and has authored chapters in major textbooks used by physicians in the field of foot and ankle surgery. Dr. Castellano is also an inventor and holds patents on products commonly used by physicians that treat the foot and ankle.

INGREDIENTS (Serves 4-6) 6 Mario’s Center-Cut Bone-In Pork Chops, 2 inches thick 2 Fennel Bulbs, with fronds, thinly sliced 2 Large Shallots, thinly sliced 1 can Diced Tomatoes (28 oz) 1/2 cup Fresh Parsley, chopped 1/2 cup Dry White Wine 1/4 cup Extra Virgin Olive Oil 2 tbsp. Capers, drained 1 tbsp. zested Lemon Salt and Pepper, to taste

DIRECTIONS Heat oil in a large skillet over high heat. Season pork chops with salt and pepper, add to skillet and cook 4 minutes on each side or until browned. Remove from pan, cover and set aside. Without draining the skillet, add fennel, shallots and 1/3 cup of parsley and simmer over medium heat. Allow vegetables to brown slightly, about 5 minutes. Add wine and scrape the bottom of the skillet. Add tomatoes, with their juices, and blend well. Place pork back into the pan, submerging in vegetable mixture, and let simmer over medium heat until fennel becomes tender and pork is thoroughly cooked, about 12-15 minutes. Fast&Frugal INTERIORS Have you ever watched a home improvement show on television and wished someone could help you freshen up your space, or fix the things that just don’t work? That’s exactly what Theresa Clements does at Fast and Frugal Interiors. Clements has been in the interior design business for more than 25 years, and was even featured on HGTV. Originally an owner of Galloway’s, Clements and Associates in Royal Palm Square, she had what she calls ‘a dream team’ of architects, designers, builders and contractors, ready to work on major projects. Fast and Frugal Interiors is a spinoff of sorts, targeting the everyday homeowner who wants to make an update but needs expertise. She says, “I’m trying to reach people who wouldn’t think of calling a designer.” Although the name has recently changed, the Fast & Frugal concept continues to be a service they extend. If a home consultation is all you have the budget for, she will come in and decide on a game plan, giving you a road map of where to go and how to achieve exactly what you want. If you need more direct assistance, the designer says, “Get on your running shoes, we’re gonna work.” She’ll take you to furniture, consignment and home improvement stores to find the perfect pieces, paint colors and more, all within your budget. Empowering Women in the Workforce By Anna Gerrol

T

he path to financial stability is not an easy one. Many struggle to transition into the workforce after completing school, especially those who are lacking the resources needed to flourish in the professional world. There are countless tools available online to help guide you in your job search, but if you have never put together a resume or can’t afford the proper attire to wear for an interview, this can be extremely discouraging. That is where Dress for Success is stepping up. Dress for Success is an international non-profit organization with affiliates in 30 countries across the globe. Nickole Hendra, Executive Director of the SW Florida location, has been with the organization since September of 2016, and recognized first-hand the outstanding need for career prep for women in our community. She explains, “When funding ended for a Walmart Foundation program we offered, we noticed there was a huge need for career prep tailored to women in Southwest Florida. We noticed the problem and came up with a solution.” Out of that insight came W.E.T.E.S, or Women Empowered Through

Employment Series, an employer driven program, which is tailored to women of all backgrounds and ages that need help polishing their professional skills. The 7-week program consists of resume building, networking tips, interviewing practice, and career advice from various mentors in the community. One of the programs greatest success stories is Sherrie Page, who was looking for a career change and needed some guidance. She says, “Three days after the hurricane I was in a terrible car accident and lost my job. I was looking on the internet, and an ad came up on Facebook saying Dress for Success SW Florida would be having classes.” Thanks to a simple post on social media, Sherrie was able to connect with numerous professionals in the community, and took note of the valuable advice they shared. “One of the mentors had a contact with Children’s Network of Southwest Florida. They gave me a great tip that I otherwise wouldn’t have even thought about. They said, ‘Make sure you research the company before going for an interview, and when you finish always be sure to get the email address of whoever you met

with.’” With that small yet monumental bit of advice, Sherrie was able to take action, a decision that would later prove to be a determining factor in her success. “When I researched Children’s Network, I realized they were having an open house for prospective foster parents. After my interview, I made sure to email the interviewers and let them know I was going to attend their open house. When I went, the hiring supervisor was there and remembered me, and she ended up calling me the very next day to offer me the job. Thanks to Dress for Success, I now have a new career in a new field and am so excited. I wouldn’t have thought about emailing them the next day if not for Dress for Success.” Not only does W.E.T.E.S provide professional guidance and support for women, but they also offer clothing to each participant in the program, free of charge. Nickole says, “Having daughters of my own, I know how expensive it is to buy an interview outfit or get ready for a job.” Their Fort Myers location resembles a well-organized closet, fully stocked with gently used dresses, suits, shoes, jewelry and handbags. Most of the items

look brand new, which is important when giving young women the confidence needed to nail their big interview. Currently, Dress for Success SW Florida serves women of Lee, Collier and Charlotte counties by referral only. However, Nickole adds, “We will never turn away a woman who needs help.” They are also beginning to offer the W.E.T.E.S program in Spanish and Creole in order to accommodate a growing demographic of Southwest Floridian’s who do not speak fluent English. Dress for Success SW Florida is always in need of donations, specifically gently worn business attire for women of all sizes. They also accept purses, shoes, and accessories to complete each outfit they give away.
