ealth care makes the headlines nearly every day, especially now that every person in the United States is required to have insurance as of Jan. 1, 2014. As a result, the face of health

care is changing — Cover Oregon is a marketplace where anyone can shop and compare insurance plans. Depending on income, applicants may be eligible for tax credits, which reduce the cost of the monthly premium.

Clinics are also evolving — most notably qualifying as a Patient Centered Primary Care Home. Inside these pages are stories about changes to local health care, as well as the contact information for clinics and hospitals.

Cover Oregon

over Oregon is an online marketplace where Oregonians can find and purchase health insurance. It went live Oct. 1, but many were required to apply on paper due to issues with the website. The marketplace was developed to enable a certain group to find health insurance — those who are uninsured, who buy insurance on their own or small businesses with fewer than 50 employees. By inputing income information and household size, the website brings up insurance plans ranging in price in a format that makes it easy to compare premiums and coverage. Through Cover Oregon, individuals

There is still time to find health insurance through Cover Oregon, which is open to applications through March 31. Simply go online to www.coveroregon.com and complete an application. After it is processed, you will receive an eligibility notice and enrollment packet. The next step is to find and choose the plan you want.

Saint Alphonsus serves Baker County and surrounding areas. On Fridays, Dr. Maynard Bronstein, an oncologist and hematologist who lives in La Grande, travels to Baker City to see patients, saving them a trip out of town. rande Ronde Hospital is a not-for-profit, 25-bed Critical Access Hospital (CAH). In addition to the hospital and its range of diagnostic, surgical and therapeutic outpatient services, the main facility also houses the Regional Hematology and Oncology Clinic, the Family Birthing Center, the Rehabilitation Therapy Department and Gym, and the Home Care Services Department which includes Home Health Services and Hospice Care. Grande Ronde Hospital is the only hospital in Union County. It is one of the top employers in northeast Oregon, with more than 500 employees. Mission

n 2012, St. Luke’s Clinic-Eastern Oregon Medical Associates doubled its size to make room for major expansion and modernization of its operations and services. The changes included a larger staff, more visiting specialists, modern equipment, electronic medical records, and a physical structure that supports a team approach in managing patient care. The expanded area (about 16,000 square feet) houses a staff of 55 employees, including 11 providers (six doctors, three physician assistants, and two nurse practitioners). Services available in the specialty

wing include: urology, pulmonology, cardiology, nephrology, oncology, coumadin clinics, a mammography mobile van, and a van provided by Mountain States Tumor Institute to drive patients to Fruitland daily for radiation treatments. In July 2013, the chemotherapy clinic, offering treatments every other Wednesday.

Wallowa Memorial Hospital named to Top 100 List By Trish Yerges

W

allowa Memorial Hospital was recently honored as one of the top 100 critical access hospitals (CAHs) in the country. The announcement was made at the 10th-annual National Rural Health Association’s Critical Access Hospital Conference in Kansas City, Mo., attended by 650 rural hospital leaders. “The National Rural Health Association is proud that the efforts of Wallowa Memorial Hospital have been recognized by this nationwide analysis and ranking,” said Alan Morgan, NRHA CEO. Morgan stated that CAHs play a critical role in delivering healthcare to the 62 million Americans who live in rural areas. “Our challenges are completely different in access while equally complex in delivery as urban hospitals,” he said. NRHA is a nonprofit organization whose mission is to

Oncology services

Dr. Bronstein travels so his patients don't have to make a special trip By Lisa Britton

Dr. S. Maynard Bronstein knows his patients don’t like to travel. He specializes in oncology and hematology. “They feel bad to start with, treatment makes them feel worse, then they have to drive,” he says. Not anymore. He joined the medical staff at Grande Ronde Hospital in September 2011. He works in La Grande but he also travels to Baker City on Fridays (except for the third Friday) and to Enterprise three times a month to see patients. This means patients no longer have to travel to Walla Walla or the Boise area for their chemotherapy treatments. Prior to moving, Dr. Bronstein discovered the need for oncology services when he saw patients at a weekly outreach clinic and through telemedicine consults. Now he offers oncology and hematology services for patients within Union, Wallowa and Baker counties. These services include diagnosis and treatments, including procedures such as therapeutic phlebotomies and bone marrow biopsies. Medical treatment includes a variety of medications regiments from oral medications to intravenous medications. Also, he often includes ancillary services within patient’s

A new option for accessing health care opened Nov. 2, 2013, in Baker City. The Saint Alphonsus Express Care clinic is located inside Albertsons, 1120 Campbell St. The clinic is open seven days a week — 11 a.m. to 7 p.m. Monday through Friday and noon to 5 p.m. Saturday and Sunday. Cathy Treadway, director of operations for Saint Alphonsus Medical Center - Baker City, emphasized the ease of access. “It’s the ability for people who are ill to be able to walk in and get service, quick and efficient,” she said. Two providers — Elizabeth Chattin and Kate Grace — will rotate days to see patients. Both are physician as-

sistants (PA-C). Express care services are for acute conditions such as colds, flu, bladder infections, sinus infections, strep throat/sore throat, ear infections and allergies. Other conditions that can be treated are minor injuries (sprains, strains, animal bites); procedures (suture removal, splinter removal); skin and hair conditions (acne, lice, cold sores, hives, insect bites, minor burns, shingles); physicals; testing and screening and wellness services (flu shot). "It continues to grow, which is exciting," Treadway said. "It seems the community appreciates the access to care." When was your last tetanus shot? By Lisa Britton

I

mmunizations aren't just for children — adults also need to be aware of their vaccination history, and whether or not they need boosters or other vaccines. Alicia Hills, nurse supervisor at the Baker County Health Department, says those vaccines include tetanus with pertussis, hepatitis A, hepatitis B, shingles (age 60 and older) and pneumonia (age 65 and older). Other vaccines adults may need are HPV (women age 26 or younger; men age 22 to 26); meningococcal (ages

19 to 21 who are first-year college students); and varicella (for those who never had chicken pox or only received one dose of the vaccine). Also, a flu shot is recommended every year. Your doctor can recommend which vaccines you may need. Chances are you won't need all these boosters, but county health departments can pull up an immunization history, or it should be with your primary care provider. Immunizations are available at county health departments, as well as many local pharmacies.

he Jonathan M. Wainwright Memorial VA Medical Center is an outpatient primary care facility based in Walla Walla, Wash., and also operates four communitybased outpatient clinics and one contract VA Clinic for area veterans, including one clinic located at 202 12th St. in La Grande, which opened in May of 2008. The La Grande VA Clinic provides veterans with primary health care and behavioral health care as well as various group therapy programs. Specialty care that cannot be provided at the Walla Walla VA in Walla Walla is provided by referral to tertiary VA medical centers or through community providers in the local area. The La Grande VA Clinic’s primary service area includes Morrow, Umatilla, Union and Wallowa counties, with approximately 1,250 Veterans from this area currently receiving care at the clinic. Hours are 8 a.m. to 4:30 p.m. weekdays (excluding federal holidays). For more information, call the clinic at 541-963-0627. The Walla Walla VA has also opened two telehealth primary care

Resources for veterans clinics in Oregon, one which is located in Enterprise, called Wallowa County VA Telehealth Clinic. These clinics were made possible through rural health funds. The Walla Walla VA is leasing space in the old Wallowa Memorial Hospital structure from Wallowa Resources, and the four-room suite has been remodeled to suit the needs of providing appropriate care to area veterans. Local clinics following the PatientCentered Primary Care Home model ■■As the name implies, this means patients are actively involved in their health care, and providers help them find the community resources they need

ealth care is changing, and clinics across Northeastern Oregon are now qualified as “medical homes.” That term is a shorter version of the official designation: “Patient-Centered Primary Care Home.” In Baker City, Valley Medical Clinic and Baker Clinic — both clinics of the Saint Alphonsus Medical System — and St. Luke’s Clinic-Eastern Oregon Medical Associates are now medical homes. In the La Grande, two Grande Ronde Hospital clinics — the children's clinic and Regional Medical Center — qualify as Tier 3. This designation means, simply, that the patient and the provider partner to coordinate their care with preventive measures, rather than wait to see a doctor when he or she is already sick. To institute this model, providers will work with high-risk patients first. “It’s working to get them healthy and the right care at the right time,” said Cathy Treadway, director of operations of the Saint Alphonsus clinics in Baker City. This patient-provider partnership would include medical care as well as other resources in the community (transportation to appointments, dieticians, Meals on Wheels, drug and alcohol treatment, etc.).

The medical home model addresses the “triple aim” goals in Oregon’s health care reform: healthy population, extraordinary care and reasonable cost. “The main piece is the medical home,” Treadway said. This concept isn’t a new one. “We’ve basically been a medical home for the past four years,” said Dr. Charles Hofmann at Valley Medical. This means the clinic providers “actively manage chronic illness” and also address acute conditions by keeping open appointments so patients can come in the same day they call. Providing care when it’s needed, in other words. To meet that need, St. Luke’s Clinic offers open appointments during an extended work day — 7 a.m. to 7 p.m. Monday through Thursday, 7 a.m. to 5 p.m. Friday and 8 a.m. to noon Saturday. Winding Waters Clinic in Enterprise has been working under the 34

PCPCH model for about three years. Johna Alford, back office manager, said the clinic created a care team of employees to help patients find the services they need, such as transportation and prescription assistance. They also offer extended hours, from 7 a.m. to 7 p.m. Monday through Friday and 9 a.m. to 1 p.m. Saturday. "Part of that is to decrease emergency room visits," Alford said. The clinic also has "health coaches," who can help patients develop plans to deal with conditions such as chronic diseases or weight loss. "It's very patient-centered," Alford said of the PCPCH model. "It's getting the patients involved in their health care — we give you all the information you need to make an informed decision." GRH's Tammy Winde said they've added clinic case management for high-risk patients and health maintenance reminders for actions such as vaccinations.

Sleep disorders addressed at hospitals in Northeastern Oregon By Trish Yerges and Lisa Britton

The three hospitals in Eastern Oregon — Wallowa Memorial Hospital, Grande Ronde Hospital and Saint Alphonsus Medical Center-Baker City — all offer programs to diagnosis and treat sleep disorders. "The most common sleep-related disorder is Obstructive Sleep Apnea (OSA)," said LeAnn E. Murray, manager of Respiratory Care at GRH. "However,," she said, "we do see a fair amount of combined sleep disorders such as OSA with Central Sleep Apnea (the brain doesn’t send the signals to your muscles to control breathing; may be a result of another condition such as heart failure or stroke) and patients with parasomnias (sleep disturbances such as sleepwalking, night terrors, sleep paralysis, and REM sleep behavior disorder)." Obstructive sleep apnea is characterized by difficulty breathing and loud snoring during sleep. It disrupts sleep for 62 percent of Americans. Responding to the need in Wallowa County, the hospital has developed a Sleep Disorders Testing Program for area residents. The hospital has contracted with Premier Sleep Professionals and Dr. Kishore Pathial, M.D. to help develop their new Sleep Disorders Program. Pathial is a board certified Sleep, Pulmonary and Critical Care specialist with more than 16 years of experience in this field. His practice includes Pulmonary and Sleep Disorder Medicine in McMinnville, but he will be working closely with Wallowa

County physicians to interpret the results of sleep studies performed in the county. "Wallowa Memorial Hospital’s Sleep Disorders Program provides a comfortable and quiet environment for patients being referred to our facility by area physicians for their sleep studies," said Ron Wulff, LRCP, RRT at the Cardiopulmonary and Sleep Department at WMH. The sleep bedroom is equipped to make the patient feel at home with cable TV and a DVR. A knowledgeable and friendly sleep technologist will explain the sleep study process to the patient. There will always be someone available to provide assistance and calm any uneasiness during the sleep disorders testing. Sleep studies are covered by most insurance plans. At Saint Alphonsus, the two sleep study rooms are decorated in a calm theme — tan and green walls, soft lighting and a bed with a satiny comforter. Almost like a bedroom — except for the camera to document a patient's sleep positions, an intercom to listen to their breathing and a machine that hooks to the 24 sensors each patient wears. Sue Ackley, polysom technician at Saint Alphonsus, said sleep apnea is a common condition they see during sleep studies. "There are pepole who stop breathing 100 times an hour," she said. When they stop breathing, their 36

oxygen level goes down — then they start breathing with a loud snore. One symptom of sleep apnea is daytime fatigue. "Your body is working overtime all night," Ackley said. Murray said some signs and symptoms of sleep disordered breathing include: • Excessive daytime sleepiness • Loud snoring • Choking or gasping during sleep • Observed apnea (stopping breathing) during sleep •Morning headaches "Sleep disordered breathing may also be a contributing factor of obesity, diabetes, and heart disease, depression, and impaired cognition," Murray said. She said a sleep study (polysomnography) consists of monitoring a patient’s brain activity, sleep stages, breathing, arm and leg movements, heart rate and rhythm, and oxygen status to determine if the patient has sleep disordered breathing or possibly another condition that may have similar symptoms as OSA. The Sleep Medicine Physician will evaluate the patient’s sleep study and determine a diagnosis and course of therapy. Some patients may be started on PAP (positive airway pressure) therapy the same night they have the diagnostic polysomnography and others may have to have an additional PAP titration study to determine optimum therapy settings. Ackley some insurance plans require a home sleep test before approving a sleep clinic study.
