SHOULDER FUNCTION
TO THOSE WHO’VE LOST HOPE His father was a professor of orthopedic surgery at the University of Havana in Cuba. When the Martinez family left the island nation, they eventually landed in Odessa, Texas. As a teenager, the future doctor would actually scrub up and accompany his father into surgery or the emergency room. He recalls, “You could do that in Texas back in the day.” Even though Dr. Martinez’s destiny seemed clear, the road nearly took a couple of turns. During medical school the doctor says, “I wanted to do everything.” First it was obstetrics. It was exciting the first time he followed a patient through her journey into the delivery room. However, he laughs, “After about twenty babies, I thought this isn’t for me.” Next was cardiology. But when he could not detect the sound of a mitral valve snap, a sound that his colleagues claimed to hear through their stethoscope, he thought once again “this is not for me.” “Every surgical rotation just felt right. I really enjoyed all of the medical tools and fixing abnormalities of the human body. In orthopedics, you can fix broken bones and abnormal muscles and people get better.” A former tennis player who blew out his shoulder, Dr. Martinez says, “Most orthopedics are frustrated athletes,” especially ones like him who were injured in their sport.
In fact, the doctor met his future wife on the tennis courts at Odessa College; she was a National Juniors Champion in Chile. After tying the knot, the two worked in his father’s medical practice for a number of years before relocating to Southwest Florida. The couple knew they wanted to live in Florida. Dr. Martinez was familiar with Miami, having visited the area numerous times with his family. However, he was not convinced that was the best place to raise his children. When he crossed Alligator Alley for a job interview more than 20 years ago, the hotel he checked into had “Welcome Bob Martinez” on the marque. He remembers thinking, “Wow they really want me.” Bob Martinez was the governor at the time… he was also visiting. In the late 80’s, Dr. Martinez was one of the first orthopedists in this region to perform a shoulder arthroscopy. Today, he continues to be among the first in his field to use cutting edge technology and unique approaches to repairing joints. The “reverse total shoulder” is one such operation. The procedure may seem counter intuitive, but it has proven successful in patients who’ve been told there is nothing that can be done to help them regain function in their arm. Dr. Martinez is one of only a few orthopedists who has not only embraced the reverse shoulder but also regularly performs the procedure. a broken or fractured humeral head, or shoulder. Dr. Martinez began doing the same locally, then gradually expanded the procedure for use in shoulders with torn rotator cuffs where arthritis had also set in. When a rotator cuff is torn and the patient doesn’t have it repaired, the muscles atrophy and the humeral head rides up against the acromion or the collarbone for the sake of visualization. Dr. Martinez is one of only a few orthopedists who has not only embraced the reverse shoulder but also regularly performs the procedure, saying in amazement, “It looks crazy but it works.” That’s because while most joint replacements are done in a manner that mimics the natural joint, this one is the complete opposite. In the past, the ball would be replaced but if the rotator was torn, the muscles still weren’t in the right spot to hold the ball or power it. Movement of the arm
was still severely compromised. In this new approach, Dr. Martinez places a socket where the ball used to be and a ball where the socket used to be. The geometry of this backwards-looking apparatus allows the shoulder to pivot and use the deltoid alone to lift the arm. It’s a process that is best understood when seen, so Dr. Martinez has a scale model of the shoulder that he takes apart for demonstration and education. Preparing for a reverse shoulder surgery, Dr. Martinez takes a CT scan of the patient’s shoulder. The image is then used to create a 3D computer model. Calculating precise measurements and placements, he actually practices the surgery online before going into the operating room for best results. “We’re working with a small bone so we have to make sure everything fits in the right way.” Candidates for this procedure are people who can shrug their shoulder

but can’t raise their hands to their head or above. This can even happen in patients who previously had a shoulder replacement. Regardless of previous surgery, the rotator cuff muscles can still give out. Besides being painful, the lack of mobility makes getting dressed nearly impossible. Patients also have difficulties rotating their arm in the manner needed to take care of important personal hygiene. People suffer for years, functioning with one arm, never seeking help because they’ve been told there is none. Others are simply afraid of surgery. Dr. Martinez cautions, “Some people are OK with having one good arm, but if one shoulder failed, the other might too and then you have nothing. It’s better to fix one while the other still works.” Surgery is the last option for treating shoulder pain according to Dr. Martinez, who first tries injections and the likes to

reduce inflammation. However, he says, “When there is a mechanical disruption, none of those things work. Mechanical problems need mechanical solutions.” Within 5-6 months after reverse shoulder surgery, most patients are able to resume most of their normal activities and wonder why they waited so long to get help. To think that our “Celebration of Smiles” all began five years ago! This past May, Park Family and Cosmetic Dentistry hosted our 5th Annual Dentistry From the Heart event. As soon as we were introduced to the story behind this local non-profit, we decided to take part in the organization. Dentis tr y From The Hear t was created in New Port Richey, Florida. It is an international charity solely dedicated to provide free dental care to those in need. Dr. Vincent J. Monticciolo is the man behind the charity, with his goal being to reach out to anyone in our community that’s in need of care but cannot afford it. In following traditions of Dentistry From the Heart’s years past, our day consisted of very few dry eyes. Tears of
nervousness, desperation and gratitude fell. This mixture of emotions overtook all that attended, as years’ worth of dental care was being provided and may not have been possible without this event. Guests arrived the night before to begin camping out for the free-care promised. Lining up as early as 10:00pm the night before provided early-comers a promised place in line when the event commenced at 6:00am. While they waited, they were awoken by the spices baked into the breakfast pastries, and odors of freshly brewed coffee provided by local donors. PFCD had initially promised to help 200 patients. Shortly after dawn, the line was capped as we hit our capacity. We knew we didn’t have the heart to continue turning

people away, so we decided to increase our capacity to 250. Sober homes, recovery programs and local community organizations car-pooled or shuttled any person in need of care. While the team at PFCD was joined by 117 volunteers, our relief grew as we each became stunned by their embodiment of PFCD’s patient-centric culture. As the day continued with lots of paper work and dental exams, the aromas lived on as hot dogs were grilled up for lunch with all of the
necessar y fixings. We exchanged warm embraces of love, and humble, tear-jerking, life-changing stories with all that attended. Contagious laughter rang out as we all posed for pictures and videos. PAINFUL Swollen Legs When You Travel? odern air travel can be challenging for any number of reasons M - long lines, cramped seats, and poor
customer service to name a few. At Vein Specialists, many of our patients have a hard time with air travel for other reasons. They have a hard time sitting for prolonged periods and develop leg swelling and pain on flights of even a couple hours duration. I have had many patients report that their legs ache by the end of the flight or that they have a hard time getting their shoes back on when it is time to exit. These symptoms are often due to a medical condition known as venous insufficiency. Venous insufficiency is a condition where the valves in the superficial venous system in the legs fail and over time this leads to impaired blood return to the heart. Blood pools in the leg veins, especially when standing or sitting for a prolonged period of time, as occurs when you are trapped with your feet down in an airline seat. This leads to abnormally high pressure in the veins or venous hypertension. It is this venous hypertension which leads to the achy, heavy, swollen legs and feet people experience after a long flight. Pooling and stasis of blood in the leg veins with prolonged sitting also can lead to deep venous thrombosis or DVT. It is recommended that you get up and walk, do calf exercises and wear compression socks during travel to minimize this risk. DVT can be difficult to diagnose and may be asymptomatic in up to half of patients. It is important to be evaluated and get an ultrasound if you

have any lingering swelling or discomfort in your legs after a flight. Venous insuf f icienc y is a ver y common and underdiagnosed condition affecting millions of Americans. It is estimated to be present in as much as 40% of the population and becomes more prevalent after age 50. Heredity is the major risk factor, as are a history of blood clots or DVT, obesity, and a standing or sedentary occupation. Female gender and prior pregnancies are also risk factors, though venous insufficiency afflicts many men as well. People are usually aware of the most common sign of venous insufficiency which is varicose veins, but many people without varicose veins also have this condition. Swelling as well as achy, heavy or tired legs at the end of the day are frequently reported and in severe cases, the skin around the ankles can become discolored, thickened or even ulcerated. Pressure in the venous system builds throughout the day when patients are on their feet, so symptoms are often worst in the evening or at bedtime. This is when the lesser known symptoms of venous insufficiency such as restless legs, leg cramps or excessive urination become problematic and can affect sleep. A trial of compression hose best predicts if these symptoms will respond to venous interventions. At Vein Specialists, we offer comprehensive, modern vein evaluation and care. We start by performing a complete history and physical examination and then offer detailed ultrasound evaluations in our office allowing us to formulate the best treatment plan for each patient. The first thing done in every ultrasound performed in our office is to exclude or rule out DVT as the cause for the patient’s leg swelling or pain. The exam then proceeds to evaluate and map the entire venous system to check for reflux or insufficiency. When a DVT is discovered, the patient is notified before leaving our office and is seen by a provider who can then initiate anticoagulation (blood thinner) if indicated to treat the DVT.

While he takes care of patients suffering from all kinds of ocular issues, such as cataracts and glaucoma, the doctor says, “My specialty is oculofacial plastics, especially the eyes and brow.” The eyes age faster than almost any other part of the face, and they just so happen to be the first thing that people focus on, literally. For many people, the eyes are one of their most beautiful physical features, but are sometimes hidden by drooping eyelids and puffy skin. This tired and even sad look can
easily be fixed through an operation known as a blepharoplasty. Small incisions are made in the crease of the eyelid to remove excess skin, resulting in a more open look. If your eyelids are folding over your lash line, or if you have trouble putting on makeup because of the excess skin on your lids, you are a good candidate for blepharoplasty. Removing this skin is not just about aesthetics; it can also greatly improve the peripheral vision of patients, according to Dr. Jones. When handling the lower lid, fat can easily be removed if it is puffy, fat can easily be removed. On the opposite end of the spectrum, if the lower lid is hollow, volume can be added. Dr. Jones says, “For most patients, surgery combined with the use of something like cosmetic Botox and fillers can give them an even greater overall result.” The use of injectables is something he takes great pride in executing at Collins Vision. “Every patient and procedure is different, and it’s important to consider all areas in and around the eye to maximize results” he says. The doctor is known for taking his time with each patient, making sure that whether it’s a surgical or non-surgical procedure, the results look natural. “It takes an artistic eye. country, he always knew he would go to college but wasn’t sure which of his parents tracks he would follow. When exploring pre-law, he was having trouble embracing the field and remembers with relief, “My Dad said one day he thought medicine would be better fit for me – I’m glad he said that.” Leaving the cornfields outside of Dayton for medical school at Ohio University, he says, “It was near the West Virginia border with these beautiful mountains. I was so happy to be there and be in medical school.” The road to ophthalmology and oculoplastics was one he “fell into” but it has proven to be a perfect fit. alk to anyone who has lived in Southwest Florida for more than 20 years and they will tell you tales of the day when Tamiami Trail was unpaved, two lanes, and surrounded by brush, not buildings. It’s a hard scene to imagine. To get a feel for what life was like “back in the day”, take a drive to the Tarpon Lodge on the northwest tip of Pine Island. After passing through a pocket of restaurants and galleries in Matlacha, you’ll meander through lanes of tropical foliage and native scrubs before arriving at the historic lodge. In 1926, it was the personal residence of an adventurous couple, passionate about fishing. Decades later and over the years, it became an inn and restaurant. When the property came up for sale, Tarpon Lodge General Manager Rob Wells, III says, “My dad had been watching the place sit there and saw a lot of potential in the building and property.” Rob Wells , Jr. and w ife Phy llis purchased the property in 1999, naming it after the beloved fish that people come from all over the world to find here — Tarpon. The Wells family helped to pioneer the Pine Island Sound area. In the mid 70’s, they purchased Cabbage Key. They relied solely on a water tower, generators and a ship-to-shore phone to live while overseeing the island’s rustic restaurant and inn; Rob III was only three at the time. He and his brother, Ken, took boats to school rather buses. He adds, “My friends used to think we were in the military. We were building tree houses, living in the woods and hiking around.” There was a time when Rob says, “I felt like I had the worst luck ever. All of these kids were doing fun stuff in town.” But as often happens, when you leave home, you see things in a different light. While studying finance at the University of Florida, he says, “My buddies were like, ‘wait a minute, you’ve got this island, that you can only get to by boat, that serves beer and cheeseburgers??’ They thought it was the coolest thing and all wanted to come down.” Rob still thinks it’s pretty cool. Today, Ken is General Manager of Cabbage Key, while Rob holds that title tarponlodgerestaurant

at Tarpon Lodge. However, the brothers constantly work together to keep the family businesses running. Their staff is also like family, many of whom have been with the Wells’ for decades. Tarpon Lodge is a white beacon in the middle of lush green grass, swaying palms, blue skies and the lapping water of the Pine Island Sound. With a bed and break fast vibe, the lodge has eight bedrooms, original wood floors, a fireplace and a cozy bar. The neighboring Island House has twelve rooms of various sizes, six of which have waterfront balconies. For guests who want larger accommodations and more privacy, the property offers a two-bedroom boathouse and a private cottage, both of which come equipped with a kitchenette. A place to unplug and enjoy nature, guests can also learn of the islands history and Calusa culture across the street at the Randell Research Center. However, a stay at Tarpon Lodge is not complete without getting on the water. Charter boats are available, be it for sight-seeing or world class sports fishing. And the sunsets — they’re magical.
Dining at Tarpon Lodge is worth the trip, even if you’re not staying overnight. Their new summer cocktail, lunch and dinner menus are full of fresh, local flavors. Treatments to Ease Your Arthritis Pain D
o you have thumb pain when opening doors and jars? Have you noticed that your strength has diminished when you try to pinch or grasp objects? If any of these sounds familiar to you then there is a possibility you may be experiencing the debilitating symptoms associated with thumb arthritis. The base of our thumb metacarpal sits on a bone called the trapezium, and together form a saddle joint called the thumb carpometacarpal (CMC) joint, which allows motion in three planes. The increased mobility at this joint predisposes it to the development of arthritis, in conjunction with the fact that we depend on our thumbs for 50% of our overall hand function. Who gets thumb arthritis? The answer is that anyone can get thumb arthritis, and the majority of the time it’s idiopathic. However, there are

certain individuals who are predisposed to the development of thumb CMC arthritis, and include: women or age over 40 history of prior trauma to the thumb CMC joint, or joint laxity inflammatory joint disease occupations that require repetitive use of the thumb What signs should I expect to see if I have thumb CMC arthritis? People with early onset of thumb arthritis may experience discomfort when grasping door knobs, opening jars, pinching and squeezing shampoo bottles, or when placing pressure over the palm of the hand during activities such as yoga. The pain can typically be localized to the base of thumb and is often activity dependent, improving with rest. As the arthritis progresses, there can be a noticeable prominence at the base of the thumb which is referred to as a “shoulder sign” because it resembles the contour of one’s shoulder. I often see patient’s in my office once the symptoms have become more constant and affect their ability to work and enjoy hobbies such as golf and tennis. How is thumb CMC arthritis diagnosed? The diagnosis is made based on clinical examination in conjunction with radiographs of the thumb.

What are my treatment options? After receiving a diagnoses of thumb CMC arthritis, the mainstay of treatment is conservative, consisting of activity modification, NSAID’s, and splinting. Quite often these treatments have been exhausted by the time someone comes to my office, and the next step on the treatment spectrum would be an injection of corticosteroid into the joint, which can be performed in the office setting. The pain relief obtained from an injection is very variable from one person to the next, and can range from a few days to months or even years. When is surgery considered? I do not consider surgery for a patient until all conservative treatment has been exhausted. If a patient is no longer experiencing relief from the corticosteroid injections, or they have had three or more injections in the past, I would discuss surgical options with my patient as repeated injections can weaken the capsular integrity of the joint and potentially weaken the surrounding tendons and ligaments. What is involved in the surgery? The surgery is very similar to other salvage surgeries for arthritis in that the arthritic bone is removed, however the hand and wrist differ dramatically from that of a knee and hip. We do not bear weight through our upper extremities. Therefore the bone that is removed does not need to be replaced with implants. There are numerous variations of the surgery for thumb CMC arthritis, but I prefer to perform a suspensionplasty which limits the number of incisions made, does not require harvest and sacrifice of a tendon, and also does not require any drilling of bone. This technique allows my patients to begin early range of motion of the thumb at two weeks following the surgery.

New Shoes By Anna Gerrol
W
ith so much noise in our daily lives, we tend to lose sight of what is truly important in the grand scheme of things. The most treasured possession for some can be considered menial by others. Something we take for granted each day, like a simple pair of shoes, can make all the difference in someone’s life, especially children in need.
It seems obvious, but there is an overwhelming lack of resources for underprivileged youth across the country. Over 60% of kids, roughly 26,500 students, in Collier County public schools are designated economically needy – that’s over three times the number of seats inside of Germain Arena! Luckily for the Southwest Florida community, Jeanne Nealon is tackling this problem head on. Laces of Love is a non-profit organization that provides new shoes to low income and disadvantages children in both Lee and Collier Counties, while educating the community about their needs. As Co-Founder and President, Jeanne is doing all that she can to find a solution to this issue. “It started when I

FOR KIDS IN NEED took an 8th grade student of mine home, and he had been wearing some of the worst shoes I had ever seen! So I gave him a new pair, and noticed it made such a difference. Then I started looking at my migrant students and the ones living just above the poverty line, and really saw the need was so great.” With the help of a volunteer Board of Directors, including two additional Co-Founders, Susan Warren and Mary Myrmo, Jeanne was able to bring her cause to life, growing little by little each year. In many schools, she is known simply as “The Shoe Lady” because she usually shows up pushing a rolling bin full of stylish, brand new sneakers. The kids light up at the sight of her, while patiently waiting to see what pair of shoes she may have for them. “We try to have an assortment of colors and styles for them to choose from, since it’s important for us to give them something that’ll boost their confidence,” Jeanne adds. Now working out of two separate storage facilities, the team keeps thousands of shoes bagged up, labeled, and ready to be distributed throughout Lee and Collier counties. Julie McLeod,
Board Member and volunteer, says, “It would be really great if someone could donate some warehouse space to us, but for now we will continue doing whatever we can, even if that means storing some shoes in our own homes!” Over the years, Laces of Love has partnered with various organizations in Southwest Florida, including PACE Center for Girls, Dress for Success, Our Mother’s Home, Salvation Army, and the Red Cross, as well as all Collier and Lee County schools. In fact, over 125,000 children have received new shoes from Laces of Love since 2004! This isn’t something that happens overnight; Jeanne has dedicated countless hours to her cause, educating our community about the overwhelming need for support of underprivileged youth. Thanks to her passion and drive, she has started a movement that shows no signs of stopping.
You may now be wondering what you can do to make a difference. If you are a member of a community association, alumnae group, athletic team, church organization, sorority or fraternity, or have a few good friends and family you can gather together, then you’re ready to host your very own shoe party. Simply invite everyone to lunch, dinner, cocktails or any event, and ask that each person brings one or more pair of new sneakers for a child in need. All sizes are needed, especially sizes 1-6 for boys and girls. Monetary donations are also greatly appreciated. You can also request a guest speaker from Laces of Love to attend, who will help introduce the organization and share their story.
