Fiber: Good For All

Lower Stress, Stay Healthy? People who learned to ease their stress used medical services 43 percent less often, concludes a recent study from the Benson-Henry Institute for Mind Body Medicine at Massachusetts General Hospital.

Fiber is a type of carbohydrate that the body can’t digest. Though most carbohydrates are broken down into sugar molecules, fiber cannot be broken down into sugar molecules, and instead, it passes through the body undigested. Fiber helps regulate the body’s use of sugars, helping to keep hunger and blood sugar in check. Children and adults need at least 20 to 30 grams of fiber per day for good health, but most Americans get only about 15 grams a day. Great sources are

Soluble fiber, which dissolves in water, can help lower glucose levels as well as help lower blood cholesterol. Foods with soluble fiber include oatmeal, nuts, beans, lentils, apples and blueberries.

Insoluble fiber, which does not dissolve in water, can help food move through your digestive system, promoting regularity and helping prevent constipation. Foods with insoluble fibers include wheat, whole wheat bread, whole grain couscous, brown rice, legumes, carrots, cucumbers, and tomatoes.

Repairing the heart with stem cells A recent study from Cedars-Sinai Heart Institute in Los Angeles suggests that stem cells may, indeed, heal damaged hearts. The researchers treated 17 heart attack survivors with an infusion of stem cells taken from their own hearts. A year later, the amount of scar tissue had shrunk by about 50%. These results sound dramatic, but are they an indication that we’re getting close to perfecting stem cell therapy? “This is a field where, depending on which investigator you ask, you can get incredibly different

answers,” says Dr. Richard Lee, professor of medicine at Harvard Medical School and a leading expert on stem cell therapy. “The field is young. Some studies show only modest or no improvement in heart function, but others have shown dramatically improved function,” he says. “We’re waiting to see if other doctors can also achieve really good results in other patients.” Studies are producing contradictory results partly because researchers use different methods to harvest and use stem cells. Some are taken from the bone marrow of donors, others

CANCER Breast Cancer and Depression

At least 35% of women with newly diagnosed and early breast cancer have clinical depression. There are times when clinical depression is not apparent. It can be exhibited as “emotional or anger suppression”. I have seen ladies smile and tell me that they feel “fine” and cry at the same time. Studies have shown that anger suppression is most common in women who are also receiving chemotherapy. Do not be afraid to discuss your emotional feelings with the therapeutic team. This team would include the treating physician(s), oncology nurses, and full time psychologist in the Cancer Treatment Center. When an individual is being treated for cancer, life can, at times, be like riding on a roller coaster. There are ups and downs. There are times of exhilaration and times of terror. But remember, just like being on the roller coaster, you do have a seatbelt. It is my job to make sure that seatbelt is fastened good and tight. Every few weeks, you randomly start sneezing and your nose runs, but you blame it on your friend’s cat. Then, as you pass by a department store’s perfume counter, the sneezing and runny nose start again. Is it allergies? Actually, you are probably suffering from a condition known as rhinitis. According to the American Academy of Allergy, Asthma and Immunology (AAAAI), approximately one in three people with rhinitis symptoms do not have allergies. There are two types of rhinitis: allergic and non-allergic. Both types of rhinitis have similar symptoms, but only allergic rhinitis involves the immune system. When an individual with allergic rhinitis breathes in a seemingly harmless substance, the immune system overreacts. It mistakes that harmless substance as something hazardous and reacts by releasing chemicals, such as histamine, into the body. Symptoms of allergic rhinitis include itchy nose, mouth, eyes, throat, skin or any area; problems with smell; runny nose; sneezing; and watery eyes. These symptoms occur shortly after the individual comes into contact with the allergen. Allergic rhinitis can be caused by a variety of substances, resulting in perennial allergic rhinitis and seasonal allergic rhinitis. Perennial allergic rhinitis is caused by common indoor materials, such as dried skin flakes, urine and saliva found on pet dander, mold, droppings from dust mites and cockroach particles. These materials typically cause symptoms year-round. Seasonal allergic rhinitis, or hay fever, is usually caused by pollen from trees, grasses and weeds. The types of plants that cause hay fever vary from person to person and also from the areas they reside. Hay fever generally runs in families, so you have a higher chance of developing it if one or both of your parents have hay fever or other allergies. An individual suffering from hay fever will generally see symptoms arise in the spring and fall. Since allergic rhinitis causes inflammation in the nasal lining, it increases your sensitivity to inhalants. Symptoms may develop from inhaling certain irritants like smoke and strong odors. Additionally, changes in the temperature and humidity of the air can be triggers for allergic rhinitis. Allergy testing, which is usually done by skin testing, can be done to determine what substance is the cause of allergic rhinitis. The best treatment is to avoid the allergen, but that may be impossible. For mild allergic rhinitis, a nasal rinse is usually sufficient to remove the mucus from the nose. Saline solution is available over the counter, or you can make your own using one cup of warm distilled water, half a teaspoon of salt, and a pinch of baking soda. For more severe cases, medication such as oral antihistamines, antihistamine nasal sprays, corticosteroid nasal sprays, oral deconges-

tants, leukotriene inhibitors and allergy shots may be required. A diagnosis of non-allergic rhinitis is made after an allergic cause is ruled out. Triggers of non-allergic rhinitis include certain odors or irritants, changes in the weather, some medications and chronic health conditions. Symptoms of non-allergic rhinitis are sneezing, stuffy nose (congestion), runny nose, mucus (phlegm) in the throat (postnasal drip). These symptoms fluctuate all year and may last for hours or days. Typically itchy eyes, nose or throat are not symptoms of non-allergic rhinitis. Treatment is the same as allergic rhinitis, except over-the-counter oral antihistamines are not effective. This is because the immune system is not causing the release of histamine like it does with allergic rhinitis. THE LONG RUN Wayne Berry's no wimp. The Navy man has jogged the 38-mile Diego Garcia Island, conquered a slew of triathlons and finished two tours in Vietnam. But, by last fall plantar fasciitis left him in such pain walking was a trial. The leading cause of heel pain, plantar fasciitis has long been an incurable ailment with treatments that range from injections to surgery with varying degrees of success. But, according to Dr. Daniel Harlin that grim prognosis may be a thing of the past. "This is a game-changer," he says of new technology — Tenex — that in some cases produces 90 percent improvement. It changed the game for Berry, who had run the full gamut of treatment options (short of surgery) for the pain. "I had given up hope," Berry says. "Cortisone shots and therapy and stretches. "Plantar fasciitis is the most common cause of heel pain and is caused by a strain on the flat band of tissue that connects the heel bone to the toes. This ligament supports the arch of the foot, and as it's strained, the plantar fascia becomes weak, swollen and inflamed. When the strain repeatedly occurs, tiny tears are formed. "The pain is caused when micro tears occur and do not heal properly causing scarring. The area is surrounded by normal tendon, which makes more traditional treatments difficult in some cases," Harlin says. Traditional treatments include steroid injections (that must be done again and again in most cases) or surgery. Tenex is an ultrasound-based device done quickly by a

radiologist like Harlin, who is the medical director of radiology at the Regional Radiology Group and locally works at Lafayette General Medical Center (LGMC) among other Louisiana locations. "We do an ultrasound-guided nerve block on the bottom of the foot, and that's usually the worst part of the whole thing," Harlin says. The doctor then punctures one to three times leaving an opening less than a centimeter, and the procedure takes about 15 to 30 minutes to complete. "They just feel pressure in the procedure, and that's it. The device is ultrasound focused on a certain frequency, and it destroys the damage," Harlin says, " I can feel it with my fingers as it melts away the bad tissue. "Berry had the procedure in November and by Christmas was doing great. He has yet to resume regular running but is certainly on the move. "I'm happy to be able to get up and get around. I found myself running after my grandson and thought 'Holy cow, I'm running. Wow.' Everything is working fine and there's no pain. It's a world of difference for me," Berry says. The biggest problem most patients have is restraint during recovery in the early days. "Getting people to be compliant is the toughest. On a pain scale of 10, people go to a zero or a two, and then they push it too hard and re-injure it," he said. "People feel so good — many are active patients and are athletes who have injured themselves running or in tennis and they feel so good they want

to get back into it." Regardless of how good they feel soon after the procedure, patients should wait 12 weeks before resuming all activity, and a boot must be worn for two weeks. Harlin says when someone's been a jogger for 30 years and feels pain-free after barely walking, it's hard to get them to take it easy. If patients allow the proper healing, they will never need the procedure again unless they are re-injured. A good example is a broken arm comparison. If you fall out a tree and break your arm and it heals, you're fine unless you fall out of a tree again. "You need to ask, 'How did you get the injury in the first place? Running is trauma to your body long term. People's bodies break down," Harlin says. In addition to running, plantar fasciitis causes include: feet that roll inward too much when you walk, high arches or flat feet, long periods of walking, standing or running especially on hard surfaces, being overweight, shoes that don't fit well or are worn out, tight Achilles tendons or calf muscles. Where is MY Doctor???

HAVE YOU BEEN RECENTLY ADMITTED TO THE HOSPITAL?

If so you may have asked yourself or even the nurse “Where is MY Doctor?” These days many patients who are admitted to the hospital chances are they were not seen by their primary care doctor. So why isn’t your primary care doctor caring for you while you are hospitalized? The reason is there is a relatively new trend in the care of the hospitalized patient. Hospitalists are the term used for the doctors who specialize in the care of patients that are in the hospital. Close to a decade ago this movement started to evolve, we are now seeing it more and more in the acute hospital settings. Why you ask? Some reason are for convenience, efficiency, patient safety, cost-effectiveness for hospitals, and the increasing need for more specialized and coordinated care for hospitalized patients. Hospitalist are board-certified internal medicine physicians who have undergone the same medical school training that your primary care physician has. The only difference is that they have chosen a non-traditional route of practicing medicine. They have chosen to work strictly in a hospital setting, rather than an office setting. The advantages to having a hospitalist care for you during your

hospital stay is that they may have more expertise in caring for complicated hospitalized patients on a daily basis. They are readily available to the nursing staff and to families to order tests, and discuss results. Hospitalist are looked at as captains of the ship. They are coordinating your care during your stay in the hospital. With the prompt ordering and results physicians can diagnose and treat conditions rapidly and hopefully expedite your discharge to home. The main disadvantage to having a hospitalist care for you is that they may not know your detailed medical history as well as your primary care physician. However, hospital systems throughout the country are working to integrate medical record information so that it is accessible throughout the system. Physicians are able to communicate via the electronic medical records. In the meantime, old fashion fax and telephone communications are used. All records are sent to your primary care physician at discharge. Always make sure you ask questions and are active in all of your health care decisions. The next time you are in the hospital and you say “Where is MY Doctor?”, remember you may have a hospitalist providing your care. Rest assured you have a qualified physician who specializes in providing optimal care during your stay.

I’m sure you’re thinking that paying for daycare for your pup is such an unnecessary expense that it probably doesn’t seem like an option at the moment. However, there are so many benefits to daycare that the investment is worth the benefits. Us as people seem to forget that our dogs and their breeds were made for a purpose. Different breeds were bred for different purposes. For example, australian shepherds were bred to endure harsh elements and herd cattle all day long. That is a constant mental and physical job which makes them some of the smartest dogs in the world. Little dachshunds were bred to chase rats and pests in the barn and the house, which again is constant mental and physical work. So when people come to me and ask, “why is my dog destroying my house?” or “why is my dog moping around?”, my answer to them is always the same. They need a job to do. Doggy daycare is that job. Just like a child going to school, your dog goes to work. Their job here is to come and socialize with other dogs. With our rotation schedule, every hour the dogs are moved to a new room. This allows their brain to be stimulated in a new environment every hour with new smells from the previous dogs as well as new surroundings. When they arrive they are broken up into groups based on size, age, temperament, and level of play. By doing this, we find where they are truly happy and will get the most out of their experience here.

Another benefit of daycare is the socialization aspect. Dogs crave being part of a pack. They love coming here and playing with their friends. Socializing is a mental and physical exercise. Remembering their furry friends, the daycare routine, and constantly monitoring the situations around them will mentally tire out your pup in ways that physical exercise never can. Of course there is physical exercise as well. The groups here are constantly monitored by someone physically in the room at all times. The dogs are never left unattended. So in short, while doggy daycare may seem like such a waste of money, the investment is completely worth the reward. You’ll notice a difference in your dog immediately. Besides there is absolutely no risk because your first day is free! Not only was The Round House by Louise Erdrich one of the best books I’ve ever read, it was also awarded The National Book Award for Fiction in 2012. The novel is set in 1988 on an Ojibwe reservation. The narrator of the story, thirteen-year-old Joe’s life is irrevocably transformed when he and his father, a tribal court judge, find his mother Geraldine covered in blood and gasoline. She’s been brutally beaten, raped, and she managed to escape before she could be set on fire by her attacker. And this is only the beginning! Geraldine is traumatized and reluctant to reveal or relive what has happened to her or to name her assailant. Joe and his father Bazil fight to find “the man whose act had nearly severed Joe’s mother’s spirit from her body.”

Louise Erdrich is one of my favorite writers, but this is not my favorite piece. Still, it was an interesting Native American bildungsroman.

that 1 in 3 Native women on reservations will be raped in their lifetime, and 86% of the perpetrators are non-Native men who are rarely prosecuted or punished because non-Natives cannot be prosecuted by tribal governments. Predators can prey with impunity. Cathy Tanguis is the only one in Between the Wines who is still actively teaching. She teaches a Native American unit in her gifted English III classes which includes works by Louise Erdrich. Cathy says that the poems and short stories by Joyce Carol Oates complement Erdrich quite nicely.

I devoured this book. From hardscrabble life on the reservation came a beautiful yet somber story of family ties, family love, and family strength.

A crime is committed and because of an issue with the jurisdiction, the criminal isn’t prosecuted. How the complex situation is resolved and hearing it told from a young boy’s prospective, made for a truly rich and interesting read.

Senior Sampler May 2016

Tropical Brown Rice INGREDIENTS

PREPARATION

3 cups uncooked instant brown rice 2½ cups reduced-sodium chicken broth pinch of salt ¼ cup unsweetened shredded coconut ½ cup fresh lime juice 1 Tbsp. packed brown sugar 16 oz can crushed pineapple, in it’s own juice ½ cup red, yellow and orange bell peppers, diced ½ cup scallions, finely chopped

1. Bring broth to a boil and add rice. Bring back to a boil, then let simmer on low, covered, for 5 min. Remove from heat, set aside. 2. Spray saute pan and brown coconut for 2-4 min., until toasted, remove, set aside. 4. Add scallions and peppers, cook for 2-3 min. until softened, set aside. 5. Drain pineapple juice into a strainer. 6. Combine all juices in a small saute pan over medium heat, brown sugar and salt. Simmer over low heat for 3-4 min. 7. Mix rice, coconut, scallions, and peppers together. Add in pineapple sauce and stir to cover rice evenly. 8. Enjoy!

Orzo, Tomato, and Feta Salad INGREDIENTS 1 package of Orzo Pasta 1 container each, red and yellow cherry tomatoes, sliced in half 1 package crumbled Feta cheese (18 oz) 1 Small Red Onion Chopped 1 can of Garbanzo Beans Juice of 1 Lemon ½ cup of Olive Oil salt and pepper to taste 3 cloves of garlic, minced

PREPARATION 1. Boil Orzo pasta until Al Dente, drain and put aside until cooled. 2. In a large bowl combine all salad ingredients mix well. 3. Mix lemon juice, garlic, olive oil and salt and pepper until combined. 4. Add Dressing to the pasta salad, serve immediately. 5. Also makes a great cold pasta salad as well.

No Regrets Chicken Salad INGREDIENTS 1 whole Rotisserie chicken, cooked and shredded ½ cup plain, non-fat Greek yogurt ¼ cup light mayonnaise 2 Tbsp. apple cider vinegar 2 Tbsp. honey ½ tsp. salt ¼ tsp. black pepper 1 cup (about 20) red or green seedless grapes, cut in half lengthwise 1 cup diced celery (about 3 stalks)

While the chicken is cooling, prepare the dressing in a large mixing bowl by whisking together the yogurt, mayonnaise, vinegar, honey, salt, and black pepper in a large mixing bowl. 2. Add the grapes, pecans, celery, and cooled chicken to the bowl, and gently toss all of the ingredients together. 3. Chill before serving.
