Understanding Acne At Every Age

Acne is often dismissed as a teenage issue, an unfortunate yet expected skin condition that fades with time. However, the reality is that acne can persist well beyond adolescence, and for some, it may even appear for the first time in adulthood. As estheticians, recognizing how acne manifests at different life stages is key to creating effective, personalized treatment plans that go beyond surface-level solutions. The triggers, patterns, and responses of acne evolve over time, making it essential to adapt treatments accordingly.

Beyond the Breakout

Teenage Acne

During adolescence, hormonal surges— especially increased androgens— stimulate the sebaceous glands, leading to excess oil production. When combined with slow exfoliation and clogged pores, the result is the classic case of teenage acne: blackheads, whiteheads, and inflammatory pustules concentrated in the T-zone. Many teens, frustrated with breakouts, turn to harsh cleansers and excessive exfoliation, inadvertently damaging their skin barrier and exacerbating the issue.

Common Teenage Acne Triggers:

Cystic acne, often appearing for the first time in perimenopause, requires a delicate balance of clearing congestion without stripping the skin. Acne is one of the most common dermatological conditions affecting adolescents. The primary factors contributing to its development include hormonal changes, excessive sebum production, disruption of epidermal keratinization, activation of Propionibacterium acnes bacteria, and the subsequent inflammatory response.

The interplay of these processes results in various forms of acne, ranging from comedones to severe inflammatory lesions such as cysts, which require more aggressive treatment. According to the World Health Organization (WHO), approximately 85% of adolescents experience acne, making it a significant factor in the development of psychological and social issues. Therefore, effective treatment—incorporating dermatological care, cosmetic procedures, and psychological support—is essential for achieving lasting results.

Modern dermatology offers a wide range of effective acne treatments, including pharmacological therapy, topical treatments, and hardware-based procedures.

Acne Solutions for Teens

However, treating adolescent acne requires a specialized approach, as this age group exhibits distinct psychophysiological characteristics, such as increased emotional sensitivity and an underdeveloped nervous system. These factors can complicate their understanding and perception of the treatment process, particularly in the early stages, when they may struggle to recognize the importance of following medical recommendations. Pharmacological treatments, which may include retinoids, antibiotics, benzoyl peroxide, or hormonal medications, often cause initial side effects such as worsening breakouts or an exacerbation of inflammation. These reactions can lead to discomfort, insecurity, and doubt about the chosen treatment, potentially resulting in skepticism and even discontinuation. Without psychological support during these stages, achieving the desired outcomes becomes more challenging. Adolescents may lack the experience to properly interpret temporary flare-ups as part of the healing process and may feel discouraged rather than reassured.

Another critical aspect of acne treatment is the involvement of a cosmetologist. Cosmetic procedures not only aid in skin healing but also help adolescents establish a consistent skincare routine. Essential steps such as preparing the skin for treatment, performing mechanical or ultrasonic cleansing, undergoing peels, and incorporating daily skincare products all play vital roles in a comprehensive therapeutic approach.

Notably, peels containing azelaic acid have demonstrated excellent results. A recommended 14-day course, alternating with non-invasive carboxytherapy, can be tailored to each patient’s needs. These treatments not only improve skin condition but also enhance the efficacy of pharmacological therapies by promoting pore cleansing, reducing inflammatory lesions, and regulating sebaceous gland activity.

Cosmetologists also provide essential guidance on proper skincare practices, including selecting appropriate personal hygiene products. Emphasizing the use of paper towels or disposable wipes for drying the skin after cleansing, regularly changing pillowcases (at least every five days), and ironing them can significantly reduce bacterial contamination. A tailored skincare regimen, considering skin type, acne severity, comorbid conditions, and ongoing dermatological treatment, ensures optimal results. A well-structured home care routine— including hydrating, sebo-regulating, and anti-pigmentation products such as kaolin-based masks—helps maintain healthy skin throughout the treatment process. Thoughtfully chosen skincare products not only reduce acne but also prevent new breakouts, promote postinflammatory healing, and minimize hyperpigmentation.

For effective acne management, it is crucial to integrate dermatological treatments, facials, and psychological support. This comprehensive approach helps adolescents better understand their treatment, reduces anxiety, and reassures them that initial flare-ups are a normal part of the healing process. By addressing both the physiological and psychological aspects of acne, this strategy fosters long-term skincare habits and ensures sustained, stable results.

Healing Post-Break Out Skin

Post-inflammatory hyperpigmentation (PIH) appears as dark brown or purplish spots that develop after a blemish heals. This occurs when inflammation triggers excess melanin production, leading to residual pigmentation in the skin. PIH is more prevalent in clients with medium to deep skin tones and can worsen with sun exposure if proper protection isn’t used.

Post-inflammatory erythema (PIE) presents as red or pink marks left behind after acne. Unlike PIH, which results from excess melanin, PIE is caused by dilated blood vessels that remain after inflammation subsides. Fair-skinned clients are more prone to PIE, and since it is vascular in nature, traditional brightening treatments like vitamin C or hydroquinone have little effect. Instead, treatments that promote vascular repair, such as LED therapy and microneedling, are more effective.

Treatment Strategies for PIH and PIE

For PIH, the goal is to regulate melanin production and accelerate cell turnover. Professional treatments such as chemical peels (especially those with glycolic, lactic, or mandelic acid), microneedling, and laser therapy can gradually lighten dark spots and improve skin tone. Topically, ingredients like azelaic acid, kojic acid, tranexamic acid, and vitamin C are excellent for breaking down pigmentation.

For PIE, treatments must focus on calming inflammation and improving vascular function. LED therapy, particularly red and near-infrared wavelengths, can help reduce redness by strengthening capillaries. Microneedling works well by promoting new collagen and reducing dilated blood vessels. Niacinamide and green tea extract can be beneficial for clients looking for topical solutions that support vascular repair.

Healing Post-Break Out Skin

Rolling scars: Wave-like depressions caused by tethered scar tissue pulling down on the skin. Treatments like subcision (a minor procedure that releases the scar tissue), microneedling, and dermal fillers can improve their appearance significantly.

Professional Treatments for Acne Scars

For clients dealing with both pigmentation and textural scarring, a multi-modality approach is often necessary. Some of the most effective professional treatments include:

Microneedling with PRP: By creating controlled micro-injuries in the skin, microneedling stimulates collagen production, while PRP (platelet-rich plasma) accelerates the healing process and enhances skin regeneration.

Microneedling with Stem Cells: Similar to PRP, microneedling with stem cells enhances collagen synthesis and accelerates skin repair. Stem cells provide powerful regenerative properties, helping to reduce inflammation, improve skin texture, and promote faster healing, making it an excellent option for acne scars and overall skin rejuvenation.

Chemical Peels: A series of chemical peels using mandelic acid, TCA, or Jessner’s solution can help improve both pigmentation and mildv scarring.

Radiofrequency Microneedling (RF

Microneedling): This combines microneedling with heat-based energy to further stimulate collagen production, making it highly effective for deeper scars.

Fractional Laser Resurfacing: For more severe scarring, fractional lasers like CO2 or erbium lasers can significantly improve skin texture, although they require more downtime.

Subcision and Fillers: For deep, tethered scars, subcision helps release the scar

tissue, while hyaluronic acid fillers can temporarily plump depressed areas.

The Role of At-Home Skincare

While professional treatments play a crucial role in scar revision, at-home skincare is just as important in maintaining results and preventing new scars from forming. A well-rounded post-acne regimen should include:

Hydrocolloid Patches: For active breakouts, hydrocolloid patches can prevent picking and help reduce post-inflammatory marks.

Guiding Clients Through the Healing Process

The journey to scar-free skin requires patience, consistency, and professional guidance. As estheticians, we play a crucial role in educating clients on realistic expectations and long-term treatment plans. Acne scars can take months to improve, and results vary depending on the severity of scarring, skin type, and adherence to treatments.

By combining advanced in-clinic treatments with effective at-home care, we can help clients achieve smoother, more even-toned skin while restoring their confidence. Acne scars may tell a story of past breakouts, but with the right approach, they don’t have to define the future of a client’s skin.

My connection to medical aesthetics spans over 15 years, starting in a boutique med spa where I learned the nuances of skin health and patient care. After university, I earned my aesthetic license and worked with luxury brands, scaling them nationally and internationally. Transitioning into medical aesthetics, I saw a huge disconnect in patient interaction, which felt impersonal. As a leader in scaling multi-milliondollar brands, what challenges have you faced, and how have you overcome them?

A loaded question to say the least! How do you get a multi-million dollar operation, hundreds of team members to have a consistent result and process, while protecting the brand values and mitigating consumer outcomes? While also ensuring you are keeping growth and expansion in the forefront for brand longevity? The greatest challenge I have faced has been maintaining consistency in delivering results while protecting brand values during growth. At TIEV, we focus on simplicity—making complex processes digestible for both the team and the client. People want to be heard and understood. We listen to our team and

clients, ensuring everyone feels heard and part of a larger vision, which allows us to grow without losing sight of our core mission of practicing responsible, patient-first medicine.

What do you believe are the key elements of successful team management in the luxury aesthetics space? Be organized. Set time aside every single month to connect with every team member. Ensure to listen to their personal goals and align with the business goals, then come up with a plan together to get them closer to where they want to go. Your team wants to be a part of something that is bigger than them and means something of great substantial value. If we can feel accomplished while also feeling good about what we do, that is the magic. Then, when we connect daily whether in passing or a simple check in there is a common expectation towards excellence. As an owner I personally meet with every department every month, ads, branding, marketing, clinical, finance, etc so the communications, company goals, and expectations stay clear.

You are known as a dedicated advocate for skin health and safety. How do you ensure these values are integrated into TIEV’s operations and client interactions?

Safety and practicing responsible medicine are non-negotiable at TIEV. It is our #1 fundamental core value actually. We empower patients with knowledge, ensuring they are part of the decisionmaking process while respecting their individuality. Beyond that, we ensure that our team undergoes rigorous training before treating patients, monthly continued education seminars and more, embodying our commitment to constant learning and refining our craft. We are obsessed with learning, we are always students. It is our commitment to our values and to the patient, and it is deeply woven into our operations.

With your background in private equity, how do you see the aesthetics industry evolving in the next few years?

The aesthetics industry is shown to grow 15% CAGR by 2033 according to AMSPA with North America being the top in the Medspa space. It is a booming field, but with that growth I think there will be pros and cons. In the consumer market, there is a rising demand for minimally invasive procedures and an increasing awareness of anti-aging and preventative treatments in younger generations than we’ve ever seen in the past. With more technological advancements in aesthetic space we are getting smarter with treatments that provide minimal down time - allowing you to take little to no time away from your everyday life.

Conversely, as the market expands, there is a growing risk of inexperienced

providers and business owners entering this space solely for financial gain, often without fully appreciating the gravity of their role. Ultimately, while this is elective medicine, it remains fundamentally a medical practice. This work demands the highest commitment to education, safety, and clinical precision, supported by a strong administrative team to ensure seamless and secure operations.

My best advice is to always properly vet anywhere you go, there are plenty of incredible providers and businesses in this space, and we are proud to say we are one of them.

Ultimately, this field offers an exciting and dynamic environment, now widely accepted compared to a decade ago.

Influencer marketing has played a significant role in normalizing these everyday procedures, helping patients under-

stand that these services are nothing to feel embarrassed or hesitant about.

What advice would you give to aspiring estheticians or entrepreneurs looking to make their mark in the wellness and aesthetics industry?

Be a sponge! Approach every opportunity with an open mind and a commitment to learning as much as you can, as early as you can. Surround yourself with people who are significantly more experienced; growth comes from being in rooms with those who are steps ahead. My journey wasn’t driven by innate talent alone—it was shaped by actively seeking out mentors and absorbing every piece of knowledge available. For instance, I learned to analyze a P&L statement at 20 because a mentor emphasized that talent without operational expertise falls short!

Find your niche, and focus on mastering it rather than spreading yourself too thin across trends. Stay informed, conduct thorough research, and specialize in areas with proven results. Early on, I recognized my passion not only for learning in this field but also for teaching and empowering others to excel— even helping them to surpass my own achievements. I strive to keep people around me, both personally and professionally, who challenge and inspire me to grow. My advice: set aside ego, embrace continuous learning, and never hesitate to broaden your skill set.

How do you maintain a balance between your professional responsibilities and personal well-being?

I personally believe work-life balance is impossible - I really believe in worklife integration. There are times when I have to attend to the needs of the business and there are times when I attend to my life and family needs. Setting boundaries, working extremely efficiently, habit stacking, and being fairly “firm” to a schedule is crucial to making things function well. Let's be real, as mothers we can plan all day long,

and sometimes unpredictability sets in. I am beyond privileged to have an excellent support system around me at TIEV where if I need to step away it is not detrimental to the business. I am beyond privileged to have an incredibly supportive and loving system at home, so if I need to attend to the needs at work my child is safe, loved and happy. Pouring into my own cup and prioritizing time for self-care is essential to my well-being. I've come to understand that when I neglect rest and the activities that nourish my soul, my clarity and ability to make sound, strategic decisions suffer. TIEV’s doors opened when I was just four months postpartum, yet I made sure to do activities that fueled me—whether it was getting my nails or hair done, going for walks, or spending time with my closest friends. True self-care is one of the most selfless acts, and while it was certainly easier before having children, I now make it a priority through careful planning and intention. This is the foundation of TIEV, taking a

moment in time to enjoy something that is a compounding gift to your aesthetic journey.
