Elmhurst Rehabilitation and Healthcare Center is a stately 4-story skilled nursing facility conveniently located adjacent to Roger William and Providence VA Hospitals. Nestled in one of Providence’s oldest neighborhoods and bordering the downtown area, Elmhurst has consistently been ranked as one of the top nursing homes in Rhode Island (and the nation), and is recognized for serving the community’s subacute rehabilitative and long term care needs with distinction.

We are Medicare and Medicaid certified, and we are proud to be a preferred provider of Miriam Hospital. Our outstanding subacute rehabilitation program successfully reduces hospital readmissions and optimizes the physical function of individuals recovering from surgery or illness. Alzheimer's & Dementia Care

Our secured unit provides a nurturing setting for individuals in various stages of dementia and Alzheimer’s disease.

We’ve created a neighborhood that is truly home to both our residents and their family members who are always welcome to visit. Our unique memory care setting is defined by our combination of qualified caregivers, comprehensive innovative treatments and specialized attention. We provide 24-hour skilled nursing services, medication management with a geriatric psychiatrist, and structured daily activities designed to meet the needs of those with memory impairment.

Cardiopulmonary Care

Our physician led Cardiopulmonary Care program, under the direction of Dr. Ibrahim Elgabry, is focused on helping patients achieve the most active life possible despite any cardiopulmonary diagnoses and/or physical limitations.

We tailor our program for individuals with heart disease and pulmonary disease, with diagnoses such as: CHF, COPD, post heart and lung transplants, emphysema, acute myocardial infarctions and acute respiratory failure.

Our comprehensive program features:

• Team consults by our board certified cardiologists & pulmonologists

• Rapid response team (RN, ACLS, BLS & full-time respiratory therapist)

• 7-day-per-week therapy services as ordered

• 24/7 access to laboratory & radiology services

Subacute Rehab

At Elmhurst, we are dedicated to proactively addressing your rehabilitation needs in a modern and tranquil setting. Reflective of our approach, we start planning your discharge immediately upon admission. Individualized care plans are crafted to ensure optimal healing – fully preparing you for your return home.

Geared towards individuals recovering from orthopedic surgery, stroke, cardiac or pulmonary condition – our goal is to restore independence and maximum function. To get you back to yourself and home as soon as possible, we deliver cutting-edge therapies up to 7 days a week. Our expert rehab professionals craft custom rehabilitation regimens utilizing the latest rehab technology and techniques to accelerate your recovery.

The Urgent SNF Service at Marquis Health Consulting Services supports a global health care expectation for the right provision of care in the right place at the right time.Therefore, our solution driven service aims to provide an alternative option to individuals not meeting acute admission criteria, however requiring and/or benefiting from supervisory skilled rehabilitation services, skilled nursing oversight or stabilization.

Private Pay: Reduced Private Pay All Inclusive Rate promoting the stabilization often necessary to avoid repeat and unnecessary ER/ED visits. All-inclusive services include room/board, meals, therapy and nursing services.
