It takes a team!

As a family medicine physician at Mankato Clinic, I am Maria Mendez De Balcazar’s primary care provider. I know her better as Carmen and her story is truly inspiring. That’s why Carmen graces our cover!

I was very concerned about Carmen’s diagnoses of diabetes and fibromyalgia. To treat and manage these conditions, I referred Carmen to our Diabetes Care Center, Physical Therapy and Psychology. Life-changing conditions also impact our mental health.

With complex and multiple health conditions, we believe a team approach to care is the best game plan.

First we added Jenny Mittelstaedt, RN care manager, to the team. As a care manager, Jenny has been instrumental in helping Carmen coordinate care with multiple providers and specialties.

Then our specialists – Holly Ellison and Hope Hartig in Diabetes Care and Jessica Denn in Physical Therapy – gave Carmen the care, tools and knowledge she needed to manage diabetes and fibromyalgia. Then Carmen did the rest!

In less than a year’s time, Carmen is feeling good and living well! Her health and quality of life have improved. This is exactly why I became a family physician – to help people be healthy and happy.

So what is a primary care provider? A primary care provider, often called a PCP, is a physician, physician assistant or nurse practitioner who sees patients for common medical issues, when they are sick and routine check-ups. A primary care provider is typically in family medicine or internal medicine. For women, your PCP can also be in gynecology and children may have a pediatrician.

Do you have a primary care provider? Visit mankatoclinic.com/specialties/primary-care-providerswelcoming-new-patients to get to know us and make an appointment. Let us be your partner in health.

CARMEN

LEARNS TO THRIVE WITH DIABETES

“MY DIABETES IS MORE LIKE A FLOWING RIVER INSTEAD OF A MOUNTAIN RANGE NOW.”

When Maria Mendez De Balcazar, who goes by Carmen, was diagnosed with type 2 diabetes in September 2021, she was afraid. She knows what diabetes can do. In her own family, she has seen diabetes lead to amputation, blindness and k idney dialysis.

So Carmen began treatment at the Mankato Clinic Diabetes Care Center. Her A1C, her average blood sugar level, was very high at 11.3%. Type 2 diabetes means the body doesn’t use insulin well which causes too much blood sugar to remain in the bloodstream. Over time, high blood sugar can lead to heart disease, vision loss and kidney disease.

Hope Hartig, APRN, CNP, CDCES, is a nurse practitioner and diabetes educator, who cares for Carmen, a Spanishspeaking patient.

“Since I started meeting with Hope, she’s treated me like family. She listened to my fears and helped me through all this. I feel like here I’ve been given the best school,” Carmen says.

“Diabetes care is all about knowledge,” Hope says. “In many cultures, people think you die from diabetes. It doesn’t have to be that way. We cut down those risks.”

To support patients, the Diabetes Care Center brings together a team of medical providers to include an educator, dietitian, and clinical pharmacist. Treatment plans combine lifestyle changes in diet and physical activity, medication, glucose monitoring, and sometimes insulin.

Cost barriers

When first diagnosed, Carmen didn’t have health insurance; she paid for all her visits and medicine on her own. To ensure Carmen received the most effective, yet affordable, treatment, Hope prescribed an older, lower-cost insulin.

“With this type of insulin, you have to be really careful and eat food at certain times. We made this compromise to keep costs the same – $25 a vial was affordable,” Hope says.

Hope taught Carmen about carbohydrates and their role in raising blood sugar. She showed Carmen how to count and reduce carbs as well as what carbs to eat – oatmeal and fruits – and what carbs to limit – refined grains such as white rice, bread and pasta. 

MEDICAL INTERPRETATION IN REAL TIME

Originally from Guanajuato, Mexico, Carmen has lived here for 16 years. She works at a bakery and has three children. Her husband drives her to the clinic for her appointments at the end of his workday. She speaks Spanish.

Imagine going to the clinic and not speaking the same language as your doctor, nurses and office staff. That’s why medical interpreters are a key partner in health care.

Elizabeth Ruiz, certified medical interpreter, of Kim Tong Translation Services is Carmen’s preferred interpreter. Elizabeth meets Carmen at the clinic. When Carmen sees Elizabeth, she smiles and relaxes. Elizabeth interprets for Carmen at check-in, appointments and when scheduling her next visit.

“The role of the interpreter is to facilitate communication between parties who do not share a common language. The nonEnglish speaking client should receive care at the same level as an English-speaking patient,” Elizabeth says.

In the exam room, Elizabeth sits directly across from Carmen and her healthcare providers who look at Carmen when they listen and talk. They pause after a few sentences to let Elizabeth interpret. With 24 years as an interpreter, Elizabeth’s brain works quickly. Information goes in in English and comes out in Spanish and vice versa – all in an instant! She often speaks with her hands to help Carmen understand.

“The interpreter is a neutral party, a conduit of communication. The conversation between the provider and the patient should feel as though the interpreter is not even there,” Elizabeth says.

Elizabeth and medical interpreters at Kim Tong Translation Services are dedicated to bridging language gaps for people who speak many different languages.

EXPERT ADVICE

 “Everything my care team has talked to me about, I’ve tried to put into action,” Carmen says.

“We can teach you all of this but you chose to make changes,” Hope says. “You’re willing to do this and that makes all the difference. If we don’t control diabetes, diabetes controls us.”

Mankato Clinic social worker Jenna Miller, LSW, helped Carmen get health insurance. When coverage began in June, Carmen was able to see dietitian Holly Ellison, RD, LD, to learn more about following a balanced eating plan to meet the body’s nutritional needs and better manage diabetes.

“I know a lot about diabetes and nutrition and our patients are experts on themselves and they know what’s going to work for them,” Holly says. “So together we work through practical tips to address daily challenges and make sure they get the nutrients their body needs to help control blood sugar levels.”

Every day, Carmen makes fresh corn tortillas, which are naturally whole grain. Whole grains have fiber to slow digestion and keep the blood sugar from rising quickly. Carmen also needed to adjust the portion size. Now Carmen makes 6-inch instead of 12-inch tortillas and eats 2 instead of 4 to 6. To the rice and beans, Carmen leaned to add more protein, vegetables and healthy fats to help control blood sugar, raise nutrition and boost satisfaction.

Carmen also eats a light snack before bed to even out her morning blood sugar. Hope and Holly taught her that the liver makes glucose when the body goes too long without eating.

“A solid partnership can be a great foundation for positive changes and better diabetes management,” Holly says. “The day-to-day management is up to our patients.”

Once Carmen had health insurance, Hope prescribed a newer, more flexible, insulin treatment and a continuous glucose monitor. Now Carmen can check her blood sugar levels instantly on her smartphone so she knows when her levels are going up or down and make adjustments. Hope can also view Carmen’s levels for more precise care.

Hope compares the older method of testing blood sugar with finger pricks four times a day to driving with your eyes open four times. With the continuous glucose monitor, you have your eyes open the entire time you are driving.

Happy & Healthy

Today, Carmen’s A1C is well controlled. Her treatment plan includes healthy eating, physical activity, medication, continuous glucose monitoring and insulin.

“This showcases what a system that’s working can do for a patient who wants to do the work and is willing to participate in their care,” Hope says. “We’re a team. And it takes all of us.”

Carmen says she feels like a new person. As a wife and mother, her family and home mean the world to her. By keeping diabetes under control, Carmen can fully be there for her family and all life has to offer.

When Carmen expresses her eternal gratitude to Hope, Hope tells her “All I want is for you to be healthy, happy and have no complications.”

“Did you know stress, anxiety, depression and lack of sleep can raise blood sugar levels? That’s why we also focus on sleep and mental health. We set up a good sleep routine and share ways to manage stress, anxiety and depression to better manage diabetes.”

FIBROMYALGIA: HOPE & TREATMENT

 Every part of Carmen’s body hurt. When people touched her, she bruised. She didn’t want to get out of bed, didn’t want to go on living.

While Carmen was learning to manage diabetes, she was diagnosed with fibromyalgia. This is a chronic condition affecting almost 5 million people in the U.S., mostly women. Fibromyalgia causes widespread pain, sleep problems, fatigue, and often emotional and mental distress. These symptoms also impacted Carmen’s ability to manage diabetes because sleep, mental health and physical activity affect blood sugar levels.

She had to quit her job at the bakery where she was on her feet for 8-10 hour shifts, pulling large pans in and out of ovens.

Carmen began physical therapy with Jessica Denn, PT, DPT, who explained how the brain and nervous system work together. People with fibromyalgia feel a heightened sensitivity to pain; brain scans show changes in neural pathways that transmit and receive pain. Jessica used pain neuroscience education (PNE) to teach Carmen about the complexities of her pain. Treatment included educational pain analogies, reeducation of pain, exercise, and lifestyle and movement modifications.

“A sensitive nervous system is like an alarm system in your home. Typically, the alarm only goes off when someone breaks in, but if the alarm system is extra sensitive, it can go off when a tree branch scrapes against the window. This is why they are experiencing flare-ups of pain,” Jessica says.

Calming techniques can lower the sensitivity of the nervous system, relax the body, decrease anxiety and prevent flare-ups. Jessica showed Carmen relaxation and breathing techniques to use when she feels anxious, stressed or pain coming on.

Getting good sleep is very helpful. Jessica advises sticking to a bedtime routine every night to relax the body, calm the nervous system and tell the mind it’s time to sleep. People sleep better when they have a fixed sleep and wake time, limit naps, unplug from electronics like the TV and phone for 30-60 minutes before bed, have a dark and cool room for sleep, and stick to a consistent routine at night when putting on pajamas and brushing teeth.

According to Choose PT, regular, moderate exercise is an important part of managing fibromyalgia. Research shows that movement like walking, stretching, strength training, yoga and deep breathing can decrease pain and improve function, general health, and sleep in people with fibromyalgia.

To start, Carmen’s goal was 5 minutes of walking a day. She learned that hurt is not harm, sore is safe, and tissues heal.

Jessica advises patients to set little goals and gradually do more activities each day. She suggests doing one load of laundry a day instead of all the laundry in one day. In November, Carmen’s goal was to get back to her job within a year. “I may still have some aches and pains but I can work,” she says.

She had been taking care of her baby nephew every day. He was good medicine for mind, body and soul. “He smiles and snuggles and brings me so much joy!”

So to return to her job, Jessica helped Carmen set mini-goals. Carmen tracked her progress in reaching her goal of two hours of walking a day. In her kitchen, her happy place, Carmen’s goal was to stand for a half hour and increase to 45 minutes by cooking and cleaning.

With physical therapy and perseverance, Carmen returned to work part-time in February. She reached her goal in three months, well ahead of schedule.

“Everything just clicked into place. …What I lived through was really horrible. But I’m here now,” Carmen says.

“Doing kettlebell swings,” Jessica adds.

She continues to work with Jessica to manage symptoms, gain strength and endurance, and improve confidence in her abilities.

“I am very thankful to all of my providers because thanks to them I’m here today. I can’t stop saying that God has sent me a group of angels here on earth. Gracias,” Carmen says. ■

WHAT IS TYPE 2 DIABETES?

Your body breaks down most of the food you eat into sugar (glucose) and releases it into your bloodstream. When your blood sugar goes up, it signals your pancreas to release insulin. Insulin acts like a key to let the blood sugar into your body’s cells for use as energy.

Type 2 diabetes, the most common type, means the body doesn’t use insulin well and too much blood sugar stays in the bloodstream. Losing weight, eating a balanced diet, being active, monitoring blood sugar, medications and insulin can help manage diabetes.

LET'S BE HONEST ABOUT

MENTAL HEALTH

Let’s be honest, no one ever recovered from mental illness without honesty. If you’ve experienced mental illness, you know it can back you into a corner of shame and disgust. It can make you shut down and lose your ability to easily communicate. Honesty is crucial to solving a mental health problem or crisis. I was born with the “honest-to-a-fault” gene, but I know this doesn’t come as naturally to some. This is what has saved me time and again in my journey with mental illness. It’s easy to want to hide feelings that don’t make sense, but these are indeed the feelings that need to be shared. It is essential to find comfortable surroundings to expel these feelings and thoughts. That’s how I choose my family, friends, and mental health practitioners I share my true feelings with.

I’ve had a 20-plus year ride with mental illness and acquired a major depression, bipolar, and anxiety diagnosis. I’ve been on 25-plus medications, been hospitalized 5 times, had 13 electroshock therapy treatments at age 18, been labeled treatment resistant, and spent 2 years away from home in residential treatment facilities out of state. On top of all that, I dropped out of high school after being at the top of my class and tennis team. Since all of this, I have earned my GED diploma, graduated cum laude from college with a B.A. in history, and I give presentations in colleges to end the stigma surrounding mental health.

When I think about what I have learned over the years, I think about honesty and how often it has helped me. I may have at one time been afraid to speak to mental health professionals but right now it comes quite easily because honesty gains respect and trust I’ve learned. There is no litmus test when it

comes to mental health, only the ability to communicate what is going on in our minds to a professional. Honesty is crucial in this exchange. I frequently phrase things like, “this might sound funny” or “is there any reason I might be feeling this way,” when in therapy or with a physician. Therapy is a critical component that complements a physician visit. It is in therapy when you shine a light on what is not only troubling you but what is also bringing you joy. Therapy illuminates the holes that need to be patched in our lives and equips us with the needle and thread to do so. It also exemplifies what is going well.

When I present my life story to a room of college students, I do not know personally, some people might be confused at the lack of privacy I must feel; however, I have been articulating feelings for years for professionals and loved ones. They are only feelings and thoughts, something we all have. I’m usually unaware I am little by little creating an open environment for listeners to share their experiences at the end of the presentation’s question time. My complete honesty and transparency do not go unnoticed. I am stunned when I hear personal tales of mental illness in families and personal strife. The best part about honesty is it breeds honesty. It’s kind of like “one-upping” someone, but all for the better. My honesty has led to some courageous student exchanges. ■

EXPERT ADVICE

EMILY THOMPSON, MSW, LICSW MANKATO CLINIC FAMILY MEDICINE

As a clinical social worker and talk therapist, I am able to support, educate and empower others. I work toward change – with my patients – and in our world. I am constantly working to break down the stigma of mental health. The National Alliance on Mental Health defines stigma as how someone views another – or even themselves – in a negative way because they have a mental health condition. Incredibly, 1 in 5 Americans is affected by a mental health condition.

On average, there are 130 suicides a day in the U.S. Still the stigma lives on. Feelings of shame, resistance or embarrassment keep people from seeking help. This means less people are in therapy and learning to manage their symptoms.

At Mankato Clinic, we want to make sure patients do not feel shame and discouragement when bravely seeking support. To meet patients where they are, I provide psychotherapy, also known as talk therapy, within the family medicine department. Together with primary care providers, I treat medical conditions that affect mental health or vice versa. I also provide short-term therapy to show patients ways to manage anxiety, depression, stress, grief, and other social or emotional issues. Typically, I meet with patients for 1 to 4 visits, but timeframes do vary. If long-term therapy is needed, I refer patients to our psychology or psychiatry department. We offer a wide range of mental health professionals and services because each person’s journey is unique.

The first step to ending stigma is by understanding that getting help and feeling better is OK no matter what condition you have – or do not have.

If you or a loved one is having a mental health crisis, call or text 988 to reach a 24/7 crisis center.

Health Rockstar: Meet Molly!

“This program works! You have to be honest. When I first started the program, I told my doctor, that I do not run, so please do not tell me that I need to run a mile. I love my carbs and I love my Starbucks coffee, so please do not tell me that I cannot have these either because then this program is not for me. My doctor laughed and said that I will not be asked to run a marathon and I can still have my carbs and coffee, but we will help you make better choices when it comes to carbs and Starbs!”

Molly Burger started gaining weight after her first pregnancy, and after having 3 kids it became progressively harder for her to lose weight. She has tried a few different programs in the past with initial success, but each time her weight rebounded. Her experience with Enara Health and Mankato Clinic has been different. With Enara’s easy-to-use mobile phone app, Molly has the support of her care team in the palm of her hand. Enara brings together a medical provider, nutrition specialist and exercise specialist. She appreciates that her team is available and responsive via in-app chats and regular check-ins.

“It is done all through an app on your phone, which is super nice for us busy moms who are always on the go,” said Molly, an RN and manager of Mankato Clinic Oncology and Infusion Center.

Since joining Enara Health in February 2022, she has lost 55 pounds. Her clothes are loose, her face is slimming down dramatically, and she’s not as short of breath when walking up stairs. She has way more self-confidence too! And she is still going strong.

Best of all, Molly is no longer considered pre-diabetic because she has lowered her average blood sugar levels. Prediabetes means blood sugar is higher than normal, but not yet high enough to be diagnosed with type 2 diabetes. A healthy body weight and physical activity can help reverse pre-diabetes.

One thing Molly has been doing differently is adding more veggies to her meals. She’s learned to enjoy vegetables and cook them in tasty ways. Another useful tool has been her anti-obesity medication, which has helped her feel full faster and curb her appetite. It often takes a multi-pronged approach to tackle weight loss, and Molly shows what success looks like when we utilize all the tools available to us. Her exercise specialist sends her exercises to focus on throughout the week.

“I can complete these exercises anytime during the day, which is super nice because I am a busy mom of three and cannot commit to a gym membership at this time,” Molly says. “The exercises are doable and you can do them in your own home. I just purchased a set of dumbbells.”

She also takes pictures of her snacks and meals every day and sends them to her nutritionist – even those days when her choices aren’t the best. It’s OK, because she knows she can get back on track the next day.

“I am so glad I finally found a program that works for me, my schedule and lifestyle. I have made better choices with food and it has not only helped me, but my husband and children too,” Molly says. ■

Did you know that 54 million Americans have low bone density or osteoporosis? As we age, bone density screening tests are important to do because osteoporosis and low bone density increases the risk of fractures and breaks with falls.

To keep bones strong, the body breaks down old bone and replaces it with new bone. Our bone mass peaks at age 30. In mid-life, our bone mass starts slowly declining and eventually bone loss will overtake bone build-up. For women, bone loss speeds up during menopause. By age 65, bone density decreases for both men and women gradually.

One of the most common ways to diagnose osteoporosis is a bone density scan, which measures the amount of mineral, such as calcium, in your bones. We use a machine called a dual-energy X-ray absorptiometry (DEXA or DXA). A DEXA scan is a simple test that’s non-invasive, painless and only takes a few minutes. The patient lies on the DEXA table while a movable arm scans the hip and spine. Patients don’t need to undress if there are no buttons or zippers in the hip and spine area.

Who should get a bone density test?

• Women age 65 years and older.

• If you’re a woman with risk factors such as early menopause, smoking, a family history of osteoporosis, or have taken certain medications, your provider may want to start bone density scans at an earlier age.

• Anyone who has broken a bone after age 50 or lost 1.5 inches in height.

• Men over age 70 are encouraged to talk with their healthcare provider about risk factors and a bone density scan.

MONTH

OSTEOPOROSIS AWARENESS & PREVENTION MAY IS BOOST BONE HEALTH

Eat foods that support bone health. Get enough calcium, vitamin D, and protein each day. Low-fat dairy; leafy green vegetables; fish; and fortified juices, milk, and grains are good sources of calcium. If your vitamin D level is low, talk with your healthcare provider about taking a supplement. Get active. Choose weight-bearing exercise, such as strength training, walking, hiking, jogging, climbing stairs, tennis, and dancing. This type of physical activity can help build and strengthen your bones.

Don’t smoke. Smoking increases your risk of weakened bones. If you do smoke, here are tips for how to quit smoking. Visit quitpartnermn.com.

Limit alcohol consumption. Too much alcohol can harm your bones. Drink in moderation or not at all.

Source: National Institute on Aging

SPRING 2023 | MANKATO CLINIC THRIVES 18

The DEXA scan measures the bone density by giving a T-score. A normal score is zero to -1. A lower score indicates osteoporosis or osteopenia, which is low bone density that’s not as severe as osteoporosis.

If your bone density score is below normal, your healthcare provider is likely to recommend a follow-up bone density scan. Based on your test results, your next scan could range from a 2-year to 10-year time frame. Talk with your healthcare provider about what is best for you based on your individual factors.

Treatment

We can improve bone density and treat osteopenia and osteoporosis with a wide range of options. To start, there are simple lifestyle measures that anyone can do at home. Weight-bearing activities, such as walking, and proper calcium and vitamin D supplementation is the foundation for bone health.

We may also prescribe medications that slow bone loss or rebuild bones. The most common medication used is Fosamax (alendronate), a once-weekly pill, that helps prevent the worsening of osteoporosis. If you don’t like pills or have a sensitive stomach, there are injectable options such as once-yearly Reclast or twice-yearly Prolia. A conversation with your healthcare provider can help you find the right medication to meet your needs and lifestyle.

Our goal in testing and treating bone density is to help prevent fractures and breaks so you can stay active and live independently. ■

DID YOU KNOW?

Bone health is often ignored before and after fragility fractures occur. Worldwide 1 in 3 Women and 1 in 5 men over age 50 will have a fracture due to osteoporosis. The bone density scan, also called DEXA or DXA for the dual-energy X-ray absorptiometry machine, only takes 5-15 minutes. The scan exposes people to very little radiation, far less than we get on a cross-country airplane flight. When your Mankato Clinic provider orders the test, our Patient Services department will help you schedule it, let you know what to expect and give you a few directions for testing. Every test is reviewed by a radiologist.

A bone density scan will quantify your bone density and allow your provider to estimate your risk of fracture. Then you and your primary care provider can discuss ways to improve your bone health.

TOP 5 TIPS FOR GOOD SLEEP

Often overlooked, sleep is key to our health and well-being. Sleep Medicine experts recommend that adults get an average of 7 to 9 hours of sleep each night. Here’s your sleep hygiene guide!

Maintain a consistent sleep schedule.

Your body has an internal clock based on a 24-hour circadian rhythm cycle. This cycle guides your body’s sleep and wake cycle. To regulate your clock, try to go to bed and wake up at the same time every day, even on weekends. If you like to nap, keep naps to 20 minutes or less so you can still fall asleep at your normal bedtime.

Make your bedroom a sleep retreat.

Studies show people sleep best when their room is dark, cool, quiet and comfortable. If your room is too light, try roomdarkening shades or curtains. If your room is noisy, the white noise of a fan can be helpful while keeping you cool too!

EXPERT ADVICE

Unplug from screens at least 30 minutes before bed.

Blue light from cell phones, TVs and tablets suppresses the production of melatonin, a hormone that helps you fall asleep and stay asleep. People often watch TV to help them sleep.

Research shows that sleeping with the TV on leads to less sleep and poor-quality sleep due to the light and sound. In fact, the brain continues to process dialogue instead of doing other jobs it needs to do during sleep. Try creating a relaxing routine such as music, meditation, prayer or reading.

When we don’t get enough sleep, we say we feel run down. When we sleep well, we feel refreshed and ready to take on the day! That’s because good sleep restores us to improve our mood, promote heart health, regulate blood sugar, support the immune system, relieve stress, maintain a healthy weight and restore our muscles. It helps us think more clearly and solve problems.

Sleep apnea is a common cause of poor sleep. Symptoms are loud or frequent snoring, pauses in breathing, choking or gasping sounds, fatigue, unrefreshing sleep, insomnia and headaches.

If you or a loved one notices these signs, you may benefit from a sleep study. Sleep studies are often done in a sleep center such as Mankato Clinic’s J. Scott Sanders

Limit caffeine and alcohol.

Skip the caffeine in the afternoon and evening. Limit alcohol before bedtime too. While a drink before bedtime can help people fall asleep faster, alcohol often disrupts your sleep later and causes light sleep. Avoid late, heavy meals too and give your body time to digest food before bed.

Center for Sleep Medicine. Getting a sleep study is like checking into a comfortable hotel room. Before bed, sleep technicians will put you at ease while they place small sensors and wires that connect to a computer. We monitor your brain and body’s activity while you sleep.

Home sleep studies are also a viable option.

As a board-certified neurologist and sleep medicine specialist, I review the sleep study, diagnose and prescribe treatment. A CPAP (continuous positive airway pressure) machine is a common treatment. The machine keeps the

Be active.

Daily exercise increases the amount of REM (rapid eye movement) sleep which aids dreaming, emotional processing, and healthy brain development. Being active during the day can help us fall asleep more quickly and sleep more deeply. Exercising outside is even better! Avoid vigorous exercise late in the day.

What is Urgency Care?

Urgency Care provides medical care for most illnesses, injuries and conditions 7 days a week with evening and weekend hours. A highly trained team of physicians, practitioners and nurses are here to care for you.

Mankato Clinic Urgency Care in our Main Street location has been remodeled to improve patient care, comfort and services.

We offer walk-in care with shorter wait times and lower costs than hospital emergency rooms. Urgency care facilities also free up emergency room resources for people who truly need critical care.

We have added large exam rooms with overhead lights for procedures, stretcher beds and bedside cardiac monitoring. For optimal patient safety, a nurses station is centered in the department and highly visible to the triage and exam rooms.

With bedside cardiac monitoring, we can treat patients with chest pain who have no history of heart issues. The cardiac monitor in the nurses station alerts us to any abnormal rhythms, heart and breathing rates, blood pressure and oxygen levels so we can immediately check on patients.

We’re still here for coughs, illness, sore throats, infections, minor injuries, sprains and stitches. We offer full lab services with testing for strep, COVID and other infections. Our phlebotomists come right to your exam room to do the tests! Our families love this. Nursing staff will call you with results and necessary instructions.

Urgency Care also offers access to full diagnostic imaging services to include x-rays, CT scans, MRI imaging and ultrasounds. Your Urgency Care provider may recommend a follow-up visit with your primary care or specialty provider. ■

MY DAD KEEPS FALLING, WHAT CAN I DO?

This can be one of the most frustrating and scary things for family members to think about. The idea that our parents may fall and get hurt and not be able to get help fast keeps many loved ones up at night.

Yes, there are fall pendants, however many individuals forget to put it on or may decide not to wear them overnight. Overnight is when many falls happen.

There is nothing you can do to guarantee that a person of any age will never experience a fall. The likelihood undoubtedly increases as one ages, with medical and physical complications. However, there are things individuals can do to help prevent falls.

1. Be aware of your surroundings and always use assistive devices such as walkers or canes. You can also mark elevation changes such as stairs, threshold, and steps to make it easier to see and adjust your body to elevation changes.

2. Keep your home safe. Remove clutter and keep at least 36-42 inches in width for safe walking paths. You should also remove or secure throw rugs and keep walkways well-lit. Home health services, like Adara Home Health, can help keep the home safe in a variety of ways. Professionals can implement a low vision program that helps those with decreased vision navigate their space. Occupational therapy can recommend assistive devices.

3. Stay active. Inactivity leads to weakness that is a significant risk factor for falls. It is however important to decrease your activity and be more careful on a “bad day.” Moreover, get plenty of rest, fluids, and proper nutrition; this helps to

maintain your strength and balance. Adara Home Health can help individuals stay active and independent. Physical therapy can help individuals safely work on strength-building and balance issues. Ongoing exercises can be recommended.

4. Review your medications. A medication review also includes over-the-counter medications and supplements. Some side effects of medications can make you light-headed, dizzy, or drowsy. This should be discussed with your physician. Home health services can help manage medications and educate patients and caregivers about new medications. Skilled nursing services can also set up medications and help monitor for adverse effects.

5. Wear proper footwear. Wear shoes with support. This lessens pain and increases balance. Non-slip soles can also help individuals avoid slipping.

If you are concerned about falling or worried about a loved one falling, start by making an appointment with your primary care provider to discuss the best fall prevention options. ■

MAKING MAGIC FOR 14 YEARS!

Turns out, I had some learning to do. Because I had served on the Board of Directors at the Mankato Family YMCA while working at Taylor Corporation, and since I had been regularly visiting the YMCA since my days in youth sports in third grade, I felt I knew what it would be like to serve as Executive Director when I applied for the position.

It turns out I knew very little. As a matter of fact, even after 14 years in the position, I think it’s safe to say I wouldn’t bet on what will happen tomorrow at the Y. What I can predict with some certainty is tomorrow will bring a challenge. It will bring a smile. It will see the Y lending a helping hand. And by the end of the day, I will have learned something I did not know when the day began.

Before serving as Executive Director, I knew the YMCA was special. I still believe that. But I think a better word than special is magical. Every day I see people coming in our doors, and chances are when they go out those same doors they are personally in a better place. This is a place people come hoping to get better. And many more times than not, better is what they get. That’s magic.

I’ll miss seeing that magic happen every day. But what I will miss the most, and I know it sounds cliché, is the people. I’ll miss the wonderful staff who could all make more money working somewhere else. But instead, they work with the young, the notso-young and the children, making the magic happen.

I’ll miss the volunteers. The board members, the mentors, the volunteer coaches do what they do at the Y because it means something; it matters to them. To them the magic matters. And of course, I’ll miss the members. With my office door located by the front door, I get the opportunity to hear from them what’s going on in their lives. I get to hear how we can do things better. I get to hear how we’re doing things right. I get to hear them. And that I will miss.

So many times, I have heard a member say they have another family because they belong to the Y. I have a wonderful family at home, and I look forward to spending more time with them when I retire this July. I know I will miss my second family, but that’s okay. I’ll know where to find them. They’ll still be making magic at the Y. ■

READING NOOK

inspires book love at Children’s Health Center

Mankato Clinic and The Creative Company are celebrating a 5-year partnership of bringing books to the Children’s Health Center on the Wickersham Health Campus.

“As publishers we want to share and give access to our books to as many children as possible — and if we can start within our own community, even better. Mankato Clinic Children’s Health Center created a thoughtful space and program to do this,” says Samantha Peterson, Marketplace Director, The Creative Company.

The Creative Company, a Mankato-based book publisher, donates books to the Reading Nook in the Children’s Health Center lobby. When children come to their appointments, they can choose a book and bring it home with them!

“The Reading Nook is a fantastic collaboration. The Creative Company donates a wide and diverse range of books to capture children’s interests,” Dr. Katie Smentek, Mankato Clinic pediatrician, says.

“We are excited to partner with the Mankato Clinic as The Creative Company began here in 1932 and remains today across four generations of family participation,” Samantha says.

Knowing how books help children thrive, Mankato Clinic created the Reading Nook in the Children’s Health Center, which opened in 2014. Children’s Health Center is home to the Mankato Clinic Pediatrics Department and a satellite clinic for Gillette Children’s Specialty Healthcare and Pediatric Therapy Services.

For Dr. Smentek, it was a dream to have a playful space at the clinic for children and families to choose a free book.

“Reading and learning begin at birth, well before children enter school. We know that reading aloud to children and exploring books together at an early age, even as infants, helps a child’s brain and language skills develop. Reading with our little ones helps children get a good start,” Dr. Smentek says. “Books are the building blocks of learning.”

If it’s hard to find time to read with your child, Dr. Smentek says it’s OK to ask for help. Ask older siblings, grandparents and neighbors to read to little ones. Once children find a love for books, they will be off and reading on their own in no time.

“For children and parents who struggle with reading, talk with your pediatrician or family medicine provider. We’re here to help you,” Dr. Smentek says.

Children of all ages and abilities are likely to find a book in the Reading Nook. Nature, animals, science, sports, unicorns, countries, biographies, careers and stories of all kinds!

The Creative Company is a dynamic group of publishers with each division specializing in different audiences, with books for all ages from toddlers to high school. Each year, The Creative Company gives to local and national organizations of all different types and sizes. Locally, families will find The Creative Company books in school libraries and The Blue Earth County Library.

“What is most important for us is seeing a continuation of our company’s values — education, creativity, reading equality and individuality — through each donation made,” Samantha says. “We are always looking for more ways to share our books within our community, so I encourage people to reach out!” ■

LisaandMayalovedigging intogethertohelptheir gardenthrive.

Seeing the fruits of her labor puts a smile on Maya’s face. Maya doesn’t mind getting her hands dirty!

Make Me A Thriver

Dig into gardening and THRIVE!

For Lisa Kaye and her daughter Maya, thriving looks a lot like dirt under their fingernails and a few stains on their jeans. It looks a lot like hard work. For this duo, thriving is gardening. Because to them it is about more than growing fruits, vegetables and flowers. Gardening is about growing a healthy lifestyle physically and mentally, while cultivating their relationship. It’s something that Lisa is happy to pass down to her daughter.

“I’ve been gardening my whole life,” Lisa says. “We had just under two acres where I grew up and my mom and dad always had a huge garden spanning the backyard. We had apple trees and flowers as well. So, it hasn’t ‘recently’ become important, it’s always been a part of our lives, growing our own food on a small scale. I’ve just in the past few years been sharing more about it and people are asking questions. Anyone can do this even on a tiny scale! Being able to take something from seed to table right in your own space is rewarding. The taste difference is incredible, not to mention you control what goes on and in your homegrown food. I know it’s an organic practice, safe to pick and eat off the plant!”

Maya follows in her mom’s footsteps.

“I have always remembered her having a garden and I used to love to go take fresh tomatoes off the vine and eat them in the garden. I guess I just started helping her while she was out there and started learning about all the ways she did things and the reasons why,” Maya says. “I love eating really good food and it gives us things to do when we are bored. There is always something to do in the garden.”

Lisa and Maya make life simple by keeping their garden accessible, but they have also embarked on a new adventure, working together on a plot at Valley Veggies & Flowers.

“We have a decent-sized garden and multiple raised beds in our backyard,” Lisa explains. “Our flowers are mostly in the front yard and around the patio. We have both been gardening at home, canning and preserving our whole lives. Valley Veggies is another extension of our gardening. This will be season two for us there and we love every minute of the hard work!”

As they help the fruits of their labors thrive, all that labor they put in, in turn helps them thrive.

“For us it’s both relaxing and rewarding,” Lisa says. “You are constantly learning something: Trial and error, weather patterns, sunlight hours, what does this seed ‘need’ to germinate and produce. We also get to spend time together and connect with other like-minded growers at Valley Veggies, garnering knowledge from them and sharing stories and tips. Connections that are made between these gardeners are so special!”

Maya agrees. “I’ve met people that are so nice and that have a lot of experience to share. I know how to help customers learn to cut their own flower bouquets. I even got to decide what to plant where! I helped plant all the sunflowers last year and even designed a raised bed all by myself and got to watch it grow!”

A mother-daughter duo, thriving together. ■

To experience locally grown veggies and flowers, visit valleyveggiesmn.com. Lisa Kaye is from Radio Mankato (Minnesota 100, KTOE and North Star 105.9).

LANIE BURETTA

Thrive Ambassador

“The Mankato Clinic emphasizes patient care. I LOVE working for the Mankato Clinic and always tell people that it is so easy to care for patients when you work for a place that truly appreciates their employees,” Lanie says.

Every year, the Mankato Clinic names a Thrive Ambassador to recognize a team member who embodies the “Together we thrive” culture. This year, it’s Lanie Buretta, Radiologic and CT Technologist.

“Lanie is an absolute joy to work with! She always comes in with a smile on her face. She is willing to go the extra mile for patients – getting a smile out of even the toughest of patients. She works mostly evenings and she is a breath of fresh air when she comes in and brightens everyone’s afternoon,” says Lindsey Walker, BS, RT, Diagnostic Imaging manager.

Lanie works the afternoon, evening and weekend shifts in Diagnostic Imaging. In the afternoons, she does CT scans, X-rays and mammograms. While many patients are scheduled, she also accommodates patients at a moment’s notice. On evenings and weekends, she performs CT scans and X-rays for Urgency Care.

“I have built a strong working relationship and friendship with the Urgency Care department,” Lanie says.

A computed tomography (CT) scan is a diagnostic tool that detects injuries and diseases. Combining a computer with an X-ray machine, a CT scan produces a very detailed 3-D image of soft tissues and bones. With a donut-shaped tube that rotates the X-ray around the body, a CT study usually has on average 700 or more images!

“In high school, I volunteered at the hospital through a career class. I went to medical records and radiology and discovered my love for the field,” Lanie says.

Lanie has worked at Mankato Clinic for 10 years. She joins team building activities and attends clinic events like the holiday party and summer picnic. When Mankato Clinic participates in community events and fundraisers such as Climb2Feed Kids, Lanie is the first to volunteer and recruits others to join. She also recommends Mankato Clinic to friends and family members who are looking for a job.

“I work with amazing coworkers, management, and providers. With all the cross communication and friendliness between departments and locations, the Mankato Clinic feels like a big family,” Lanie says. ■

Meet Nancy Dobson

Clinic Foundation Board Member

“I’ve been on the board since shortly after I retired! I’m a big fan of the Mankato Clinic and all that they do in our community. So, it was an easy ‘YES’ when I was asked to be on the board. We each bring in a unique perspective - and I’m happy to bring mine to the board,” reflects Nancy.

Nancy’s perspective is so important to the board as she grew up in the community and chose to raise her family here. Along with growing a family here, she also grew professionally.

“Professionally I am a physical therapist with a passion and heart for kids. I started Pediatric Therapy Services in 1991 – and over the years it continued to grow to meet the needs of children with a wide range of abilities and challenges,” she explains. “I loved it all – the kids and families we served, helping them achieve meaningful milestones in development, and working alongside my amazing colleagues. A few years ago, as I was starting to think about retirement, PTS became part of the Mankato Clinic. And now I’ve been officially retired for almost two years!”

Retirement for Nancy though, didn’t necessarily mean slowing down.

“My husband Joe and I spend the summers in Elysian at our lake house and working at our ice cream shop Scoops! We have LOTS of grandkids that we love to spend time with. And we also love to travel over the winter months. We’ve hiked and biked many trails across the U.S.,” says Nancy.

Joining the Foundation board was the perfect addition to her retirement lifestyle.

“It is especially rewarding to be a part of approving grants that help the kids/youth in our community. The backpack food program has grown to meet the needs of so many kids. Kids should never have to go hungry,” she says. “I really love the Mankato and the surrounding communities here in southern Minnesota. The last few years have been especially challenging in many ways – and there are some significant needs right here in our community. There is no time better than now for each of us to share our time, talents and treasures with others. It’s really a privilege to be a part of it all.” ■

Our Community

Giving back to the community is part of who we are at the Mankato Clinic. A small, dedicated group of physicians started the Mankato Clinic Foundation in 1965, making it one of the oldest foundations in Minnesota.

Our mission is to encourage and support the health and well-being of the communities we serve through efforts that promote and improve community wellness. In the last 10 years, the Mankato Clinic Foundation has provided more than $1.8 million in scholarships and grants to worthwhile organizations and programs.

During the first quarter of 2023, the Mankato Clinic Foundation awarded $96,125 in community grants to:

• Centenary United Methodist Church Holy Grounds Breakfast ($3,000)

• Connections Ministry Emergency Shelter ($5,000)

• Educare Foundation ($25,000)

• Greater Mankato Area United Way First Steps ($42,125)

• Greater Mankato Area United Way Project Community Connect ($1,000)

• Life-Work Planning Center Women in Transition ($4,000)

• Mankato Youth (MY) Place ($7,500)

• Mankato YWCA Girls on the Run scholarships ($3,000)

• St. Peter North Elementary Kindness Retreat ($3,500)

• Southern MN Crisis Nursery ($2,000)

The foundation receives the majority of its funding from Mankato Clinic physicians, along with giving by Mankato Clinic staff and the greater Mankato community.

If you would like to help build a healthier tomorrow for greater Mankato communities, you can give to the Mankato Clinic Foundation at: Mankato Clinic Foundation 1400 Madison Avenue, Suite 324B Mankato, MN 56001

Grants are awarded on a quarterly basis. The next grant deadline is June 1. To learn more, visit mankatoclinic.com/mankato-clinic-foundation

A new place for MY Place

The mission of the Mankato Clinic Foundation is to encourage and support the health and well-being of our community through health initiatives that promote and improve community wellness. That’s why when the grant request from MY Place landed in front of the Foundation board, it was clear their missions aligned. Mankato Youth Place, MY Place as it is commonly known, is committed to providing a nurturing environment that empowers youth to build the confidence, knowledge and skills needed to reach their full potential. Executive Director Erin Simmons says the need is greater now than ever before.

“MY Place is serving 60% more kids this school year than last. A typical day is between 55 and 65 kids. While we do not have "qualifiers" to attend MY Place, the majority of our kids are experiencing poverty and the majority of our kids are kids of color,” says Simmons.

MY Place provides care at no cost to students during non-school hours. Supper is served every day before children go home. And yearround programming includes academics, STEM, art, music and healthy lifestyle programs.

As a nonprofit, MY Place relies on donations from the community to open its doors and serve children in need. Funding from the Mankato Clinic Foundation helps MY Place continue to break down barriers.

“We cannot do the work we do without gifts like the Mankato Clinic Foundation's to our general operating budget,” explains Simmons. “We are dependent on donations to do the work that we do, whether it is a donation of art supplies or money; everything works together to allow us to maximize our impact.”

As the organization serves more children each year, the need for space also became critical. To that end, MY Place purchased a new building, which of course increases their funding needs.

“We are entering into a capital campaign to support the purchase and remodel of our new building. The new building is located at 1315 Stadium Road and will bring us into closer proximity to more kids and is in a more walkable, bikeable area. The new building is larger than our current location so over time we'll be able to increase the number of kids we are serving. “
