Scholarship Application Getting Started This form provides the step-by-step process of application for privately funded scholarships through various donors and Mt. Hood Community College Foundation.

You need only to fill out this application each term to be considered for all scholarships you apply for.

You must be taking classes toward a MHCC degree or certificate. You are not eligible if you have received a degree in the last three years unless you are in the nursing program.

Read the enclosed insert carefully before submitting your application to be sure that all the proper documentation is attached. Incomplete applications will not be accepted or considered.

Respond to the following points. Use complete sentences and answer as completely as you can. Neatness and clarity of thought will also be considered.

Step IV

Letters of Recommendation

You may need to request at least two current letters of recommendation (written within the last year). This requirement may vary for some scholarships. Be sure to check the information listed for each scholarship carefully. Letters from faculty, employers, church leaders, etc. should be written on letterhead. If you submit a personal reference, please have the person recommending you add their address and phone number. This step is also very important to the selection committee. Carefully choose the individuals recommending you.

Step V

Transcripts

If the scholarship requires proof of credit hours, include with your applications any unofficial grade transcripts from schools you have attended since high school. It is the responsibility of the student that outside transcripts are included in this application. High school seniors must submit a current HS transcript. You do not need to request grade transcripts from Mt. Hood Community College.

MHCC SCHOLARSHIP APPLICATION TIPS The following advice is given in the hope that you will successfully complete the MHCC Scholarship Application and move forward to being awarded free money to help you pay for college.

Getting Started • • • •

You must use blue or black ink ONLY. If you don’t, your application will be shredded.

You must turn in ALL pages of the application. If you don’t, your application will be shredded.

If your application is missing any of the required documents or requested information, your application will be shredded.

We do not accept late applications.

Step I • • • •

Use only blue or black ink—NO PENCIL. Answer every question.

Write legibly—if we can’t read it, we can’t award you.

The major you write down on this part of the form must be the major you have declared at MHCC, and it should also show on your MHCC transcript.

Step II • •

If you did complete the FAFSA for the term covered by the scholarship, fill in your EFC (Estimated Family Contribution).

You can access this number from your FAFSA at http://www.fafsa.ed.gov/ If you did not complete the FAFSA or if you are not eligible to submit a FAFSA, use an EFC calculator. An EFC Calculator is at http://apps.collegeboard.com/fincalc/efc_welcome.jsp.

It’s always best to type the answers to the four questions. If you must print, do so neatly and legibly.

Answer each of the four questions completely and thoroughly.

Ask a friend, teacher or parent to proofread your answers and to check your grammar and spelling.

Neatness counts!

If your answer to one of the four questions is abstract, support it with a concrete example that illustrates your point. The scholarship sponsors want to see evidence that you satisfy their criteria, not just unsupported statements.

If you are a parent or work fulltime, you may use those examples as your involvement in community activities.

You may need two current (within the last year) letters of recommendation.

Each letter should be written on college or company letterhead.

Each letter should address your academic potential, with particular, specific details that are uniquely YOU. Ask for the letters at least four weeks before the application deadline.

Carefully and thoughtfully choose who you ask for a letter of recommendation.

Transcript copies must be legible.

Sign and date application. Failure to do so will result in your application being shredded.

Turn complete application in to the Office of Financial Aid by 5pm on the deadline. Applications found in the drop box or on the fax after 5pm will not be considered.

SCHOLARSHIP DONOR INDEX FOUNDATION AMERICAN ASSOCIATION OF UNIVERSITY WOMEN, GRESHAM AREA BRANCH (QAUW) The American Association of University Women advances equity for women and girls through advocacy, education, philanthropy and research. This scholarship given by the Gresham Area Branch recognizes and honors a Mt. Hood Community College student of excellence and need in the 2nd year of a planned program of study. The student must be on track to graduate in the spring of the second year of study. The scholarship is for a student who is pursuing her first degree and holds no prior degrees.

The scholarship will be used for 2012-2013 Academic Year and the amount is $1,500.

Applicant must: • Be an MHCC registered female student in the second year who has completed 36 credit hours at the time the scholarship is in effect.

• Carry a minimum of 12 credit hours at the time the scholarship is in effect.

• Be in a planned program of study leading to a degree in a declared major. Student must be seeking her first degree.

• Submit a brief statement outlining student's goals.

• Submit two letters of recommendation supporting nomination. One must be from an instructor teaching in the planned program of the declared major.

• Maintain a 3.0 GPA in order to remain eligible for the scholarship each term.

• Scholarship funds will be distributed equally between three terms. Winter and spring term funds are contingent upon student maintaining full-time status with a minimum 3.0 GPA • Scholarship may be used for tuition, books, student/technology/college service fees (course/lab fees are not included); no cash will be disbursed.

ANGELINA AND PETE COSTANZO VOCATIONAL SCHOLARSHIP (ACOS) The Angelina and Pete Costanzo Vocational Education Scholarship fund was established at the Oregon Community Foundation by Frank and Laura Costanzo to honor Frank's parents. It was the Costanzo's hope that the spirit, ingenuity and willingness to work that was exemplified by Angelina and Pete during the early part of the 20th century could still be found in our young people today. This scholarship is intended to encourage those young people.

The scholarship will be used for 2012-2013 Academic Year and the amount is to be determined.

Applicant must: • Be a resident of Clackamas, Multnomah or Washington Counties.

• Be a high school graduate or GED recipient.

• Be enrolled in MHCC Automotive Technology program.

• Not be a candidate for a bachelor's degree.

• Have and maintain a minimum cumulative GPA of at least 2.0.

• Attend either full or part time.

• Have the potential to successfully complete the program and/or be making satisfactory progress.

• Have personal qualities: strong work ethic; self-starter; follows through; responsible/dependable.

• Be recommended by his or her instructor.

• Scholarship funds will be distributed over three terms.

• Scholarship funds may be used for any school related expenses and tools.

FOUNDATION AUTOMOTIVE TECHNOLOGY STUDENT SCHOLARSHIP FUND FOR FORD ASSET SCHOLARSHIP (DAUT) The Automotive Technology Student Scholarship Fund provides assistance to students enrolled in the Ford ASSET program at Mt. Hood Community College.

The fund awards scholarships to students to help defray their tuition costs and other activities and technology fees (no course/lab fees are included).

Participating students must enroll or be enrolled in classes and make consistent progress toward their degrees or certificates in automotive technology.

The scholarship will be used for Fall Term 2012 and Spring Term 2013 and the amount is Tuition and activities and technology fees (no course/lab fees are included) for two terms.

Applicant must: • Be registered (or be committed to registering) as a second-year student at MHCC carrying a minimum of 12 credit hours per term at the time the scholarship is in effect.

• Be committed to pursuing a degree or certificate in automotive technology.

• Demonstrate financial need.

• Show ability to succeed in the field of automotive technology.

• Have a cumulative GPA of at least 2.0.

• Submit one letter of recommendation from an Automotive Technology faculty member supporting nomination.

• Scholarship funds will be distributed equally between two terms. Spring term funds are contingent upon student maintaining full-time status with a minimum 2.0 GPA • Scholarship shall be used for tuition and student/technology/college service fees (course/lab fees are not included); no cash will be disbursed.

FOUNDATION BOB WILSON SCHOLARSHIP The Bob Wilson Scholarship recognizes and honors an outstanding second-year MHCC student studying physics, engineering or math. One scholarship will be awarded to a physics major, one scholarship will be awarded to an engineering major and one scholarship will be awarded to a mathematics major.

The scholarship will be used for 2012-2013 Academic Year and the amount is One Academic Year Tuition.

Applicant must: • Be a full-time student at MHCC carrying a minimum of 12 credits hours each term the scholarship is in effect.

• Have completed a minimum of 36 credit hours by the time the scholarship is in effect.

• Have a 3.0 cumulative GPA from previous high school or college courses (submit transcripts) in order to apply.

• Submit two letters of recommendation supporting nomination. One must be from a program instructor.

• Scholarship funds will be distributed equally among the three terms. Must maintain a satisfactory 3.0 GPA each term the scholarship remains in effect. Student will forfeit all or remaining scholarship if student withdraws from MHCC. • Financial need will be a consideration.

• Scholarship may be used for tuition, books, student/technology/college service fees (course/lab fees are not included); no cash will be disbursed.

FOUNDATION BOEING COMPANY MANUFACTURING TECH PREP ASSOCIATE DEGREE INTERNSHIP SCHOLARSHIP (QBOE) The Boeing Manufacturing Tech Prep Associate Degree Internship scholarship recognizes and honors a Mt. Hood Community College student of excellence entering or returning to MHCC’s Machine Tool Technology or Mechanical Engineering Technology program.

The scholarship will be used for Fall Term 2012 and the amount is $500.

Applicant must: • Be a Boeing Manufacturing Tech Prep Associate Degree Internship student who is entering or returning as a full-time student in MHCC’s Machine Tool Technology or Mechanical Engineering Technology programs.

• Maintain a 2.0 GPA in order to remain eligible for a scholarship each term.

• Any student withdrawing from full-time status in the Machine Tool Technology or Mechanical Eng ineering Technology programs will forfeit all or remaining scholarship monies, which will be returned to the fund.

• Scholarship may be used for tuition, books, student/technology/college service fees (course/lab fees are not included); no cash will be disbursed.

FOUNDATION BOEING COMPANY SCHOLARSHIP FOR STUDENTS IN THE MHCC MACHINE TOOL TECHNOLOGY OR MECHANICAL ENGINEERING OR ENGINEERING TRANSFER PROGRAM (QBMT) The Boeing Company Scholarship recognizes and honors a Mt. Hood Community College student of excellence and need in the Machine Tool Technology, Electrical Engineering, Computer Science, or Mechanical Engineering Technology or Engineering Transfer program.

The scholarship will be used for Fall Term 2012 and the amount is $1,000.

Applicant must: • Be a registered full-time MHCC student in the Machine Tool Technology, Electrical Engineering, Computer Science or Mechanical Engineering Technology/ Engineering Transfer program during the term the scholarship is in effect.

• Have authorization to work in the United States on a full-time basis for other than training purposes (student visas or temporary visas do not qualify).

• Demonstrate financial need.

• Submit a brief statement outlining student's goals.

• Submit two letters of recommendation supporting nomination. One must be from an instructor verifying excellent leadership and communication skills.

• Have an overall minimum GPA of 2.5.

• Be of high scholarship merit and potential as defined by institution.

• Boeing scholars are encouraged to apply for Boeing internships if positions are available.

• Have a strong interest in the aerospace industry.

• Possess leadership and communication skills.

• Scholarship may be used for tuition, books, student/technology/college service fees (course/lab fees are not included); no cash will be disbursed.

FOUNDATION BURLINGAME FAMILY SCHOLARSHIP (QBLG) The Burlingame Family Scholarship recognizes and honors a student of excellence and need.

The scholarship will be used for 2012-2013 Academic Year and the amount is $1,000.

Applicant must: • Be a registered MHCC student carrying a minimum of 12 credit hours each term the scholarship is in effect.

• Submit a brief statement outlining student's goals.

• Submit two letters of recommendation supporting nomination.

• Maintain a 2.5 GPA in order to remain eligible for the scholarship each term.

• Demonstrate financial need.

• Scholarship shall be disbursed equally between fall, winter and spring terms.

• Scholarship may be used for tuition, books, student/technology/college service fees (course/lab fees are not included); no cash will be disbursed.

FOUNDATION COMPLETE YOUR DREAM SCHOLARSHIP (DCOM) The scholarship will be used for 2012-2013 Academic Year and the amount is One Academic Year Tuition.

Applicant must: • Be a full-time student (12 credit hours minimum).

• Have a minimum GPA of 2.75.

• Have completed a minimum of 36 credit hours in a two-year degree-granting program, or 12 credit hours in a certificate program.

• Submit two letters of recommendation supporting academic and occupational promise in the selected program area.

• Scholarship funds will be divided equally between three terms. Winter and spring term funds are contingent upon maintaining satisfactory academic progress.

• Scholarship may be used for tuition only. Fees are not included. No cash will be disbursed.

FOUNDATION DAVID H. SPOONER MEMORIAL SCHOLARSHIP (QSPO) The David Spooner Memorial Scholarship for Visual and Performing Arts recognizes and honors a returning student in visual and performing arts studies with a scholarship to continue studies.

The scholarship will be used for Fall Term 2012 and the amount is One Term Tuition.

Applicant must: • Be a student enrolled in Visual and Performing Arts studies, having completed at least one term at MHCC in the program and have a minimum GPA of 2.5 at time of application.

• Demonstrate financial need.

• Submit two letters of recommendation supporting nomination, one of which must be from an MHCC Visual/Performing Arts faculty member.

• Scholarship may be used for tuition, books, student/technology/college service fees (course/lab fees are not included); no cash will be disbursed.

FOUNDATION DONNA HAND MEMORIAL SCHOLARSHIP (QHDD) The Donna Hand Memorial Scholarship recognizes and honors an MHCC Visual Arts student of excellence and need.

The scholarship will be used for Fall Term 2012 and Winter Term 2013 and the amount is $1,000.

Applicant must: • Be a registered MHCC full or part-time student carrying a minimum of six credit hours during each term the scholarship is in effect. Student must have a declared major in the arts.

• Submit a brief statement outlining student's goals.

• Submit two letters of recommendation supporting nomination.

• Maintain a 2.5 GPA in order to remain eligible for the scholarship each term.

• Demonstrate financial need.

• Scholarship may be used for tuition, books, student/technology/college service fees (course/lab fees are not included); no cash will be disbursed.

FOUNDATION ENGLISH AS A NON-NATIVE LANGUAGE (ENL) SCHOLARSHIP (DENL) The scholarship will be used for Fall Term 2012 and the amount is $500.

Applicant must: • Fill out the application.

• Be at least a half-time student.

FOUNDATION KAISER PERMANENTE LATINO STAFF ASSOCIATION SCHOLARSHIP (QKPL) The Kaiser Permanente Latino Staff Association Scholarship recognizes and honors a Latina student who has completed the Transitions program and intends to pursue a career in healthcare.

The scholarship will be used for 2012-2013 Academic Year and the amount is $1,000.

Applicant must: • Have completed high school or have obtained a GED. • Be eligible to enroll in college-level courses at a level higher than Reading 90, Writing 90, or ENL 201.

• Be enrolled in or have completed the Transitions program.

• Complete at least six credits per term while receiving the scholarship.

FOUNDATION KERRI FOLAND SCHOLARSHIP (QFOL) This scholarship is in memory of Kerri Foland, a wonderful wife, mother, friend and physical therapist assistant. Hope is that this scholarship will help Kerri's caring legacy live on by giving future physical therapist assistants opportunities that otherwise may not be available. The Kerri Foland Scholarship recognizes and honors an MHCC Physical Therapy Assistant student of excellence and need.

The scholarship will be used for Fall Term 2012 and Winter Term 2013 and the amount is $1,000.

Applicant must: • Be a registered MHCC Physical Therapy Assistant student carrying a minimum of 12 credit hours during each term the scholraship is in effect.

• Achieve a minimum grade point average of 3.0.

• Submit two letters of recommendation from instructors, counselors, or community members.

• Recipients of this scholarship are limited to a maximum of eleven credit hours. ENROLLMENT OF MORE THAN ELEVEN CREDIT HOURS WILL RESULT IN THE FORFEITURE OF THE SCHOLARSHIP. Hobby and general interest classes are not supported by this scholarship.

The scholarship will be used for Fall Term 2012 and Spring Term 2013 and the amount is Tuition and activities and technology fees (no course/lab fees are included) for Two Terms and $1,000 for tool expenses.

Applicant must: • Be a full-time, second-year student in MHCC’s Ford ASSET program at the time the scholarship is in effect.

Applicant must: • Be a full-time, second-year Nursing program student at MHCC at the time the scholarship is in effect.

• Have accumulated enough credit hours by the time the scholarship is in effect to demonstrate that the student will graduate from the Nursing program with the required 103 credits at the end of two years.

The scholarship will be used for Fall Term 2012 and the amount is $1,500.

Applicant must: • Be a registered MHCC student with a minimum of 35 completed credit hours toward an associate degree in Funeral Service Education at the time the scholarship is in effect.

• Submit two letters of recommendation supporting the individual's desire and abilities in funeral service.

• Maintain a 3.0 GPA in order to remain eligible for the scholarship each term.

• Submit a typed, 300-word essay on the topic of "Why I want to be a funeral director."

OREGON TRAIL CHAPTER DAR SCHOLARSHIP (LDAR) You do not need to be affiliated with the Daughters of the American Revolution to apply for this scholarship. The Oregon Trail Chapter of the Daughters of the American Revolution is sponsoring a scholarship to recognize an East County resident who has completed one year at Mt. Hood Community College and is enrolled as a second-year student.

The scholarship will be used for Fall Term 2012 and the amount is One Term Tuition for 12 credits.

Applicant must: • Be a second-year student who is pursuing a degree in the following: Certificate Program-Early Childhood Education, Allied Health, Transfer Program-Education or History.

• Have a minimum GPA of 2.5.

• Be a citizen of the United States.

• Submit two letters of recommendation from people other than family members.

• Be a resident of East County and have been residing in East County for at least one year.

FOUNDATION PACIFICORP SCHOLARSHIP FUND FOR FISHERIES TECHNOLOGY (QPCF) The PacifiCorp Scholarship Fund for Fisheries Technology awards a scholarship to a student of excellence who is pursuing a fisheries education at MHCC. Provided the Fund has adequate assets, a scholarship will be awarded to the recipient for a second year, if the student continues to meet the scholarship restrictions and conditions.

The scholarship will be used for 2012-2013 Academic Year and the amount is to be determined.

Applicant must: • Be a member of the Yakama Nation, a Yakama descendant or have other tribal affiliation.

• Have a declared major in Fisheries Technology.

• Maintain a 2.5 GPA in order to remain eligible for the scholarship each term. A student is allowed a one-term probation if the GPA is less than 2.5 but greater than 2.0.

• Carry a minimum of 12 (full-time) credit hours at MHCC during each term the scholarship is in effect.

• Submit a brief statement outlining student’s goals.

• Submit two letters of recommendation supporting nomination; one letter must be from a Yakama Nation tribal council member.

• Scholarship funds will be distributed equally between terms. Each term allocated funds will be placed in student account and will pay for tuition and activities and technology fees (no course/lab fees are included) first. Balance of funds will be distributed to student by check.

• Scholarship funds must be used for tuition, student/technology/college service fees (course/lab fees are not included), books, and/or required materials for the Fisheries Technology program. FOUNDATION PAUL AND CAROL KREIDER SCHOLARSHIP FOR PERFORMING AND VISUAL ARTS (DKRE) The purpose of the Paul and Carol Kreider Scholarship Fund for the Performing Arts shall be to attract some of the region’s most talented young artists to begin their training at MHCC. The scholarship will be used for Fall Term 2012 and the amount is One Term Tuition.

Applicant must: • Be a full-time student at MHCC with a minimum of 12 credit hours per term.

• Maintain a minimum 2.5 GPA during the term covered by the scholarship.

The scholarship will be used for Fall Term 2012 and the amount is Tuition up to 12 credits.

Applicant must: • Be residents of Oregon or the reciprocal areas of Washington or Idaho.

• Attend MHCC and enroll for at least three or more credit hours. Awards may cover up to and including 12 credit hours of in-district tuition costs.

• Submit written recommendations from two MHCC staff members indicating the student's academic potential and progress.

• A maximum of three recognition awards may be received if satisfactory academic progress is maintained.

The student will receive the cost of tuition and activities and technology fees (no course/lab fees are included) for three terms for the MHCC Cosmetology program plus a one time award of $300 for the cost of the supply kit.

The scholarship will be used for 2012-2013 Academic Year and the amount is One Academic Year Tuition plus $300 for the supply kit.

Applicant must: • Be a registered second-year MHCC Cosmetology program student carrying a minimum of 12 credit hours during each term the scholarship is in effect.

FOUNDATION SCHOLARSHIP FOR PART-TIME STUDENTS (DPAR/QPAR) The scholarship will be used for Fall Term 2012 and the amount is Tuition and student/technology/college service fees (course/lab fees are not included) to 11 credit hours plus books and supplies not to exceed $75.

Applicant must: • Achieve a minimum grade point average of 2.0.

• Submit two letters of recommendation from instructors, counselors or community members.

• The MHCC Foundation Scholarship Committee will select scholarship recipients. The committee may recommend more than one recipient, or may recommend that no award be made in a particular term, if no meritorious applications are received.

• Selection is based on appraisal of the contents of the application, educational plans, potential for development, letters of recommendation and financial need.

• Recipients of this scholarship are limited to a maximum of 11 credit hours. Hobby and general interest classes are not supported by this scholarship.

• Enrollment in more than 11 credit hours will result in the forfeiture of the scholarship. Scholarship awards are contingent upon funding.

FOUNDATION SCI OREGON FUNERAL SERVICES INC. (QOFS) The SCI Oregon Funeral Services Inc. Scholarship recognizes and honors an MHCC Funeral Service Education program student of excellence The scholarship will be used for 2012-2013 Academic Year and the amount is $1,000.

Applicant must: • Be a registered MHCC student enrolled in the Funeral Service Education program during each term the scholarship is in effect.

• Be enrolling from one of the following states: Oregon or Washington.

• Demonstrate financial need.

• Submit one letter of recommendation from school instructor or counselor (or someone employed within the funeral services industry) supporting academic and occupational promise.

• Maintain a 3.0 GPA and must fully adhere to the Funeral Service Education Student handbook in order to remain eligible for the scholarship each term. SCI will contact program director each term to confirm adherence to the handbook.

• Scholarship funds will be distributed equally among three terms.

• Scholarship may be used for tuition, books, student/technology/college service fees (course/lab fees are not included); no cash will be disbursed.

FOUNDATION SHIRLEY LABINOWICZ SCHOLARSHIP (DLAB) The Shirley Labinowicz Scholarship recognizes and supports students having outstanding academic and/or leadership potential as they pursue their goals of becoming teachers, counselors, or psychologists.

The scholarship will be used for 2012-2013 Academic Year and the amount is $1,500.

Applicant must: • Be a part-time student at MHCC carrying six credit hours each term the scholarship is in effect.

• Have Associate of Arts Oregon Transfer (AAOT) degree as declared major in Education, Counseling or Psychology.

• Have a 3.0 cumulative GPA from previous high school or college courses (submit transcripts).

• Submit two letters of recommendation supporting nomination. One letter must be from MHCC instructor in applicant’s field of interest.

• Maintain satisfactory 3.0 GPA each term the scholarship is in effect. Student will forfeit all or remaining scholarship monies if the student withdraws from MHCC classes with fewer than six credit hours and/or does not meet the 3.0 GPA requirement.

• Write a 1-2 page essay outlining reasons for becoming a teacher, counselor or psychologist. Include a description of paid employment and/or volunteer experiences relevant to goal.

• Preference will be given, but not limited, to Transitions graduates and students demonstrating financial need.

• Recipients of the scholarship are eligible to reapply for a second year of support.

• Scholarship funds will be distributed equally among the three terms.

• Scholarship shall be used for tuition and student/technology/college service fees (course/lab fees are not included); no cash will be disbursed.

FOUNDATION TICHY ESL TO ENL SCHOLARSHIP (QENG) The Tichy ESL to ENL Scholarship provides financial assistance to ESL students entering into ENL94 classes or above.

The scholarship will be used for Fall Term 2012 and the amount is $500.

Applicant must: • Demonstrate financial need.

• Take at least one ENL class at the time the scholarship is in effect.

• Submit a brief essay describing student’s goals.

• Submit one letter of recommendation from an ESL/ENL instructor attesting to the student’s potential to be successful in ENL94 classes.

• Take the college CPT or LOEP exam and submit the test scores.

• Any student withdrawing from classes with fewer than five credit hours remaining will forfeit all or remaining scholarship monies which will be returned to this scholarship account.

• Scholarship may be used for tuition, books, supplies, student/technology/college service fees (course/lab fees are not included); no cash will be disbursed.

FOUNDATION TICHY NURSING SCHOLARSHIP (QTIC) The Tichy Nursing Scholarship recognizes and honors a Nursing student who has completed his or her first year in the Nursing program, is enrolled for a second year in the Nursing program and who has financial need.

The scholarship will be used for Fall Term 2012 and the amount is $500.

Applicant must: • Be a registered MHCC student carrying a minimum of 12 credit hours during each term the scholarship is in effect.

• Submit a brief statement of goals.

• Maintain a 2.5 GPA in order to remain eligible for the scholarship each term.

• Submit two letters of recommendation supporting nomination.

• The application will be graded on two areas: Financial need will account for 50 percent of the criteria for awarding the scholarship. Fifty percent of the criteria will be based on the likelihood of the student to successfully completing the Nursing program. Letters from the applicant’s instructor will be used in this determination.

FOUNDATION TRANSICIONES GRADUATE SCHOLARSHIP (QTCG) The Transiciones Graduate Scholarship recognizes and honors Transiciones Graduates with scholarships to allow them the opportunity to pursue training at MHCC. The scholarship will be used for Spring Term 2012 and the amount is $500.

Applicant must: • Be a graduate of the Transiciones program.

• Have a 2.5 GPA (not required if no GPA available).

• Enroll or have enrolled in Transitions program classes the term following completion of ENL classes.

• Carry a minimum of five credit hours during each term the scholarship is in effect and maintain a 2.5 cumulative GPA. • Submit two written recommendations for funding. One recommendation must be from a staff member of the Transitions or Transiciones program, which includes verification of the above criteria.The second recommendation may be from any other professional, such as an instructor, counselor, clergy or supervisor who has knowledge of the applicant’s skills and character.

• Have not received a Transitions or Transiciones scholarship for the 2012-2013 Academic Year.

Applicant must: • Be an enrolled and active TRiO participant for at least three consecutive terms to apply for this scholarship.

• Submit a brief statement outlining student's goals and including a description of current financial barriers.

• Be a registered MHCC full-time student carrying a minimum of 12 credit hours to apply for this scholarship.

• Submit two letters of recommendation supporting nomination. One letter must be from TRiO adviser indicating student has participated in TRiO three consecutive terms, and one from instructor.

• Have a 2.75 cumulative GPA in order to apply for scholarship.

• Maintain a 2.75 GPA in order to remain eligible for the scholarship award.

• Scholarship may be used for tuition, books, supplies, student/technology/college service fees (course/lab fees are not included); No cash will be disbursed.

FOUNDATION WESSINGER FAMILY SCHOLARSHIP (QWGF) The Wessinger Family Scholarship recognizes and honors a student of excellence and need.

The scholarship will be used for Fall Term 2012 and the amount is One Term Tuition.

Applicant must: • Be a registered MHCC full- or part-time student with a declared major during the term the scholarship is in effect.

• Submit a brief statement outlining student's goals.

• Submit two letters of recommendation supporting nomination.

• Maintain a 2.5 GPA in order to remain eligible for the scholarship each term.

• Demonstrate financial need.

FOUNDATION WINTZ FAMILY FOUNDATION SCHOLARSHIP FOR TRANSICIONES GRADUATES (QWCG) The Wintz Family Foundation Scholarship recognizes members of the Transiciones program with scholarships to allow them the opportunity to pursue training at MHCC. The scholarship will be used for Fall Term 2012 and Winter Term 2013 and the amount is $1,000.

Applicant must: • Be an MHCC student and a graduate of the Transiciones program.

• Demonstrate financial need.

• Submit a written recommendation for funding from the director of the MHCC Transiciones program or from a member of the Transiciones staff team stating that student is a Transiciones Graduate.

Applicant must: • Be an MHCC student and a graduate of the Transitions program.

• Demonstrate financial need.

• Submit a written recommendation for funding from the director of the MHCC Transitions program or from a member of the Transitions staff team stating that student is a Transitions Graduate.

ZELMA GORDON NURSING SCHOLARSHIP (AZEL) The scholarship will be used for 2012-2013 Academic Year and the amount is to be determined.

Applicant must: • Be enrolled in the Associate Degree Nursing program at MHCC. • Be a full-time student (minimum 12 credit hours).

• First-year students must have successfully completed the first quarter of the Nursing program, but not more than three quarters.

• Have a cumulative GPA of 2.65 or better.

• Demonstrate a significant financial need.

• Preference will be given to: a) First-year students; b) Students who have been selected for the immediately preceding year.

• Submit two letters of recommendation.

• The scholarship committee may interview scholarship applicants.

• The award considers all required texts and printed materials, uniforms, and equipment and fees. The award does not consider transportation, room and board, and recommended texts or printed materials.
