pproximately 25% of Americans have varicose veins. Not only are they unsightly and painful, but more importantly they are a sign of underlying venous disease called venous insufficiency, according to Dr. Joseph Magnant, founder of Vein Specialists. Varicose veins are those thick, bluish veins that bulge just below the skin surface of the legs and feet. They look twisted and may be painful. Often related to heredity, they can also worsen during pregnancy, with standing for long periods of time, advancing age, and obesity. Varicose veins are not a disease unto themselves. According to Dr. Magnant, they are a sign of venous disease, also referred to as leak y valves, venous insufficiency, or venous reflux disease. So what causes them? There are one way check valves in veins that, when functioning properly, help keep the blood moving up the legs back toward the heart. Sometimes they become weak and leak, resulting in high pressure in the leg veins. Dr. Magnant says, “A branch of the vein may dilate and become ropey developing into a varicose vein. Even though varicose veins often appear first in the calves, the pressure that creates them comes from above. They start small and can wind all of the way up to the thigh where the branch is connected to the leaky vein.” It happened to Laura Oliver who works in the shoe depar tment of Dillard’s and spends long hours on her feet. In fact, her varicose veins had started to appear years ago. She states, “My father had varicose veins and he waited until he was 70 to have them fixed. I wasn’t going
Varicose Veins to wait that long.” She was not only self-conscious about the appearance of her legs, but they hurt and would cramp at night, and sometimes they would swell. Dr. Magnant says those symptoms and signs are common in people with venous disease, even if they don’t have visible signs such as varicose veins. Laura adds, “If mine weren’t bulging, I would have just thought my legs swell at end of the day and would not have made the connection to vein disease. I don’t think I would have sought help because I didn’t know swelling was a sign of vein disease.” When talking to customers who c ame into her shoe depar t ment wearing compression stockings, Laura says, “They all went to Vein Specialists. I heard nothing but great things about their team.” Af ter she made an appointment with Vein Specialists she says, “All of the great things I heard were true.” Laura had a comprehensive ultrasound performed on both legs and a treatment plan was decided upon. She had an endovenous ablation in which heat is used to seal the vein. Dr. Magnant says, “These veins are superficial, located outside of the muscle. They are only responsible for about 5% of the return from the legs when the valves are working.” After the endovenous ablation, the blood no longer pools in the vein, immediately reducing the pressure. “I went back to work af ter the procedure, standing all day with compression hose on and it didn’t hurt,” says Laura. Not only was the process quick and with ver y little discomfort, she saw an improvement in the appearance of her varicose veins almost immediately.
“About 25 million people in the U.S. have varicose veins,” says Dr. Magnant, “and another 25 million have venous insufficiency that presents with signs other than varicose veins.” If left untreated, venous insufficiency can progress to the point of causing skin changes which can lead to open wounds. “Vein disease may not be 100% curable,” says Dr. Magnant. “But we can keep it under control. Venous reflux disease is a medical condition. Spider veins are usually considered cosmetic, however, varicose veins are a sign of a medical problem that should be investigated.” If you have varicose veins, leg cramps, leg fatigue or swelling, skin discoloration, or ulcers, you many have venous disease. Chris doesn’t have a dog yet, but he plans on getting either an English Mastiff or English Bulldog soon. “I had an English Mastiff before, and I love them.” Although Chris says that he loves English Mastiff dogs, he still has a soft spot for English Bulldogs. “If I decide to get an English Bulldog, I want to get a female dog, and I’ll name her Lola.” Chris, who is originally from Washington D.C., moved down to Southwest Florida for the beautiful sunshine, and the endless fishing spots. “I like to fish, play volleyball, and run.” O ne of Chr is’ favor i te hobbies is to play beach volleyball. “Every weekend, I like going to the Naples Pier to play beach volleyball,” Chris says that he’s the nicest guy that you will ever meet, and we can attest to that.

REDUCING PAIN ENHANCING LIFESTYLE
P
eople suffering from injuries and degenerative diseases are often told surgery is the only way to alleviate or reduce pain, but Dr. Ronald Gardner, founder of Gardner Orthopedics says that’s simply not the case and regenerative therapy is one reason why. He has been using regenerative therapies for decades with success rates of 90%. Many patients are familiar with or have heard of a regenerative variety known as biologics, which includes the use of platelet rich plasma and stem cell therapies. There are also non-biologic regenerative therapies that most commonly f all under t he umb rella heading of prolotherapy. things like tendonitis, bursitis, and early osteoarthritis; conditions that need tissues to be stimulated to heal themselves.” This course of treatment typically involves a series of precisely placed injections of the tissue stimulating compounds. However, the most exciting new findings when it comes to regenerative therapies, according to Dr. Gardner is, “In biologics, and particularly with platelet rich plasma, there are new applications that appear to have some utility in the areas of treating peripheral neuropathy, erectile dysfunction, hair regrowth, and skin regenerative therapies used in aesthetic anti-aging.” Dr Gardner comically admits that he has no intention to treat ED, but notes that as for hair regrowth, these modified treatments are done at a
fraction of the cost of hair transplant or implants, and even better— it appears that THEY WORK. Dr. Gardner says, “You would not believe the before and after pictures of some of these patients, some of whom were distinctly balding, and afterwards had a full head of hair.” L i ke w i s e , t h e u s e o f b i o l o g i c therapies for peripheral neuropathy is a budding science that adds hope to people who have had almost no effective therapies until now. Skin anti-aging experts are frequently using “PRP facials” (ever heard of a “Vampire Facial”?) as part of their skin rejuvenation protocols, and Dr. Gardner has assisted some of them in gaining the necessary expertise to properly perform these treatments. While hair regrow th, treating peripheral neuropathy, and anti-aging therapies might seem a curious extension of treatment at an orthopedic office, Dr. Gardner explains, “We have more expertise than anyone else in this part of the state with the use of regenerative therapies. It makes sense for us to pilot programs for hair regrowth, peripheral neuropathy, and anti-aging interventions due to our safety record and experience as part of a lifestyle enhancement effort, even though it really has no musculoskeletal application. We will, on a selected basis, begin to offer some of those protocols soon and those interventions will likely also include nutritional support and even hyperbaric oxygen therapies as part of the algorithm.” For the majorit y of or thopedic patients, regenerative therapies are used with great success to treat conditions such as bursitis, plantar fasciitis, tennis elbow, golfers elbow, rotator cuff tendonitis, iliotibial band syndrome, and certain specific areas of arthritis to name a few. But according to Dr. Gardner, “Knee and shoulder arthritis are by far our highest areas of success for these treatments.”
The type of biologic regenerative therapy used at Gardner is called autologous, meaning the injections are derived from your own cells. Dr. Gardner explains, “We take the cells from your own blood, fat, or bone marrow, so the risk associated is very very low.” For PRP, the process begins with a blood draw from the patient. The platelet s are separated and then injected into the injured or arthritic joint. A similar process involves using liposuction to harvest fat cells that are emulsified then centrifuged to get the most concentrated specimens possible. Dr. Gardner uses the stem cell rich layer from fat to inject into the joint. A similar process is used with bone marrow. These procedures are done under local anesthesia right in the office. “The injections stimulate your body to heal itself and by replenishing the cellular material in damaged tissues, it further stimulates or augments the healing process,” explains Dr. Gardner. He adds, “Patients often go from barely being able to walk to walking without a limp. TURKEY with Chef Harold Balink
T
he turkey is typically the star guest for Thanksgiving and also the cause for much anxiety. So how do you get it right this time? We’ve turned to award winning Southwest Florida Chef Harold Balink for turkey tips guaranteed to make YOU the star in the kitchen this year. Besides making sure the bird is completely thawed and its cavit y cleaned out, his best bird tip is to brine it overnight in a mixture of one part sugar, two parts salt and 10 parts water; rinse well before cooking. Chef Balink explains, “It doesn’t matter how you cook it, brining ads so much more moisture.” OVEN ROASTED: Chef Balink says go low and slow to keep the bird moist, “Set the oven on about 200 ºF or 225ºF, rub the turkey with butter, and put it in.” Get the temperature in the thickest part of the breast up to 155-158 degrees, then take it out and turn up the heat to 450 º F. As the bird sits “carry over cooking” takes place. Chef Balink says, “It should get up to about 165ºF after it comes out of the oven. Then rub it with

some more butter and pop it back in for 10-15 minutes to get that beautiful golden brown.” DEEP FRIED: A much faster method, first check that your pot is big enough to easily fit the bird. Get the oil temperature up to 400 degrees and completely thaw and dry the turkey, including its cavity, before submerging (never put a frozen turkey in hot oil…very dangerous!). Chef Balink says, “You don’t need to inject it (the turkey) with anything if you brine it. It’s brown, moist and beautiful.” DEBONING AND STUFFING: Chef Balink says, “Deboning is a pain, but you have this beautiful flat canvass of turkey that you can layer with anything from sausage to stuffing, and roll it up….It doesn’t need to be perfect, people will be wowed that it is all in one.” The cooking method is once again slow and low. Chef Balink says serve your bird with a glass of festive Beaujolais Nouveau and you are good to go. He continues, “Man it’s tough to beat a homemade Thanksgiving meal when you do it right.”
TIPS About Chef Harold Balink Harold is the chef/owner of Harold’s, a small farm to table restaurant in Fort Myers with a big following. His exposure to the culinary world began as a young man working in his grandparent’s steakhouse in Colorado. An apprenticeship with King’s Crown at South Sea’s Island Resort on Captiva Island brought the popular chef to the area but he didn’t stay initially. After working around the country he eventually returned to Southwest Florida where he has had quite a successful run with restaurants on Fort Myers Beach, downtown and at The Bell Tower Shops, before opening Harold’s three years ago. His culinary degree comes from Johnson & Wales University in Rhode Island and his Hotel and Restaurant Management degree is from Boston University. He was also invited to cook at the esteemed James Beard House in New York City. With two grown daughters and two dogs, Harold not only feeds his patrons stomachs but also their souls. He says, “I’m passionate about serving people, not just food and wine. Everyone has a niche and for me it’s being a servant. I find that is when I seem to be happiest.”

The Personal Touch of Dentistry by Gina Birch
“I’m allergic to dentists,” laughs Patsy Capps. But the Sanibel resident actually looks forward to traveling over the causeway for treatment by Dr. Katrine Farag, owner of Advanced Dentistry of Fort Myers. Patsy says, “It was very difficult for me to go to her the first time because I had some bad dental experiences etched in my memory.” She had been a patient of the dentist whose practice Dr. Farag bought two years ago and says, “I thought she’s on trial with me and I guess I’m on trial with her.” The trial has turned into a relationship of trust and admiration. “She (Dr. Farag) was so genuine and that is hard to find these days,” gushes Patsy. During a routine visit Dr. Farag found that a crown on one of Patsy’s molars had cracked. She has a small mouth making it a challenge to reach the tooth. Then there are those bad dental experiences locked in her memories. She says, “I told Dr. Farag the only way I can do this is if you do it right this minute.” It just so happened there was a cancellation, so on a moments notice, Dr. Farag switched gears and went to work on the tooth. Patsy remembers, “It was amazing. Her professionalism is top shelf all of the time and for the first time in my life I came away from a dental procedure with a good feeling when it was over.” Her husband Van also needed dental work, but his was more extensive. Medication he had been taking over the years actually caused damage to his teeth. Dr. Farag used digital technology and visual models to thoroughly explain her comprehensive treatment plan for

Patsy says, “She made sure we could see what she was going to do with mockups and made sure we knew what the end results would be.” Patsy laughs, “I had no idea what a bridge was, but what she did was beautiful. My goodness he (Van) looks ten years younger.” Dr. Farag is known for attention to detail and for the welcoming, family atmosphere in her office. In fact Patsy says she feels so close to Dr. Farag and the Advanced Dentistr y staf f that she never arrives to an appointment without gifts in hand; baskets of muffins, cookies, and even trinkets for the dentist’s two children. She says, “I’ve adopted their kids. I’ve never met them but I get a blessing out of it, you know. It makes me happy that I’m making them happy.” Patsy’s story is a testament to how Dr. Farag cares for her patients, and cares for their health in general. “She is a perfectionist. She is diligent in making sure everything is right in every way. She doesn’t leave it at ‘good enough,’” says Patsy. The Southwest Florida dentist has breathed new life into the business she now owns, and has designed the space to feel comfortable and welcoming. Patsy

About Dr. Farag: Dr. Farag was born in London but grew up in Charlotte, North Carolina—she still says ya’ll. ouston, 86, hadn’t been to a food pantry in a long time. But now he needed help for himself and his disabled adult son who lived with him. His social security and the $45 a month he received in food stamps wasn’t able to cover his bills and buy groceries. That’s when he decided to come to a pantry in Bonita Springs that partners with the Harry Chapin Food Bank. He chose a few food items, loaded them into his 18-year-old car, and left. He was full of gratitude. “If I hadn’t got people helping me, I couldn’t make it,” he said. Houston is one of thousands of clients the Harry Chapin Food Bank serves annually through its more than 150 partner agencies, 30 monthly mobile pantries, and senior programs like the Care & Share Senior Feeding Campaign.

In the last fiscal year, the Harr y Chapin Food Bank distributed 24 million pounds of food throughout Lee, Collier, Charlotte, Glades, and Hendry counties. With the holidays approaching, the food bank faces its most critical time of the year as it pushes forward on its mission, to lead our community in the fight to end hunger. The three months bet ween November 1st and the end of January, the Harry Chapin Food Bank will receive about 50 percent of the total monetary donations it collects for the fiscal year. These donations not only meet the heightened need of the holidays, but provide the means to carry on the food bank’s mission throughout the year. “ We depend on charit y at the holidays, the time when families draw together around food and focus on

First his mother-in-law, and then his wife, became ill and had to go into a nursing home. The bills drained all their retirement savings. Just recently, Jeff and his 3-year-old daughter went to one of the emergency mobile pantries on Fort Myers Beach, that was set up to specifically help those economically impacted by the water quality crisis. “I work in a restaurant. Our business right now is down,” he said. “This is a tourist place. If there are no tourists, there is no business. Without business, there is no money, and with no money, there is no food.”

The smallest donation from the smallest person can help. At holiday time, the food bank also welcomes the community “angels” who step up to meet the need, like little Joy Youngblood. At age 5, Joy broke into her piggy bank and brought in $8 as a holiday donation. “You give the money away so people can eat, because that’s being a nice and kind friend,” she said. In each subsequent year, Joy has returned with more holiday donations to build on the blessings she seeks to give others.
