Dr. Rekha Gohel Medical Director

Dr. Rekha Gohel, M.D. is a Geriatrician and Internist under the practice named Elite Medical Care, located in Freehold, NJ, and is affiliated with CentraState Medical Center. She has extensive experience in providing diagnostic, therapeutic & preventive medical services to geriatric population, taking care of patients in office, hospital, nursing homes & assisted facilities. She is involved in clinical trials specializing in Alzheimer’s dementia-performing neurocognitive assessments on the research participants & procedures involved from the informed consent stage to patient’s evaluation, reviewing clinical data, diagnostic studies, & supervising the staff. Dr. Gohel completed a Geriatric Fellowship in Flushing Hospital Medical Center, Flushing NY, and has had hospital appointments in Raritan Bay Medical Center, Old Bridge and CentraState Medical Center, Freehold.

Neuro Recovery Program

Dr. Jia Cheng

Our Neuro Recovery Program is directed by Director of Movement Disorder Dr. Jia Cheng of Centra State Medical Center.

With consultative presence and support, our interdisciplinary team of rehabilitation experts assists those recovering from neurological events, disease, and common diagnoses. Our program is designed to assist those affected by neurologically disabling conditions to improve abilities and return to their most optimal functional levels.

Patients can expect an individualized recovery plan that will support a return to more routine daily functions.

Pulmonary Rehabilitation

Our Pulmonary Rehabilitation Program under the leadership of Shore Pulmonary, is tailored for individuals with pulmonary disease, with diagnoses such as COPD, post-lung transplants, emphysema, and acute respiratory failure.

Orthopedic Rehabilitation

Dr. Eric J. Buxbaum

Our Orthopedic Rehab Program was designed with you in mind and is led by a board-certified orthopedic surgeon, Dr. Eric J. Buxbaum. The program’s clinically proven orthopedic care pathways, coupled with our signature rehabilitation experience, is an optimal option for your post-hospital rehabilitation.

• Our Benefits Specialists will review your coverage plan and payment options.

• Our Rehabilitation Team will provide an overview of your anticipated treatment plan and what you can expect in your recovery.

• Our Physiatrist will closely monitor your care as you progress through therapy.

• Our Discharge Planning Specialist will assist you with planning your discharge from day one of your stay.

Memory Care

Our certified skilled nursing facility features a 30-bed Memory Care wing dedicated to residents with Alzheimer’s & Dementia that is under direction of an Associate Professor, Dr. Zeeshan Khan, Centra State Medical Center affiliation.

Our combination of qualified caregivers, luxurious amenities, innovative activities, and special attention to Jewish tradition creates a care setting like no other. Our residents are comforted by the familiar values they hold dear… even when cherished memories start to fade.

Dr. Khan has years of clinical experience and specialization in the area of Alzheimer’s and dementia. Dr. Khan is affiliated with Centra State Medical Center as the Acute Care for Elderly (ACE) Unit Medical Director. He is an Associate Professor with Rutgers RWJMS at CentraState Family Medicine. In addition to Memory Care oversight, Dr. Khan will be introducing his residency program to The Jewish Home. This provides new physicians the opportunity for hands-on learning in the field of geriatrics.

Chronic Illness Program

Our Program is under the medical direction of Dr. Vandana Sharma. We have partnered with VNA Healthcare & Hospice for palliative medical care and pastoral support to address the pain, symptoms, and stress of chronic illness. Our proactive efforts, inclusive of an organized set of interventions are focused on the needs of a chronically ill and compromised patient population.

The Urgent SNF™ Service at Marquis Health Consulting Services supports a global health care expectation for the right provision of care in the right place at the right time. Therefore, our solution driven service aims to provide an alternative option to individuals not meeting acute admission criteria, however requiring and/or benefiting from supervisory skilled rehabilitation services, skilled nursing oversight or stabilization.

HMO: Willingness to accept without prior authorization with select HMO/ MCO Medicare Primary: Qualifed 3-night Stay within 30 Days

Private Pay: Reduced Private Pay All Inclusive Rate promoting the stabilization often necessary to avoid repeat and unnecessary ER/ED visits. All-inclusive services include room/board, meals, therapy and nursing services.
