2009 ith summer overEstablished and the back to school routine underway, stocking up on school supplies and updating wardrobes aren’t the only things that should be on your list. This is the perfect time of year to check in with the pediatric professionals at Park Family and Cosmetic Dentistry to get on track and/or stay on track with your child’s oral hygiene. According to the Academy of Pediatric Dentistry, a child should have their first dental visit by the age of one. While that may seem young to some parents, it’s important to set the precedent early for good oral hygiene according to Park Family’s pediatric specialist, Dr. Leanet Ascunce. She says, “It’s important to see your pediatric dentist by this age so that parents can be educated on how to keep their child’s oral health in tip top shape. We teach techniques which can make brushing a baby and toddler so much easier on both the parent and the child.” Her aunt was a dental assistant in Cuba and as a young girl, Dr. Ascunce would go to the dental office during school breaks

rather than go home. She says, “At Park Family, our goal is to have our patients feel as though they are family. Pediatric dentistry is the first exposure to good oral health. We want to make this experience as good as possible to shape a child’s view of the dentist as a positive one.” Park Family goes the extra mile to make sure that happens, beginning from the time your child walks through the door. The waiting room is kid friendly and the staff is trained to be fun and outgoing. Dental experiences here are tailored to each age group from toddlers to teens. Positive reinforcement is provided throughout the dental visit. Dr. Ana Scopu knows from personal experience how important that first impression is. Growing up in Serbia, her first experience was horrific; “I had 14 cavities, a root canal, AND I left with a scar on my lip.” Specializing in orthodontics, Dr. Scopu can see skeletal patterns forming in babies and toddlers. This is another reason why early dental care is important, since some of these patterns can predict the eventual need for braces. This allows

Dr. Scopu the time to plan ahead with parents in devising treatment plans. Orthodontic consults can begin as early as 8-9 years of age, while braces aren’t typically applied until a child hits puberty. Braces do much more than straighten teeth. Applying them when bones are still growing allows Dr. Scopu to subtly change the skeletal structure of the mouth and help prevent more serious problems from forming as an adult. Park Family is the only dental practice in Naples to use Insignia Custom Braces, made individually for each patient. They allow for a more predictable outcome and can shorten the amount of time a child wears braces by approximately six months. When planning a child’s first dental appointment, Dr. Ascunce advises parents to, “Hype up the visit as lots of fun, with new and exciting adventures. Keep the dental terms friendly. Tell them we are going to brush their teeth and maybe take some pictures with our special dental camera.”

She adds, “I really go out of my way to make sure kids have a positive experience so they do not become adults who fear the dentist. Dental health is an integral part of one’s overall health and it’s important that I create a positive feeling towards dentistry early on.” Dr. Scopu says, “ I love seeing kids from very young ages, because if they have a pleasant experience, then they grow up knowing that going to the dentist is not a bad thing and they are hopefully consistent with practicing good oral hygiene.” She makes visits fun and when the parents see what she creates for their child they often become her patients too. BEST HOME SERVICES EMBARKS ON AN AMBITIOUS EXPANSION PROJECT by Gina Birch

B

est Home Services has come a long way from its humble beginnings in 1980 as a small family owned electrical company. Over the years the company has not only added heating, cooling, and plumbing services to its list of specialties, but now there is also an ambitious expansion plan on the books. H eadquar tered in Naples, t he business already has offices in Fort Myers, Sarasota, and Tampa with more than 200 employees. In June, Best Home Services Co-CEO’s and brothers, Chadd and Keegan Hodges, brokered a deal with Alpine Investors in San Francisco giving them more than $100 million dollars to expand throughout Florida and the United States. Ke e g a n e x p l a i n s , “ F r o m t h a t partnership we formed a company called Apex Service with the plan and vision of buying companies across the U.S. that are like Best Home, large platform companies.” Best Home is the platform company for the state of Florida and its first acquisition is Frank Gay Services in Orlando.

The commercial heating and air conditioning company began in 1981 as a one-man plumbing business and now has 175 employees. Now as part of Best Home Services, the Orlando company will be able to not only expand its commercial side, but also add residential services. The result of the acquisition is the creation of one of the largest air conditioning companies in Florida. Keegan says, “We had a vision that we could be much larger than where we are in Southwest Florida. We know there are other like-minded business owners that see the value of joining a group, so if we partner with the right ones, we would have their expertise and capital to be largest in Florida. Likewise, we partner with others outside to be largest in each of the other states where we are looking to expand.” The value to those businesses includes things such as de-leveraging risk. Keegan explains, “You get to a certain size or you’re a certain age in life, this is a way to reduce risk and it

ein disease or venous insufficiency is most commonly thought of as being something that shows up as we age. While it is true that it more commonly presents at an older age, this disease can affect people of all ages. Josh Bolton, a local ER Physician Assistant is a healthy 35-year-old that had been suffering from swollen, achy, and discolored legs for 18 years following an ATV accident. The accident resulted in multiple injuries to his legs and a DVT (blood clot). Josh recalls being only 20 years old and looking like he had “old man legs” because of his swelling and discoloration. He eventually developed venous ulcers, the most advanced sign of venous insufficiency. “Because of this accident, my legs were always swollen, achy, heavy, itchy, and painful. On top of these symptoms they were also horribly discolored and would turn purple, blue, and all different colors.” Josh remembers at his first appointment with Dr. Magnant just how “old” his legs looked for being such a “fit” guy. Dr. Magnant states, “Josh had an ulcer so deep on his one ankle, I could almost put the entire tip of my finger inside his wound.” Josh’s venous disease progressed over the years and resulted in ulcers on both of his ankles. Despite being

in the medical field as a PA, he was unsure of what t ype of specialt y physician to consider visiting, so he procrastinated and self-treated his wounds. As his venous disease worsened, he first visited a vascular surgeon, a dermatologist, and later a wound care specialist. He seemed to be on a rotation of seeing these physicians without any conclusive diagnosis. The wound care doctors would debride his ulcers weekly, applying different prescription creams, and wrap his legs with bandages. According to Josh, “all the doctors I saw treated my wounds locally but never got to the root of my problem, so my ulcers never seemed to make much progress.” After his move from North Carolina to S ou t hwes t Flor ida , J osh was referred to Vein Specialists by his new Dermatologist. An ultrasound was immediately ordered to evaluate the blood flow in Josh’s legs, which revealed that his underlying problem was venous insufficiency. After meeting with Dr. Magnant and discussing a variety of treatment options, an endovenous radiofrequenc y (RF ) ablation was recommended for both legs, which essentially is a heat sealing of his problem veins. Neida Carvalho, owner of Davidson Irrigation, is a mastermind when it comes to landscape lighting and design. Lighting doesn’t just boost curb appeal; it also adds a layer of security to your property and home. She explains, “Imagine your neighborhood and you’re a bad guy. You drive by a home that’s dark and one next door has lighting around it. Where do you think the bad guy is going to go? Growing up, Neida’s stepfather owned a marine salvage company in Virginia. She began working with him at the age of nine and by the time she was a teenager knew how to fix boat motors and pumps, she even learned how to weld.

After graduating from high school, she joined the United States Coast Guard where, thanks to her experience growing up, she transitioned into the field of diesel mechanics with ease. When she lef t the Coast Guard in 1989, Neida formed a lawn care business, later adding irrigation services. As the business took on a new life, she got a contractor’s license enabling her to take on bigger projects; operating the only woman and veteran owned business of its kind in Southwest Florida. The outdoor lighting part of her company is something she has become more and more passionate about over the last few years and she has become quite the expert at it. Neida is now highly sought after for her vision and design. he was suffering from hip pain for years and knew she needed to take action, but Helen Haver, like many people in pain, was afraid of surgery. That is until she met Dr. Peter Ameglio. She says, “I was totally afraid, but I had a comfortable feeling that he would take care of me.” And he did. Helen underwent her first hip surgery at the age of 76 in March via the SuperPATH® technique. Dr. Ameglio introduced SuperPATH to Southwest Florida over 2 years ago and since has received the attention of physicians, nurses, and physical therapists in our community due to the fast recovery of his patients. SuperPATH is not easier for the surgeon to perform, but for the patient the benefits are big. Dr. Ameglio explains, “All other hip replacements require a hip dislocation except for SuperPATH. I always use the analogy of how much effort it takes to remove a drumstick from a turkey at Thanksgiving time.” With the SuperPATH approach the hip joint is never dislocated, preserving the joint capsule. Also, no muscles or tendons are cut. Af ter surger y with all other hip replacements patients are instructed in precautions or certain positions that they should not place their leg or hip, as doing so could risk joint dislocation. Dr. Ameglio’s patients have NO hip precautions or restrictions post-op because

the hip is never dislocated. Helen healed quickly from her SuperPATH procedure, but her other hip needed surgery too. Just three months after her first surgery, Helen went in for her second SuperPATH procedure. The surgery was once again successful, however, this time when she returned home to recuperate, she began experiencing discomfort. “I couldn’t sit, I couldn’t sleep, my butt hurt so much, there was tingling, and my lower back and the right thigh was hurting too.” She went back to Dr. Ameglio who has also become well known for his treatment of the sacroiliac (SI) joint. Dr. Ameglio injected Helen’s SI joint with an anesthetic to eliminate the pain and a steroid to reduce inflammation. She says, “Within a couple of days I started feeling much better and in 5-7 days it was almost gone.” Today she is pain free. Dr. Ameglio’s interest in the SI joint came af ter searching for relief for patients still experiencing pain after hip replacement surgery and/or had unresolved pain in the lower back buttocks, thigh, and/or groin. He states, “We are blessed with many talented orthopedic surgeons in SW Florida that perform hip replacements. In fact, hip replacement surgery, in my opinion, has the best outcome of all orthopedic surgeries. Unfortunately, some patients still experience residual

pain after a successful hip replacement regardless of the approach. When this occurs, the surgeon confirms the hip replacement is not loose in the bone or the pain is not related to an infection among other reasons. Often, no reason for the pain is found and the patient is left reporting they have pain despite results of testing being normal. This discrepancy motivated Dr. Ameglio to find an answer to this unresolved pain. “The SI joint can mimic symptoms of an arthritic hip. I have seen patients who’ve had a hip replacement but still have symptoms of hip arthritis even though the joint has been replaced.” Problems with the SI joint can result in pain in the lower back buttocks, thigh, groin, and can radiate down the leg making it difficult to diagnose. Some symptoms are similar to those found in hip arthritis. The sacroiliac joint functions as a shock absorber for the spine. It dissipates the force to the sacrum, pelvis, and lower extremities; however, as we get older the SI joint may become less able to absorb shocks. The SI joint may become arthritic or dysfunctional from injury such as childbirth, motor vehicle accident, a fall, or twisting. “15-30% of people with low back pain have sacroiliitis and in those patients who have arthritis or a fusion in the

lumbar spine, up to 45% have sacroiliitis,” says the surgeon. “It’s a sizeable number of patients and it is critical to understand because of the overlap in symptoms between the SI joint and hip.”

“Dr. Ameglio has been so kind and caring. He listened to me and took his time,” says Helen. Laughing, she adds, “I felt like I was one of his children and here I could be his mother.” Her friends are amazed that Helen had both hips replaced within a span of three months and in less than 2 months time she is already back to her regular routine which includes volunteering at the Edison & Ford Winter Estates, as well as ushering at the Barbara B. Mann and Florida Repertory Theatre. She says, “I tell people don’t hesitate, you can’t live life if you are so uncomfortable and hurt so much. You can’t live on pills for pain. I’m really blessed, fortunate, and so glad I found Dr. Ameglio.”

Homestead & Your Estate Plan

F

lor ida law prov ides special protection from creditor claims for a person’s “homestead,” i.e., their primary residence. These protections even extend beyond death. That is to say, when a Florida resident dies, they can pass their primary residence on to their heirs at law–usually a spouse or child–and the decedent’s creditors cannot touch the property. APPEALS COURT: Homestead Order Did Not “Extinguish” Sons’ Life Estate in Mother’s Property.

It is important to be aware of the homestead rule when making an estate plan. For instance, if you have a spouse or minor child, the homestead rule grants them certain rights to your homestead even if your will or trust says otherwise. And while the homestead rule does not require you to leave your house to any adult children, it can still complicate the administration of your future estate. Here is a recent Florida court case on point. In Mullins v. Mullins, three siblings went to court over what to do about their late mother’s house. The mother died and left a will, which said her two sons–Robert and Kenneth–could continue to live in the house for as long as they wished, provided they paid the property taxes and other expenses. In legal terms, the mother gave her sons a “life estate” in the property.

Around the time of the mother’s death, Kenneth moved out of the house. Kenneth and his sister Carla later sued Rober t, claiming their brother’s actions “forced [Kenneth] to vacate the property.” The lawsuit asked a judge to force a sale of the house, with the proceeds divided equally among the three siblings. Robert opposed this move, arguing their mother’s will gave him a valid life estate in the property. Kenneth and Carla responded by pointing to an order previously issued by a judge confirming the homestead exemption applied to the property. Such homestead orders are typically necessary to clear title to a property so it can be sold or mortgaged. Critically, the homestead order issued in this case did not mention the mother’s will or the life estate. The Florida Fifth District Court of Appeal, reversing a trial judge’s prior ruling, said that did not matter. The mother’s will still controlled. All of the siblings agreed the will created a life estate for Robert and Kenneth. The homestead order did not change this; it only confirmed the property was still protected by the homestead exemption. As the appeals cour t explained, the homestead order “did not create new rights, but rather explained or clarified the rights that already existed by operation of law.”

Takes the Lead in Early Detection of Breast Cancer

F

or 20 years, Dr. Mary Kay Peterson has been waging the war against breast cancer at Radiology Regional Center. As the Director of Women’s Imaging, Dr. Peterson not only preaches the ‘early detection’ gospel to women everywhere, she is also passionate about education. During breast cancer awareness month, she says there are three critical things for women of almost any age to do: get an annual mammogram, do a monthly self-exam, and get an annual physician exam. It’s also important to know about advances in technology and impressive new diagnostic testing options, such as 3D Mammography and Abbreviated Breast MRI or AB-MR. “3-D mammography is the newest technology; we have it in all of our offices,” says Dr. Peterson. She explains, “Patients don’t necessarily know the difference, but I can see so much more.” The imaging allows the doctor to see what is in the breast layer by layer. “It’s like scrolling through the pages of a book, instead of just looking at the book from the front through the back cover,” says Dr. Peterson. The 3D imaging views the breast tissue layer by layer, reducing callbacks for more testing.

Initially radiologists thought 3D screening was only good for women with dense breast tissue, however, 3D is now recommended for everyone. Dr. Peterson comments, “We’re finding cancer we probably would never have detected. Why would you not want the best technology if it’s available.” Last year Florida became the 34th state to pass a breast density law. Dr. Peterson says, “Even though we had been reporting a patient’s density all along, now the state mandates the information be included in a patient’s report.” This is important because it opens the door for patients to seek supplemental imaging, such as AB-MR (abbreviated breast MRI), a shortened version of the standard breast MRI. “If you have a lot of glandular tissue, cancer can hide in there. Breast MRI’s offer the most sensitivity, meaning you can pick up disease, and the best specificity which means you can decide whether its good or bad.” This does not mean women with dense breast tissue should skip the mammogram in lieu of an MRI. The two tests are done in conjunction. It’s the mammogram that picks up calcifications in the breast, the most common form of cancer. Mammograms

detect 4-5 cancers per 1000 women while MRI’s detect an additional 16-23 cancers per 1000 women. Most insurance companies do not cover the AB-MR screening for women who have been diagnosed with dense breasts. However, because it is considered a supplemental screening, patients do not need a doctor’s prescription for the AB-MR. The test must be scheduled between one to ten days after your menstrual cycle. If you are one of many women who believe the amount of radiation received from mammograms is more harmful than helpful, the doctor shakes her head and explains, “If you live on the earth for 7 weeks, you’ve received the same amount of radiation (from environmental sources) as a mammogram.” She says 3D imaging emits only a minimal amount more radiation than 2D imaging. It is recommended that women get their first mammogram at age 40, and annual thereafter. If a first-degree relative, like a mother, has been diagnosed with breast cancer before the age of 40, then a woman should get her baseline mammogram 10 years earlier than the age of the diagnosis. For example, if my mother was diagnosed at age 37, my baseline mammogram would be at age 27.

Dr. Peterson recommends that mammogram be 3D. She says, “The detail and the specificity of 3D is like the difference between driving a car while sitting behind the wheel as opposed to sitting in the back seat and trying to drive. Wouldn’t you rather drive from the front seat?” If you’re uninsured or are worried about the cost of a breast screening, Dr. Peterson says, “Patients need to know that there are many organizations offering accessibility to screenings in our community. We have links on our website to help connect you. Don’t pass up screening because of financing.” While mammography screening is important, Dr. Peterson says with passion “Mammograms aren’t everything. See your physician for annual exam. Don’t by-pass your monthly self-exams, which should be performed the week after your period when hormones are calmest.” She cautions, “Make sure you check all of the way into your armpit and around the nipple.” Look for areas of hardness, ridges, lumps, dimpling, or nipple discharge.
