Introduction Community Care Services & Home Care Packages In today’s ever-changing healthcare environment, it is a key focus in our community to assist our elderly to remain at home as long as possible and facilitate their ongoing independence. This is only achievable if the environment in which they live in, is safe and supported. Asking for help doesn’t mean losing your independence, it’s quite the opposite.


Government funded services in the form of a Home Care package are provided following an assessment of the consumer’s circumstances and needs, to enable them to achieve this outcome. Home Care assistance may look different for different people. It may mean getting help with shopping and cooking. Or it could be receiving personal care to assist with activities of daily living or getting in and out of bed. It may even mean getting modiďŹ cations to improve your safety and movement around the house.

Access to the funding in a Home Care package in many circumstances requires an Allied Health assessment to assess the risk and determine a solution. This service delivery could be completed by a Physiotherapist, Occupational Therapist, Dietician, or Podiatrist, depending on the particular situation.
