TIP 2:

The wash-hand basin and infection control

Wash-hand regimes and the requisite infrastructure are a critical cornerstone of infection prevention and control in any hospital setting because many hospital-acquired infections are transferred by hand from one person to another. Yet, uninformed wash-hand basin and tap selection or poor installation can actually contribute to hygiene Figure 1 – acceptable consulting room layout

problems and the spread of disease. A clinical wash-hand basin should therefore be preferred over a “normal” wash-hand basin. So what are the distinguishing features of a clinical wash-hand basin? From figure 3, it can be seen that a clinical wash-hand basin does not have an overflow or tap-holes, that it is not a drop-in or counter-top unit. The water stream from the taps must be aimed to avoid falling directly into the outlet, to avoid aerosolisation. Pressure compensating flow restrictors are advisable. These features allow for ease of decontamination and help avoid biofilm developing, which can harbour harmful microorganisms. In at

MEDICAL SANITARY WARE Mac Brothers consulted with healthcare practitioners, engineers and architects and it became clear that major re-engineering would be required to meet aspired infection and safety standards in surgical theatres and wards.

The new product ranges are

The manufacturer made use of two fibre optic lasers cutters and a very unique fully automated panel bender to mention but a few. The technology required in sheet metal production differs quite a lot from other stainless steel products where pressing technology is used to produce a required outcome. Some of the new innovations include:

HEALTHCARE FACILITY GUIDELINES TO WINDOWS AND DOORS Use of natural light and ventilation for patient care facilities is to be maximised when designing a health care facility. By maximising natural light (daylight) with window opening onto an atrium, courtyard can be beneficial to both the patients and the healthcare staff.

showers, a mimimum clear opening of 1000mm is recommended. Door openings must take into consideration requirements for special equipment that may be moved into patient accessed rooms, such as bariatric equipment and lifting apparatus. Emergency access

Certain rooms that are used by patients should be equipped with doors and hardware that will permit emergency access (panic doors) from the outside. These rooms can be defined broadly as follows:

• Rooms that are used independently by patients, have only one door and are less than 6m2

• Rooms where there is less than 2.5 metres of clear space behind the single door

• Patient bedrooms, bathrooms and

ensuites in mental health facilities or mental health components of other health facilities

• Secure rooms in mental health facilities

When rooms have only one opening, the door should be capable of opening outwards or in a manner that will

negate the need to push against a patient who may have collapsed within the room. An example of this is; if the door opens inwards, in case of emergency, the staff must be able to open the door outwards without any need to use a key. In all areas except mental health secure rooms, surface sliding doors installed on the outside of the room may satisfy the requirements of this section. This can be achieved if:

Push / Pull Plates – for doors where a door lock or latch is not required, a push plate with pull handle may be installed. Examples include scrub rooms where staff may be carrying items or have wet hands.

Locks – door locking may include

keyed locks, electronic locking systems, snibs, push buttons and privacy latches with indicators. Door locking should always allow escape from inside a room accidently locked. Fire exit doors should be openable from the inside with a single action.

to remain open may require hold open devices, to prevent them closing, particularly in main travel and circulation pathways. Hold open devices should be installed according to relevant fire regulations, building codes and standards. Designers should consider the following:

• The devices should be activated in

• Door catches should not be installed on doors that are required to stay closed

• Devices should not be installed on doors where distributed patients may lock themselves in a room

• Door catchers should not be

installed in mental health patient rooms where it can be used for self harm

• Electronic hold open devices linked to a fire / smoke alarm system are

acceptable and must have a manual release button as part of the device. Self-Closing Devices – closing devices are fitted to doors that are required to stay closed that may include:

• Pressurised rooms such as isolation rooms

• Birthing rooms

• Clean and dirty laundry utility rooms • Disposal rooms

Self-closing devices should not be used in the following rooms: • Offices

• Patient bedrooms

• Bathrooms, ensuites, toilets, showers • Rooms used independently by people with disabilities

• Meeting rooms and interview rooms Door grilles or undercuts may be

required to facilitate air-conditioning systems, to allow for return air or for balancing of air pressurisation between rooms.

Glazed panels may be provided in doors where visual observation for

reasons of safety, security or patient observation is required.

Automatic sliding or swing doors are favourable in areas with lots of movement such as the main entrance and delivery points. Automatic doors may also be used in areas where rapid, hands-free access is preferred in entries to emergency units or operating theatres. •

you need to take into account the traffic

and use anticipated for the specific location in the hospital. In general, ceramic tiles and rubber or vinyl flooring options are a more sensible choice as they are easier to clean and hard wearing. National Product Manager: Flooring, says FloorworX Self Leveller has proved extremely popular throughout South Africa and was also specified for other large flooring contracts recently, including the Polokwane Hospital (20 000m2), and Saldanha Bay Military Academy (also 20 000m2). Concrete repair and performance enhancing products supplied by a.b.e. to Group Five for the Nelson Mandela Children’s Hospital include durarep FC (fairing compound), a cementitious polymer modified fairing coat that covers concrete surface imperfections and provides a smooth finish, says Rens Pelser, Technical Sales Consultant for a.b.e. in Boksburg. “In addition, durarep FR (fibre reinforced), a cementitious structural repair mortar, is being used by Group Five for a wide variety of applications. The product provides excellent bonding to concrete when used with the appropriate primer, does not shrink, is chloride-free, and apart from being easy to apply, also has high ultimate strength. It is suitable for repairing voids and honeycombing in concrete, and for profiling as well as general concrete repairs required during the construction stage of a project,” Pelser adds. FloorworX Self Leveller as well as durarep FR and durarep FC are part of the growing range of a.b.e. products manufactured at Boksburg in a special

plant with adapted formulas and raw materials to eliminate unhealthy dust emanating from these products when used on building sites. Finally, a.b.e. epidermix 344 as well as epidermix 345 structural wet-todry concrete adhesives are also being used at the Nelson Mandela Children’s Hospital. epidermix 344 provides an ‘open time’ (the time it remains workable before setting hard) of about two hours, while the 345 version has an impressive 18 hours open time capacity. “These two products, suitable for structural situations, provide high ultimate tensile strength as well as lap sheer strengths. It can be used to bond fresh concrete to existing concrete, plaster to brickwork, or to provide a damp-proof barrier in the concrete matrix,” Pelser states. NELSON MANDELA CHILDREN’S HOSPITAL Union Tiles was privileged and honoured to be involved in this wonderful project. The vision that was so successfully depicted by the architect and board, was carried out and ended in a fairy tale of clean lines, yet welcoming and fresh. Union Tiles supplied a ‘true’ Terrazzo tile

600x600mm in size and 20mm thick. The

background was a soft light grey with beautiful

almost luminescent white marble chips in, these were polished to a beautiful shine. Union Tiles’ product is absolutely unique and, has been recognised by the Green Building Council.

STANDING DESKS COULD DECREASE HEART DISEASE Sitting for more than four hours a day could increase an individual’s risk of death from any health issue - including heart disease and diabetes - by 46% when compared to those who sit for less than two hours a day.

According to a study by the Journal of Circulation, the Implementation of standing and walking desks in South African offices could decrease the risk of physical health issues drastically as well as improve the mental and intellectual state of the workforce. This is according to Dr Robert Delgado, a corrective care chiropractor and lifestyle coach at Delgado Chiropractic, who says that the brain is like a battery that gets charged up by movement, especially movement of the spine.

Little did the researchers who discovered PETase in soil samples taken at a Japanese plastics facility know that their examination of the enzyme had tweaked its structure, making it far more efficient at breaking down plastic than before. PETase is named for its ability to digest polyethylene terephthalate (PET), the most common strain of plastic, but the improved strain even made short work of polyethylene furandicarboxylate (PEF). In the last five years an unprecedented amount of research has been poured into finding solutions to the problem, and a number of possible solves

Handling virulent, clinically derived, multidrug resistant isolates on a daily basis requires extreme vigilance and strict adherence to safety measures. Currently, the seminal guide to preventing these types of infection is the third edition of the World Health Organisation’s Laboratory Biosafety Manual, first published in 2004. Among its recommendations are for handling potentially infectious materials are:

Contrary to popular belief, most drugresistant isolates are only classified as bio-safety level 2 (BSL 2) under the WHO’s classifications. However, this doesn’t mean that any less precaution needs to be taken with them.

POSSIBLE SUPERBUG CURES New discoveries raise the possibility that antibiotic resistant superbugs could be effectively managed or even eliminated by new advances in antibiotic medicine. Virulent strains such as methicillin resistant staphylococcus aureus have been successfully neutralised by a modified strain of teixobactin recently. A synthetic version of teixobactin has been created which may tip the balance against drugresistant strains. Physicians are looking into utilising tiny, 3D-printed implants infused with antibiotics to prevent deadly infections postsurgery, but until a cure has been definitely produced, it is best to adhere to extra-strict safety controls when handling dangerous biological samples.

at varying depths in incisions and body cavities. A set-up consists of a single or multiple-lighthead assembly attached to a suspension arm. The surgical light fixture can either be mounted at a fixed point on a ceiling or wall or even positioned along a ceiling mounted track. Types of lamps include tungsten, quartz, halogens and light-emitting diodes (LED). Sterilizable handles allow the surgeon to easily adjust the light position. Most models can accommodate a mounted camera and the latest lights have an integrated camera. Integration into the operating room infrastructure and control system is now possible. Surgical lights are designed to operate for extended periods of time without radiating excessive heat. In case of light failure during surgery, some models come with a reserve lamp that automatically switches on when the primary lamp fails. Reported problems with operating lights include patients reportedly

burned by lights in which heat protection filters were removed. Burns were reported when multiple surgical lights operated at maximum intensity were focused on the same area. Bumping lights can crack or fracture pieces that could fall onto the operating table. Dirt or fingerprints on quartz-halogen lamp can cause the bulb to fail prematurely, be discoloured or explode. Other reported problems include disconnection and or falling of fixture components. In hospital environments, using circadian lighting techniques with changing colour temperature and light intensity can show positive results. Human-centric lighting has the potential to support the well-being of patients as well as the workers in the healthcare facility. With the right lighting solutions the healthcare building’s energy efficiency and human comfort can heal and be an essential infrastructure development.

NOISE AFFECTS HOSPITAL CARE Sleep is fundamental to human health in general, and critical to patient recovery. The World Health Organization (WHO) stipulates that for a good night’s sleep, continuous background noise should not exceed 30 decibels, and individual noise events exceeding 45 decibels should be avoided.
