Challenges facing Research Ethics Committees in the Middle East Mrs. Corinne Aad Naba, BSN MHA, Global Operations Manager, ClinServ International-ClinGroup Holding.
The Middle East has witnessed a tremendous increase in the number of clinical trials in the past ten years. On the other hand, Research Ethics Committees (RECs) aren’t keeping pace with the increase in clinical trials. In response to increased research being performed, many RECs have been established but their actual functioning in regards to protecting the rights and welfare of participants remains questionable. The importance of the role of the RECs comes from the concern with the potential for exploitation in the area of
international research, considering the presence of extreme poverty and lack of access to health care among a high proportion of the populations in the Arab countries. The role of the Research Ethics Committee as an independent actor protecting human research participants requires that the REC: be properly constituted, has competent members, has adequate resources and regulatory framework. But is it the case in the Middle East? What are the rules? The purpose of Ethics Committees www.middleeasthospital.com

is to review research and establish if the rights and welfare of human participants involved in research are adequately protected. The REC has authority to approve, require modification, or disapprove all research activities. Consequently it should guarantee the following rules: Respect for persons: persons must be treated as autonomous agents. Informed consent must be obtained and research subjects ought to be respected and not used as means. Special protection should be entitled to vulnerable groups including children, illiterate, poor, pregnant women, mentally disabled and prisoners. December 2015 | 9

Specialist article Beneficence: maximize possible benefits and minimize potential harms. The risk-benefit ratio of the research should be favorable and it should have a social value and scientific validity. Justice: Persons ought to be treated fairly, implying fair distribution of benefits and burdens. This has bearing on the selection of subjects. Therefore, the RECs should ensure the following when approving a clinical research study: social and scientific value, scientific validity, fair subject selection, favorable risk-benefit ration, informed consent, respect for enrolled subjects, independent review and community perspective. What is the status? In 2008, the WHO conducted a retrospective data analysis to evaluate the ethical practices for health research in the eastern Mediterranean region. They concluded that Ethics Review and Informed Consent are imperfectly realized in the Middle East. They investigated furthermore the adequacy of informed consent documents and they found several insufficiencies in the following: duration of subject’s participation, description of risks, disclosure alternatives, availability of medical care for research injury, subjects’ rights or complaints, voluntariness of participation and ability to withdraw. Regarding member composition, many RECs lack adequate diversity, more than 80% of members are physicians and other membership categories (scientist/non-physician, nurses, ethicist, and community member) are not well represented. www.middleeasthospital.com

Furthermore, the majority of REC members lack proper training in research ethics. Therefore, more formal training is needed. Training topics should include: basic ethical principles, role of REC (authority, mandate and responsibilities), informed consent with vulnerable populations, ethics review conduct, applicable laws and regulations, evaluating informed consent processes, relevance of research to local health needs and evaluation of scientific validity and value.

What are the problems?
Some protocols are being conducted under a US IND, and therefore US regulations and requirements are being applied. The 1572 Form that the principal investigator signs for such a study includes a commitment (Section 9 of 1572) that “… (PI) will ensure that an IRB that complies with 21 CFR Part 56 will be responsible for the initial and continuing review and approval of the clinical investigation.”
The challenges to proper functioning of RECS in the Middle East can be summarized in the following: lack of national regulations, inadequate member diversity, membership competence, human and capital
21 CFR Part 56 is United States regulation, and as such, it is likely that the ethical review committee (IRB/EC/ REB) that approved the protocol in the MENA cannot fully comply with the stated US regulations. This being the www.middleeasthospital.com
case, the FDA may grant a waiver for investigative sites that are compliant with ICH GCP principles, or equivalent measures without being fully 21 CFR Part 56 compliant. The problem is that not all the sponsors are asking for the waivers and the findings come with the inspections and audits. In the absence of an accreditation process, evaluating the performance of an REC remains a challenging task. In the meanwhile, a self-assessment mechanism would provide RECs a way to review their policies and processes against recognize international standards. Or to create their policies and procedures as some are acting without having any written procedures. Also there’s a lack in tracking and reporting as not all the EC have minutes recorded for their meetings. December 2015 | 11

Specialist article How to enhance ethical research? An important infrastructure element of ethics capacity at the national level is the establishment of National Ethics Committees (NECs). Such NECs could focus on providing guidance and advice to policy makers, developing normative instruments in ethics, establishing training programs in ethics, and reviewing research protocols, either those to be conducted in the country or only those that are international in scope. Enhancing ethical research can also be done by: establishing national standards and codes, thus improving the awareness and adherence to them, implementing reliable institutional policies and procedures, transparent processes for approval and monitoring and applying straightforward mechanisms for handling complaints and investigations.
Further efforts are also needed to ensure compliance with existing laws in addition to the development of national regulations in countries that lack such regulations. These challenges affirm the widespread concern that despite the existence of international research ethics standards, guidelines need to be developed that are sensitive and relevant to the local context of countries in the Region.
Ethics is also about integrity of researchers which involves commitment to contributing to knowledge, pursuit and protection of truth, reliance on appropriate research methods and honesty.

Middle East at MEDICA The Anti-Doping Laboratory Qatar (ADLQ) occupied the majority of the space in the Qatar Pavilion alongside co-exhibitors Weill Cornell Medical College-Qatar, Aspetar Qatar (orthopaedic and sports medicine hospital), and Sidra Medical and Research Center. The new anti-doping lab is officially opening on 10th December
and was showcased at MEDCIA to introduce ADLQ to the market and to meet prospective partners for possible future research collaborations. ADLQ is aiming for full WADA accreditation by 2015. Saudi Food and Drug Authority The Saudi Food and Drug Authority (SFDA) took a stand at MEDICA this year to raise awareness of the new regulations governing the importing of medical devices to the Kingdom. After serious concerns over the quality of medical imports into what was a previously unregulated market, manufacturers seeking access to the booming healthcare market in Saudi Arabia are now required to meet strict criteria. Paper Medical Records Vs. Electronic Medical Records A paper patient record is identified by name, some kind of medical record number, and other identifiers that make it easier to find in the filing system. An EMR provides distinct identifying information for each patient, and identifiers to locate the digital record among any number of other records. Paper typically contains demographic and insurance information, along with a list of medical problems, medications, and allergies. These must be readily updated and should stay current and accurate. An EMR maintains this information, and shares any updated
information wherever it is needed. The big difference is the workflow — when updated insurance information is provided, that information is automatically passed to billing, reducing the need for duplicate data entry. In addition, clinical information, such as problem lists and medication lists, are readily updated without duplicate data entry, so that changing medications within the EMR application automatically updates the patient’s medication list everywhere. A paper medical record contains office or progress notes in chronological sequence. These are “browsed” by literally flipping through pages, until the desired entry is located. This can take a LOT of time.

Progress notes in a traditional paper record might be produced by dictation/transcription, free handwriting (hopefully, neat handwriting), or form completion. An EMR stores progress notes and provides quick access by date of visit, provider, or other search criteria and the ability to browse by diagnosis and prescription. A fully-functional EMR automatically creates the progress notes as the visit is produced. Laboratory and radiology reports, as well as correspondence, are filed in more or less chronological order. Access to specific entries is no more efficient than it is with progress notes. An EMR stores reports in any number of ways to provide rapid access and quick reference, such as scanned images, direct lab result posting, even

Specialist article on-line lab information applications. Using common demographic and identifying information, access to specific lab results or other patient reports is highly efficient and useable. If a medical record is filed correctly in the medical records system, a staff member must go to the stacks of detail and, using some quick identifier code, locate the correct last name. The first name is located and confirmed, and then the chart is “pulled”, but not before a placeholder is inserted, in order to 1) make re-filing easier and 2) record where the detail is headed. The issues surrounding finding a chart that is “out” somewhere, or has been incorrectly filed, are easily imagined. After all there is only one access, what if two people need it at the same time? An electronic record is never lost, out, or misfiled. It is always exactly where it should be, even if you aren’t. That is to say that an electronic record may be accessed from any point in a healthcare facility that has access to medical records. Plus, it can be accessed by multiple people if needed. In a paper-based system, a healthcare provider typically writes a paper prescription for the patient to take to a pharmacy. There are often one (or more) added steps, 1. Health News - Eastwood Park Training Topping out of new multi million pound training facility A landmark stage has been reached with the construction of Eastwood Park’s new multi million pound training facility in the UK. To mark progress on reaching the highest point of the building and also the half way stage of the building project, a traditional “topping out” ceremony recently took place in glorious sunshine. A symbolic yew branch was attached to the tallest point of the building to promote good luck. Eastwood Park, with its headquarters in Falfield, South Gloucestershire, is renowned worldwide for its delivery of specialist hospital support services training in decontamination, medical equipment, estates and facilities management. When complete the new 3,000 m2 centre will simulate a hospital environment allowing health professionals to familiarise themselves with new medical equipment and technologies as well as benefiting from practical training. The centre will be the only one of its kind in the world.
Many of the individual specialist training buildings that have stood since the centre was operated by the Department of Health as the NHS training centre for hospital engineering during the 60s through to late 90s, will soon be demolished having recreated brand new facilities brought together in one unique, purpose built building. John Thatcher, Eastwood Park’s Chief Executive Officer said: “We are delighted to start to see the building taking shape and would like to take this opportunity to thank everyone who has been involved. We are actively planning the fit out in some detail and are excited about the next stage.” Members of the Eastwood Park Board and representatives from those companies working with construction company Pollard Ltd, and architects APG Architecture, attended. Eastwood Park Training Study for your degree in 2016 at Eastwood Park Three new awards for healthcare engineers and estates teams Eastwood Park Training in the UK is delighted to announce a formal partnership with Staffordshire University, marking the launch of two new Foundation Degrees and a BSc (Hons) Degree in 2016. Specifically for the healthcare engineering and estates sector there is a new and unique Foundation Degree in Hospital Engineering only available from Eastwood Park. It complements the Foundation Degree in Medical Equipment Technologies that has been designed for EBME engineers. Both are two year work-based learning programmes that allow learners to continue working while studying. Learners can progress to an optional third year to attain a BSc in the Management of Healthcare Engineering Technologies & Facilities.
Potential learners can start registering for the programme now, although the full details will be available later in the autumn, with the first cohort starting in April 2016. GCC News HMC First Middle East Body To Receive ACGME-I Accreditation Hamad Medical Corporation (HMC) has become the first hospital system in the region and the whole Middle East to achieve the institutional accreditation from the Accreditation Council for Graduate Medical Education – International (ACGME-I). This makes Qatar the second country outside of the United States (US) to have the prestigious accreditation conferred upon it which indicates that it meets the highest and most rigorous standards for physicians’ speciality training. The accreditation is effective from 1 July, 2012 so that medical residents specializing in the current year will be educated under this accreditation. The ACGME is the body responsible for accrediting the majority of graduate medical training programs, such as internships, residencies and fellowships for physicians, in the US and the ACGME-I is the international arm of the ACGME. Under the ACGME-I structure, doctors in training programs (residents) undergo structured specialist training supervised by dedicated senior clinicians after graduating from medical school. They are regularly assessed on their medical knowledge, clinical skills, professionalism, interpersonal and communication skills. This ensures the aspiring specialist doctors are well-trained and equipped with the necessary skills and knowledge to provide excellent care by the time they finish their training which spans for 4-6 years. “I am immensely proud of our team for the collaborative effort in achieving this accreditation,” said
