Delivering help to the most deserving of communities himself when he would state the person had suffered this problem for X number of “years”. The on-site Australian nurse manages a very efficient health clinic, combining local and foreign staff, as well as a mix of invaluable and lively translators. Three nurses and three doctors were working at any one time, with Khmer translators provided where necessary. Also, there’s a pharmacy on-site, providing the community with essential medicines for free. A positive and effective vibe pervades the entire clinic.

I’ve just returned from volunteering as a nurse in Siem Reap, Cambodia. The organisation I was fortunate enough to work for was called ‘New Hope Cambodia’, in association with DocTours. Mornings in the Helen Bonner Health Clinic were quite busy, mostly tending to the usual complaints of hypertension and chest infections. Occasionally though, things like monkey bites and possible Malaria were thrown into the mix just to remind us that we were dealing with a very different population. One of the stunning things was how long people had often had their ailment. I often asked my translator to repeat

The afternoons usually involved home visits, some 10-15km out of town. This is where I would awkwardly re-apply my mosquito repellent, whilst balancing on the back of a moving motorbike. Malaria is found on the outskirts of town, and I would probably rather fall off a ‘Moto’ than take my chances with those Mosquitos!

First stop was a discouragingly despondent young woman. I waded through the mud, the chickens, and the smaller siblings to approach her sickbed. She avoided my eye contact as I pulled across her Sari. Seeing her gaping wound, I fell prey to the predictable mindset of many new volunteers, “Quick! Let’s get her to a hospital for advanced wound healing techniques.” It seemed entirely inconceivable to leave her in these extraordinarily basic conditions; with an open wound the size of a fist. As I dressed her wound, I noticed small ants trailing across her body. Now, in theory I might accept that she cannot access sophisticated healthcare; but in no way, practical or theoretical, would I accept her to be crawling with

insects. As I smacked the ants off her, the translator was kind enough to explain the situation. He calmly explained that in Cambodia the medical system is very different; this spiel I can tell he has said many times before. “Our healthcare system is different in Cambodia... if people cannot afford treatments, they simply do not receive them.”

This sentiment of medical inaccessibility was echoed the next day in the clinic. A very unwell looking teenager shuffled in, supported by the arms of her mother. Mercifully, the other nurse sat the patient down and started taking her history. With a recent Leukaemia diagnosis, she was unable to afford treatment, and it was difficult to ascertain what treatments had been received thus far. The Khmer staff knew the routine; if you cant afford treatment, you don’t get treatment. The exception to this rule is if the affected is under the age of 15. The nurse hesitated to ask, “What’s your age?” “16,” replied the child. A hush fell upon the room, as we contemplated the consequences of this fateful and damning number. But for all the dismal aspects of being irrevocably sick in a developing nation, there are innumerable daily victories. The provision of basic medical care undoubtedly changes lives. Simple medical procedures, the access to antibiotics, correct diagnoses and the dispensing of medication for chronic conditions all work to keep the majority as healthy as possible. Holistic care and attention to detail is ingrained in the daily running of the clinic that I was blessed to have worked with. I would so highly recommend a volunteer stint at New Hope Cambodia - you will think very differently after your return and with any luck you will have delivered help to the most deserving of communities. Menzies School of Health Research New app to improve Indigenous mental health An innovative iPad app based on tools developed by Indigenous people for Indigenous people is hoping to address one of the nation’s fastest growing health problems. National health surveys show that Indigenous adults are more than twice as likely to experience emotional distress as non-Indigenous adults. Similarly, mental health disorders are the second largest contributor to the total disease burden of Indigenous Australians accounting for 15 per cent of the total disease burden. Developed by the Menzies School of Health Research 10

(Menzies), in partnership with the Queensland University of Technology (QUT), the AIMhi Stay Strong iPad App has been designed to promote wellbeing by looking at strengths, worries and the goals or changes people would like to achieve in their lives. Menzies Associate Professor Tricia Nagel said the AIMhi Stay Strong App will assist services to deliver costeffective, evidence-based substance misuse and mental health interventions to Aboriginal and Torres Strait Islander clients. “It’s important to remember that mental health is more than just treating mental illnesses; it is about our overall emotional wellbeing which is so important for positive


health and life outcomes.” “The app centres on the client’s strengths and worries and sets goals for change,” Assoc Prof Nagel said. Clients are first asked to identify the people in their life that help keep them strong, their relationships and the role they play in the person’s life. They are then asked to identify their strengths in four areas of their life and this is represented visually as leaves on a tree. As they input more strengths, the leaves grow stronger and healthier. Similarly, clients are asked to identify things in their life that take away their strength in the same four areas. As they input more worries, the leaves on the tree wilt and change colour. “Clients are then left with a visual representation of the areas in their life where they are strong and the areas in their life where they are not as strong,” Assoc Prof Nagel said. “The process is assisted and supported by the health provider and provides help text and audio instructions to reduce the impact of low computer or English literacy. “A summary of the Stay Strong Plan can then be emailed and printed to keep a record of the session for clients and health providers.” Menzies and QUT are in the process of trialing the Stay Strong iPad App with selected health service providers and a limited preview is available at www.menzies.edu.au/aimhiapp. Training in the app is being delivered without charge to primary care providers in the Northern Territory. It’s anticipated the app will be publically available through iTunes by 2014. Since its launch in 2003, AIMhi has given Indigenous service providers and communities a range of tools to recognise and overcome the problems causing mental illness, such as social issues, work and family stress. PTSD Coach Australia mobile app PTSD Coach Australia is a mobile app developed by the Departments of Veterans’ Affairs and Defence to support serving and ex serving Australian Defence Force (ADF) personnel experiencing posttraumatic stress disorder (PTSD). Why an app? Younger veterans particularly have an expectation of digital access and flexibility and the app seeks to give them greater capacity to support their own recovery. A mobile app can be stigma-free, used privately and anonymously. Mobile devices are always on, accessible and available when needed most. They can offer support when clinicians are not available, can be personalised, and multi-media presentations, game-based learning, location-based services can enhance the user experience. 14

Why PTSD? The 2010 ADF Mental Health Prevalence & Wellbeing Study found that PTSD was the most prevalent anxiety disorder in the ADF. The PTSD Coach Australia app helps serving and ex-serving ADF members and their families understand and manage symptoms that commonly occur after trauma. By providing individuals with the education about PTSD, information about self-assessment and professional care, and tools to manage the stresses of daily life with PTSD, this app gives them greater responsibility for their own self-management and recovery process. How does the app work in treatment? Mental health practitioners are using PTSD Coach Australia in conjunction with traditional treatment for


trauma. They use the app to record real time assessments based on the PCL and provide necessary skills training including basic psychoeducation, replacing workbooks, audio etc. Subsyndromal problems can be addressed as can intermittent symptoms. Patients can receive reminders to attend appointments, rehearse skills and coping behaviours, or self-assess between sessions. Assessment results can also be emailed to a health clinician to allow for monitoring between consultations. What type of patient is it for? As a clinician, you need to decide whether PTSD Coach Australia is appropriate for individual patients. Ideally, the patient will have a diagnosis of PTSD and be in treatment for the condition. People with Acute Stress Disorder or subclinical presentations may find the app of benefit, but it is important to note that if your client does not have PTSD, there are options for mood management tools (including apps and web resources) that may be more appropriate to their condition. What can you do?
