as a complex building type, and, and engineering is a specialised endeavour. This is important to be aware of, because, provision of good infrastructure in the context of healthcare service delivery can literally be a matter of life and death. For example, if the hospital power supply is interrupted – even for a short period of time – and there is not a well-conceived redundancy plan, failure of mechanically ventilated breathing apparatus could result in asphyxiation, patients
or contracting an illness they did Hospital design, engineering and specification is not the only factor, but research has produced an evidence base to show how these can contribute positively to safety, health and healing. Here are just four of the many ways informed specification can reduce risk and result in better healthcare built environments.
TIP 1:
Understand the clinical procedures
on the operating table could be left vulnerable and lifts could stop, delaying patients on their way to receive critical care. This seems fairly obvious. Yet, observations in the field show that, all too often, insufficient care is taken in the planning, design, engineering, operation and maintenance of hospital infrastructure. The World Health Organization notes that, globally, at least 10% of hospital admissions result in adverse events which see

Healthcare Design 2020
Good hospital design should balance the need for the building fabric to be loose-fit and adaptable with a need to support highly specific and specialised procedures. Did you know that, internationally, clinicians are always taught to approach from the patient’s right-hand side when doing general clinical procedures, such as taking vital statistics? The set-up of examination beds and all the medical paraphernalia in consulting rooms

TIP 2:
The wash-hand basin and infection control
Wash-hand regimes and the requisite infrastructure are a critical cornerstone of infection prevention and control in any hospital setting because many hospital-acquired infections are transferred by hand from one person to another. Yet, uninformed wash-hand basin and tap selection or poor installation can actually contribute to hygiene Figure 1 – acceptable consulting room layout
problems and the spread of disease. A clinical wash-hand basin should therefore be preferred over a “normal” wash-hand basin. So what are the distinguishing features of a clinical wash-hand basin? From figure 3, it can be seen that a clinical wash-hand basin does not have an overflow or tap-holes, that it is not a drop-in or counter-top unit. The water stream from the taps must be aimed to avoid falling directly into the outlet, to avoid aerosolisation. Pressure compensating flow restrictors are advisable. These features allow for ease of decontamination and help avoid biofilm developing, which can harbour harmful microorganisms. In at

Beware gas
Compressed gas is useful for a variety of purposes in hospital settings and can be fixed and piped in the very fabric of the infrastructure, or mobile and deployed to where it is needed. Only suitably qualified and skilled professionals should work on these installations and designers and engineers must work closely with healthcare service providers to understand their specific needs, and follow applicable standards and regulations. All compressed gas cylinders (regardless of size) need to be secured to racks, walls, work benches, or trolleys by means of a strong chain

MEDICAL SANITARY WARE Mac Brothers consulted with healthcare practitioners, engineers and architects and it became clear that major re-engineering would be required to meet aspired infection and safety standards in surgical theatres and wards.
The new product ranges are
produced using the very high tech equipment used by Mac Brothers to produce a leading brand in the catering industry. It required more than just a laser cutter to produce the new range. The manufacturer made use of two fibre optic lasers cutters and a very unique fully automated panel bender to mention but a few. The technology required in sheet metal production differs quite a lot from other stainless steel products where pressing technology is used to produce a required outcome. Some of the new innovations include:
Slop hopper with closing grid
The new Surgeon Scrub unit with sloped edges for improved hygiene. Sensors were added to the scrub sink to make it hands-free where required. (Elbow action mixers remain an option). All exposed areas are fully closed to prevent the collection of waste. Surgeon Scrub Unit

• Secure rooms in mental health facilities
When rooms have only one opening, the door should be capable of opening outwards or in a manner that will
negate the need to push against a patient who may have collapsed within the room. An example of this is; if the door opens inwards, in case of emergency, the staff must be able to open the door outwards without any need to use a key. Allen key or special device, whilst managing security in normal use. In all areas except mental health secure rooms, surface sliding doors installed on the outside of the room may satisfy the requirements of this section. This can be achieved if:
• The door can be easily and safely removed off the track
• Door removal is not prevented by the door locking mechanism.
Door hardware and accessories Handles – in all doors within a
healthcare facility should be given to the shape of the door handle so that it is not caught by pockets in clothing of staff, patients or visitors.
Push / Pull Plates – for doors where a door lock or latch is not required, a push plate with pull handle may be installed. Examples include scrub rooms where staff may be carrying items or have wet hands.
Locks – door locking may include
keyed locks, electronic locking systems, snibs, push buttons and privacy latches with indicators. Door locking should always allow escape from inside a room accidently locked. Fire exit doors should be openable from the inside with a single action.
Door Catches – doors that need

to remain open may require hold open devices, to prevent them closing, particularly in main travel and circulation pathways. Hold open devices should be installed according to relevant fire regulations, building codes and standards. Designers should consider the following:
• The devices should be activated in
easy reach without bending down
• Door catches should not be installed on doors that are required to stay closed
• Devices should not be installed on doors where distributed patients may lock themselves in a room
• Door catchers should not be
installed in mental health patient rooms where it can be used for self harm
• Electronic hold open devices linked to a fire / smoke alarm system are
acceptable and must have a manual release button as part of the device. Self-Closing Devices – closing devices are fitted to doors that are required to stay closed that may include:
• Pressurised rooms such as isolation rooms
• Birthing rooms
• Clean and dirty laundry utility rooms • Disposal rooms
Self-closing devices should not be used in the following rooms: • Offices
• Patient bedrooms
• Bathrooms, ensuites, toilets, showers • Rooms used independently by people with disabilities
• Meeting rooms and interview rooms Door grilles or undercuts may be
required to facilitate air-conditioning systems, to allow for return air or for balancing of air pressurisation between rooms.
Glazed panels may be provided in doors where visual observation for
reasons of safety, security or patient observation is required.
Automatic sliding or swing doors are favourable in areas with lots of movement such as the main entrance and delivery points. Automatic doors may also be used in areas where rapid, hands-free access is preferred in entries to emergency units or operating theatres. •

FLOORING
FLOORING When specifying flooring for hospitals,
you need to take into account the traffic
and use anticipated for the specific location in the hospital. In general, ceramic tiles and rubber or vinyl flooring options are a more sensible choice as they are easier to clean and hard wearing. Seamless floor coatings, laminated flooring or cork sheeting are also worth considering depending on the intended application and budget. Take into account factors such as sound deadening qualities, slip-resistance, anti-bacterial properties and longevity. We recommend that specifiers refer to Internal Floor Finishes in Healthcare Facilities by IUSS. Infrastructure Unit Support Systems (IUSS) aims to optimise the acquisition and management of South Africa’s public healthcare infrastructure. Usually vinyl, or other coverings, can be applied onto 10mm thick FloorworX Self Leveller screed after only 24 hours,” Nel added. The screed is being applied at

the hospital by specialist flooring contractors, H&M Flooring, and Universal Flooring. Peter Jones, a.b.e. National Product Manager: Flooring, says FloorworX Self Leveller has proved extremely popular throughout South Africa and was also specified for other large flooring contracts recently, including the Polokwane Hospital (20 000m2), and Saldanha Bay Military Academy (also 20 000m2). Concrete repair and performance enhancing products supplied by a.b.e. to Group Five for the Nelson Mandela Children’s Hospital include durarep FC (fairing compound), a cementitious polymer modified fairing coat that covers concrete surface imperfections and provides a smooth finish, says Rens Pelser, Technical Sales Consultant for a.b.e. in Boksburg. “In addition, durarep FR (fibre reinforced), a cementitious structural repair mortar, is being used by Group Five for a wide variety of applications. The product provides excellent bonding to concrete when used with the appropriate primer, does not shrink, is chloride-free, and apart from being easy to apply, also has high ultimate strength. It is suitable for repairing voids and honeycombing in concrete, and for profiling as well as general concrete repairs required during the construction stage of a project,” Pelser adds. FloorworX Self Leveller as well as durarep FR and durarep FC are part of the growing range of a.b.e. products manufactured at Boksburg in a special

plant with adapted formulas and raw materials to eliminate unhealthy dust emanating from these products when used on building sites. Finally, a.b.e. epidermix 344 as well as epidermix 345 structural wet-todry concrete adhesives are also being used at the Nelson Mandela Children’s Hospital. epidermix 344 provides an ‘open time’ (the time it remains workable before setting hard) of about two hours, while the 345 version has an impressive 18 hours open time capacity. “These two products, suitable for structural situations, provide high ultimate tensile strength as well as lap sheer strengths. It can be used to bond fresh concrete to existing concrete, plaster to brickwork, or to provide a damp-proof barrier in the concrete matrix,” Pelser states. Group Five handled the application of the concrete repair products, following initial a.b.e. training and guidance. For more information, call 011-306-9000.
FLOORING
NELSON MANDELA CHILDREN’S HOSPITAL Union Tiles was privileged and honoured to be involved in this wonderful project. The vision that was so successfully depicted by the architect and board, was carried out and ended in a fairy tale of clean lines, yet welcoming and fresh. Union Tiles supplied a ‘true’ Terrazzo tile
600x600mm in size and 20mm thick. The
background was a soft light grey with beautiful
almost luminescent white marble chips in, these were polished to a beautiful shine. Union Tiles’ product is absolutely unique and, has been recognised by the Green Building Council.
The Polished terrazzo is regarded as extremely
favourable internationally and locally by architects due to its sustainable and stain resistant features.
As with natural stone, after years of wear and tear terrazzo tiles can be rejuvenated by re-grinding,
revealing a beautiful new surface. Now there is a lifetime of service!
The specific tile LG21HPSL, that was used
allowed for designers to incorporate many fun aspects into the childrens’ wards. The colour
and depth of this tile created a background and palette for all the other colours to look brighter, clearer and in many ways allowed for a gentler fun feeling.
Union Tiles Flooring prays that all the natural
beauty in our Terrazzo product will give pleasure
to those who walk those passages and rooms for many years to come.

STANDING DESKS COULD DECREASE HEART DISEASE Sitting for more than four hours a day could increase an individual’s risk of death from any health issue - including heart disease and diabetes - by 46% when compared to those who sit for less than two hours a day.
According to a study by the Journal of Circulation, the Implementation of standing and walking desks in South African offices could decrease the risk of physical health issues drastically as well as improve the mental and intellectual state of the workforce. This is according to Dr Robert Delgado, a corrective care chiropractor and lifestyle coach at Delgado Chiropractic, who says that the brain is like a battery that gets charged up by movement, especially movement of the spine.
48

“This is why we feel invigorated after exercises and lethargic after sitting all day. By not moving enough the brain becomes deficient in one of its most important raw materials namely proprioception, which can negatively affect focus, concentration, emotion and overall health and wellbeing.” Dr Delgado explains that sitting for too long can also damage spinal joints and ligaments which sends stress signals, or nociception, to the brain. “This in turn can activate the stress response and the release

the user to adjust the height to the individual’s personalised needs and body structure. The electric models also have a height memory feature, so workers can switch from sitting to standing easily.” In addition to the utilisation of standing desks, individuals who sit for extended periods must also take their spines through its full range of motion in order to feed the brain with vital nutrition, says Dr Delgado. “Whilst sitting, rotate your head to the right and to the left and flex the head down towards the floor and up towards the ceiling holding each position for five seconds. Extending the arms and flexing wrists and shoulders will also assist in turning the power of the brain back on.” He stresses that the most important thing to do is to stand up every thirty minutes and recommends setting an Outlook reminder on your computer to go off every half an hour. “If using a standing desk is not a viable option, workers can use a slightly dynamic backrest for their chairs, which ensures that the core is engaged

HVAC SERVICES
HVAC - HEATING, VENTILATION AND AIR-CONDITIONING HVAC services are exceptionally important in terms of hospitals and clinics, with stringent requirements in place to ensure optimal and
contamination-free operation. These days, in line with green building principles and energy-saving initiatives, it is advisable to look into natural ventilation in hospital areas where this may be possible. Refer to IUSS HEALTH FACILITY GUIDES Building Engineering Services http://bit.ly/IUSS-building-engineering-services. The Infrastructure Unit System Support – IUSS – project is an initiative of the National Department of Health and provides valuable guidelines on all aspects of healthcare design.

LAB ACCIDENTS: FAILS AND WINS Those of you watching Strange

Parsons, might be disappointed to learn that (spoiler alert) Parsons blew himself up in a home laboratory accident. While working on a rush order of explosive material for a film set, Parsons miscalculated so severely that paramedics discovered him minus a forearm and most of his face. Thankfully, not all lab accidents lead to calamity. Thanks in large part to today’s stringent modern safety and hazard control measures, lab accidents have become few and far between. Some have even been instrumental in the history of science and human progress. BUNSEN DISCOVERS
BURNER, CHEATS DEATH Robert Bunsen, the

But Bunsen was equally famous for escaping death in a series of lab disasters. His discovery that cyanogen gas can be formed in blast furnaces, for example, came about largely because he opened a test tube embedded in a furnace and nearly choked to death on its contents. He was prone to eye-related accidents – while working with organoarsenic compounds a tube of cacodyl and dimethylarsine cyanide blew up in his face, permanently blinding him in one eye. The other eye almost suffered the same fate when Herr Professor brought a lit candle too close to occluded hydrogen gas. Thankfully his remaining eye recovered and Bunsen could continue his completely safe practice of climbing into Icelandic water geysers and measuring their temperatures.

A recent lab accident may one day remove plastic from our oceans. An enzyme called PETase, which could break down and digest plastic, was first discovered during an examination of the bacterium Ideonella sakaiensis. It is believed that this enzyme was useful in the very distant past to strip the outer layers of plants. Unfortunately, PETase works too slowly to affect the largescale environmental contamination we currently experience. Little did the researchers who discovered PETase in soil samples taken at a Japanese plastics facility know that their examination of the enzyme had tweaked its structure, making it far more efficient at breaking down plastic than before. PETase is named for its ability to digest polyethylene terephthalate (PET), the most common strain of plastic, but the improved strain even made short work of polyethylene furandicarboxylate (PEF). It is hoped that future modifications of PETase could completely remove unwanted plastic from the environment. ASSISTANT SPILLS SELTZER, CREATES SOFT DRINK EMPIRE

SUPERBUGS IN THE LAB Besides hospitals, the most common breeding ground for antibioticresistant superbugs is laboratories. Now that superbugs have been discovered in 80% of supermarket meat in the US, the need to implement effective safety and preventative measures has never been greater.
The threat of drug-resistant superbugs

years ago when medical researchers
should a particularly virulent infection
first came to the public’s attention five sounded the alarm on virulent bacterial strains that appeared to be resistant to most known antibiotics. The possibility of a super-infection arising from these virulent strains has therefore become a real possibility, with several deaths from untreatable illnesses already recorded on the Indian subcontinent. One of India’s biggest hospitals reported that two out of three deaths in children at the facility related to drug-resistant infections. According to information from Johns Hopkins University, antibiotic use has increased 65% since the year 2000, exponentially increasing the risk that drug-resistant infections will occur. Additionally, a projected 10 million deaths a year will take place 10 years from now if nothing is done to stop the threat of superbugs. In the last five years an unprecedented amount of research has been poured into finding solutions to the problem, and a number of possible solves

capable of being deployed en masse occur, though. LAB SAFETY MEASURES
While hospitals are the most likely place for these infections to occur, laboratories are also prime locations for a superbug outbreak. Handling virulent, clinically derived, multidrug resistant isolates on a daily basis requires extreme vigilance and strict adherence to safety measures. Currently, the seminal guide to preventing these types of infection is the third edition of the World Health Organisation’s Laboratory Biosafety Manual, first published in 2004. Among its recommendations are for handling potentially infectious materials are:
• Specimen containers must be made
of glass or plastic (preferably plastic).
• Plastic is preferred because glass could break and the shards cut through protective materials, causing infection.
• Primary specimens must be opened
LABORATORY FITTINGS AND FIXTURES
in a biological safety cabinet.
• Air should never be blown through a
liquid containing infectious material.
• Microbiological transfer loops should have a diameter of 2–3mm and be completely closed.
• Food and drink must not be consumed or stored in the laboratory.
• Cosmetics should not be applied in the laboratory.
• Liquids should not be forcibly expelled from pipettes.
• Centrifuge buckets, rotors and bowls should be decontaminated after each use.
• Equipment sharing between labs should be discouraged.
Contrary to popular belief, most drugresistant isolates are only classified as bio-safety level 2 (BSL 2) under the WHO’s classifications. However, this doesn’t mean that any less precaution needs to be taken with them.
POSSIBLE SUPERBUG CURES New discoveries raise the possibility that antibiotic resistant superbugs could be effectively managed or even eliminated by new advances in antibiotic medicine. Virulent strains such as methicillin resistant staphylococcus aureus have been successfully neutralised by a modified strain of teixobactin recently. A synthetic version of teixobactin has been created which may tip the balance against drugresistant strains. Physicians are looking into utilising tiny, 3D-printed implants infused with antibiotics to prevent deadly infections postsurgery, but until a cure has been definitely produced, it is best to adhere to extra-strict safety controls when handling dangerous biological samples.

Light is critical to human functioning to allow us to see and perform activities. But light also affects psychologically and physiologically, therefore incorporating light into healthcare settings can be beneficial for patients as well as staff who work in the building. Studies show that higher light levels are linked with better performance of complex visual tasks and light requirements increase with age. Light impacts

circadian rhythm is a natural, internal wake cycle and repeats roughly every 24 hours. By controlling the body’s circadian system, light impacts outcomes in healthcare settings by reducing depression among patients, decreasing length of stay in hospitals, improving sleep and circadian rhythm, lessening agitation among dementia patients, easing pain and improving the adjustment to night-shift staff. The presence of windows at the healthcare facility as well as daylight have been

linked to increasing satisfaction with the work environment. Further importance is that exposure to light is critical for Vitamin D metabolism in the human body. Light exposure is also used to treat neonatal hyperbilirubinemia (jaundice). SURGICAL LIGHTS
The right surgical lighting in the operating theatre is critical for patient safety and staff comfort. The lighting design should enable the medical team to see small, low-contrast objects
“The right surgical lighting in the operating theatre is critical for patient safety and staff comfort”
at varying depths in incisions and body cavities. A set-up consists of a single or multiple-lighthead assembly attached to a suspension arm. The surgical light fixture can either be mounted at a fixed point on a ceiling or wall or even positioned along a ceiling mounted track. Types of lamps include tungsten, quartz, halogens and light-emitting diodes (LED). Sterilizable handles allow the surgeon to easily adjust the light position. Most models can accommodate a mounted camera and the latest lights have an integrated camera. Integration into the operating room infrastructure and control system is now possible. Surgical lights are designed to operate for extended periods of time without radiating excessive heat. In case of light failure during surgery, some models come with a reserve lamp that automatically switches on when the primary lamp fails. Reported problems with operating lights include patients reportedly
burned by lights in which heat protection filters were removed. Burns were reported when multiple surgical lights operated at maximum intensity were focused on the same area. Bumping lights can crack or fracture pieces that could fall onto the operating table. Dirt or fingerprints on quartz-halogen lamp can cause the bulb to fail prematurely, be discoloured or explode. Other reported problems include disconnection and or falling of fixture components. In hospital environments, using circadian lighting techniques with changing colour temperature and light intensity can show positive results. Human-centric lighting has the potential to support the well-being of patients as well as the workers in the healthcare facility. With the right lighting solutions the healthcare building’s energy efficiency and human comfort can heal and be an essential infrastructure development.

NOISE AFFECTS HOSPITAL CARE Sleep is fundamental to human health in general, and critical to patient recovery. The World Health Organization (WHO) stipulates that for a good night’s sleep, continuous background noise should not exceed 30 decibels, and individual noise events exceeding 45 decibels should be avoided.

There is a significant difference
in a quiet or noisy environment. In a noisy environment the performance is approximately 50% less accurate. Consider how this may affect work in emergency departments and operating rooms, where the average sound level often exceeds 70 decibels.
CEILINGS, ROOFING & INSULATION
SHOUTING JUST TO BE HEARD In general, clear speech
communication needs to be at least 15 decibels louder than the background noise. Since the average sound levels in emergency departments and operating rooms often exceed 70 decibels, this means communication must be conducted at around 85 decibels. Or, in other words, people need to consistently speak as loud as the sound from a circular wood saw. LESS NOISE – LESS MEDICATION Studies show that more patients receive medication in noisy environments. In a specific study of

In neonatal sound environments, the American Academy of Paediatrics recommends sound levels lower than 45 decibels. But numerous studies from NICUs report average sound levels in the range of 55–89 decibels. The high sound levels increase the infants’ heart rates, respiration rates and blood pressures, as well as lowering their oxygenation levels. It is also known that hearing impairment is around seven times higher among children that have been discharged from NICUs.
