Unleashed Hounds and Hops Waiver and Release Date: __________ I hereby acknowledge that I have voluntarily applied to participate and use the dog park and taproom (“UHH”) operated by Unleashed Hounds and Hops LLC (“Company”). I understand that the act of unleashing my dog(s) and being physically present within the Park necessarily involves a risk of injury to me, persons that accompany me and dogs that accompany me. I understand that these risks are entirely my responsibility and I knowingly assume this responsibility. I hereby release, waive, discharge and covenant not to sue the Company, and its respective members, managers, employees, or agents from any and all claims, liabilities, losses or expenses, including reasonable attorney’s fees, and disbursements whatsoever arising out of or related to any loss, damage or injury, including death that may be sustained by me, or to any property belonging to me, whether caused by my negligence, the Company’s negligence or otherwise, occurring while or arising out of my use of UHH. When this Waiver and Release refers to “My dog”, it includes dog(s) accompanying me even if I am not the legal owner of the dog. I am fully responsible for my dog while using UHH. I understand that the registration process is established to determine that dogs in UHH are healthy, but does not guarantee that docgs in UHH are healthy. There is a risk of serious injury, including death, to my dog, myself, other persons, or other animals. I understand that dogs, may act or react unpredictably at times based on circumstances, and these actions cannot always be foreseen. I agree to assume the risk of any injury to me, my dog(s), any individuals (including children) that accompany me to UHH. I understand and assume the risk that all dogs in UHH may not have been vaccinated for Bordetella, distemper, parvovirus, or rabies. Additional risks include, but are not limited to: the inexperience of a dog owner/handler; the inability to predict a dog’s reaction to stimulus present at UHH; the possibility of dog fights, dog bites; the possibility of dog theft, dog escapes, slip and falls, premises defects, equipment failure, failure in supervision, and other circumstances inherent to activities that take place at UHH. I knowingly and voluntarily accept responsibility for all these risks in exchange for use of UHH. I hereby certify that my dog has been tested, vaccinated/treated of the illnesses listed above and determined in good health by a qualified professional. I understand that food and beverage items that may be poisonous or unhealthy for dogs will be present at UHH and assume the risk of My Dog ingesting such items. I understand and agree that no agents or employees of the Company are required supervise UHH at any time and I expressly assume responsibility for all risks associated with UHH. I hereby represent and warrant that I am not aware of any incidents in which My Dog(s) has/have shown aggressive, fighting, or biting behavior towards people or other animals. I have carefully read this waiver and release and understand its contents. I represent that I am at least 18 years of age, or as a legal guardian agreeing on child’s behalf, and am competent to sign this Release and Waiver. Name: ____________________________
