If You Get Sick, Call Us First!
As your Millennium Physician Group Care Team, it’s so very important to us that you always receive the best medical care and as quickly as possible. We know you can become ill when you don't have an appointment or when the office is closed, when you're traveling or when it’s just plain inconvenient. However, we're here to make things easier. If you need medical assistance, Call Us First at your medical office. Even if it's after hours, still call us, and the on-call healthcare provider will speak with you, evaluate your needs and provide appropriate instructions. We have the ability to not only offer medical advice but also schedule a same-day appointment, offer you a telehealth visit, direct you to the nearest walk-in medical center, or even schedule a home visit.
In the Event of an Emergency: The emergency room should be avoided except in true emergency situations. If you have a serious injury or illness that requires immediate assistance, we encourage you to go directly to your nearest emergency room or dial 911. Upon arrival to the emergency room or hospital, please ask for a Millennium doctor or notify the staff that you are a Millennium patient. We have hospitalist-doctors who are part of our group and will communicate directly with your Millennium primary-care physician. They have access to your medical records and will provide the same quality care you know and trust. Your physician will provide you
with a Millennium ID Card to present to the hospital as a reminder that Millennium physicians will monitor your care during your stay. After your discharge, our care management nurses will continue to follow up with you.

What is a Wellness Visit? Who's eligible for The wellness visit is a preventive Wellness Visit? screening service offered annually to Medicare patients. These patients are eligible for two different types of wellness visits, a “Welcome to Medicare Visit” and an “Annual Wellness Visit”. These are opportunities to uncover potential critical health issues and tailor a preventive plan to begin early intervention. During your visit you will be assessed on different factors that contribute to a healthy life. This includes depression screening, fall risk, home safety, and cognitive impairment. Your physician will update your current conditions, medical and family history, prescriptions and more. You'll also discuss preventive care and immunizations.
This is a free preventive screening visit at your healthcare provider’s office. It is available annually to Medicare patients at no charge.
What is the “Welcome To Medicare Visit?" The “Welcome to Medicare Visit” is performed within your first 12 months of enrollment. It's like your introduction to Medicare! During the visit, your physician will update your current conditions, prescriptions, medical and family history. This visit is a one-time event, so take advantage of this offer before your qualification period expires!
What is an “Annual Wellness Visit?” The “Annual Wellness Visit” is a preventive screening service to help you plan for a healthy future. This should be an annual appointment after your “Welcome to Medicare Visit” has taken place.

Why Should I Go? These visits are a great opportunity to speak to your provider about your overall health. They also strengthen the relationship between you, your healthcare provider and care team! Many visits occur when patients are sick, but in this case, we want to see you when you're well and work together to create a personalized health plan uniquely designed to keep you healthy!
Note: These visits are 100% covered by Medicare; however, should your provider choose to perform further testing, you may be subject to a co-pay or a deductible.
Chronic Care Management (CCM) is a program created by Medicare for members with two or more chronic conditions that are are anticipated to last for at least 12 months or for the remainder of the patient’s life. Millennium Physician Group takes special care of patients with Diabetes, Chronic Obstructive Pulmonary Disease (COPD), Congestive Heart Failure (CHF), Hypertension, as well as those experiencing depression because of their chronic illness, those with overburdened caregivers and those lacking care-giver support. Our goal is to provide the care and resources needed to help you stay healthy, prevent flare-ups, and avoid unnecessary emergency room visits or hospitalizations. CCM registration is simple and involves filling out an enrollment consent form. A nurse case manager will call you to assist in completing the form and will also work with your provider to develop

develop an individual plan of care. The RN Case Manager is available Monday - Friday, from 8 a.m. to 5 p.m. to assist with your healthcare needs. You will also receive assistance with education of your disease, coordination of care and physician appointments. Furthermore, the case manager will continue to check in on you to see how you're doing! And you will also have 24/7 access to a triage nurse to assist with urgent healthcare concerns. The Chronic Care Management (CCM) service is covered under your Medicare benefit at 80%. The patient is responsible for the additional 20%, but most secondary insurances cover the remaining cost. Please speak to your provider to see if Chronic Care Management services are right for you.
