The University of Western Australia Glue ear mystery solved and researcher hears applause Perth researchers are planning to end the sleepless nights that families face when ear infections strike and won’t go away. Dr Ruth Thornton of UWA’s School of Paediatrics and Child Health is part of a team whose findings could reduce the need for antibiotics and surgery, and help tackle hearing loss in Indigenous communities. Dr Thornton is one of 12 national finalists – and the only one from WA – in the annual Australia-wide Fresh Science competition. Nearly 60 early-career scientists were selected for the first round of the competition at State level. Dr Thornton and her team discovered that sticky nets of 6

DNA hide the bacteria in the ears of kids with recurrent middle-ear infections, and evade antibiotic treatment by creating impenetrable slimy biofilms. The researchers are targeting these nets with a drug that has already proven its ability to help kids with cystic fibrosis by breaking up thick secretions in their lungs. A study on the DNA net discovery was published in the online science journal Plos One in February and clinical drug trials are now underway. “Bacteria in the ear hide in a sticky glue made up of big nets of DNA from the children’s own immune system,” Dr Thornton said. “It is similar to what happens in the lungs of people with cystic fibrosis, where a treatment known


as Dornase alfa is used to break up this sticky DNA.” “We are now trialling this treatment in the ears of children when they have grommets inserted. We believe this could get rid of these bacteria and stop children getting more infections and needing more ear surgery.” Middle ear infection, or otitis media, is one of the commonest childhood complaints. It occurs in the space behind the eardrum where fluid collects and bacteria can cause painful infections. For some children, this fluid doesn’t go away and becomes like egg white over time, leading to “glue ear”. However, some children strike it doubly unlucky with chronic infections that resist antibiotics and require the insertion of ventilation tubes, or grommets, in their eardrums. Thirty per cent of these children need repeat grommet surgery due to re-infection, and the condition’s chronic nature has been associated with hearing loss and learning difficulties. The clinical trials at UWA involve 60 children under the age of five, who will each receive Dornase alfa in one ear during grommet surgery. The other ear will be used as a comparison and receive only surgery. Results will be collected over two years, with a larger national trial planned later. There are also plans to trial the treatment in indigenous children who suffer from high rates of severe chronic middle ear infections, and which can cause extensive hearing loss and chronically weeping ears. “This is the first potential change in treating middle-ear infections for a long time, and more effective treatments will hopefully lead to improved hearing, better learning outcomes and a reduced burden on children and their families,” Dr Thornton said.

Australian Red Cross Against all odds: homeless to employed in six months “I had no house, couldn’t afford food or my bus tickets and I started crying when I came into College that day,” says Glodie. Her trainer, Amanda Wilkins, quickly saw that she was in need and jumped into action. Amanda helped the teenager find somewhere to stay and supported her whilst pushing her to achieve what Amanda knew she was capable of. Glodie explains, “Amanda took me to the homeless centre to get general support and find accommodation.” “From finding me a place to stay when I lost my home to finding me a work placement,” Glodie says that Amanda went above and beyond to help her finish her course.

Glodie, 19, moved from the Republic of the Congo to Adelaide five years ago and decided to study a Certificate III in Aged Care at Red Cross College. She absolutely loved the course and had settled into the learning environment well but came across a massive stumbling block when she was made homeless half way through. 10

I could see that she wasn’t going to give up, even when the odds were stacked against her and if we didn’t look after her, I knew she would fall through the cracks.

Compatible Care Providing quality of care to our Aged Care Within the industry, aged care has always been considered a second rate level of nursing, where Registered Nurses go when they are getting to the end of their careers, where they work with unlicensed care staff and where they feel that there is no positive outcome for the residents under their care. For many, it is perceived as an unexciting area of nursing where you do not get to use your “so called� expertise.

One of the main problems Trish encountered as DON in aged care, is a lack of parity between nurses who work in aged care and those within the acute sector, with no mandated staffing levels.

In aged care, you look after frail and often very sick people that have multiple medical conditions plus physical, emotional and end-of-life situations. 12

Trish was not comfortable with retirement and saw the need during those years of experience for better provision of agency nurses to aged care homes. She felt there were better ways to service this need in aged care and decided to open her own agency, now known as Compatible Care Nursing Agency.


“During my 28 years as a Director of Nursing, I believe along with the staff doing the day to day care of my residents, that we made a difference in the final years of those we cared for and took the pressure off their relatives.“ On Trish leaving her position to retire, or so she thought, she continued to examine the years she had spent in the area of nursing that had become such an important part of her life and an area that she could do very little about in the position she held. “We relied on agency staff to assist in filling our rosters when we were understaffed, when staff were sick, or on annual leave. However, it never quite fulfilled the necessary requirements, not because of a lack of ability but more from a lack of direction in some cases.“ This encouraged Trish to begin Compatible Care Nursing Agency, where she hoped to mould a standard of staff that she would have liked coming to her facility to care for the frail residents. “I needed my agency staff to become as committed to a facility as they are with the residents they care for and as far as possible, to be a reliable appendix to permanent staff.” The following are the important changes she believed were required: • • • • •

Quality not quantity of care staff Keep the agency personal enough to develop a close relationship with my staff Staff that are committed to caring and don’t just see it as a “pay packet” Staff that will work harmoniously with permanent staff to provide a team outcome Ability to build up closeness with facilities and assure them that as far as possible, they would get the same selection of staff on a regular basis. This would give continuity of care to the residents and assist permanent staff

“Having worked in aged care for 30 years, I have always felt fulfilled in caring for one of the most vulnerable groups in our community. Caring for people who have had a full and productive life and now due to age and failing health are unable to care for themselves. We owe our ageing community more care and respect than they sometimes receive.”

All staff would be orientated to aged care by understanding their responsibilities as per the Aged Care Act. This is achieved by giving all staff an orientation prior to their first shift and regular ongoing education and training Staff that can service shifts regularly at very short notice To ask our clients to provide assistance in giving our staff adequate handover procedures and other routine duties pertinent to the facility

Services for Australian Rural and Remote Allied Health Rural placement is key to boosting aged care workforce After your experience in Broken Hill, what is the main message you would tell others about working in rural aged care? I feel that working in the aged care setting as an allied health professional seems to get a bit of a bad rap. Prior to my experience in Broken Hill, I can honestly say I had very little interest in working in the aged care setting. I can now proudly say that with my wonderful rural Broken Hill experience and with the constant support and engagement of all staff, health professionals and aged care residents, I would happily work within the aged care setting. Not only is it a wonderful opportunity to help and improve the health of our elderly, it is also a fantastic opportunity to listen and learn from them. I like to think that from listening to their incredible stories, I have returned to Adelaide much more open minded, and perhaps even a little wiser. What were your first impressions of rural aged care? Erin Healy, a student dietitian at Flinders University in Adelaide, and SARRAH Scholar.

Erin Healy, a student dietitian at Flinders University in Adelaide, thought she was not cut out for aged care. That is, until she spent seven weeks in Broken Hill on a rural placement scholarship offered by Services for Australian Rural and Remote Allied Health (SARRAH).

I was very impressed with the aged care facility, as I can honestly say that I did not know what to expect. All areas of the facility (dining room, lounge room, individual bedrooms) felt very homely and welcoming. All residents seemed comfortable in the facility and the staff were very accommodating. I found it a little overwhelming at first, however after speaking with many residents, it became clear that being in aged care gave them a sense of security. They had a strong sense of community, with activities, performances, morning teas and other entertainment held daily. This was fantastic as it kept all of the residents active and social, with one lady


saying to me she had more of a social life now that she had moved into the aged care facility! Had you ever lived or worked in a rural area prior to this experience? Yes, I have had a lot of experience living and working in rural towns during my childhood. I lived in the town of Roxby Downs for five years prior to moving to the Clare Valley in 2000, so I am a country kid at heart despite moving to Adelaide in 2009 for university. What were some of the daily duties you were given on your rural placement? Initially, I was involved in screening all of the aged care residents for malnutrition and getting an overview of their knowledge of food and nutrition, and the role that nutrition plays in falls prevention. If a resident was screened as “at risk of malnutrition” or “malnourished” both myself and my project team were responsible for then referring them on to see a dietitian at the Broken Hill hospital. In addition to this, myself and the project team spent considerable time designing, preparing and delivering information and education sessions to both staff at the aged care facility and the residents. These sessions again focussed on nutrition and falls prevention in the elderly and aimed to eventually improve the nutritional status of the aged care facility residents and therefore reduce their risk of falls in the future. How did you find the patients? The residents were incredibly welcoming and always up for a chat! It was humbling how encouraging and supportive they were of our team’s efforts. I love speaking with people and I found that in this setting, perhaps more so than a busy hospital setting, there was more time to sit and speak with each individual resident about their interests, life stories, families and much much more. This allowed me to get to know the residents and helped me to build strong professional relationships with them. This then translated into allowing the residents to build trust in me and the nutrition program which was so important.

It seemed that in the rural health care setting, communication between different disciplines was very highly regarded and well implemented. I think this is very important and hope to see in my future as a dietitian that this “multidisciplinary approach” to patient care continues to be adopted. Would you consider a rural career after this experience? I have always been very open to the idea of working in a rural setting as a dietitian due to growing up in rural towns. However, my experience in Broken Hill has cemented this and I would now jump at the opportunity to work in a rural setting. Not only did I love my experience working in an aged care setting in a rural town, I also loved the relaxed way of life in Broken Hill and my personal achievement of moving away from home for seven weeks and continuing down the very exciting road to becoming a dietitian!
