Policies and Procedures We strive to provide the best medical services in a friendly atmosphere. Employees are expected to be courteous, friendly, and helpful at all times. We expect a mature and professional manner to be exhibited at all times. All employees share in the responsibility of observing a code of ethics. This code requires truthfulness and honesty.

Millennium’s core values summarize how we treat others, patients, and fellow community members. Millennium CARES for every patient, every time.


Ethics and Professionalism All employees share in the responsibility for observing the code of ethics that regulates the activities of both physicians and employees.  Only physicians and advanced providers (ARNPs and PAs) have the training and legal right to make a medical diagnosis and treat human illnesses and injuries.  All information concerning patients or organization business must be held in strict confidence.  An employee’s professional life is expected to be conducted according to the highest moral and ethical standards.

Policies to Know Tobacco Free Workplace

ADA/ADAA Compliant

All MPG campuses, facilities, and employerowned vehicles are tobacco free environments.

Drug Free Workplace Compliance with MPG’s Drug Free Workplace Policy is a condition of employment. The following types of drug screens are conducted by MPG: • • •

Performance Improvement MPG reserves the right to impose appropriate discipline, up to and including termination, for misconduct. Employees who violate any policy or who participate in misconduct will be disciplined in several stages.

Consists of an oral warning of disapproval delivered by the supervisor. The supervisor will place a memo to file in the employee’s personnel file.

Consists of a written warning and oral explanation delivered by the supervisor. This form will be signed by the employee and placed in their personnel file.

Consists of a written warning and oral explanation delivered by the supervisor. It includes a notice that another warning may result in termination. This form will be signed by the employee and placed in their personnel file.

Immediate termination by the supervisor. A written exit report will be placed in the employee’s personnel file.

Employee Performance Improvement Plan The Employee Performance Improvement Plan (ePIP) is a plan that is put together by both the employee and supervisor to try to avoid any future performance issues. Both individuals will sit down, discuss the issue, and brainstorm ideas to avoid a reoccurrence. The plan will be written and included with the written warnings previously mentioned. The warnings and ePIP will be filed into the employee’s personnel file.


Dress Code Employees must project a neat, clean, and professional appearance consistent with MPG’s image. All employees are required to wear their MPG ID badge at all times.

Uniform Vouchers • • • • • • •

Vouchers can be used at YurStore.com. Any individual purchases are also allowed. Vouchers will be issued at the end of each month for all eligible employees during the entirety of the month. Vouchers will expire at the end of 90 days. All purchases can be mailed directly to the employee’s residence or office. Any special requests must be made by the practice manager to Becca Bradley. Exchanges can be made. Please email Becca Bradley for assistance.


Vouchers

Medical Staff Most medical personnel will receive a voucher for 3 scrub pieces and 1 lab jacket after 90 days of employment. Those medical personnel will also receive an additional 3 scrub pieces for each anniversary.

Administrative Staff Select administrative personnel will receive a voucher for a variety of tops after 90 days of employment. Those personnel will also receive an additional voucher for each anniversary.

Technology Use Texting is only permitted during authorized work breaks. Personal cell phones should remain on silent or turned off during working hours. In case of emergency, please ask permission from a supervisor to use a phone for personal uses.

Uniform Vouchers

Internet Use Policy MPG reserves the right to review, audit, access, or disclose content of its IT resources at any time, without user notification. Misuse of IT resources may result in disciplinary action, up to and including, termination. Internet Access Use of MPG IT resources for accessing the internet is restricted to approved workrelated purposes such as: • • • •

Accessing electronic timecard Ordering supplies Research and data mining Communicating with MPG employees and non-employees


Absences Employees unable to report to work as scheduled must notify their supervisor or supervisor’s designee by phone before his/her shift begins. Ask your supervisor how they prefer to be notified. Supervisors are responsible for tracking and monitoring staffs’ absences. Employees must bring in a doctor’s note stating a “return to work date” if they have been absent for 3 or more consecutive days for medical reasons.

On the Job Injuries It is your responsibility to: • • • •

Notify your supervisor immediately Complete an incident report ASAP using Compliance 360 (to access, click the Report It icon on your work desktop) Call HR to notify them of the incident and start Worker’s Comp claim Get treatment (including post-injury drug test) from an approved facility within 24 hours

Compensation Salaries and Wages Employees are paid bi-weekly on alternating Fridays for work performed through Sunday of the previous week. Overtime Pay Employees must receive supervisor’s authorization prior to working additional hours. Donated time or unscheduled work without pay is neither expected nor permitted. In accordance with the provisions of the Fair Labor Standards Act • Non-exempt employees are paid straight time for all hours worked up to forty (40) hours in one week; • Non-exempt employees are paid overtime at time-and-one-half the regular rate of pay for hours worked in excess of forty (40) hours in one week; • Only “hours worked” will be used to calculate overtime pay. Paid time off and holiday pay are not considered “hours worked”; • Compensatory time off in lieu of overtime payments will not be granted.


Evaluations and Increases 90 Day Evaluation •

Each employee will receive a 90-day introductory evaluation (not tied to compensation)

Annual Performance Evaluations •

MPG conducts performance evaluations for all employees annually during the summer

Any potential merit increase, if approved by the Board of Directors, will be distributed annually after the evaluation cycle is complete

Internal Job Postings MPG fosters an environment of internal promotion by offering all current employees first opportunity to advance should they possess all the skills and abilities for open positions. •

To apply for an open position, employees must: Go to the Millennium Website, www.millenniumphysician.com

Click on the “Careers” tab. Look at the “Open Jobs” to determine your interest.

Once you have found a position you are interested in applying for, go back to the “Internal Applicants” tab and fill out “MPG Internal Applications Only” form. (Do not complete the application under “Open Jobs”)

Your application will then be looked at by the HR Recruiter and sent over to the Hiring Manager immediately.

*Exception: Employees may move after 3 months of acceptable service provided that an unusual need is being met and that HR has approved. **Definition of Good Standing: No disciplinary notices within past 12 months or since being hired (whichever is more recent); acceptable performance appraisal; good attendance.


Preceptorships/Internships Goal of the Program

Preceptorship Process

To provide students with a professional learning experience with a Physician or Advanced Provider in hopes of retaining the student as a permanent MPG employee.

All students should complete the online application found on our website. Once the application is received, HR will begin processing and credentialing the student. HR will: • Check to validate the school has an approved college program • Verify if we currently have an Affiliation Agreement with the school • Check to see if the school is on our current list • Complete all pre-rotation paperwork • Conduct a background check

• Have the student do a 10-panel drug screen

Once the student is vetted, HR will contact the Provider and their office to set up the official start date. After the rotation is completed the Provider is to send a copy of the student’s evaluation to HR. These will be sued for if the student would like to do another rotation or if the student applies for employment after graduation.
