From the Editor... Welcome to Issue 19! This issue’s front cover competition winner was submitted from the staff at St John of God Hospital Murdoch when they turned blue on Monday 6 September to help raise awareness for Prostate Cancer. Joining in the theme promoted by the Prostate Cancer Foundation of Australia, caregivers around the hospital wore blue and enjoyed a BBQ lunch with gold coin donations collected to support prostate cancer awareness. The photo shows the staff from St Joseph’s Wards sporting blue attire in their bit to spread the word. Congratulations ladies for participating and for taking such a great photo! The Coles Group and Myer $500 Shopping Spree competition has finally come to an end. We received a lot of entries and would like to thank everyone that entered the competition. We have picked one lucky winner from the various entries, so turn to page 22 to find out if you’ve won! I must also clarify an error regarding the front cover competition winners for Issue 16 and 17. In the last editorial, I had mistakenly attributed Annie Piper for the photo that won in Issue 17 when really it was Issue 16 that Annie won, while Kiran Deep was the actual winner for Issue 17. The Diabetes Educator provides expert diabetes education, support and clinical care to patients and their families. The Diabetes Educator communicates directly with the patients and their families providing relevant information regarding patient treatment and future care. As part of the Chronic Disease unit, you will act in a training capacity, organising and delivering educational programs to other health professionals in the organisation. The role also has QC and QA functions for point of care testing and additionally you will collaborate with the Health Promotion Unit to develop materials and participate in community events to increase Diabetes education.

Whistleblowing:

histleblowing can be defined as any attempt to report an issue or wrongdoing within or outside an organization in order to bring about a necessary change to protect an individual or group of individuals. Within the profession of nursing, whistleblowing is “the action taken by a nurse who goes outside the organization for the public’s best interest when it is unresponsive to reporting the danger through the organization’s proper channels” (Lachman, 2008). Incident reports and verbal reporting to supervisors are examples of whistleblowing. Sometimes these common methods of whistleblowing are not adequate; in other words, the problem reported is not addressed in a manner that satisfies or appeases the whistleblower. When an incident occurs that the whistleblower feels exposes patients or others to danger and is not dealt with appropriately, the whistleblower may take the problem further by informing someone higher up the chain of command or perhaps outside the organization. When this occurs, the whistleblower exposes themselves to several possible consequences, some of them unpleasant. Generally, whistleblowers who go beyond voicing a concern verbally or in writing to their immediate supervisors feel that they have no choice- a danger is clearly present and must be removed or resolved.

In these situations, whistleblowers are themselves at risk, as they may lose their jobs or face censure from colleagues or coworkers. It takes great courage to follow this course of action, and a certainty of the facts as well as hard proof is necessary. There are usually laws to protect whistleblowers from being punished if they reveal a truth that others would rather remain hidden. Providing there is no malicious intent to harm the reputation or standing of the organization, whistleblowers are usually entitled to some protection. Before deciding on such a course of action, the whistleblower may wish to review the laws or statutes that govern whistleblowing in the state or country in which they practice. Nurses have an ethical obligation to report wrongdoing that endangers patients or workers. They may have not only an ethical obligation, but also a professional obligation under the licensing body of the area where they practice. When contemplating such an action, nurses may wish to consult their licensing body as to their best course of action. While never an appealing option, whistleblowing has in the past been viewed as the only option possible in some situations. Whenever possible, whistleblowers should attempt to resolve conflicts by following organizational policy up until or up to the point that nothing further can or will be done.

The National Enrolled Nurse Association (NENA) will hold their 9th National Conference ‘Moving Mountains’ at the Hotel Grand Chancellor Hobart, October 18 & 19, this year.

Hobart August 15th 1995 was, and still is, considered by many to be the “birthplace” of NENA. This is where so many nurses came together and agreed strongly that NENA should be the voice for our level of nurse. It was then that we set ourselves the challenge of changing the way we were perceived. From those early days, we gained the respect of many in the wider nursing community who still continue to ask for, and value, the input from us. For many of us that passion and commitment is still as strong and relevant today, which is why we would love to see you there. We feel that each of you has something important to add to our profession and you may very well meet

The 15years have provided so many opportunities to highlight the value of our level of nurse at a local level and at a national one. We have challenged traditions at times and we have achieved so many rewards, so you could say that NENA and the Enrolled nursing community have “moved mountains”. We still have challenges to overcome and a very effective way to face those challenges, is by keeping the association between us all strong. The health arena is an ever evolving one & in co-operation with nursing leaders of Australia, we can effect change!! Simulation and Beyond Creative teaching approaches for improving patient safety.

Patients requiring critical care are a vulnerable population in terms of medication errors. Critical care patients are the sickest patients in the hospital, requiring intensive nursing care and frequent medications and therapies. Furthermore, their therapy is subject to frequent changes, especially in terms of medications.

What risk factors are cited as being frequent causes of medication errors in critical care? Risk factors can be divided into categories based on contributing factors, as found in Camire et al (2009): • Patients- Critical care patients are often at the extremes of age, often require mechanical ventilation or sedation, are severely and acutely ill and are subject to frequent medication changes. • Providers- may be sleep deprived, working overtime, inexperienced in critical care and may lack knowledge in regards to the medications they administer. • Medications- medications used in critical care may have a high propensity to cause harm and are often given in parenteral form, including cardiovascular drugs,

antibiotics, anticoagulants, sedatives and analgesic medications. • Critical Care Environment- patients in the CCU or ICU typically receive numerous medications that are constantly being changed in regards to types of medications and dosages. Care is fast-paced and urgent, often “life or death”. Patient conditions may change quickly, requiring the addition of new therapies and drug regimens. New and novel treatments are often used and the environment of critical care units is often chaotic. • Organization- Critical care units are subject to stress from organizational problems, such as inexperienced staff, lack of staff, high nurse-to-patient ratios, frequent personnel changes and frequent turnover of care. Working conditions can be stressful and supervision may be lacking. All of the factors cited have been implicated as possible contributing factors to medication errors in the critical care setting. In identifying the most common causes of medication errors in this environment, steps can be designed to prevent these errors from occurring, reducing morbidity and mortality.
