Chronic Kidney Disease Management

At our chronic kidney disease program, we are dedicated to providing comprehensive care and support for individuals living with kidney disease. Our team of experienced healthcare professionals is committed to helping our patients manage their condition and improve their quality of life. We offer a range of services and resources to meet the unique needs of each patient.

Our approach:

• Multidisciplinary Care- We believe in a collaborative approach to care, involving a team of specialists including nephrologists, dietitians, social workers, and nurses. This ensures that all aspects of our patients' health and well-being are addressed.

• Personalized Treatment Plans- Each patient is unique, and we tailor our treatment plans to meet their specific needs. Our team works closely with patients to develop a plan that includes medication management, dietary recommendations, and lifestyle modifications.

• Education And Support- We understand that living with kidney disease can be challenging, both physically and emotionally. That's why we provide education and support to our patients and their families, helping them understand the condition and cope with its impact on their lives.

• Regular Monitoring- We closely monitor our patients' kidney function and overall health through regular check-ups and laboratory tests. This allows us to detect any changes or complications early on and adjust the treatment plan accordingly.

Through treatment and education, our clinical team of professionals assists patients in a better understanding of their conditions, enabling them to enjoy a greater level of independence and improved quality of life.

Ventilator Program

Our ventilator program is under the direction of Dr. Ansari, a leading area pulmonologist specializing in pulmonary disease and critical care medicine at Tufts Medical Center and Melrose Wakefield Hospital, along with Dr. Jin Chae, an Internal Medicine physician affiliated with Spaulding Cambridge. North End’s respiratory team is changing lives daily due to their advanced skills in pulmonary care.

Our NEW dedicated ventilator unit features a highly skilled multi-disciplinary team of health care professionals which consists of a pulmonologist, a respiratory therapist, RNs, LPNs and other professional caregivers. Every member of our ventilator program team works with the resident and family to help recover maximum function for residents with needs for a ventilator. Our dedicated vent unit works in tandem with our pulmonary rehab program to design a treatment program that is best for our residents. Each individual has different needs and requests, and our staff of professionals is here to help each person regain the most breathing ability possible.

Our ventilator program and services include:

• Dedicated ventilator unit led by a board-certified pulmonologist

• Focus on increasing mobility and weaning dependence whenever possible

• 24-hour respiratory therapists

• Direct oversight from our internal medicine physicians

• Nurse practitioner or hospitalist on unit daily for medical interventions

• Speech therapy for bedside swallow evaluations and communication strategies

• Physical and occupational therapy to increase endurance and ability

Cardiopulmonary Rehabilitation

Our physician-led Cardiac and Pulmonary Rehabilitation program, under the direction of a pulmonologist, as well as our team of respiratory therapists are focused on helping patients achieve the most active life possible despite any cardiopulmonary diagnoses and physical limitations. We tailor our program for individuals with heart disease and pulmonary disease, with diagnoses such as CHF, COPD, post heart and lung transplants, emphysema, acute myocardial infarctions and acute respiratory failure.

Our comprehensive program features:

• Team consults by our board-certified cardiologists & pulmonologists

• Advanced cardiac life support (ACLS) certified nurses

• 7-day-per-week therapy services as ordered

The Urgent SNF Service at Marquis Health Consulting Services supports a global health care expectation for the right provision of care in the right place at the right time.Therefore, our solution driven service aims to provide an alternative option to individuals not meeting acute admission criteria, however requiring and/or benefiting from supervisory skilled rehabilitation services, skilled nursing oversight or stabilization.

URGENT SNF PAYMENT OPTIONS:

Medicare A: (qualified 3 night) stay within 30 days

HMO: Willingness to accept without prior authorization with select HMO/ MCO

Private Pay: Reduced Private Pay All Inclusive Rate promoting the stabilization often necessary to avoid repeat and unnecessary ER/ED visits. All-inclusive services include room/board, meals, therapy and nursing services.

In partnership with our patients and or responsible party, every member of the team works collaboratively to develop an effective Care Navigation plan. Our belief in providing a better patient experience through quality and satisfaction will ensure successful navigation through the health care continuum.

Roadmap to successful navigation:

• Patient-centered needs/preferences identified on admission with assessments and continues throughout the stay

• Choosing the appropriate placement in the Marquis continuum

• Provides financial resources
