From the Editor... Welcome to Issue 9 We would like to wish all nurses for “International Nurses Day“ on 12 May last week. We think nurses do a great job everyday and should have a day to recognise the hard work done to provide for our loved ones during hardships. This years theme is ‘Closing the Gap: Increasing Access and Equity’. Congratulations goes to Amanda Harwood, our winner for the Front Cover competition this issue. Amanda has taken out a Student of the Year award at Challenger Institute’s Student Awards Ceremony in Fremantle last April. Amanda completed a Challenger Diploma of Nursing last year and has since commenced her enrolled nursing graduate placement at Hollywood Private Hospital in Nedlands.

Principal Objective

DDHS has an exciting opportunity for a registered midwife to work within our Mothers and Babies program based at our Women’s Clinic in Palmerston. Hours of work are Monday to Friday and no on-call work required! This position will contribute to the quality and delivery of primary health care within the Mothers and Babies program, by providing high quality, comprehensive and culturally appropriate midwifery care to patients with the aim of improving maternal and birth outcomes. The midwife will provide antenatal, postnatal clinical midwifery services as well as provide maternity education and workshops to our patients. An experienced and knowledgeable clinician, you will also undertake a mentoring role with other clinicians in the organization to enhance their knowledge and skills in relation to all aspects of midwifery care.

This position involves coordinating the day to day running of the Kanyini Polypill Study at Danila Dilba Health Service. The Kanyini Polypill study’s primary aim is to assess whether fixed dose combination cardiovascular medications improves clinical outcomes and adherence for Aboriginal people at high risk of CVD when compared with usual care. The successful applicant must:

What is a mental health professional?

Mental health professionals are responsible for providing clinical assistance to patients who are diagnosed with a severe mental illness. Any person who suffers from a mental illness is described, on a general note, as having feelings, thoughts and behaviours that can cause themselves or others pain. Here is a list of some mental illnesses: • Depression • Anxiety • Schizophrenia. • Bipolar disorder • Eating disorders • Personality issues • Post-traumatic stress/shock • Duel illnesses: people who may suffer from a physical and psychological illness • Mental illnesses coupled with drug or alcohol related problems

What do mental health professionals do?

It is a mental health professional’s objective to be able to understand and interpret correctly a person’s most private thoughts within their own mind to appropriately treat and care for the patient. This may prove challenging as any individual’s personal feelings and thoughts aren’t always displayed through their behaviours. Some interventions offered to patients include; mental health and risk assessments, case management, ongoing monitoring of the client’s mental state, wellbeing and treatment needs, delivery of treatment including

individual, group and family therapy ,focussed psychological strategies, advocacy, family / carer support, education, rehabilitation and finally, support services in conjunction with other service providers. Mental health professionals are also involved in completing screenings, teaching, specialist counselling, psychotherapy, medication management and direct nursing care. Within a workplace environment, mental health professionals are required to monitor the biological dimensions that the patient suffers from through medication administration, behavioural observations and general interpretation of the results from these actions. It is also their responsibility to care for and support the patient’s family and loved ones, as well as protect the rights of the patient. Mental health care professionals are required to: • Support the patients throughout their illness • Request relevant interventions based on close observation to promote psychological well-being and emotional health • Encourage patients to improve their relationships with others and to establish own peace of mind, thus fuelling the recovery process of the patient • Work within a multidisciplinary team of other healthcare professionals that includes psychologists, occupational therapists, doctors, nurses, social workers, speech pathologists, dieticians, indigenous health workers, consumer consultants and other allied health professionals • Protect the human rights of the patient • Support carers / family members and loved ones of the patient

What are the job prospects and opportunities?

There is currently a shortage facing almost all health facilities across Australia, which is why mental health is in demand as the ideal healthcare profession. As a result of these shortages, mental health services have entered mainstream and general health facilities to create more job opportunities and open mental health as an accessible outlet to the Australian public. As an example, some general hospitals are creating a mental health management team that focuses on care in the community with support from acute care and short stay units. There has already been a 57% increase in employees within the public sector of mental health services thanks to the Third National Mental Health Plan in 2008, yet there is still a shortage of mental health professionals in Australia. Here’s a list of what current mental health professionals are earning within various specializations: - Clinical Psychologist ($66,129) - Psychologist ($60,000) - Occupational Therapist ($60,000) - Social Worker ($41,099) - Registered Nurse ($64,090) - Mental Health Clinician ($66,093) - Registered Psychiatric Nurse ($59,089)

How do I get there?

For mental health nursing, there is currently no direct entry to study mental health nursing; therefore it can only be completed as a post graduate qualification from previously studying nursing. You can also apply for graduate transition programs provided by some metropolitan and regional hospitals. These programs are usually partnered with select universities for accreditation of postgraduate subjects. They offer graduates the opportunity to complete educational preparation and receive clinical placements.

Some hospitals may also offer experienced registered nurses the opportunity to complete a transition program that the hospital offers as another pathway for registered nurses to qualify for and practice mental health nursing. Here are some handy tips if you’re set on becoming a mental health professional: - Choose mental health electives throughout your course of study when at university - Look into any programs either for work experience or traineeships as an allied health assistant in mental health - Register for transition to mental health programs - Apply for a mental health cadetship

The program developed by The Marian Centre is designed to prepared students to enter their mental health clinical practice with more confidence and encourage a positive experience. This innovation has resulted in a first for Western Australia where all five Universities have collaborated in an education program. In conjunction with the Department of Health and Aging, all 5 WA Universities (Notre Dame, ECU, UWA, Curtin and Murdoch), ARAFMI and consumers, The Marian Centre is delivering a unique program to positively promote Mental Health and the satisfaction gained working in this specialised area, with its own unique challenges and rewards.

A spike in Mental Health cases following recent natural disasters

Dr Michael Monisse-Redman, Lecturer Mental Health Edith Cowan University, said “The success of this application now means that we all benefit from an initiative that will enhance university based learning and close the divide between tertiary settings and the workplace.” Student feedback has been overwhelming and includes; ” I thoroughly enjoyed the workshop. It has sparked my interest in mental health. [I] think it should be compulsory for all nursing students.” “This course has succeeded in relieving my fears and concerns with my next prac placement.” “Interaction and the opportunity to collaborate about ideas and concepts in mental health.” The program has the support from the Commissioner of Mental Health, who identified programs like these as a positive move in delivering care in the mental health sector. According to the Australian Medical Association of Queensland (AMAQ), almost 60 per cent of doctors have reported that there has been a severe and sudden increase in mental health cases after the devastating summer of natural disasters in Queensland. The survey also showed that approximately half of the doctors that were surveyed believed the increase in cases reported will continue for at least the next twelve months. The survey also revealed a quarter of people who know that they do have a mental illness refuse to receive treatment or they do not know how to seek help. The president of AMAQ, Dr Gino Pecoraro has stated that a state-wide campaign will be launched with the hopes of encouraging and supporting friends and family to look after and take care of each other. Mr Pecoraro also said that every community should keep an eye out for signs, both for themselves and for their loved ones. “We know that about aw quarter of patients are saying they are aware that they’ve noticed feelings of anxiety or they’re not managing. We also know that a quarter of the people we’ve surveyed said that they wouldn’t feel comfortable coming out and asking for help, so what we’re trying to do is enlist the help of the community to look after each other.” “They won’t go to social engagements whereas once upon a time they did and little things will set them off. If a small setback occurred at work, once upon a time they could deal with it - now it blows up into a major thing. They’re simple things that people can see every day. If they see it in someone they love, we want them to engage with that person and encourage them to come and see somebody who can help them.” 31

Its Role in Nurse Education Nurses constantly have to adapt to challenges in the workplace while also keeping up with their primary role: care of the patient. New graduates require support to bridge the theory-practice gap as they move over into the clinical setting. Experienced nurses require extra professional development if they change practice areas, or need support in the rapidly changing health care environment. These challenges however, provide a chance for nurses to mentor and support one another, assisting knowledge transfer. All of this contributes to a strong learning environment that is beneficial to everyone.

VALUE Often seen as a tool for students and new graduates, mentorship is important for all nurses. It offers practical guidance and a venue for experienced nurses to pass on their knowledge before retiring or moving on. Unlike the teacher-student relationship, which usually involves some form of evaluation, mentorship is a long-term relationship that cultivates active learning. A positive mentorship is based on guidance, partnership, consistency and mutual respect. Some qualities of a good mentor include: > Being a good role model for best practice > Feeling genuine concern for mentee > Aims to inspire mentee > Is keen and enthusiastic about job > Has a positive attitude > Is motivated to teach and support > Has realistic expectations

MEANINGFUL EXCHANGE Effective learning occurs when the mentor and mentee have a good relationship. In the same way, low morale in the mentee can occur if there is a mentor/mentee mismatch, or because of poor mentoring traits. Some of these attributes can include: > Lack of knowledge and expertise > Breaking promises > Delegating unwanted jobs to mentee > Overprotects, or “throws in the deep end” > Often dislikes job and may be disliked by other staff > Poor teaching skills with little structure It’s important to remember that the mentee is 34

not a passive entity in the mentoring relationship. To get the most out of the connection, the mentee should drive the relationship, taking on the following roles: > Identify specific skills, knowledge and /or goals to achieve > Develop a plan, in conjunction with your mentor to achieve these goals within a specified time frame > Take advantage of any opportunities for learning > Identify resources to aid you in your learning and to achieve your goals. Resources may include other people, books and websites > Provide feedback on the relationship and learning experience

MUTUAL BENEFITS One of the biggest benefits of a mentoring relationship is that mutual learning can occur. Mentoring nurses may find themselves learning new theoretical knowledge, or simply benefiting from the enthusiasm of the mentees. In addition the relationship creates shared value, with older nurses often feeling valued as a result of passing on their knowledge. Nevertheless, just as the mentee has a role in the relationship, so too does the mentor. The following are some of the responsibilities of a mentor: > To provide guidance and support to the mentee based on their developmental needs – without “spoon feeding” > Provide feedback on mentee’s development and learning: constructive criticism is very important > Act as a source of encouragement and support > Play devil’s advocate: the mentor should aid the mentee’s learning through stimulating thought processes

The nursing environment also plays a key role in its success or failure. As practicing nurses, both mentors and mentees will have competing demands on their time. There will be a conflict between providing patient care and fulfilling the mentor/ mentee role. This may be particularly true for the mentor who could have additional responsibilities as a senior staff member. Both need to feel supported in their roles by direct and upper management, as well as by colleagues/team members. For effective learning to occur as part of the mentoring relationship, both the mentor and mentee need to take time out of the clinical environment to meet, discuss learning and provide feedback. This requires both understanding and cooperation on the part of team members, as a workplace which is not supportive of mentorship will not bring about healthy mentoring relationships, and may instead cause feelings of bitterness and jealousy in other staff, at what may be perceived as “slacking off”. Running an education session on the roles and benefits of mentoring may be of help in this situation.
