Fall is finally here! Although we haven’t felt a shift in the weather quite yet, we know it’s here by the start of a new football season. I just love this time of the year – so grateful for the memories my son and I have shared and look forward to creating many more. September is also a month that finds families trying to get back into a groove, settling into a routine now that school is back in session. While residents are recovering from the summer, many restaurants report a slow down so it’s a great time to walk into a local eatery and actually find a seat. In short, it’s a month where living local is extra important for many businesses. On the more serious side, September is also Gynecologic Cancer Awareness Month in the State of Florida so this month we are spotlighting the Betty Allen Gynecologic Foundation in our “For A Greater Cause” column. Every five minutes a woman in the U.S. is diagnosed with a gynecologic cancer. Since no screenings exist for most of these cancers, the foundation is on a ART DIRECTOR

“Heel pain is one of the most common conditions that podiatrists treat, as well as ingrown toenails,”
says Dr. Castellano, who specializes in treating the foot and ankle at Gardner Orthopedics. Most heel pain is caused by plantar fasciitis according Dr. Castellano. There is a band of tissue that spans across the arch of the foot from the heel to the toes. He says, “It’s a thick ligament and if it is damaged it doesn’t heal well. This has a lot to do with arch support, and over time, the arch tends to fall a bit as things settle.” livinglocalfl.com | SEPTEMBER 2018
This causes the band to stretch a further distance while other muscles get tighter and we become less flexible. The combination doesn’t work so well. Many factors come into play when diagnosing heel pain. Individuals suffering from heel pain of ten go to retail stores spending hundreds, even thousands of dollars, on arch supports in hopes of easing their discomfort. While these devices can offer relief for some, more often than not, people in pain continue to spend time, effort, and money trying various types

of arch supports when the arch might not be the source of the problem at all. If you are suffering from heel pain and want to try arch supports, you should not invest more than $50, according to Dr. Castellano. He cautions if pain persists with the arch support or you have been rolling a tennis ball on your heel and it’s not feeling better, it’s time to seek help from a physician. The condition will likely worsen the longer you put off seeing a doctor. Dr. Castellano explains, “Ligaments can get thick and degenerate, which makes

healing time longer and more difficult.” The pain you are feeling could be coming from a number of other areas than the heel. This is where you need the expertise of a physician. “We will evaluate everything from the hips down. Sometimes you have a bone bruise or stress fracture and arch supports won’t help.” He continues, “Sometimes there is a problem in the joint between the ankle and heel bone that masquerades as plantar fasciitis. It’s another reason why an x-ray is important. In this case injections can help.”

“It’s not the first time the doctor has created something from scratch. He actually built a a 1974 Honda CB360.” Another misconception is that heel spurs are a primary source of pain. Dr. Castellano says, “People assume if they have a shot it will dissolve the spur, but that is not always the case. I’ve seen people with very large spurs and they are in no pain at all.” There are a myriad of treatments available once the source of your pain is determined. Dr. Castellano goes the extra mile to not only get to the root of your pain but also to find creative solutions for relieving it. Case in point, the “boot.” Many patients are put in a cumbersome walking boot so they are able to heal following an injury or surgery. The bottom of the boot is very thick so it can rock when you walk and protect the foot. The boot causes the thick soul to raise your foot higher than the shoe on the uninjured foot. The result is a discrepancy in the length of the legs. Dr. Castellano explains, “Invariably, people would stop wearing the boot because the discrepancy hurt their hip so much. Some patients would even go to cobblers and have extra soles glued on to their walking shoe to level their gait. However, Dr. Castellano says, “It was both expensive and wasteful. Patients only needed a fix for 6-8 weeks and then they’d throw away the raised shoe.” Enter the EVENup™ Shoe Leveler, a detachable device that fits securely around a flat shoe, lifting it to the height of the boot for balance. He laughs, “I didn’t know a thing about pistons and valves, but you can learn anything on YouTube.” Several years ago, Dr. Castellano left Southwest Florida for Atlanta, to be with his father who was battling prostate cancer. That is where he learned of the Distinguished Gentleman’s Ride to raise money and awareness for men’s health issues. Instead of buying a motorcycle to join the ride, he built one and is currently building his second. You can find rides in 650 cities worldwide in September, including Miami, which Dr. Castellano plans to participate in. But on a daily basis, he says he often thinks twice before hitting the streets in Southwest Florida, “It’s crazy here. I’ll ride short distances, but I get more enjoyment out of building the bike than riding it.” @moreoptionslessrisk
Giving back includes more than participating in the Distinguished Gentleman’s Ride. Dr. Castellano has traveled to Guatemala with colleagues to treat children with foot and ankle problems. He says, “It was overwhelming to me. It felt so good to help a child born with a clubfoot. But what left an even greater impression was the ones we couldn’t help. It hit me hard.” About his career and his work at Gardner Orthopedics, he says, “I feel really blessed. ary Anne Simpson’s story is one of perseverance, not accepting “no” for answer, and how Dr. Joseph Magnant, founder of Vein Specialists, restored her health and sanity. An active mother who plays tennis several days a week, Simpson was suffering from chronic itching below the knees. As time went on, the itching became intolerable, eventually manifesting into a rash, that at times covered a large portion of her legs. She remembers, “I had to wear long tights playing tennis because someone asked if I was contagious. The itching was so bad I felt like I was losing my mind.” It wasn’t just the physical discomfort that made her feel that way, but also the parade of physicians she went before, many of whom told her there was nothing wrong with her or nothing they could do to help her. Simpson lives in the Chicago area. Over the course of two years she spent thousands of dollars and saw numerous, highly recommended physicians, who offered little to no relief and provided few answers. What she was actually suffering from was stasis dermatitis, a condition where blood pools, or is static, rather than circulating. She did extensive research and even asked physicians about the possibility of stasis dermatitis, but the condition continued to go undiagnosed. Initially Simpson called upon dermatologists saying, “I went to five different doctors who prescribed lots of expensive ointments and antibiotics.” One doctor even tried to tell her she had fleas.

Extensive allergy tests came back negative. She tried changing her diet, even made her own soap, but the itching persisted. “I thought I was going to suffer the rest of my life. It sounds so trivial but when it flares up, you literally think you are going to go out of your mind.” One particularly itchy evening, she turned to the Internet looking for support. That is when she found an old blog on venous insufficiency and stasis dermatitis, written by Dr. Magnant. He explains, “Stasis dermatitis, in general, is a skin condition, but it’s a manifestation of an underlying venous problem.” The first stop for most people who are suffering from this is indeed, a dermatologist, who typically prescribes a steroid cream. Simpson says, “I was using steroid cream like a moisturizer.” However, Dr. Magnant says, “You have to identify and fix the underlying problem so it will not recur.” Desperate for help, Simpson reached out to Vein Specialists and within weeks bought a plane ticket to Southwest Florida for an evaluation with Dr. Magnant. After nearly three hours of comprehensive ultrasound testing, she remembers, “I could tell by the face of Brian Sapp, the Vascular Technologist performing the ultrasound, that he found something. I felt like crying. I thought, oh my God, I DO have this and I’m NOT losing my mind.” She had a blockage in the lef t common iliac vein and there were issues with additional veins in both legs and the pelvis.

Why did Simpson slip through so many cracks? Dr. Magnant says, “Stasis dermatitis is not that hard to diagnosis, but physicians, in general, don’t receive a lot of education on it or venous issues.” Because of that, Dr. Magnant spends a lot of time reaching out to other physicians in the area saying, “Sometimes you have to be imaginative and I want them to know of the things we can fix. Venous disease intersects with dermatology; skin and veins collide. What I like to refer to as dermatovenous disease.” Venous Insufficiency often progresses as people age. However, Dr. Magnant says, “I have seen patients in their 20’s and 30’s who have these problems.” Simpson adds, “Except for the nagging rash and some discoloration in my calf, I was not the stereotype of someone who has vascular problems.” Since finding Dr. Magnant, she has traveled to Southwest Florida twice for venous treatments, with a third procedure scheduled in the future. “It is so good to feel normal again,” says Simpson. “Another unexpected benefit is that my energy level during long tennis games is markedly better. I had no idea that would be the case.” Not only has she found hope and relief at Vein Specialists, but she’s also made new friends during the process. Simpson concludes, “I hope that my story will help other women out there who are like me. Dr. Magnant gave me my life back.”

Expanding Treatments to Improve Aesthetics

the abdomen, buttocks, love handles, inner thighs, back of the thighs, and arms. The procedure helps to firm skin, minimize f ine lines and wrinkles, improves microcirculation, and even the appearance of acne scars. seem like it, but we’re working hard to get to the customer’s house as soon as possible.” Cole explained how all the technicians, managers, and dispatchers work together to make sure that they get the job done fast and efficiently. Cole has learned in his position as a dispatcher that there’s a lot that goes on behind the scenes. “There’s a lot more involved in my daily activities as a dispatcher.” Cole talked about how he’s constantly checking to make sure that technicians are on time to the customer’s house. “Daily, I’m updating the customer on the technician’s whereabouts.” Cole, who is originally from Minnesota, moved to Southwest Florida a week before he started working for Best Home Services. “I’m from a small town in Minnesota that has about 1,200 people.” Cole said that he moved to Southwest Florida for more job opportunities. “Most people in the small town where I’m from, never leave it.” Cole said that he wanted

to do something different with his life, so that’s why he came to Southwest Florida. “I have one grandma that lives in Estero, and I have some cousins that live in the area too.” Cole says that she sees his grandma frequently. Cole loves his dog named “Dom” short for Dominic. “Dom is a big baby.” Cole says that Dom is basically his best friend. “Dom is very well behaved, and he’s a great dog.” When Cole is not working, he can be found working on cars. “I’ve had 25 cars in the 6 years that I’ve been driving.” Recently, Cole was in a car accident, so now he’s looking for another car. “I like to restore and modify cars.” Cole said once he can find a place that has a garage, he’s going to bring his 1961 Buick Lesabre from Minnesota. Perhaps one of the biggest myths about extensions is that they will damage your natural lashes or cause infections. or some people with vision problems, putting in and taking out contact lenses every day is as second nature as brushing your teeth. But for others, it seems an impossible task that sometimes doesn’t deliver the desired results. Dr. Jay Rosen has made a name for himself in Southwest Florida over the last twenty-plus years. As an expert in his field of optometry, he specializes in finding contacts that work for patients with cornea issues, such as keratoconus, and those who have other challenging conditions. Keratoconus is a progressive thinning of the cornea that causes it to bulge. Dr.
Rosen explains, “The surface becomes irregular. Neither glasses nor soft lenses will work because they translate a bad image and blur vision. Hard lenses help to create a new surface, but most doctors have trouble fitting them.” That’s one of the things that happened to Susan Kane. Struggling for years trying make contacts work, she laughs, “I was a contact failure. I couldn’t get them in; I couldn’t take them out. They would stick to my eyes.” Then she met Dr. Rosen and remembers, “He said ‘you won’t be a failure with me.’ ” She had three laser surgeries, which changed the surface of her eye. As she aged, the surface continued changing

at a rapid rate. Kane remembers, “I was seeing Dr. Rosen every month to get things stable and get the right contacts. We kept working until we got it right.” “If anybody can fit you correctly, it is Dr. Rosen; and if anyone can teach you how to put in contacts, it’s his assistant Carrie. They are the perfect team,” says Kane. As a law enforcement officer, seeing clearly is crucial to her job. Now she doesn’t need to keep multiple glasses on hand and she is able to put her contacts in with ease every day. “Contact lenses are one of those things that people have a laissez-faire attitude about,” says Dr. Rosen. “But they are medical devices that can cause permanent changes to your eyes, so there is a responsibility to take care of them correctly.” That includes removing them and wearing them as instructed by your doctor and according to manufacturers directions. Dr. Rosen cautions, “A lot of people over-wear them and get infections that in the worst-case scenario, can lead to blindness.” He adds, “One of the worst things you can do is sleep with contacts in.” While they improve your vision, contacts decrease the amount of oxygen that gets to the eye. If you don’t take them out when you are supposed to, that lack of oxygen can compromise the eye, leading to bacterial infections, swelling, and a host of other problems. “Contacts are like a sponge pulling moisture out of cornea,” says Dr. Rosen. If dry eye is something you suffer from and want to wear contacts, it is very important to treat that condition. He also advises the use of artificial tears (not natural tears) throughout the day for contact wearers. But his number one advice is, “Get your eyes checked regularly.” Besides his expertise in the field, Dr. Rosen’s patients almost always comment

on his patience, his caring bedside manner, and his wit. A personable doctor who remembers faces and names, he says, “I enjoy people. I talk to my patients about their lives not just their eyes.” Not to mention, he says, “It feels good to help people who can’t see. It changes their lives.”
About Dr. Rosen A self-proclaimed foodie and “mosttime vegetarian,” Dr. Rosen recently moved his practice to Collins Vision where he acts as Clinical Director. Originally from Long Island, he is a graduate of Cornell University and received his Doctorate of Optometry at State University of New York in Manhattan. Following his residency, he was hired full time, eventually overseeing the university’s contact lens and cornea department. A
s an orthopedic surgeon, the topic of bone health and osteoporosis typically arises after my patients have sustained a traumatic fracture of their bones. As a woman and an orthopedic surgeon, bone health and fracture prevention are near and dear to my heart. I hope this article can shed some light on the importance of bone health and ways that you can prevent a fracture. The following are a few common questions I encounter frequently by my patients:
Why does bone health become so important as we age?
Our bones are living and dynamic structures. They are continuously remodeling, which means that new bone replaces old bone. When we are younger, we actively build new bone so that there is net positive bone growth. The opposite phenomenon occurs with aging, where we favor more bone resorption than formation. As the end result with aging, our bones become more porous and the strength decreases; thereby predisposing individuals to hip, spine, and wrist fractures.
How can I maximize the strength of my bones?
Peak bone mass is achieved by the late teenage years, and it occurs slightly earlier in women than men.

There is slow, inevitable decline in bone mineral density after the age of for t y in both women and men. Considering that our bodies are not able to produce calcium, and 99% of our calcium stores are in our bones, we will mobilize calcium from our bones when we are calcium deficient. Therefore, daily consumption of calcium via supplements and our diet are the most effective way to maintain our balance of calcium and prevent resorption from bone. In addition to calcium, vitamin D is integral to maintaining our calcium homeos tasis, as it promotes the resorption of calcium from our intestines. Therefore, without vitamin D we cannot absorb the calcium we take in. In addition to maintaining a healthy diet and weight, incorporating weight bearing exercises such as walking, running, and tennis to your daily
routine will help to maintain your bone strength. Avoiding tobacco use and excessive alcohol consumption will help reduce bone loss. The American Academy of Orthopedic Surgeons’ website is a great source for information on recommended daily values of calcium and vitamin D intake.
What is the difference between osteopenia and osteoporosis?
Both osteopenia and osteoporosis are defined by a decrease in bone density. However, osteoporosis is more severe, and has been coined as the “silent disease” as it often is not recognized until a fracture or break in the bone occurs following a low energy fall. Falls are the leading cause of injury among persons aged 65 and older in the United States. Fractures are the primary cause of hospitalization or death following a fall, and the mortality rate is approximately 30% following a hip fracture. One in two women and one in four men older than age 50 will have an osteoporosis-related fracture in her or his lifetime.
How do I know if I have osteopenia or osteoporosis?
Bone density exams, also known as a DEXA, are screening tools to monitor an individual’s bone density. In general, it is recommended that all women over the age of 65 and men over the age of 70 obtain a DEXA scan. Based on the test results, your primary care physician can help guide you regarding treatment options if necessary.

Grilled Steak with Tomato-Basil Salsa INGREDIENTS (4 Servings)
DIRECTIONS
• 2 lbs. skirt steak
Preheat grill to medium-high heat.
• 4 plum tomatoes
Slice the tomatoes and place in a bowl. Drizzle 2 tbsp. of olive oil over the sliced tomatoes and season to taste. Place sliced tomatoes on the grill and cook until charred and soft. Remove tomatoes and let cool.
• 4 tbsp. olive oil • 2 tbsp. balsamic vinegar • 1/4 cup fresh basil, minced • 2 tbsp. fresh flat-leaf parsley leaves, minced • 2 garlic cloves, minced • 2 tsp. sea salt • 1/2 tsp. black pepper • 1/4 tsp. crushed red pepper
Dice the tomatoes and place in a bowl. Add basil, parsley, vinegar, garlic, crushed red pepper, 2 tbsp. olive oil, then toss to mix all the ingredients. Season the steaks to taste. Grill 3 to 4 minutes per side. Serve the steak topped with the tomato-basil salsa.

Gynecologic C Cancer

eptember is Gynecological Cancer Awareness Month in the state of Florida. The roots of this designation lie in Southwest Florida with the Betty Allen Gynecologic Cancer Foundation, the brainchild of Sharie Kearns.
State Representative Heather Fitzenhagen is a board member of the organization and sponsored a resolution last year hoping to foster more awareness of these types of cancer. The foundation was first established in 2011 as the Betty Allen Ovarian Cancer Foundation. Betty Allen is Kearns’ mother. Twelve years ago, Allen was diagnosed with stage 1 ovarian cancer and not only did she survive, but she is still thriving today thanks to early detection. The experience was so impactful that it set Kearns on a new life path.
Ovarian is the deadliest of all gynecologic cancers and there is no screening for it. On top of that, most women are unaware of the symptoms. Kearns’ mother had just lost her husband and was experiencing fatigue and bloating. She never mentioned it to anyone, attributing the symptoms to grief. Then one day she fell in a store and injured her hip. An MRI revealed there was more going on in her body than a bruised hip. Through the ordeal, Kearns became a relentless advocate for her mother and ovarian cancer. With the encouragement of her mother’s physician, Dr. James Orr, she channeled her energy into creating a foundation to help others. She remembers, “He knew (Dr. Orr) there was a need for education and there was just nothing out there at the time.” Dr. Orr currently sits on the foundation’s medical advisory board.

“I was 46 years old when my mom was diagnosed, and I knew nothing about ovarian cancer or the symptoms,” explains Kearns. “It just shocked me that smart educated women think ‘because I get my pap I’m fine.’ They walk out with a negative pap smear but could still have ovarian cancer.”
After the foundation incorporated in 2011, inquiries began flooding in for help with other gynecological cancers such as cervical, uterine, vulvar, and vaginal.
“According to the American Cancer Society, every five minutes a woman in the U.S. is diagnosed with a gynecological cancer,” Kearns says.

Although Kearns’ vision is a big one, the current focus of the Betty Allen Gynecologic Cancer Foundation is on education. Kearns and her board are armed and ready to go into the community offering free educational workshops (Know the 5 and Survive) to residential communities, colleges, businesses, religious organizations, and women’s groups. Betty Allen herself is known to make cameos at these events. She says, “I’m so proud of my daughter for doing this. It gives me chills.” It is estimated that in 2018 more than 110, 000 women will be diagnosed with a gynecological cancer and more than 32,000 will succumb to it.
She continues, “I can’t prevent cancer but I can educate.” While a high percentage of local women are handed a stage 3 or 4 cancer diagnosis according to the advocate, she says, “Hopefully in a couple of years Dr. Orr will call me and say they are coming in at stage 1 or 2 instead. The earlier gynecologic cancer is detected and treated, the higher the survival rate. Then I know we’re doing our job.” The foundation encourages everyone to visit its website at GynCancerFL.org to educate yourself on the symptoms and risks of gynecologic cancers and sign-up to attend the Know the 5 and Survive Workshop. The foundation also has a variety of volunteer needs, sponsor, and donor opportunities.
