ir Conditioners may run year round in Southwest Florida, but most systems go into over drive in the hot, humid summer months. It’s time to give your cooling system a little TLC, and the team at Best Home Services is ready to assist. Brothers Dave McClanahan and Adam McClanahan are a new part of that team. They got their start in the air conditioning field while living in Virginia. Dave says, “My first job was in the heating and cooling business right out of high school.”
Adam followed in his big brother’s career footsteps saying, “I started helping in new construction, then took classes. I love seeing the smiles on customers’ faces when they are nice and cool.” The brothers have a few tips on how to stay cool while managing your energy use this summer. The most important thing, hands down, is regular maintenance. Adam says, “It’s like going to the dentist or changing the oil in your car. It’s something you want to do on a regular basis, every 6 months.”

In between scheduled maintenance, the McClanahan’s say it’s important that homeowners do a little ‘homework’ such as regularly changing air filters. When facing down the variety of filters in the retail aisle, Dave says, “Everyone has their own opinion on which ones are best, but I say you don’t have to buy an expensive filter, cheap ones are fine and the pleated ones put more strain on your unit. He says, “It’s wet and dark and stuff likes to grow in there, it’s a common thing. However, bacteria can cause health issues and allergies.” And about that service agreement, he adds, “In Florida air conditioning systems sometimes only last 8-10 years, so those maintenance agreements are pretty important.” When it comes to setting your thermostat, extremes are not good according to the brothers. For instance, if you turn your thermostat up high when you leave for the day then turn it way down when

you get home, your system has to run longer to get to the proper temperature. They recommend not exceeding a high of 78 degrees. hen Best Home Ser vices technician, Dave McClanahan, went on a call to the home of an elderly woman known as Ms. Marie, he says, “It was over 90 degrees in her home.” She was struggling on several levels and could not pay for a new AC. He says, “We come across a lot of people who need help, but there was something about her; I had to help.”
Dave called the technician who had seen Ms. Marie a few weeks earlier and they hatched a plan, personally calling Goodman Air Conditioning for a donation. Then they approached the managers and employees of Best Home. Within minutes of sending out a text thread, Dave says, “It blew up and everyone in the company got involved.” They donated time, money, anything they could.

Zach Mersch, Brand Building Manager says, “One of the biggest core values that we instill in our employees is family. In everything we do, we treat others like a member of our family, and they took it to heart.” The volunteer project was not simple. They had to reposition the air handler and used the extra funds to pay for new ducts, hoses for her leaking water heater, and more. They even called in help from electric and plumbing friends so Ms. Marie would be safe and not have to worry about mounting home repairs. Zach marvels, “These guys gave up their whole weekend and more to help her.” She has since become like an adopted Grandmother. The project has done more than help a customer in need. Zach says, “It lit a spark in us, opened our eyes to start a new program to help our neighbors.” Best Home is currently working with vendors to partner in an ongoing program to help residents like Marie. Dave says, “This is the best company I’ve ever worked for. They are helping the community. It’s a good feeling, helping another human being.”

Don’t Let Them Bug You This Summer It’s of f icially summer time here in Southwest Florida, bringing an abundance of sun, shorts, and for many bothersome spider veins. Our practice treats all types of venous disease, the most common being varicose veins and spider veins. Spider veins can present in a number of different ways, and although they are usually not painful, the noticeable appearance of them can be bot her some. Many t imes spider veins are even considered “cosmetic”, however, in some cases, spider veins can be a manifestation of a deeper problem called venous insufficiency. Venou s in su f f icienc y ( V I ) i s a common disorder that occurs when the valves within our veins begin leaking blood, causing a backward flow of blood to the lower extremities and an increase of venous pressure in our legs. Commonly experienced signs and symptoms of VI include bulging varicose veins, swelling, leg cramps, restless legs, tingling and numbness in the feet, heaviness and achiness of

the legs, changes in skin texture, skin discoloration, and even ulcerations. If lef t untreated, these signs and symptoms of VI have the potential to worsen over time. Spider veins, on the other hand, can present in a number of different ways. They often appear as red or purple branches on the superficial parts of the skin, either lying flat on the skin or with a raised appearance. In some cases, groups of spider veins can take on a “bruise” like appearance, as they cluster into groups called telangiectasias. Spider veins in the thighs or even the calves are a cosmetic issue that can be safely managed in the office setting with injection sclerotherapy. This is an effective treatment using an injected chemical irritant with a very small needle into the spider veins to cause them to sclerose or scar down over several weeks to months and slowly disappear. It may take several treatments and periodic maintenance therapy to achieve or maintain optimal results.

While most cases are cosmetic, for some patients spider veins present as signs of venous insufficiency which may need to be corrected prior to injection sclerotherapy. These patients may experience a larger amount of spider veins lower down the legs towards the ankles and feet which can progress to the point of spider veins making the feet look purple, especially when sitting or standing. These patterns of spider veins are often related to high venous pressure in the legs resulting from underlying venous insufficiency. A complete treatment of VI can be accomplished with an endovenous c l o s u re u s i n g e i t h e r a l a s e r o r radiofrequency catheter that allows the heating and permanent sealing of the underlying leaky veins. This will decrease the venous pressure in the legs and significantly improve vein related signs and symptoms which may, in turn, assist in the disappearance of spider veins in the lower legs. The procedure is minimally invasive and is performed under local anesthesia. Patients are able to resume their normal activities in just a short period of time after treatment. Several weeks after the underlying venous pressure has been corrected, injection sclerotherapy can
be performed to clear up any residual spider veins. Removing excess fluid might make sense for the reduction of swelling, but how does wound care fit into the picture? It is hard for wounds to heal when the body has poor circulation. Parker says, “It’s also important to use proper dressings. Individuals trying home remedies to heal a wound without proper dressings are at a higher risk of infection, which may lead to hospitalization.” Shanaberger says, “They gave me these bandages that helped to absorb moisture from my wound while the pump was working. It was kind of a one-two approach to my healing. The bandages took care of the outside and the pump took care of the inside.” The process helped to dry out his wound in just a couple of weeks. Parker adds, “I’ve seen cases where patients have been unsuccessfully treating a wound for as long as a year. With the right treatment and specialty dressings, our goal is to get the wound healed within 45 days.” Shanaberger uses the compression pump one to two times a day and says, “It makes my leg feel good. It’s kind of like a gentle blood pressure cuff where the pressure builds up then moves from the foot to the thigh, pushing all of that fluid up and out.” He still uses a topical cream on the wound area and still bandages his foot before going to work to keep it dry but says, “The pump has made a world of difference in the size of my leg and my quality of life.” The compression pumps are portable so patients can travel with them. r. Peter Ameglio is creating a buzz in Southwest Florida amongst his patients, their families, friends and physical therapists, for the remarkable recovery of his patients. The surgeon uses minimally invasive techniques for hip replacements and treating sacroiliac (SI) joint dysfunction. In fact, he is the only orthopedic surgeon in SWFL to perform both the SuperPATH® approach for total hip replacements, that he brought to our community nearly 2 years ago, as well as iFuse SI joint surgery to resolve pain below the belt. SuperPATH is a revolutionary approach that was developed at New England Baptist Hospital in 2003 by Dr. Stephen Murphy. Dr. Ameglio was invited to observe Dr. Murphy during one of his studies where he witnessed first-hand that the SuperPATH technique resulted in less injury to the patient than hip replacements requiring hip dislocation and larger incisions. In traditional hip replacements a dislocation of the hip is required. This results in injury to the joint capsule and small muscle attachments of the hip. Following surgery with traditional approaches, the patient is instructed in hip precautions or certain positions that may cause joint dislocation.
The SuperPATH approach does not carry any of these restrictions because the joint is never dislocated in the first place. In addition, the capsule and muscles are not injured during surgery allowing for a faster recovery. This technique is not easier to perform for the surgeon, but is significantly easier on the patient. A leading cause of hip pain is arthritis. However, hip pain can also be a sign of sacroiliac joint dysfunction. Both can present with similar symptoms such as groin pain and decreased hip range of motion. An example of this is a patient who has had a hip replacement, yet they continue to have pain. Dr. Ameglio became interested in SI joint dysfunction as he found patients with hip pain were often resolved with SI joint injections. The symptoms of sacroiliac joint dysfunction or sacroiliitis can mimic those of an arthritic hip, even though the hip is okay. Sacroiliitis can present with posterior hip pain, groin pain, thigh pain, and pain radiating down the leg. Sacroiliitis is a diagnosis that can fall between the cracks leaving patients in chronic pain and on an exhaustive search for answers and relief. This includes patients who have had back surgery. Dr. Ameglio says, “Of the patients who have had a back fusion, 45% may

What are the symptoms associated with trigger finger?
Often individuals will notice that they have a painless catching or clicking in their finger when they bend and straighten it. This can progress to pain in the finger and palm associated with the clicking, or triggering. Symptoms are typically worse in the morning and improve as the day progresses with increased use of the hand. As symptoms become more severe, they can lead to a locked digit and inability to flex and extend the finger.

What are the main causes of trigger finger?
M os t of t he t ime t he c ause is unknown. However, trigger finger can be associated with diabetes, rheumatoid arthritis, gout, or amyloidosis. It commonly occurs during the 5th to 6th decade of life and it is more common in women than men. The underlying pathology consists of a thickening of the flexor tendon, which bends the finger, and the tunnel system in which the tendon travels. As the space within the tunnel narrows, it
makes it more difficult for the tendon to enter and exit the tunnel system. This causes the mechanical symptoms known as “triggering”.
What are the treatment options for trigger finger?
There are both non-operative and operative treatment options for trigger finger. If someone presents early in the disease process, the symptoms can sometimes be abated by taking oral anti-inflammatory medications. However, the mainstay of conservative treatment typically consists of a corticosteroid injection around the start of the tunnel system in the palm of the hand. Occasionally, more than one injection is necessary to obtain resolution of the symptoms. Unfortunately, the injections are not as efficacious in diabetic patients. If a trigger finger is not responsive to conservative treatment with injections, then surgical intervention can definitively resolve the triggering.
What does surgery for a trigger finger entail?
The surgery for a trigger finger can be per formed in a surger y center under minimal anesthesia. Through a one centimeter incision in the palm of the hand, the beginning of the tunnel system is enlarged to allow the tendon to successfully pass through it. The triggering resolves immediately following the surgery, and is a very successful and reproducible procedure. A recent stor y from Canada highlights the potential “worst case scenario” for passwords lost due to an owner’s death. This case involves the estate of Gerald Cotten, a Canadian citizen who reportedly died while in India this past December. Cotten ran a “cryptocurrency exchange” based in the Canadian province of British Columbia.

Even if you have never dealt with cryptocurrencies yourself, you likely have read about them in the news. A cryptocurrency is basically a complex computer program that creates virtual units that can be bought, sold, and used in a manner similar to traditional money. But cryptocurrencies rely on a decentralized ledger, rather than banks, to record all transactions. Cryptocurrency users typically store their account information in a “wallet,” which can be either a software program running on a computer or a piece of hardware. These wallets do not store the actual cryptocurrency–that is the function of the ledger–but rather contain the decryption keys necessary for the user to receive or send currency units. If the password to the wallet is lost, it is virtually impossible for anyone to access the cryptocurrency associated with it. And that is what apparently what happened in the Cotten case.

The widow said her husband kept most of the exchange’s keys in offline hardware wallets–and she does not know their passwords. This means the exchange’s customers cannot withdraw funds from their accounts. The exchange has subsequently filed for creditor protection, according to the Canadian Broadcasting Corporation. Keeping Your Passwords Safe– But Accessible to Your Estate
Setting aside the question of whether it is wise to invest your money with cryptocurrencies in the first place, t he Cot ten s tor y raises a more general concern regarding the use of passwords. Most of us carry around laptops and smartphones that rely on

encryption to keep our data secure. The problem is that if someone needs to access that data after we die, they may be unable to do so unless we leave some record of the passwords behind. One way to mitigate this problem is to use a password manager. This is a program that allows users to store all of their passwords in a database. Many password managers are “cloud” based, so the database is actually stored on a third-party server. The user then only needs to remember a single “master” password to access the database and retrieve the individual passwords stored as needed. For estate planning purposes, you should consider keeping a written copy of your master password in a safe place that your personal representative or family can access after your death. hen Abelino Garcia fractured his wrist, the Urgent care physician told him he would need surgery. That surgery was estimated to cost $15,000. His source of income is from contract work as a carpet installer. $15,000 is a bill Garcia could not even think of paying with their income level and on top of that, he was now out of work. The Bonita Springs resident sought help from Café of Life, which connected him with We Care. We Care is a program administered by the Salvation Army Fort Myers Command in collaboration with United Way, Lee Health/Lee Community Clinics, Lee Count Medical Society, and the Department of Health that helps low income men and women with medical services. Patient Coordinator Jacqueline Galindez says, “To qualify for help mylivinglocal.com | JULY/AUGUST 2019
from We Care, a person must be a Lee County resident with a life-altering condition. Their income must fall below 200% of the federal poverty level, be uninsured and not eligible for any other government programs, and be between the ages of 19 and 64.” Once a person is accepted, Galindez reaches out to one of the approximately 200 medical specialists in the program to see if any are willing to help, without compensation for their time or skills. In Garcia’s case, Galindez contacted o r t h o p e d i s t , D r. Pete r A m e g l io. Although he is known for SuperPATH® total hip replacement and treating SI joint dysfunction, orthopedic surgeons are trained in the care of all bones and joints. Dr. Ameglio not only enjoys maintaining those skills but also likes treating the whole person.

“Mr. Garcia had a fracture of the distal radius eight or nine months prior to seeing me. The ends of the bones were not even in the same zip code so it left me very few options,” explains Dr. Ameglio. Basically, he could not use his hand because it was no longer properly connected to his arm. Dr. Ameglio fused Garcia’s wrist and he is now back to work and living without pain. The surgeon took Garcia’s case for a number of reasons, “He is Spanish speaking and he could not get or afford care. I c o m e f ro m P a n a m a; I understand that level of poverty, as I’ve seen it first hand. He came here for a better life and I felt for him.” Garcia has been out of work since last September. He has a si x- year- old daughter and his wife has taken on grueling jobs such as picking tomatoes to suppor t t he family. Not able to provide for his wife and daughter and holding back tears, Garcia says, “I was depressed and felt like I was less of a man. Dr. Ameglio gave me hope that my hand will be OK and that I can get back to work. It will change us financially and mentally, being able to provide for my family as I always have.” Dr. Ameglio says, “I think everyone should have access to healthcare. The fact that he is 47 years old and one fracture takes him out of commission for life is tragic. The technology is available to fix these things, it’s just access to care.” Although We Care connects those in need with physicians who can help solve an issue on the front end, patients still need assistance on the
back end. For instance, Galindez says, “Our specialist can remove a ca ncerou s tu m o r, b u t w e d o n’ t h a v e the means to pay for chemo if that is needed, or any other medication that a patient may need.” The operating expenses for We Care come through grants and donations. Galindez gets 100 calls a day asking for help and her cases are backed up for months with people like Garcia waiting for help. More funding would allow We Care to hire another patient coordinator to help. Her vision is to see other Salvation Army organizations, across the state and nation, adopt We Care to serve the medical needs of low-income residents so that they can get back into the work force. We Care also helps with dental. Galindez says, “All of the credit goes to the specialists. Without them we wouldn’t have a program. They take their time to see patients for doctor visits and surgeries so they can go back to work, provide for family and contribute to our society.
