controlling cholesterol What works, from the tried and true to the new

By Karen A. Jamrog n awful lot of us struggle with high cholesterol. In fact, according to the Centers for Disease Control, about one in every six adults has too much blood cholesterol, placing those individuals at an increased risk of heart disease. Anyone can develop high cholesterol, even kids.

C

holesterol is considered a significant risk marker for cardiac events, says Steven L. Schwartz, MD, FACC, medical director of Foundation Cardiology, located at Southern New Hampshire Medical Center. “And it is likely not just a marker but also an active player in that risk,” Schwartz says. Excess cholesterol can settle in arteries and eventually clog them, which impedes blood flow and can lead to a heart attack.

“There are two sources of cholesterol,” says Richard A. Boss, MD, a cardiologist at Concord Hospital Cardiac Associates. “There’s the cholesterol you eat and the cholesterol you make.” Cholesterol is a waxy substance manufactured in the liver that contributes to body functions, including hormone production and maintenance of cellular structure. “It’s in every cell in our body,” Boss says. “[But] it’s very important that we watch our diet and not add unnecessarily to the cholesterol our bodies are already making.”

Further contributing to cholesterol’s somewhat contradictory nature — our bodies need and produce it, yet too much is harmful — are two of its main components, LDL and HDL. LDL, which stands

for low-density lipoprotein, is considered a “bad” type of cholesterol because it transports cholesterol through the bloodstream and into arteries, contributing to the dangerous plaque build-up that leads to heart disease. “We know that plaques that have a lot of cholesterol in them are more likely to have plaque rupture,” Schwartz says, “and the ones that rupture are more likely to cause heart attacks.”

HDL, or high-density lipoprotein, on the other hand, protects against heart disease by removing cholesterol from the blood and returning it to the liver, where it is broken down and passed from the body. Given the consequences of LDL and HDL, it is best to minimize the amount of LDL in your body, Boss says, and maximize the amount of HDL. We run into trouble, however, when lifestyle and genetics skew cholesterol levels. Smoking and a lack of exercise, for example, contribute to plaque build-up in the arteries, and poor dietary choices — such as eating animal products laden with saturated fat; or baked goods, processed foods or oils that contain trans fat — stimulate our liver to produce more cholesterol. And people who have a family his-

HEART HEALTH tory of cholesterol problems might find that even if they make all the right lifestyle and diet choices, their body continues to produce too much cholesterol. “It’s almost like there’s a thermostat in the body that is set too high,” Boss says, “that pushes cholesterol production at a high level.”

For those patients, doctors typically recommend medication such as statins, which lower cholesterol by inhibiting the natural production of cholesterol in the body. “We use them in a lot of people, with tremendous impact,” Boss says.

PCSK9 inhibitors are another cholesterolmanagement option now available to doctors and patients. They work by deterring degradation of LDL receptors in the liver so that more receptors are available to “catch” LDL and aid its release from the body, Schwartz says. PCSK9 inhibitors are administered via injection every two to four weeks and are not as popular as statins given their short track record — doctors prefer to see long-term outcome results before prescribing new medication — and high expense, particularly when compared to statins.

Patients who take cholesterol medication should be careful not to rely solely on their prescription to keep cholesterol in check. “You want to use as little medicine as possible,” Boss says, and at the same time recognize that cholesterol poses an ongoing challenge. “I don’t want [patients] relying on medication and ignoring diet and exercise,” he says; lifestyle habits such as not smoking, eating heart-friendly foods and consistently getting aerobic exercise play a key role in maintaining healthy cholesterol levels and a healthy heart. Food Matters It’s well-established that the food we eat plays a significant role in the amount of cholesterol we have. To help limit cholesterol: Avoid consuming saturated fat, which occurs naturally in a variety of foods that can be traced back to animals — think red meat, butter and ice cream, for example.

Avoid eating trans fat, often found in the form of partially hydrogenated oil in many commercially produced baked goods and fried foods.

Saturated fat and trans fat contribute to plaque build-up that clogs arteries and can lead to heart attacks. Trans fat is considered so dangerous to our health that the Food and Drug Administration has deemed partially hydrogenated oil as not “generally recognized as safe” in human food, and in 2015 banned its use in industrially made food. The FDA says its action “is expected to reduce coronary heart disease and prevent thousands of fatal heart attacks every year.” Food manufacturers have three years to comply with the FDA’s regulation, though, so not all have yet modified their recipes to eliminate trans fat from their products. In addition, naturally occurring trans fat will remain present in some foods.

The American Heart Association recommends that all adults age 20 or older have their cholesterol checked every four to six years.

Ways to Reduce Your Cancer Risk Lifestyle choices can help safeguard health

ome cases of cancer can be traced back to a likely trigger, such as an inherited genetic fluke. In many other instances, however, cancer appears to be random — the luck of the draw. “Most cancers are sporadic; we don’t know why they happen,” says Brian Knab, MD, medical director and radiation oncologist at the Elliot Regional Cancer Center. But even though much about cancer remains uncontrollable and unknown, it is clear that choices we make each day can significantly decrease our odds of being diagnosed with the dreaded disease.

Unfortunately, we cannot wholly prevent cancer through the way we live our lives. Even individuals who exercise daily, maintain an ideal body weight and rarely put red meat on their plate can develop cancer. But “doing the right things helps,” Knab says.

For starters, do not overlook the power of early detection. “Early cancer detection and treatment is one of the most effective means of reducing the risk of developing and dying from cancer,” Knab says.

Mammography, colonoscopy and other health screenings are capable of detecting cancers at an early stage or even before a cancer develops, Knab says, and have helped to dramatically reduce cancer deaths in the United States.

In addition to taking part in screening, consider the lifestyle choices you regularly make. “Some studies suggest that approximately one-third of all cancers can be attributed to environmental and lifestyle risk factors such as tobacco use, obesity, diet, inactivity and air pollution,” Knab says. Alcohol consumption also contributes to your likelihood of developing cancer. Lifestyle factors have been linked to a variety of cancers, Knab says, including those most commonly diagnosed in the United States: breast, colorectal, prostate and lung cancer.

Among the ways individuals can influence their cancer odds, tobacco use tops the list. Smoking, Knab says, accounts for one-third

of all cancer deaths in the United States.

But weight also plays a significant role in the development of cancer. According to Knab, excess weight has been associated with up to 20 percent of all cancers. Increased body fat causes insulin resistance and can promote cancer growth, Knab says, and body fat can fuel the development of hormone-related cancers such as breast and prostate cancer.

And while the food you eat, of course, affects your weight, when it comes to cancer, diet by itself is also important. A diet high in fat has been associated with development of prostate cancer, Knab says, and a diet high in red meat has been linked with an increased risk of colorectal cancer and an increased risk of dying from cancer. You’ve heard it before, but consuming vegetables, fruits and whole grains while minimizing sweets and red and processed meat is the way to go. In fact, a wholesome diet can help lower your risk of all sorts of health trouble, including cancer, heart disease, diabetes and stroke, according to the American Cancer Society.

Meat is considered processed if it has added preservatives or has been cured with salt or smoke, or heated at a very high temperature to extend the product’s shelf life, Gilbert-Diamond says.

Graying in the Granite State Seniors looking ahead at where to work, play and live

As a large percentage of New Hampshire’s baby boomers reach retirement age and beyond, health care workers are beginning to take a closer look at how to serve the state’s graying population.

ccording to a study by the New Hampshire Center for Public Policy Studies, nearly half a million of the state’s residents – fully one third of New Hampshire’s population – will be over the age of 65 by 2030. The study also found that during the next 20 years, the fastest-growing age group will be those age 70-74, and that there will also be significant growth in the number of people aged 75 and older.

The coming of the so-called “Silver Tsunami” is expected to affect housing, transportation, state and local budgets, Social Security and the economy. However, there is good news: Professionals who work with seniors every day say people are living longer, doing more and are happier and are making more of their later years than ever before.

Sixty really is the new 40, and that affects how seniors will work, play and decide where to live as they age.

This generation of seniors has much greater expectations of what’s in store for their retirement years than any before it, says Cathleen Toomey, vice president of marketing at the RiverWoods Group – an Exeter-based retirement community that oversees a number of communities throughout the state.

“They are generally more well-travelled, are healthier and have more hobbies and interests,” Toomey says. “Their interest and participation in fitness is much more significant than other generations.”

That evolving dynamic is reflected in the wider range of living options available to seniors. From independent living and assisted living facilities that provide lively and engaging communities with dynamic cultural offerings, to continuing care retirement communities (CCRCs) that offer increasing levels of medical care, these are not your parents’ nursing homes.

“They are vibrant, active communities with residents living life to the fullest, while contributing positively to the overall community they live in,” says Tina Partridge Vincour, director of senior services and lifestyle at the Calamar Communities, which manages senior communities in Bedford, Londonderry and Concord. “There are more choices and different types of senior housing options to choose from.”

People investigating senior housing essentially choose from several options. Independent living communities are for people comfortable taking care of themselves and their day-to-day activities such as making

meals and housekeeping. Assisted living is just that – a community for seniors who have reached the point where they may need help with their daily tasks such as bathing, dressing and keeping up with medications.

Assisted living facilities typically have a medical professional available at all times. People age 62 and older can now choose to move into CCRCs such as Silverstone in Nashua or Birch Hill Terrace in Manchester, which recently merged with RiverWoods in Exeter.

“Assisted living and CCRCs are very different things,” Toomey says. “One of the challenges for us is explaining the differences between retirement communities, CCRCs and assisted living. People need to know the difference between the communities they’re looking at.”

CCRCs, which usually include independent living, assisted living and skilled nursing options, are offered in three types of contracts: Type A, which typically includes housing, residential services and amenities, including unlimited health care services at little or no increase in the monthly fee; Type B, which is typically offered at a lower entrance and monthly fee, and limits health care services accessed without an increase in the monthly fee; and Type C, which is essentially a fee-forservice contract, including similar housing, residential services and amenities, but requires residents to pay market rates for health services.

“It’s really not about finding a place to die, but finding a place to live and excel in your retirement years,” says Ben French, marketing manager at RiverWoods.

For many seniors, the emphasis is on the living and the giving.

They continue to add plenty of color and texture to the fabric of society as outdated stereotypes of whiling away the days in a rocking chair fast become obsolete. The AARP of New Hampshire offers “Life Reimagined Check-Ups” several times a year, providing a roadmap wherein seniors can pursue new goals.

The entirety of northern New England is also older than much of the country. According to the 2015 US Census, New Hampshire is now tied with Vermont for the second oldest population in terms of median age, edged out slightly

by Maine. Additionally, different regions of the state present different challenges. Coös County, which has the highest poverty rates, also has the highest percentage of seniors, and the population there is aging at a significantly faster rate than counties in the southern tier.

Not only are the patients getting older, so are doctors. The New Hampshire Center for Public

Policy Studies found that those practicing here are already significantly older than the nationwide physician population. It found that the impact of aging will touch virtually every critical question the state is likely to face in health policy and in other policy areas in coming years, and offered several suggestions. It said the state must look at the long-term care system to analyze the available infrastructure and future demands; health care systems need policy support in developing systems that will be sustainable in a market in which Medicare accounts for an ever-increasing share; and health care workforce issues need to be more aggressively addressed through a systems approach. “[Current retirees] will live longer than any
