MCL INJURIES
The most commonly injured structure is the medial co-lateral ligament, or MCL. The medial ligament runs down the inside of the knee, where one knee would touch the other if both knees where touching each other. We grade these injuries in terms of severity; Grade I-II and III. Grade I injuries are minor tears or overstretching of the ligament and they generally respond well with early management and in most cases, the person can return to skiing after a day or two. Grade II is a larger tear and Grade III is a complete rupture. Grade II and III require immediate bracing and it’s usually only advanced and expert skiers who can return to skiing on the same trip. Fortunately, the MCL has good blood supply and if it is the only injured structure in the knee no surgery is required. ACL INJURIES

If you’re a physio and you’ve spent the past 14 winters tending to injured skiers in Niseko, if there is one condition you become very good at managing, it is knee injuries. Although skiing is an overwhelmingly safe sport, with injury rates of only 2.8 injuries per 1,000 ski days, the knee is the most commonly injured body-part. Most knee injuries occur when a skier loses control briefly and the ski twists, or ‘catches an edge’ and crucially, the boot does not release from the binding. The length of the ski amplifies the torsional force of the fall, and that amplified force generally ends up at the knee. Although fractures are rare, ligament tears are common.
The second most common injury is the dreaded anterior cruciate ligament (ACL). The ACL is the main stabilizing ligament, deeper inside the knee than the MCL, and injuries tend to be complete ruptures or no injury at all. There is a saying that ‘doing your ACL’ is like getting pregnant: you’ve either done it or you haven’t. An ACL rupture is often characterized by a an audible ‘pop’ and feeling of instability or having a wobbly knee. Pain is a poor indicator as many tears are not overly painful. The swelling and stiffness tends to be delayed somewhat, so we find it easier to assess these injuries on the same day rather than 2-3 days afterwards. Due to the instability and swelling people often require swelling management, gait-training and bracing to assist them to get home. Only expert skiers who are very determined not to waste their holiday who use a high-end knee brace will be able to return to skiing on their same trip. The ligament has poor healing ability, so people who have instability and wish to continue participating in sports involving twisting, often choose to have an operation to replace or repair the ligament – commonly known as a knee re-construction. The return to sport time after a knee reconstruction is 9-12 months. MENISCUS INJURIES
The meniscus is a type of cartilage and is known as the ‘shock absorber’ in the knee. It functions to disperse the force arising from the ground as we move. It is commonly damaged by twisting under weight-bearing, so is a common knee injury and often damaged at the same time as another structure. These injuries have variable healing power, but most research now is suggesting it is better not to operate in most cases. PREVENTING KNEE INJURIES
To prevent knee injuries ensure you are fit enough to ski, begin with an easier run to warm up, don’t ski too hard when tired and consider a knee brace to help stabilize the knee.
